Advance directives: development, opposition, and criticism.

Since the PSDA became law on December 1, all Medicare and Medicaid health care providers are required to develop policies and procedures to comply with the law. The principles of autonomy and distributive justice are basic to the Act, and the social worker's vital role will continue to develop as the law is implemented.

Tuberculosis in AIDS patients: an ethical dilemma for discharge planning.

Discharge planning with AIDS patients has become more complex since the resurgence of tuberculosis in this group. Such psychosocial problems as drug use, homelessness, and poverty have also contributed to the difficulty in discharge planning because of issues of noncompliance with medical regimens and the subsequent development of drug-resistant strains of TB. Ethical conflicts, resulting from balancing respect for patient autonomy with the obligation to cause no harm to society, arise for the health care professional who coordinates the discharge plan. Every effort to decrease the incidence of rehospitalization, especially through the emergency department, should be taken to control the cost of inpatient care. These efforts should focus on arranging for directly observed therapy and placing the homeless in shelters that promote outpatient treatment.

Antimicrobial agent resistance in bacterial isolates from patients with diarrhea and urinary tract infection in the Sudan.

Antimicrobial sensitivity tests were performed on four-hundred and ninety-seven bacterial isolates from Sudanese patients with diarrhea or urinary tract infections. Shigella dysenteriae type I and enteropathogenic Escherichia coli showed high resistance rates (percentage of isolates showing antibiotic resistance) against the commonly-used antimicrobial agents: ampicillin, amoxicillin, chloramphenicol, tetracycline, cotrimoxazole, nalidixic acid, sulfonamide, and neomycin, and were completely sensitive to ciprofloxacin. Eighteen resistance patterns against nine antimicrobial agents tested were observed in enteric pathogens. Resistance to ampicillin, amoxicillin, tetracycline, cotrimoxazole, and sulfonamide was the most frequent pattern. The common urinary pathogens, E. coli, Klebsiella pneumoniae, and Proteus mirabilis showed high rates of resistance to ampicillin, amoxicillin, cotrimoxazole, tetracycline, sulfonamide, trimethoprim, streptomycin, and carbenicillin. We recommend that physicians seek updated knowledge of the common antibiotic-sensitivity patterns when starting empirical antibiotic therapy in Sudanese patients with diarrhea or urinary tract infection.

Prevalence of iodine deficiency disorders in the United Arab Emirates measured by raised TSH levels.

The United Arab Emirates National Screening Programme for Congenital Hypothyroidism was established in January 1998. The programme measures neonatal thyroid-stimulating hormone (TSH) levels of blood samples collected on filter paper on day 5 by heel prick. The prevalence of raised TSH levels (> 5 microU/mL whole blood) during 1998 and 1999 was used to evaluate the degree of iodine-deficiency disorders (IDD) in the population in different regions. The ratio of TSH profile in the present study and goitre rate in schools in a 1994 study were discrepant, although there was good correlation between the ratio of TSH profile and urinary iodine. The prevalence of raised TSH levels was < 3% in the Emirates overall, which is normal, and IDD varied from mild to normal problems in different regions.
