The judge's view of competency evaluations. II.

Twenty trial court judges were surveyed to determine what information they considered pertinent in psychiatric examinations for competence. These judges showed a clear understanding of what they were asking for in ordering the examinations but also showed a significant tendency to use the competency exam to advise them about other issues in addition (e.g., dangerousness or the need for treatment). As a group the judges appeared to be eager for psychiatric input. Typical judges could be described as pragmatic in their views of psychiatry in the courtroom, having a relatively low level of expectation but a high degree of satisfaction with the psychiatric opinions they receive.

The influence of age on the fragility of the cornea.

Corneal fragility was measured with the Cochet-Bonnet aesthesiometer on 171 eyes of various ages. It was found to remain the same until about age 40. It increased significantly and progressively afterwards. It was compared to corneal sensitivity which diminishes significantly after about the same age.

Sensitivity and fragility in keratoconus.

38 persons with various degrees of keratoconus were examined. Using the photokeratoscope an index of the severity of the cone was established and compared to a group of normal eyes. Sensitivity was measured in the centre and periphery of the cornea. It was found to be significantly lower than in normal eyes and those eyes wearing contact lenses exhibited the lowest sensitivity. There was no correlation between peripheral CTT (Corneal Touch Threshold) and the severity of keratoconus, but there was a significant correlation between central CTT and severity, though only for the group of eyes not wearing contact lenses. Corneal fragility was also found to be greater in keratoconus than in normal eyes and more so in those eyes wearing contact lenses. There was a significant correlation between the severity and the fragility.

Mood and affect: a semantic confusion.

Although semantic clarity is a cornerstone of scientific psychiatry, psychiatrists often use technical words with the false assumption that their meanings are clear. The common use of the terms "mood" and "affect" illustrates this point. The authors attempt to show how unclear definitions of these words have led to confusion and how they could be more clearly understood by careful semantic and phenomenologic analysis.
