Assay method for myeloperoxidase in human polymorphonuclear leukocytes.

A simple assay method for measuring myeloperoxidase (MPO) has been developed. MPO is found in polymorphonuclear leukocytes and is important as a bactericidal agent in the presence of H2O2 and halide ions. This improved assay method is based on work of Andrews and Krinsky using tetramethylbenzidine (TMB) a noncarcinogenic substrate. By assaying MPO under optimal conditions of TMB at 1.6 mM, H2O2 concentration of 0.3 mM, pH 5.4, and incubation temperature of 37 degrees C, sensitivity of MPO measurements increased eightfold in comparison with the original TMB method. A method has been established to determine absorbance at 655 nm of the reaction mixture by incubation for 3 min and then stopping the reaction by the addition of pH 3.0 buffer. An attempt was also made to raise the sensitivity by using 3,3'-dimethyoxybenzidine (DMB), a carcinogenic substrate. The improved TMB method was 34 times more sensitive than the DMB method.

A preliminary study on clinical application of Nd:YAG laser to the face and head.

Preliminary experiments were carried out for use of the Nd:YAG laser therapy of malignant tumors of the head and face. Because bone and brain tissue lie under the soft tissues in the face and head, the authors prepared two experimental models and examined the thermal effects of laser irradiation using three irradiating methods, single-pulse, automatic running, and manual. In experiments using results from these models, a temperature rise inducing protein coagulation did not occur under the surface of the soft tissue under the bone in both models. With automatic running irradiation, the temperature rise in the surface of the soft tissue under the bone was about 5 degrees C at 60 W with a running speed of 4 mm/sec in model I. These conditions were considered to be sufficiently within safe limits. In the second model, it was estimated that no thermal effects would be exerted on the soft tissue under the bone at 60 W or less with a running speed of 8 mm/sec or less. Using manual irradiation, no thermal disturbance of the soft tissue under the bone was induced in the first model, but without a shortening of irradiation time or an increase in irradiation distance this method could cause thermal damage to the second model and must be taken into consideration.

Endocrinological and histological aspects of the process of polycystic ovary formation in the rat treated with testosterone propionate.

To elucidate the process underlying polycystic ovary (PCO) syndrome, we tried to produce experimental PCO by sterilizing rats with androgen and investigated the formation of PCO endocrinologically and histologically. Wistar-Imamichi strain female rats were given an injection of testosterone propionate 1.25 mg on 5 days of age. On 5, 10, 20, 40, 60, 80, 100, 130, 160, 200 days after the injection the ovaries were obtained. Ovarian wedge resection was also performed at 100 and 200 day-rats. Serum gonadotropins, prolactin and steroids were measured with RIA and compared with ovarian pathology. Serum prolactin was increased drastically 40 days after the injection and maintained at a constant level higher than control. Through wedge resection, the serum LH level returned to normal, but FSH slightly increased. Serum estradiol decreased more markedly than estrone did. The 40 day-ovaries showed polycystic features, lack of corpus luteum, hyperthecosis, luteinization of stroma, while no thickening of tunica albuginea. In conclusion, the rats treated with testosterone propionate had many endocrinological and histological aspects similar to those of human PCO syndrome.

[Tissue penetration and clinical effects of ceftazidime in the field of obstetrics and gynecology].

To study concentrations in the gynecological organs, ceftazidime (CAZ) was given intravenously by one shot of 0.5 g to 1 patient and of 1.0 g to 10 patients who underwent simple hysterectomy because of uterine myoma. Also, clinical effect of the drug was studied in 3 cases. The levels of CAZ in sera in uterine artery and elbow vein, and tissue concentrations in various sites of the gynecological organs obtained during 50 to 425 minutes after administration were determined by the paper-disc method with P. mirabilis ATCC 21100 strain. Concentrations of CAZ at 92 minutes after administration of 1.0 g i.v. were 39.8 mcg/ml in serum of uterine artery, 47.6 mcg/ml in serum of elbow vein, 20.5 mcg/g (tissue/serum ratio 0.43) in the ovary, 18.0 mcg/g (0.38) in the fallopian tube, 16.1 mcg/g (0.34) in the endometrium, 34.5 mcg/g (0.72) in the myometrium, 30.8 mcg/g (0.65) in the uterine cervix, 36.4 mcg/g (0.76) in the portio vaginalis and then gradually decreased time-dependently. Tissue concentrations were higher than those in serum in the endometrium, uterine cervix and portio vaginalis at 147 minutes after administration. CAZ concentrations of adipose tissue between 353 and 365 minutes after injection of 1.0 g were lower than 0.8 mcg/g. The clinical responses to CAZ in intrapelvic infections were good in all cases. Slight elevation of total bilirubin, to 1.5 mg/dl, was observed in a case. In the light of these clinical findings, CAZ appears to be a potent antibiotic effective in the clinical application.

[Comparative study of x-ray and endoscopy in the detection of polyps and cancer of the large bowel with office computer].

A comparative study of X-ray and endoscopy in the detection of polyp and cancer of the large bowel was done with the use of office computer, and following result was obtained. False positive and negative rate of X-ray in the detection of polyp was 31.5% and 16.3% respectively when X-ray preceded endoscopy. False negative rate of X-ray and endoscopic diagnosis was 40.8% and 28.6% respectively in the detection of polyp, when endoscopy preceded X-ray. But the possibility that the false negative of endoscopy might be the false positive of X-ray was not excluded in many cases. The false positive and negative rate of X-ray diagnosis was 21.0% and 7.0% respectively in the detection of cancer when X-ray preceded endoscopy. The false negative rate of X-ray diagnosis was 2.8% in the detection of cancer when endoscopy preceded X-ray. It was emphasized that X-ray should be preceded by endoscopy in the detection of polyp and cancer of the large bowel in order to reduce the false positive and negative rate of X-ray diagnosis to the minimum.

[Clinical studies on 6059-S in the field of pediatrics (author's transl)].

6059-S was administered to 32 children with various acute bacterial infections (bronchopneumonia 11, bronchitis 1, pyelonephritis 5, acute enteritis 6, purulent infection 4, secondary infection due to agranulocytosis 5) at the dose of 21 to 190 mg/kg/day for 2 to 12 days. The clinical response of 6059-S was very satisfactory in all 17 cases with the injection of respiratory tract or urinary tract infection, but it was not so favourable in 5 cases of secondary infection due to agranulocytosis. The overall clinical response was excellent in 5, good in 20, fair in 4, and failure in 3 with effective rate of 78%. As to side effect, each one case diarrhea and elevation of GOT and GPT was noted.

[Prevalence of osteoarthrosis of the hip and other joints in Japanese population (author's transl)].

A population study on osteoarthrosis (OA) was made in Iwata district of Kamitonda Wakayama. In the present survey, 1335 inhabitants aged 30 years and over were surveyed from August 1972 to May 1974. The completion rate was 69.1%. The examinations consisted of measurements of blood pressure, body height and weight, blood and urine test and radiographs of hand, cervical spine and pelvis. Osteoarthrotic changes of wrist and finger joint, cervical spine and hip joint were detected and assessed according to Kellgren and Lawrence's criteria by two observers respectively. The results obtained were as follows. 1) The prevalence of OA of the hip joint in this population sample of 737 persons was 0.94%. 2) Primary OA of hip joint was found in 0.27% of the same sample. 3) Compared with Caucasians, the incidence of OA of hip joint was low and similar to that of the Chinese in Hong Kong. 5) The incidnece of radiological osteoarthrosis of wrist, finger joints and cervical spine was comparable in the British studied by Kellgren and Lawrence. 6) The incidence of OA of the first carpometacarpal joint was very low in Japanese regardless the sex.
