[Multilocular erythema migrans in borreliosis].

BACKGROUND: Borreliosis is the most common vector transmitted disease in childhood. Although the disease manifests with an erythema migrans in 80 % of the patients, multilocular skin manifestations are only observed in 2-18 % of these. Differential diagnoses of erythema migrans include erysipelas, persistent insect bite reaction, and fixed drug eruption, in particular when the clinical history does not reveal a tick bite. PATIENT: We report on a 5-year-old boy showing nine erythemas with central pallor on his face, trunk, arms and legs. He recalled a tick bite 3 weeks before. RESULTS: Serological studies revealed an acute infection with Borrelia burgdorferi. After antibiotic treatment with orally administered amoxicillin skin manifestations resolved within three days. During a follow-up period of six months the patient revealed no signs of persistent borreliosis. CONCLUSION: Multilocular erythema migrans is a possible manifestation of borreliosis and is classified as disseminated early infection which is frequently associated with systemic reactions, including malaise, arthritis, carditis, headache and even meningeal signs. Treatment is based on antibiotics, which should preferably be given intravenously in case of systemic signs.

Contribution to the study of tetracycline bone side-effects. Absence of calcium deposition impairment in the trabecular bone of a patient treated during 3.5 years with doxycycline.

Tetracyclines are easily incorporated into bone; whether bone formation is impaired during this process is still a matter of debate. We therefore studied, by histomorphometry, fluorescence microscopy, and microradiography, a sample of trabecular bone from a patient with a total hip arthroplasty, treated without interruption for 3.5 years with doxycycline because of a pertrochanteric abscess. Absence of osteomalacia and a low-mineralization score of the edges of the trabeculae similar to that observed in a control group of 12 subjects suggest no interference by doxycycline with calcium deposition in bone.

Adult septic arthritis with Hemophilus influenzae.

Two cases of Hemophilus influenzae septic arthritis are reported, one in a rheumatoid patient and the other in a healthy young woman after meningitis. The evolution of this rare infectious arthritis was not as good as usual in spite of early therapy. The first patient later presented a Staphylococcus albus bacteriemia and the other a post-infectious persisting arthritis.

Age-specific doses of lormetazepam as a night sedative in cases of chronic sleep disturbance.

Lormetazepam, a new benzodiazepine derivative, was tested under double blind conditions in order to find the optimal dosage for different age groups of out-patients. 120 patients suffering from chronic sleep disturbance were included in the study: a younger group (age 20 to 55) and an older group (age 56 to 85 years). Four different doses were given to each age group: 0.5, 1.0, 2.0, and 3.0 mg to the younger group and 0.5, 1.0, 1.5, and 2.0 mg to the older group. A pre-placebo week (i.e. when all patients received placebo) in which baseline data were recorded preceded the two verum weeks, and these were followed by a post-placebo or withdrawal week (again all patients receiving placebo). The level of significance accepted for statistical decisions was alpha = 0.05. No differences in effects between the different doses were observed with regard to sleep pattern variables (sleep latency, sleep duration, frequency of awakenings, sleep quality, occurrence of 'bad' dreams) with the exception of sleep quality which was better in the older group than in the younger group after 0.5 mg in week 2. Considerable differences with regard to hangover feelings the next morning and during the next day (morning feelings, tranquility, alertness, and concentration), comparison of the effects of discontinuing therapy upon the above-mentioned sleep pattern variables and small differences in side effects--which were few--led to the following conclusion: --0.5 mg stood out as the best dose for the older group. --None of the dosages given to the younger group emerged clearly as superior. However, it would seem that the 1 mg dose should be the dose recommended, since fewer unfavourable scores and side effects appeared after this dose.

[Extraspinal erosions of tendon insertions in ankylosing spondylitis].

A study based on the clinical observation of 9 adults carrying the HLA B27 antigen. Seven of these patients had a typical ankylosing spondylitis with bilateral sacro-iliitis. The other two cases were considered to have a minor form of the disease, without sacro-iliitis. In various zones of tendinous or ligamentous insertion (on the calcaneal and ischial tuberosities, the anterior tibial tuberosity, the patella, the lateral femoral condyle, the metataŕso-phalangeal epiphyses of the index finger, the head of the humerus), the patients complained of pain and presented radiological images of more or less developed bony erosion. These erosion of insertions are not specific to ankylosing spondylitis, but they can be suggestive of the diagnosis. They can be an important element in the diagnosis of pre-spondylitic forms, especially in adolescents. Certain histological findings suggest that these lesions are a form of localized algodystrophy.

Symmetrical reactive oligoarthritis after Campylobacter jejuni enteritis--case report and study of the synovial complement.

A 34-year-old man, in whom sacroiliitis had been diagnosed 5 years previously, presented in July 1982 with reactive arthritis following Campylobacter jejuni enteritis. Diarrhoea was stopped by erythromycin but joint effusion recurred. In order to clarify the relationship between Campylobacter jejuni and the immunological system, we proceeded with a study of the synovial complement. The results were compared with those obtained in some other arthropathies.
