[Therapy of advanced endometrial cancer].

In recent years, the incidence of endometrial cancer has a tendency to increase gradually in our country. Its majority (stage I and II) is a case to be treated by hysterectomy alone. However, to patients with advanced inoperable cancer (stage III and more), a radiotherapy, which is not so sensitive to endometrial cancer, has been used. Since the progression of endometrial cancer is dependent on sex steroid hormones (estrogen and progesterone), anti-tumor effects of progestogen are expected to be effective to patients with estrogen receptor (ER) positive and progesterone receptor (PR) positive cancer or with well-differentiated adenocarcinoma (G1 type) histologically. The most widely used progestogens are medroxyprogesterone acetate (MPA) and megestrol acetate (MGA). Many investigators have reported that progestogen with high dosage shows a good response to advanced endometrial cancer. On the other hand, the monochemotherapy responsive to endometrial cancer is adriamycin (ADR), cyclophosphamide (CPA), 5-fluorouracil (5-FU) or cisplatin (CDDP). The drugs in polychemotherapy regimens are used to be combined basically the above anti-cancer agents. The polychemotherapy is more effective than monochemotherapy. The combined chemotherapy regimen (CPA, ADR and CDDP; CAP regimen) obtained a good clinical results to advanced endometrial cancer. Thus, in recent years, the combined hormonochemotherapy of high dose progestogen and polychemotherapy was recommended as the best therapy to advanced endometrial cancer, and reported a good results. In conclusion, the treatment of advanced endometrial cancer is based on the use of progestogen therapy and on polychemotherapy. The choice of treatment is made on the basis of patients' conditions and the biological characteristics of the endometrial carcinoma.

[An experience of anesthesia in a case of WPW syndrome].

WPW (Wolff-Parkinson-White) syndrome is a rare disease characterized by electro-cardiographic anomalies associated with a history of recurrent supraventricular tachycardia. ECG abnormalities consist of a short PR interval and a broad QSR complex with a slurred upstroke. We experienced general anesthesia in a 29-year-old male with this syndrome for operation of maxillary cyst. Physical laboratory examinations of this patient revealed his conditions of within normal limits with the exception of ECG findings. After premedication with atropine, hydroxyzine and pethilorfan intramuscularly one hour prior to anesthesia, anesthesia was induced with intravenous thiopental 325 mg and the trachea was intubated with intravenous succinylcholine 40 mg. Thereafter anesthesia was maintained with 2.0% enflurane and 67% nitroxide in oxygen under controlled ventilation. During 1 hour operation, vital signs remained stable and paroxysmal tachycardia was not recognized on the ECG. The postoperative course was uneventful.

[A case of general anesthesia with Pierre Robin syndrome and scoliosis].

We experienced a case of general anesthesia in a one-year-old boy for palate plasty with a slight micrognathia, cleft palate and scoliosis. At the first anesthesia, we tried two times oral intubation under the spontaneous respiration after slow induction with halothane, nitrous-oxide in oxygen, twice but were unsuccessful. In site of easy intubation with succinyl choline chloride rales and bronchial secretions increased and the operation was postponed. Ten months later, the same operation was planned and re-tried using same method as the first anesthesia. As this time, it was relatively easy to intubate, blood pressure and heart rate were stable during the operation. The recovery from anesthesia was smooth. From the experience of this case, we recognized again that preoperative respiration management was very important for these patients.

[Lactating mammary tissue in a mature cystic teratoma of the ovary].

Discussed is a mature cystic teratoma that was surgically removed from the left ovary, said teratoma containing lactating mammary tissue of a 30-year-old Japanese lady, primigravida and in her 36th gestation week. The tumor measuring 3 X 3 X 3 cm, was accidently found when she underwent a cesarian section. In the lactating glandular tissue of the teratoma, the application of an anti-alpha-lactalbumin serum revealed diffuse and/or reticular cytoplasmic stainings of almost all the luminal cells and secretory materials. Further, myoepitheliall cells were clearly visualized with anti-actin serum. References are made to 2 other case reports of similar tumoral tissue.

[Study on the local estrogen biosynthesis in human uterine leiomyoma].

The growth of uterine leiomyoma is regulated not only by the estrogen levels in blood, but also by estrogen production in the tumor itself. In this study, we investigated both the estrone formation (estrone sulfatase activity) and the transformation from estrone to estrone sulfate (estrone sulfotransferase activity) in human leiomyoma tissue. We compared them with those in the myometrium and endometrial tissues overlying and opposite a uterine leiomyoma node. We also measured the tissue concentrations of estrone, estradiol and estrone sulfate. Estrone sulfatase activity in the uterine leiomyoma was 0.49 +/- 0.08 nmol/h/mg protein (Mean +/- SE), and was lower than that in the myometrial tissue (0.76 +/- 0.09). Moreover, the enzyme activity was higher in the endometrium overlying the leiomyoma node (2.62 +/- 0.29) than in the endometrium opposite to it (2.10 +/- 0.24). On the other hand, estrone sulfotransferase activity in the myoma (20.2 +/- 2.4 pmol/h/mg protein) was higher than in myometrial tissue (16.7 +/- 2.3). The concentrations of estrone and estradiol in leiomyoma tissue were lower than in the tissues surrounding the leiomyoma. The tissue concentration of estrone sulfate was higher in leiomyoma tissues. These results suggest that estrone sulfate is hydrolyzed mainly by estrone sulfatase in the endometrium and myometrium surrounding leiomyoma nodes and the estrone formed may affect the growth of leiomyoma.

[The diagnostic critical value in DFO low dose loading test on patient with aluminum accumulation compared to each parameter of ROD].

One hundred two hemodialyzed patients were examined to determine the standard level of delta Al value which is the difference of serum Al concentrations between pre and post DFO loading test. We applied low dose of DFO 15 mg/kg in this loading test. Some significant negative correlations were found between delta Al and MCI, sigma GS/D, osteocalcin, free-Hydroxyproline (dialysate) and free-gamma-Carboxyglutamic acid (dialysate). Each correlation rate was -0.58, -0.44, -0.76, -0.57 and -0.51 respectively. In addition tendencies of correlation were found between delta Al and ALP and between delta Al and %QCT (QCT/mean QCT in patient's age x 100). And statistical significant differences were found between (0 less than delta Al less than or equal to 150 micrograms/l) group and (150 micrograms/l less than delta Al) group in each osteobiochemical parameter. These results indicate that 150 micrograms/l is the lower diagnostic standard level of delta Al in 15 mg/kg DFO loading test.

[A case of recurrent ovarian cancer markedly responding to daily oral administration of low dose etoposide].

A 67-year-old woman with recurrent ovarian cancer, who had carcinomatous peritonitis and large abdominal cystic lesions, was treated by daily oral etoposide at a dose of 50 mg/day for 21 consecutive days at 2-week intervals in the outpatient department. After 15 days of administration, the serum level of CA125 decreased from 2,220U/ml to 445U/ml, and that of LDH from 2,725 WU to 906 WU. After the third course of treatment, the abdominal cystic lesions were remarkably reduced, and the performance status improved grade 1 from 3 prior to the administration. No severe side effects except for alopecia and mild myelosuppression were noted during the 3-month treatment.

Multiple forms of adenylate kinase in pig brain.

The rate of ATP formation from ADP by adenylate kinase is known to be easily followed by determination of the increase in fluorescence due to the NADPH that is formed by combined reactions of hexokinase and gluc-6-p dehydrogenase. We found that the rate of CTP formation from CDP also can be followed similarly by use of more units of hexokinase and extension of the reaction time. A crude enzyme sample containing most of the activity of adenylate kinase was prepared from pig brain and eluted from a CM-cellulose column. Enzymatic activity of ATP formation and that of CTP formation were compared for all the fractions. Two fractions were found in the eluate; one was rather specific for ADP as substrate, and the other was less specific for ADP and could form CTP at an appreciable rate.

[A case of tracheal calculus in the pathogenesis of which fungal infection was probably involved].

A 63-year-old woman with a past history of bronchial asthma was admitted to our hospital with a diagnosis of status asthmaticus. Examination revealed a ring-shaped, calcified stone within the trachea immediately below the vocal cords. As the patient complained of increasing difficulty in breathing, emergency tracheotomy was performed in an attempt to save her life, with success. Bronchial calculus (broncholithiasis) has been occasionally reported, and it is hypothesized that the calculus is mostly caused by intra-bronchial perforation of calcified lymph nodes around the bronchus. Tracheal calculus is a rare disease, and our case is only the third ever reported in Japan. However, perforation of calcified lymph nodes was probably not responsible for this case, which probably resulted from repeated chronic inflammation and fungal infection.

[An autopsy case of multiple myeloma accompanied by extensive nodular infiltration into the extraskeletal tissue].

Discussed is the case of a 41-year-old Japanese man dead at 4 months after start of his clinical course of multiple myeloma of the Bence-Jones protein lambda type of the skull. Autopsy findings revealed that atypical plasmacytes had infiltrated extensively into the testes, the retroperitoneum, including the pancreas and the renal pelvis, the bilateral pleura, the right subclavian soft tissue, and the meningism, but not to the liver, spleen, and lymph nodes. In both testes, which weighted 120 g and 100 g, respectively, myeloma cells were found disseminated throughout the testicular tissue. Such a testicular involvement in cases of a multiple myeloma is extremely rare. The pertinent literature also is reviewed.

[An autopsy case of postirradiation cutaneous angiosarcoma of the hip].

Presented is the case of a 63-year-old woman who developed a cutaneous angiosarcoma of the hip after receiving adjuvant radiation therapy for a squamous cell carcinoma of the uterine cervix after a hysterectomy. The time lapse between the radiation therapy and the occurrence of the angiosarcoma was 15 years, and its systemic metastasis was seen on autopsy. This autopsy was carried out 26 years after the patient had received the initial irradiation and no recurrence of the uterine cancer was found. Factor involved in the etiology of an angiosarcoma are discussed, and 34 previously reported cases of a postirradiation angiosarcoma are reviewed.

[Urothelial tumor in dialysis patients; report of four cases].

Four cases of urothelial tumors diagnosed during hemodialysis were reported. Patients' ages ranged from 43 to 74 years old. One case was female and others were male. Durations of dialysis at the time of diagnosis were 13 to 24 months. Chief complaints of these four cases were gross hematuria. One case with bladder tumor underwent emergency operation because of repeated bladder tamponade. Partial cystectomy was performed on this case. Other two cases with bladder tumor were treated with endoscopic method. Simple nephrectomy was done in the case with renal pelvic tumor. Post operative course in all four cases were uneventful without major complications.

[Effect of propiverine hydrochloride on the function of the bladder in decerebrated dogs].

The effect of propiverine hydrochloride (P-4) on the function of the bladder in decerebrated dogs was compared with that of propantheline, an anticholinergic drug which is used for the treatment of micturitional disorders. P-4 (5 and 10 mg/kg, i.v.) significantly increased maximum vesical volume (Vmax); and at the dose of 10 mg/kg, i.v., it also significantly increased effective vesical volume (EV) when determined by cystometry. However, at the dose level of 2, 5 and 10 mg/kg, i.v., P-4 had no effect on residual volume (RV) after micturition contraction. Propantheline (0.25 and 0.5 mg/kg, i.v.) also significantly increased Vmax, but showed no effect on EV. At doses higher than 0.016 mg/kg, i.v., propantheline significantly increased RV. These findings indicate that P-4 is a useful drug for the treatment of pollakiuria.

An arginine carboxypeptidase generated during coagulation is diminished or absent in patients with rheumatoid arthritis.

A novel arginine carboxypeptidase that is generated during blood coagulation is diminished in sera obtained from patients with rheumatoid arthritis. The enzyme, which is unrelated to carboxypeptidase N, is more effective than lysine in removing terminal arginine from small synthetic substrates and may function in vivo in the removal of terminal arginine from inflammatory peptides such as C3a and C5a. Either diminished levels of this enzyme or an inability to generate it may be an important consideration in the mechanisms involved in the local inflammation that is observed in patients with rheumatoid arthritis.

[Pharmacokinetic and clinical studies on aztreonam in the perinatal period in obstetrics and gynecology].

Pharmacokinetic and clinical studies on aztreonam (AZT) in the perinatal period in obstetrics and gynecology were performed with the following results. 1. Concentrations of AZT in maternal serum, umbilical cord serum, amniotic fluid and neonatal serum were determined after 1 hour intravenous drip infusion of 1 g. The maternal serum concentration was 32.2 micrograms/ml at 26 minutes after administration, gradually decreasing thereafter to 13.2 micrograms/ml at 2 hours 33 minutes, 4.9 micrograms/ml at 3 hours 21 minutes and 2.9 micrograms/ml at 5 hours 3 minutes. Umbilical cord serum concentration was 17.0 micrograms/ml at 36 minutes after drip infusion and still remained at 4.0-16.1 micrograms/ml at 5 hours after administration. Amniotic fluid concentration was 9.9 micrograms/ml at 3 hours 21 minutes after drip infusion and showed 3.3 micrograms/ml at 16 hours 26 minutes after administration. Most of the maximum serum concentrations of newborns between 3 to 24 hours after delivery were not detectable, with only one case with 2.2 micrograms/ml at 9 hours after delivery. 2. AZT 1 or 2 g x 2/day was given by intravenous drip infusion to 12 cases of perinatal infections in obstetrics and gynecology for 5 to 8 days. Clinical efficacies were evaluated as excellent in 8 cases, effective in 2 and not effective in 2 with 83.3% efficacy rate. With respect to side effects, minor degree of urticaria was observed in 1 case. Another case showed a transient elevations of GOT, GPT and Al-P in laboratory tests.

[A recurrent gastric cancer improved by radiation therapy].

In March, 1988, a 65 year-old male underwent a total gastrectomy for a gastric cancer (Borrmann 3: a poorly differentiated adenocarcinoma; IIc: signet ring cell carcinoma). In October that year, however, an anastomotic recurrence of this tumor was found. Thus, from November, 1988 to January, 1989, to treat this recurrence, the patient received a course of radiation therapy frontally, towards the upper abdomen, with a total dose of 46.5 Gy (linac X-ray). His symptoms improved, and a CT, an upper gastrointestinal barium study, and an endoscopic examination indicated that the tumor had disappeared. No further recurrence was found until January, 1990, when the patient died of a metastasis that had spread throughout his body.

[Valvuloplasty for acute mitral insufficiency caused by torn chordae (Carpentier method): report of a case].

A 66-year-old female was admitted to our hospital with acute heart failure (NYHA III), and diagnosed as an MR + Tr. The cause of MR was torn chordae of the posterior leaflet of the mitral valve. A prolapse part of the posterior leaflet was resected and sutured by McGoon's method. Valvuloplasty was performed by using a Carpentier ring. Postoperative course was uneventful. She recovered well after the operation. Histological examination of the valve showed myxomatous degeneration.

Enzymatic properties of isozymes and variants of glucose dehydrogenase from Bacillus megaterium.

Three glucose dehydrogenases (GlcDH) from Bacillus megaterium, GlcDH-I, GlcDH-II and GlcDH-IWG3, were purified from Escherichia coli cells harboring one of the hybrid plasmids, pGDK1, pGDK2 and pGDA3, respectively, pGDK1 and pGDK2 contain two isozyme genes, gdhI and gdhII, respectively, from B. megaterium IAM 1030 and pGDA3 contains an isozyme gene from B. megaterium IWG3; GlcDH-IWG3 is a variant of GlcDH-I. GlcDH-I and GlcDH-II have similar pH/activity profiles and the profile for GlcDH-IWG3 is identical to that of GlcDH-I. The pH/stability profiles of these enzymes show that GlcDH-IWG3 is the most stable enzyme in the acidic region, while GlcDH-II is the most stable in the alkaline region, and GlcDH-I is the most unstable throughout the entire pH range examined. As for thermostability, GlcDH-II is the most resistant against heat inactivation at pH 6.5. The values of the first-order rate constant for heat inactivation at 50 degrees C are 0.27 min-1, 0.05 min-1 and 0.11 min-1 for GlcDH-I, GlcDH-II and GlcDH-IWG3, respectively. Kinetic studies show that these enzymes have similar kinetic constant values except that there are some differences in Kia for NAD(P) and Ka (the limiting Michaelis constant) for NAD; the values of the ratio of Kia for NAD and NADP are 11,340 and 8.7 for GlcDH-I, GlcDH-II and GlcDH-IWG3, respectively. GlcDH-I and GlcDH-IWG3 have very similar substrate specificities and GlcDH-II has a slightly higher specificity for D-glucose and 2-deoxy-D-glucose than the others. The results are discussed on the basis of the amino acid substitutions between the enzymes.
