[A case of histological complete remission by preoperative intraarterial infusion chemotherapy].

We experienced a case of histological complete remission by preoperative intraarterial infusion chemotherapy in the treatment of histologically diagnosed endometrial cancer of well differentiated type. MRI demonstrated invasion of the myometrium to a depth of more than 1/2 and left partial parametrium. This patient was given intra-uterine arterial infusion chemotherapy twice (CDDP & ADM ia, CPA iv). Radical hysterectomy was done 3 weeks after intraarterial infusion chemotherapy. The extirpated uterus was histologically complete remission. Thus, in this case shows that intraarterial infusion chemotherapy is effective for neo-adjuvant chemotherapy of endometrial cancer.

[Neurons in monkey parietal association cortex sensitive to axis orientation].

Patients with parietal lesions may fail to adjust the orientation of the hand to that of the target object or may make errors in judging the orientation of the bar. This suggests that the parietal cortex functions to discriminate the orientation of objects. Therefore, we studied orientation selectivity of the visual neurons of the posterior parietal association cortex of alert behaving monkeys. We recorded 23 neurons that showed selectivity in the axis orientation of visual stimulus in the lateral bank and fundus of the caudal part of the intraparietal sulcus (IPS). Almost all the neurons tested with a slit on the screen responded better to a longer or narrower stimulus. There were also neurons which responded only to three dimensional objects, such as a bar presented in the sagittal orientation tilted forward or backward. The majority of these neurons had wide receptive fields and their responses were position-invariant. These results suggest that the axis orientation selective neurons of the parietal cortex represent orientation of the longitudinal axis of objects in 3-dimensional space.

Application of 13C NMR spectroscopy to paratope mapping for larger antigen-Fab complexes.

For the purpose of engineering the antibody combining site, mapping residues that are involved in antigen binding provide us with valuable information. By use of 13C NMR spectroscopy with selectively 13C-labeled Fv fragments, we have established a general strategy to identify the residues that are perturbed upon binding of small antigen (hapten) molecules [(1990) Biochemistry 30, 6604-6610]. In the present paper, we demonstrate that this strategy can be extended to molecular structural analyses of the complexes of an Fab fragment and a larger antigen molecule such as Pseudomonas aeruginosa exotoxin A with a molecular mass of 67 kDa.

Nonsense mutations at Arg-1947 in two cases of familial neurofibromatosis type 1 in Japanese.

We report two familial cases of NF1 presenting as C to T transitions changing an Arg-1947 codon to a stop codon. In one of the two families, cosegregation of the mutation with NF1 was demonstrated, indicating this mutation causes the disease in this family. As the same mutation at Arg-1947 has been reported previously in three cases of unrelated Caucasians (two are sporadic; the origin of the other is not reported), the codon at Arg-1947 (CGA) in the NF1 gene is considered to be a hotspot common among different ethnic groups and also among familial and sporadic cases.

Pilonidal sinus of the axilla: report of five patients and review of the literature.

Five patients with a pilonidal sinus of the axilla--thought to be the 9th to 13th described in the literature--are reported. All patients were nonhirsute, healthy women aged 17 to 30 years. One or two hairs were seen macroscopically and histologically in 3 of 5 patients, and 1 hair was seen macroscopically in the remaining 2 patients. Friction (abduction-adduction) or suction, as well as assorted factors such as shaving, minor infection, and maceration, seemed to be the most probable cause of pilonidal sinus of the axilla. Complete resection, including the openings of 1 or 2 sinuses, is the only suitable treatment for axillary suppuration.

Oral florid papillomatosis and leukoplakia of the esophagus associated with keratoderma and showing transepidermal elimination.

A 37-year-old male presented with a cauliflower-like tumor in the right buccal mucosa, leukoplakia of the tongue and esophagus, and keratotic papules or plaques on the extremities including bilateral palms and soles, associated with borderline diabetes mellitus and a repeated history of infections. A multi-system keratinizing abnormality was suspected, although no distinct disease category has been found showing similar involvements.

Difference in allelic expression of genes probably associated with tumor progression in murine fibrosarcomas and cell lines.

Allelic expression was examined by single-strand conformation polymorphism analysis in murine fibrosarcomas from inter-subspecific F1 mice between C57BL/6 and MSM. Ten genes encoding p53, mdm2, E-cadherin, 72 kD metalloproteinase and its inhibitor (Timp2), thymidine kinase and four glucose transporters (Gluts) were examined. These genes were chosen because of their probable association with tumor development and progression. In some of the tumors and cell lines, p53, E-cadherin and Glut3 genes showed remarkable differences in allelic expression, one allele being poorly expressed. The allele-specificity persisted in nine cell lines obtained by repeated transplantations from one tumor. These results suggest that expression of some genes is allele-specific in tumor cells and the pattern of specificity is stable. Such a decrease or a loss of expression in one of the alleles may be functionally equivalent to the loss of heterozygosity of the gene, and therefore this may confer malignant properties on tumor cells. It is also suggested that differential expression of two alleles is a common event in tumor cells.

Amplification of a c-yes-1-related oncogene in canine lymphoid leukemia.

Genomic DNAs from 4 canine spontaneous lymphoid neoplasms were examined by Southern blot hybridization using 7 kinds of oncogene probes. Analysis using a human c-yes-1 cDNA probe revealed amplification of the proto-oncogene in a DNA sample from a dog with lymphoid leukemia. Its degree was about 4- to 8-fold in comparison with a control DNA sample from a healthy dog. The genomic abnormality of canine c-yes-1-related oncogene may have a role in the tumorigenesis of this neoplasm, although the significance of the structural change in this oncogene remains unclear.

Mutation of the p53 gene in gallbladder cancer.

Genetic alteration of the p53 gene was examined in tissue specimens of primary gallbladder cancer (GBC) using PCR-single strand conformation polymorphism (SSCP) and direct sequencing analyses. The p53 gene mutation was detected in five of the 16 GBC cases examined (31%). All five cases showed single point mutations; four were mutations resulting in amino acid substitution and the other was one base-pair deletion. Of the five mutations detected, four occurred at evolutionarily conserved regions and the remaining one at a well conserved region among species. Our result is similar to those of previous reports for various human cancers in mutation scattering over four of the five conserved domains.

Gastric carcinoma, an endoscopically curable disease.

Following refinement of endoscopic diagnosis of gastric cancer, the concept of 'early gastric carcinoma' and its endoscopic features have become established. After the wide acceptance of the techniques of endoscopic mucosal resection (EMR), laser therapy and accurate endoscopic-endosonographic assessment of the depth of invasion, curative endoscopic treatment of intramucosal gastric carcinoma has become established. On the basis of the analysis of lymphatic metastasis, the indications for endoscopic treatment of gastric carcinoma have been defined as (1) well-differentiated, intramucosal adenocarcinoma of a superficial elevated lesion (diameter < 2 cm), and (2) well-differentiated, intramucosal adenocarcinoma of a superficial depressed lesion (diameter < 1 cm) having no ulcer or scar inside. The preferred procedure of endoscopic treatment is EMR with careful histological examination of the resected specimen. It is supplemented by repeat EMR, laser therapy, or surgical gastrectomy in case of incomplete resection. For detection of residual cancer and recurrence, periodic endoscopic follow-up is necessary. Early gastric carcinoma is endoscopically curable.

Electronic endoscopy in perspective.

Electronic endoscopy, developed in 1983, enables the endoscopic image to be transmitted through electric signals that can be easily processed by computer. An electronic endoscope is composed of three vital parts, i.e., a charge-coupled device (CCD) that converts the image to electric signals, a video processor that converts analog electric signals to the digital and processes them to become video signals, and a television monitor. The introduction of electronic endoscopy has enabled computer management of endoscopic images, on one hand, and provided an opportunity for image processing and analysis, on the other. A digital image management system will further develop into a database of endoscopic images. Combined with the technologies of remote control and remote sensing, the introduction of CCD to endoscopy has made it possible to transform the endoscope, so that it could become a capsule. Future advances in the research of image processing and analysis will lead to the development of a system of automated endoscopic diagnosis.

The effect of recombinant human erythropoietin (r-HuEPO) on left ventricular mass and left ventricular hemodynamics in hemodialysis patients.

The purpose of this study was to examine the effect of recombinant human erythropoietin (r-HuEPO) on left ventricular mass. Twenty-seven hemodialysis patients (13 men and 14 women) were given r-HuEPO for renal anemia. Blood pressure and heart rate were measured before and after the 16-week course of r-HuEPO, and at the same time echocardiography was performed to measure left ventricular dimensions and wall thickness. These measurements were used to calculate left ventricular volume, cardiac output (CO), and left ventricular mass (LVmass). Diastolic blood pressure (DBP) increased after administration of r-HuEPO (from 75.8 +/- 10.8 mmHg to 85.6 +/- 12.7 mmHg), but there was no change in systolic blood pressure (SBP) or heart rate. LVmass increased significantly in seven cases (from 194.7 +/- 40.0 g to 240.3 +/- 47.3 g). These cases, Group I, showed no decline in stroke volume (SV) or CO, and showed significant increases in SBP. In the remaining 20 cases, Group II, LVmass decreased or was unchanged. In this group SV and CO decreased, but there was no increase in SBP. We conclude that increases in LVmass may be associated with elevated systolic blood pressure and hypertrophy of the left ventricular wall, when hemodialysis patients with severe renal anemia are given r-HuEPO.

A variant family of mouse minor satellite located on the centromeric region of chromosome 2.

A variant sequence of minor satellite that exhibits a polymorphism in strains and a subspecies of the mouse by pulsed-field gel analysis has been cloned. A segregation study using intrasubspecific backcross mice reveals that the variant maps to the centromeric region of chromosome 2. A long-range physical analysis of the region shows that the variant family is probably located between the proximal telomere and the major satellite. The sequence probe provides an important marker of the chromosome 2-specific centromere.

Protective activity of anti-exotoxin A monoclonal antibody against mice infected with toxin-producing Pseudomonas aeruginosa.

The neutralizing and protective effect of murine monoclonal antibody (MAb) 3C7 (IgG1) against Pseudomonas aeruginosa exotoxin A was examined in an experimental mouse model of infection with exotoxin A-producing strains. Treatment with MAb 3C7 blocked the reduction of functional elongation factor 2 (EF-2) in the liver of mice but could not clear the bacteria. Administration of gentamicin caused bacteria to be cleared but did not block reduction of hepatic EF-2 level. Treatment with either MAb 3C7 or gentamicin individually did not prolong time to death; however, the combined therapy with both MAb 3C7 and gentamicin cleared bacteria and blocked the reduction of hepatic EF-2 level, resulting in a significant increase in the survival rate of mice. These results suggest that anti-exotoxin A MAbs show effectiveness against pseudomonal infection caused by exotoxin A-producing strains.

Carney's complex with primary pigmented nodular adrenocortical disease and spotty pigmentations.

Carney's complex is composed of myxoma, spotty pigmentation and endocrine overactivity. A 27-year-old male was diagnosed to have Carney's complex on the basis of intense spotty pigmentations on his face, soles and palms, and bilateral adrenal nodular hyperplasia on computed tomography scanning (CT) and magnetic resonance imaging (MRI). Total bilateral adrenectomy was done; histological findings were compatible with primary pigmented adrenocortical disease (PPNAD). Recently, his sister and one of his brothers were suspected to have Carney's complex with PPNAD. We report the first familial case of Carney's complex with PPNAD and spotty pigmentations in Japan.

[An experimental and clinical study of chorioretinal photocoagulation using a frequency-doubled Nd: YAG laser].

We developed a new frequency-doubled Nd: YAG laser system which works on 100 volts AC and emits continuous wave green laser light of 532 nm. Histological observation of rabbit eyes revealed no difference in chorioretinal lesions produced with the frequency-doubled Nd: YAG laser or with an argon green laser. This device was then clinically applied to 69 eyes of 59 patients with several disorders such as diabetic retinopathy, retinal vein occlusion, Eales' disease, Coats' disease, age-related macular degeneration, lattice degeneration in the peripheral retina, macular hole, and retinal tear. The ophthalmoscopic findings showed no visible differences in the lesions made with this device or with the argon laser. No complications were observed in these patients except for one case of central retinal vein occlusion, which developed vitreous hemorrhage after the treatment. This solid-state laser system has some mechanical advantages over the gas laser, i.e. reduced cooling requirements, easy maintenance, and long life, and it is considered to be usable clinically.

Thrombin generates monocyte chemotactic activity from complement factor H.

We have recently found that the complement factor H (H) was the precursor of the major macrophage chemotactic factor in the delayed-type hypersensitivity (DTH) reaction site in the skin and was converted to the factor by an unidentified trypsin-like protease in plasma. Thrombin and plasmin are also present in the site, and we, therefore, examined the possibility that these proteases converted H to be monocyte chemotactic. Intact H caused no monocyte migration, although it was able to do so after incubation with thrombin, but not with plasmin. The activity was chemotactic rather than chemokinetic and was absorbed by an anti-H IgG-conjugated column. The generation of monocyte chemotactic activity from H was dependent on incubation time with thrombin and also the protease activity of thrombin, and the activity was seen at concentrations of H lower than 10(-8) M. The inhibitory activity of H for C3b-Bb was not affected by incubation with thrombin or plasmin. Incubation of H with thrombin, but not with plasmin, generated a hydrophobic molecule, in a time-dependent manner, which had monocyte chemotactic activity. These results show that H becomes a monocyte chemotactic factor due to cleavage by thrombin, which converts H to a more hydrophobic molecule and also suggest that thrombin-treated H induces monocyte migration in the DTH reaction site.
