[Glomerular apolipoprotein B deposition in glomerular diseases].

In an attempt to elucidate the relationship between the progress of glomerular injury and abnormalities of lipid metabolism, we investigated glomerular deposition of apolipoprotein B (apo B) in renal biopsy specimens from 60 patients with glomerular diseases by indirect immunofluorescence using antihuman apo B-100 monoclonal antibody in comparison with clinical and histopathological findings. The patients were divided into 2 groups according to the intensity in staining of apo B in glomerulus (group A: negative or weakly positive; and group B: definitely positive). Staining of apo B in glomerulus was found in 37 patients (62%). The levels of serum total cholesterol, phospholipids, low density lipoprotein cholesterol and apo B in group B were significantly higher than in group A. The urinary protein excretion in group B was greater than that in group A. Group B was also shown to have a significantly decrease in renal function. Light microscopy revealed severe mesangial proliferation in patients with IgA nephropathy of group B. These findings suggested that glomerular apo B containing-lipoprotein deposition may play an important role in the progression of glomerular injury.

Indium-111-antimyosin antibody imaging for detecting different stages of myocardial infarction: comparison with technetium-99m-pyrophosphate imaging.

The diagnostic value of 111In-antimyosin (AM) imaging for identifying myocardial infarction was evaluated in comparison with 99mTc-pyrophosphate (PPi) imaging. Twenty-four patients with various stages of myocardial infarction, ranging from three days to nine months after the onset of infarction, underwent both AM and PPi scans. Of 26 infarct lesions AM scan identified 22 (85%), while PPi scans detected 10 (38%) (p less than 0.01). When less than a week had passed since the onset both scans demonstrated all infarct lesions. For seven subacute lesions studied within one to two weeks of onset, AM scans detected (100%), while PPi scans identified only 2 (29%). Furthermore, AM scans showed discrete myocardial uptake in 7 (64%) of those studied more than two weeks after onset. The intensity of AM uptake in the infarcts studied more than seven days after onset was less than that in acute infarcts studied within seven days of onset (p less than 0.05). These preliminary data indicate that the abnormal myocardial uptake of AM persists beyond the first two weeks when PPi no longer accumulates. Thus, AM scans can be considered to provide a sensitive diagnosis of subacute as well as acute myocardial necrosis.

Significance of fill-in after thallium-201 reinjection following delayed imaging: comparison with regional wall motion and angiographic findings.

To identify reversible defects, reinjection of a small amount of thallium-201 (201Tl) following 3-hr delayed imaging was performed in 60 patients with coronary artery disease who had perfusion abnormalities on their post-exercise 201Tl images. Thallium-201 uptake was visually scored and judged as normal (Group 1), reversible defect (Group II), new fill-in after reinjection (Group IIIa) and no fill-in even after reinjection (Group IIIb). New fill-in after reinjection was observed in 27 segments of the 85 segments (32%), showing persistent defect on the stress and delayed images. The wall motion in Group IIIa was worse than Group II but better than Group IIIb. Group IIIa showed Q-wave on ECG more often (69%) than Group II (27%) (p less than 0.01), but less often than Group IIIb (85%) (p less than 0.05). These data indicate that the reinjection 201Tl imaging often identifies new fill-in in the areas of no redistribution on the delayed images and it may hold promise for assessing tissue viability which the conventional imaging may underestimate.

[The role of cholesterol and phosphatidylcholine on very low density lipoprotein secretion from rat hepatocytes].

The role of cholesterol and phosphatidylcholine on hepatic very low density lipoprotein secretion was investigated with rat hepatocytes. Hepatic very low density lipoprotein secretion (apo B, apo E, cholesterol, triglyceride, phosphatidylcholine) was decreased by the reduction of hepatic phosphatidylcholine content. However, cholesterol loading into hepatocytes did not affect hepatic very low density lipoprotein secretion. Hepatic apolipoprotein contents were constant and were not influenced by a change of hepatic cholesterol and phosphatidylcholine contents. These results suggest that lipids, which are constituents of lipoproteins, play different roles on hepatic very low density lipoprotein secretion.

Effects of dietary cholesterol and fatty acids on plasma cholesterol level and hepatic lipoprotein metabolism.

The effects of dietary cholesterol and fatty acids on the plasma cholesterol level and rates of very low density lipoprotein (VLDL) cholesterol secretion and low density lipoprotein (LDL) transport through LDL receptors in the liver of the hamster were investigated. Increases of plasma VLDL- and LDL-cholesterol levels and VLDL-cholesterol secretion from hepatocytes were observed in animals fed a diet enriched with 0.1% cholesterol for 2 weeks in comparison with animals fed a control diet. The addition of dietary palmitic acid accelerated the effect of dietary cholesterol on plasma VLDL- and LDL-cholesterol levels and VLDL-cholesterol secretion from hepatocytes. Dietary linoleic acid accelerated the effect of dietary cholesterol on VLDL-cholesterol secretion from hepatocytes and diminished the effect on the plasma LDL-cholesterol level. Hepatic LDL receptor activity was considerably suppressed by a control diet containing 0.05% cholesterol and a further small suppression was induced by a diet enriched with 0.1% cholesterol with or without 5% palmitic acid. However, dietary linoleic acid diminished the effect of dietary cholesterol on the suppression of hepatic LDL receptor activity. These results suggest that dietary palmitic acid augments the effect of dietary cholesterol in elevating the plasma LDL-cholesterol level through acceleration of VLDL-cholesterol secretion from the liver, and that dietary linoleic acid diminishes the effect of dietary cholesterol in elevating the plasma LDL-cholesterol level by preventing the suppression of hepatic LDL receptor activity induced by cholesterol.

[Assessment of resting thallium-201 reinjection after stress-delayed Tl imaging (I): Comparison with 24-hour scan and regional wall motion].

Clinical value of resting reinjection of 1 mCi (37 MBq) of Tl after stress-delayed scan was assessed in comparison with 24 hr delayed scan and regional wall motion (RWM) in 37 patients with coronary artery disease. Of 101 segments with initial perfusion abnormality, concordant findings were observed after Tl reinjection in 67 segments (66%). But redistribution (RD) after Tl reinjection was observed in 19 of the 52 persistent defect (PD) segments (37%), and complete RD was observed 15 of the 43 segments (35%) where 3 hr scan showed incomplete RD. On the other hand, concordant findings were observed on 24 hr delayed scan in 11 of the 20 segments with perfusion abnormality (55%). RD on 24 hr delayed scan was observed in 5 of the 13 PD (38%), but it was not apparent in 3 of the 7 segments where 3 hr scan showed RD. Furthermore the segments which showed RD after Tl reinjection in PD segments tend to have less severe RWM abnormality than the sustained PD segments (RWM score: 1.65 +/- 1.17 vs. 2.29 +/- 1.05, p less than 0.05). These data indicate that Tl reinjection which permits assessment of RD with good quality images on the same day is considered as a valuable means for assessing myocardial viability.

[A parathyroid hormone related protein (PTHrP) implicated in hypercalcemia associated with malignancy: research of the PTHrP for novel hormonal tumor marker].

A cDNA complementary to the parathyroid hormone related protein (PTHrP), the humoral hypercalcemia factor in malignancy, was recently isolated and sequenced. PTHrP expression in human carcinomas was examined (determined) by Northern blot hybridization, Southern blot hybridization and radioimmunoassay (RIA). Expression of PTHrP mRNA was detected in three out of four lung squamous cell carcinomas, two out of ten breast carcinomas and the one adenosquamous carcinoma of the maxilla. No clinical hypercalcemia was found in these PTHrP mRNA positive carcinomas. No expression of PTHrP was detected in normal human or rat tissues. In Southern blot hybridization, no amplification of PTHrP gene was found in PTHrP positive cases. An insertion in one allele of promotor region of the gene was identified in one PTHrP positive lung squamous cell carcinoma. The serum level of PTHrP was examined using human PTHrP (1-34) RIA in lung carcinomas. We found no correlation between the level of PTHrP and clinical hypercalcemia or the histopathological diagnosis. We discuss some problems of the PTHrP assay as a novel tumor marker for malignancies. A new RIA assay study using recombinant human PTHrP expressed in Escherichia coli is also reported.

[Fatty acid metabolism using C-11 palmitate: (1). Resting study].

Serial dynamic images were obtained at rest following intravenous injection of 185-370 MBq (5-10 mCi) of 11C-palmitate for 40-50 minutes in 3 normal subjects and 6 patients with coronary artery disease. Due to the rapid clearance of activity from the blood, myocardial images were observed within 5 minutes after the injection in each case. The time activity curve of each myocardial region indicates biexponential clearance. The early clearance half time (Te 1/2) was similar in normal myocardium (18.8 +/- 6.0 min), ischemic myocardium (19.3 +/- 5.8 min) and infarcted myocardium (15.5 +/- 6.0 min). The late clearance half time (Tl 1/2) was also similar among these groups (81.4 +/- 36.0, 66.9 +/- 25.7 and 57.8 +/- 37.4, respectively). However, the residual fraction (%E) was slightly lower in the ischemic myocardium (19.6 +/- 11.0%) (p less than 0.05), indicating decreased beta oxidation in the former. Although positron emission tomography using 11C-palmitate may permit assessment of fatty acid metabolism, the parameters in this study were greatly fluctuated at rest among the normal, ischemic and infarcted myocardium. Further investigations under various interventions may be warranted.

[Evaluation of complete left bundle branch block with normal coronary artery using positron emission CT (PET) and Tl-201 scintigraphy].

False-positive Tl-201 scintigraphy in patients with complete left bundle-branch block (CLBBB) and normal coronary artery had been reported by several authors. However the reason for this phenomenon remained to be clarified. We investigated 2 patients (case 1; a 65-year-old male, case 2; a 58-year-old male) with CLBBB using positron emission computed tomography (PET). Coronary angiography revealed normal coronary artery in both cases. In both cases, Tl-201 stress scintigraphy demonstrated definite perfusion defect at the anteroseptal region without redistribution, although PET study using F-18 fluorodeoxyglucose (18F-FDG) revealed defect at the septal wall. This indicates that perfusion defect detected by Tl-201 scintigraphy relates rather not to ischemic myocardium with a viable region, but to necrotic or fibrous tissue. Thus, PET is a useful tool in investigating viability of abnormal myocardium when other study has failed to do so. Our cases with CLBBB suggested that there was abnormal flow and metabolism at the anteroseptal region, possibly relating to the pathogenesis of CLBBB.

Expression of lacto series type 2 antigens in human renal cell carcinoma and its clinical significance.

We performed immunohistochemical examination of serial sections of human fetal and adult renal tissue as well as human renal carcinoma tissue, using monoclonal antibodies T5A7, 1B2, FH2, FH4, and FH6. These monoclonal antibodies were directed to lacto series type 2 antigens with sugar-chain structures: lactosylceramide, lactoneotetraosylceramide (paragloboside), Lex (a chemically well-defined fucosyl carbohydrate antigen), difucosyl Lex, and sialosyl-difucosyl Lex, respectively. The staining pattern in fetal renal tissue changed significantly according to the stage of organogenesis. In addition, expression of the antigens, especially paragloboside and sialosyl-difucosyl Lex, was closely related to the prognosis of the patient. These results suggest that the expression of a series of oncofetal antigens in development or differentiation of organs is reflected in the reversion to an immature pattern of antigenic expression in tumor tissue. The pattern of antigen expression in renal tumors offers a good criterion for ascertaining the degree of tumor differentiation and malignancy and is valuable for determining prognosis.

The influence of ouabain on twitch contractions in the presence of veratridine.

1. Direct stimulation evoked twitches in mouse diaphragm muscles in presence of 10 microM D-tubocurarine in vitro. Effects of ouabain and their dependence on K+ were examined on the twitch responses and action potentials in the presence and absence of twitch potentiators. 2. Ouabain inhibited twitch contractions only in the presence of veratridine, aconitine and monensin while it had no inhibitory effect on control twitches. The interactions between ouabain and these twitch potentiators depended on the presence of external K+, except in the case of monensin. 3. Removal of Ca2+ from a bathing solution accelerated the potentiating effect of veratridine and the antagonizing effect of ouabain. 4. Caffeine further potentiated the twitches which had been attenuated by ouabain combined with veratridine. 5. Ouabain combined with veratridine consistently decreased resting membrane potentials, action potentials and overshoot potentials and prolonged time to peak of and duration of the muscle action potentials. 6. Tetraethylammonium, 4-aminopyridine, and caffeine produced twitch potentiation which was insensitive to ouabain or the removal of K+. 7. These results suggest that twitch contractions in the presence of activators of sodium channels link with activation of Na+-K+-ATPase. Accumulation of Na+ inside the muscle fibres may uncouple the excitation-contraction system. 8. This uncoupling may not include the caffeine-sensitive process that controls the release of Ca2+ from the sarcoplasmic reticulum. Na+ accumulation may decrease transmembraneous gradient of this ions, thereby causing a reduction in excitation coupled with twitch contraction.

Microvascular changes in the stroma of human colorectal carcinomas: ultrastructural histochemical study.

Ultrastructural histochemical studies were performed using Ulex europaeus agglutinin-I lectin (UEA-I) and anti-endothelial monoclonal antibody BMA 120 in order to morphologically characterize the microvasculature of human colorectal carcinomas. In the normal mucosa, UEA-I and BMA 120 were bound to luminal plasma membrane of endothelial cells, usually continuously. Capillaries in the stroma of invasive adenocarcinomas showed remarkable structural changes such as swelling of endothelial cells with well-developed cell organelles and narrowing of the lumen. Reaction products for UEA-I and BMA 120 were both observed along the luminal plasma membrane of endothelial cells, usually discontinuously, partially retaining the features of normal capillaries. Furthermore, we have confirmed an occurrence of solid capillary buds composed of a strand of large endothelial cells with a trace of lumen. They showed almost no reactivity to UEA-I and BMA 120. Conventional electron microscopy revealed that they were present frequently in invasive carcinomas, but infrequently in intramucosal carcinomas and inflammatory lesions. Our results suggest that the stroma of invasive colorectal carcinomas abounds in immature capillaries and their precursors, which may indicate active tumor-induced angiogenesis.

The influence of ouabain on twitch potentiation by anticholinesterases in the phrenic nerve-diaphragm muscles of mice.

1. (+)-Tubocurarine, hexamethonium, atropine, ouabain, and removal of potassium from the bathing medium were examined for their effects on indirectly evoked twitches (IT) of mouse phrenic nerve-diaphragm muscles in the presence or absence of neostigmine. 2. Neostigmine increased the amplitude of IT. The twitch potentiation was reduced by (+)-tubocurarine at low concentrations that had no inhibitory effect on normal IT. Hexamethonium (10-100 microM), but not atropine (0.1-1 microM), partially inhibited the twitch potentiation. Neither hexamethonium nor atropine had an inhibitory effect on IT in the absence of neostigmine. 3. Ouabain (5 microM) abolished the twitch potentiation by neostigmine while having no inhibitory effect on directly evoked twitches in the presence of neostigmine and (+)-tubocurarine together. 4. The potentiating effect of neostigmine was less in a potassium-free bathing solution. The inhibitory effect of ouabain disappeared in this solution. 5. Reinclusion of KCl at 2.5 mM restored both the potentiating effect of neostigmine and the antagonistic effect of ouabain. This reinclusion did not potentiate IT in the absence of neostigmine. 6. An interaction resembling that between ouabain and neostigmine was obtained between ouabain and physostigmine or paraoxon. 7. Both endplate potentials (e.p.ps) and miniature e.p.ps increased in terms of their amplitude and duration in the presence of neostigmine. Ouabain did not reduce the enhanced endplate responses. 8. These results indicate that the potentiation of IT by anticholinesterases may occur via nicotinic receptors which are sensitive to both (+)-tubocurarine and hexamethonium, and that the interaction between anticholinesterases and ouabain depends on the presence of K+. It appears that the mechanisms of twitch potentiation are dependent on the ionic gradients maintained by Na+-K+-ATPase.

[An outbreak of enterocolitis due to Clostridium perfringens in a hospital for the severely disabled].

We had an outbreak of 14 cases of enterocolitis due to Clostridium perfringens (Cl. perfringens) in a hospital for the severe multiply-disabled, where the 100 disabled were admitted, in summer in 1985. The signs and symptoms shown by this enterocolitis were primarily diarrhea without fever and loss of appetite. The feces of 10 cases were examined bacteriologically. The test showed 10(3) to 10(6) cells of Cl. perfringens per one gram of their feces and all the strains isolated were untypable by the classification of Hobbs. Nine out of 10 cases were randomly selected and all of the 9 cases were proved to have enterotoxin producing strains. All the strains were highly sensitive to many kinds of antibiotics except kanamycin and gentamicin. Eleven out of the 14 cases were admitted in the same ward and the 7 out of the 11 cases were in the same room of this ward. Considering the spreading route of this infection, it is unlikely that this outbreak occurred due to food supplied from kitchen in this hospital, because all of the disabled, admitted in this hospital, had little chance by which some of the disabled only in a specific ward or room were supplied with bacteriologically contaminated meals from the point of view of cooking and supplying system of this hospital. Adding to this fact, if this outbreak was due to food-born infection, the symptoms of most patients should occur within 1-2 days, because the incubation period of this disease is within a day, however, the patients increased day by day for more than a week.(ABSTRACT TRUNCATED AT 250 WORDS)

Production performance of laying hens fed L-tryptophan.

The purpose of this study was to determine whether supplementing L-tryptophan to a nutritionally complete diet would influence egg production rate and egg shell quality in laying hens. One hundred and twenty layers (Rhode Island Red x White Leghorn) were randomly allocated into three groups. Birds were fed a basal diet or diets supplemented with L-tryptophan at 250 or 500 mg/kg for 405 days from 25 wk of age. Hen day egg production and daily egg output were proportionately higher with higher levels of supplemental dietary L-tryptophan; the differences were significant (P less than .05) for layers fed the 500 mg tryptophan/kg diet. The improvement in egg production by dietary L-tryptophan was greatest during the latter part of the laying period. These findings suggest that supplemental L-tryptophan improves the egg production rate in laying hens.
