Ultrasonographic anatomy of veins draining the left lobe of the liver. Feasibility of live related transplantation.

The anatomic variations of the left hepatic vein (LHV) in 182 patients with normal liver functions were analyzed using ultrasonography in order to clarify the feasibility of live related hepatic transplantation (LRHT). LHV was demonstrated in 151 patients (83%). Based on the location of the confluence of the middle hepatic vein (MHV) and LHV and the ramifications of the left medial vein (LMV) and the left superior vein (LSV), the livers were classified into 3 types: a) intrahepatic confluence (42/151 livers, 27.8%); b) extrahepatic confluence (69/151 livers, 45.7%); c) separate insertion (40/151 livers, 26.5%); and 23 subtypes. The length of the common trunk and the intrahepatic common trunk of MHV and LHV and the distance from LMV or LSV to the confluence of MHV and LHV were measured. Overall, the control of LHV in situ for LRHT appeared easy in 34/151 patients (22.5%), while in 44/151 patients (29.1%), the length of free LHV and the common trunk was less than optimal for procurement of a left hepatic lobe graft with complete venous outflow and sufficient extraparenchymal length of LHV.

[Comparison of intravenous drip infusion excretory urography using ionic and non-ionic contrast media].

Ionic and non-ionic contrast media were compared for use in intravenous drip infusion excretory urography. Sixty consecutive cases were classified alternately into two groups, ionic and non-ionic, excluding cases which were known to have factors affecting contrast ability adversely, such as hydronephrosis, renal failure and so on. Each group consisted of 30 cases. A hundred ml of the ionic contrast medium (Diatrizoate--60% Urografin) or non-ionic contrast medium (Iopamidol--Iopamiron 300) was administered I.V. by drip infusion, each infusion taking less than 10 minutes, using a 18 G needle, and 0-minute, 10-minute and 20-minute films were obtained. Visibility of nephrogram, calyceal system, renal pelvis, ureters and bladder were evaluated, ranging from 0 to 3, by six radiologists who were not informed of the contrast medium used. Some detailed factors such as radiographic contrast and fullness of the collecting system and of the bladder were also analyzed. Visualization rate of the ureters was defined as the length of visualized ureters divided by the distance between the renal pelvis and the ureteral orifice of the bladder. There was no significant difference in visualization of nephrogram between the two groups. However, the non-ionic group was superior to the ionic group in visualization of the calyceal system and ureters with statistical significance, probably due to higher radiographic contrast of non-ionic medium. There was no statistical significance between the two groups, in fullness of the calyceal system and visualization rate of the ureters, whereas the ionic group was superior to the non-ionic group in fullness of the bladder.(ABSTRACT TRUNCATED AT 250 WORDS)

[Diagnostic capability of digital radiography of the chest using scanning laser stimulated luminescence].

Diagnostic capability of Fuji Computed Radiography (FCR) of the chest was compared to the conventional radiography (CoR) using regular film-screen system. FCR utilizes imaging plates of scanning laser stimulated luminescence. Visibility of 14 structures of the chest radiography was evaluated by 3 radiologists in 100 pairs of FCR and CoR which were taken at the same time with the same exposure factors. FCR was superior to CoR especially in observation of the mediastinum and areas behind the heart and diaphragm. The minor fissure was better seen on CoR. Superiority of FCR to CoR was thought to be mainly due to the processed image of FCR, and the so-called normal image of FCR had little diagnostic advantage. Simulated abnormal densities: nodular, alveolar, and interstitial densities with a chest phantom, were made, and detectability of alteration of these densities on FCR and CoR was evaluated by 19 radiologist, using ROC curve analysis. FCR and CoR revealed the similar diagnostic capability in nodular and interstitial densities. In alveolar densities, however, FCR was statistically inferior to CoR. Nineteen radiologists were divided into 3 groups by their experience in radiology, and comparison was made as to their diagnostic capability with FCR and CoR in the same manner as described above. There was no significant difference among the groups with FCR in either density pattern.

Ultrasonography of benign gastric ulcers. Characteristic features and sequential follow-ups.

Ultrasonography was performed for 15 patients with gastric ulcers, after tap water ingestion using 5-MHz and/or 7.5-MHz transducers. Sonographic signs of gastric ulcer were classified as gastric wall edema associated with ulcer crater (six cases) and gastric wall edema only (nine cases). The latter nine included two cases of perforation of gastric ulcers that were depicted as gastric wall edema associated with fluid collection. Ultrasonography proved useful for detecting benign ulcerations and can be used to supplement follow-up examinations, but it cannot replace endoscopy and contrast radiography.

Treatment of desmoid tumors in Gardner's syndrome. Report of a case.

Aggressive desmoid tumors present difficult problems among patients with Gardner's syndrome. Recently, attention has been directed toward metabolic or hormonal manipulation of these tumors. A 21-year-old woman with Gardner's syndrome was admitted because of recurrent abdominal wall tumors. She was treated with nonsteroidal anti-inflammatory drugs, tamoxifen, and ascorbate for seven months. During this therapy, CT scan showed a gradual increase in size of the tumors. Subsequent resection of the abdominal tumors and the colon was performed. Although these three types of drugs were administered to prevent postoperative recurrence, an abdominal wall desmoid tumor that invaded the mesentery developed within nine months. The known treatments, that is, chemotherapy, irradiation, and surgical resection are discussed in view of this experience.

[Imaging diagnosis of hepatobiliary system and pancreas--computed tomography, ultrasound and angiography].

CT, US and angiography have become the most important examinations in the diagnosis of hepatobiliary and pancreatic diseases. Therefore updated and precise knowledge of these modalities are mandatory not only for the diagnosis but also for the interventional radiology in this field. General review on the advantages and limitations of these medical images are described.

Posttranscatheter embolization computed tomography in hepatocellular carcinoma. Evaluation of embolization effects.

The postcontrast CT scans and angiography of 26 lesions in 23 patients with hepatomas who underwent transcatheter arterial embolization therapy were reviewed. Newly developed low density and high density areas and gas were noted on CT scans of the tumors after embolization in 100, 8 and 58% of the lesions, respectively. The CT findings of residual tumors correlated well with those of follow-up angiography. When discrete smooth round low density zones were observed on CT, there was no residual neovascularity on follow-up arteriography. When there were nodular or thick septum-like structures in low density lesions, arteriography revealed residual tumor vascularity in the lesions. CT proved useful not only in evaluating the effects of embolization, but also in providing important information about residual tumors.

Computed tomography of renal metastases.

The CT manifestations of renal metastases from nonurinary tract carcinomas were evaluated in five autopsy-proven cases. Three cases resembled renal inflammatory disease or renal cysts. The wide variety of CT manifestations of renal metastases underscores the need to bear renal metastases in mind when renal abnormalities are observed in any case of advanced malignancy.
