[Effects of etoposide in testicular tumors].

A clinical study of a new semisynthetic podophyllotoxin etoposide (NK 171) was performed by administering the drug to four patients with testicular tumors. The drug was given intravenously at a dose of 80-120 mg/m2/day for five days or orally at a dose of 200 mg/body/day for five days. Among the four patients, 1 CR, 1 NC and 2 PD were observed. One patient with a testicular tumor who was given oral etoposide achieved complete remission for 23 months. Patients experienced the following side effects: alopecia (75%); anorexia (50%): stomatitis, phlebitis, anemia and liver dysfunction (25%). None of these side effects, however, was serious or permanent. Currently, we are testing etoposide in combination chemotherapy.

[Behçet's disease with Budd-Chiari syndrome. Apropos of a case. Review of cases autopsied in Japan and of the literature].

A case of Behçet's disease with Budd-Chiari's syndrome in 31-year-old japanese is reported. In the Annuals of Pathological Autopsy Cases in Japan from 1974 to 1983, 122 autopsy cases of Behçet's disease were recorded. Among them, 4 cases showed Budd-Chiari's syndrome including our case. This manifestation in Behçet's disease will not be incidental but a disease process of vascular type, itself. Although the autopsy rate of all the death in Japan is 4.4%, in the last 10 years (1974-1983), that of Behçet's disease is 14.2% (1976-1983). So our data from the autopsy series of all over Japan will show the pathologic lesions of the fatal cases and statistically be significant.

Nucleotide sequence and gene organization of ColE1 DNA.

The primary structure of the plasmid ColE1 DNA has been determined. The plasmid DNA consists of 6646 base pairs (molecular mass of 4.43 MDa) and is 48.46% in GC content. The phi 80 trp insert of the composite plasmid of ColE1, pVH51, has also been determined. The determination of the nucleotide sequence of ColE1 DNA provides the basis for examining the relationships between the DNA sequence and the gene organization of the plasmid. The focus of this paper is to use this sequence data coupled with a review of the literature and our own work to examine the nine known functional regions of ColE1: imm (colicin E1 immunity), rep (replication function), inc (plasmid incompatibility and copy number control), bom (basis of mobility), rom (modulator of inhibition of primer formation by RNA I), mob (plasmid mobilization), cer (determinant for conversion of plasmid multimers to monomers), exc (plasmid entry exclusion), cea (structural gene for colicin E1), and kil (structural gene for the Kil protein).

Mechano-electrical transduction currents in isolated vestibular hair cells of the chick.

Properties of a mechano-electrical transduction channel were studied in enzymatically dissociated chick vestibular hair cells by using a whole-cell recording variation of the patch voltage-clamp technique. The apical hair bundle was stimulated by a glass rod which moved along a one-dimensional axis when stimulated by either a triangular or a trapezoidal command voltage. The motion of the glass rod was monitored optically using a photodiode. In response to triangular stimuli, the hair cell generated a current of triangular wave form with occasional step-like spiky or zigzag-appearing events. Control experiments confirmed that the current was generated only when the hair bundle was displaced towards the tallest stereocilium. The mechano-sensitive current was blocked by streptomycin and by neomycin. The blockage by streptomycin was clearly voltage dependent: the reduction of the current became larger with hyperpolarization of the membrane. This suggests that the positively charged antibiotic molecules plug the mechanically gated channels. From the evidence presented in 3 and 4 above, the mechano-sensitive current recorded here was identified as the mechano-electrical transduction (m-e.t.) current. The permeability of the m-e.t. channel to various monovalent cations was determined from reversal potential measurements. Since a CsCl-EGTA intracellular medium was used, all the permeabilities were calculated relative to PCs. The sequence of permeabilities was Li greater than Na greater than or equal to K greater than or equal to Rb greater than Cs greater than choline greater than TMA greater than TEA. External Ca ions were indispensable for the recording of transduction current and Sr ions could replace Ca ions without loss of the transduction activity. The minimum [Ca]o for stable generation of the m-e.t. current was 20 microM in Cs saline. The addition of 50-200 microM-Ca to the isotonic Ba saline could maintain the m-e.t. current. The m-e.t. current was observed in isotonic Ca and in Sr salines. Isotonic Ba, Mg and Mn salines were enriched with 1-2 mM-Ca in order to generate the m-e.t. current. The permeabilities of the divalent cations relative to Cs were calculated from the reversal potentials, and the sequence of permeabilities among divalent cations was Ca greater than Sr greater than Ba greater than Mn greater than Mg. Step-like m-e.t. currents were observed in Cs saline. The smallest step amplitude with clear resolution had a conductance of 49.7 +/- 4.5 pS (mean +/- S.D., n = 7 cells). Augmentation of in vitro antibody response by disulfide compounds. I. Comparison between intermolecular and intramolecular disulfides.

The following disulfide compounds: 2-hydroxyethyldisulfide (2-MEox), 2-aminoethyldisulfide (cystamine) and oxidized dithiothreitol (DTTox) were found to augment the in vitro antibody response to sheep erythrocytes in murine lymphocytes as effectively as their reduced forms when they were added to the culture medium. We, however, found out that the mode of action of DTTox was greatly different from that of 2-MEox or cystamine. 2-MEox and cystamine showed augmenting effects on the proliferative response to lipopolysaccharide and on the uptake of (35S)-cystine by the lymphocytes. In contrast to these intermolecular disulfides, DTTox, an intramolecular disulfide compound, was found to be inactive in these systems. 2-MEox and cystamine, but not DTTox, were reduced to thiol forms by the intact lymphocytes or by the cell homogenate. Thus, it is likely that DTTox did not behave as the reduced form in the lymphocyte culture in contrast to 2-MEox and cystamine, suggesting that the disulfide form itself plays an important role in augmenting effects of DTTox.

[Intravesical chemotherapy with 4'-EPI-adriamycin in patients with superficial bladder tumors (randomized study)].

Thirty-seven cases of superficial bladder cancer were treated with 4'-epi-adriamycin (EPI) by intravesical instillation to investigate the optimal dosage and treatment schedule. Patients were randomized into either group A: 50 mg/day for 6 days (regimen A) or group B: 60 mg/day for 3 days X 2 courses with 4 days interval (regimen B). Seventeen cases were evaluated in both groups, three cases were ineligible in group B. Patient characteristics such as age, sex, size of tumor, stage and grade were not significantly different between the two groups. Response rates were 41.2% (group A) and 70.6% (group B), while incidence of toxicity was 47.1% and 50.0% respectively. There is a trend in favor of regimen B which deserves to be more extensively verified.

[Phase II study of recombinant human leukocyte A interferon on urogenital cancer patients].

A phase II study on recombinant human leukocyte A interferon (rIFN-alpha A) was carried out in 30 patients with urogenital cancers. Each patient received rIFN-alpha A by i.m. injection every day for at least 4 weeks. The initial daily dose was 3 X 10(6) U, being escalated at intervals of 3 days or more up to 50 X 10(6) U. The results are summarized as follows: In aged patients, the daily dose appropriate for everyday i.m. injection was considered to be 9 X 10(6) U or below, judging from the adverse reactions observed. According to Koyama and Saito's response criteria, partial response (PR) and minor response (MR) were obtained, respectively, in 3 and 1 out of 12 patients with renal cell carcinoma, while PR was seen in 1 out of 9 with urothelial cancer. No response was observed in patients with testicular cancer and in those with prostatic cancer. Various kinds of adverse reactions were recognized and each patient showed one reaction or more. Fever, fatigue, leukopenia, anemia, thrombocytopenia and elevation of GOT and GPT were observed relatively frequently. Among these, fatigue and thrombocytopenia were regarded as dose limiting factors.

[Combination chemotherapy with cisplatin, ifosfamide and adriamycin in patients with advanced transitional cell carcinoma of the urinary tract].

A combined chemotherapy with cisplatin, ifosfamide and adriamycin (CIA therapy) was performed in 20 patients with advanced transitional cell carcinoma of the urinary tract. Adriamycin 30mg/m2 and ifosfamide 1.5g/m2 were administered on day 1 and cisplatin 50 mg/m2 on day 2, which was repeated at intervals of 3 weeks, in principle. According to Koyama and Saito's response criteria, complete response (CR), partial response (PR) and minor response (MR) were obtained in 1, 4 and 5 cases, respectively; CR+PR+MR in 10 cases out of 18 evaluable cases (56%). The duration of response was 2 to 14 months (median; 4 months) in the cases with CR and PR. The survival time of the responders ranged from 3 to 23 months (median; 11 months), being significantly longer than that of the non-responders (p less than 0.05, Generalized Wilcoxon test). Adverse reactions observed were of a slight or moderate degree, with only a few cases of damage to renal function.

[Cerebral perfusion imaging with N-isopropyl-(123I) P-iodoamphetamine (123I-IMP): its planar imaging and semiquantitative analysis in cases with internal carotid occlusion].

Occlusion of internal carotid artery (ICA) is one of the most common cause of stroke, and the single common arterial lesion considered for extraintracranial bypass. The symptoms of ICA occlusion are not uniform; some shows transient ischemic attacks (TIAs), others completed stroke at the onset. So, measurement of cerebral blood flow might be needed to evaluate the prognosis, or to select surgical candidate in ICA occlusion. Several methods, such as Xe-133 intracarotid method, Xe-133 inhalation method etc, have been applied for the assessment of cerebral blood flow in cases with ICA occlusion. No previous report was found by the newly developed 123I-IMP in this connection. The purpose of the present study is first to demonstrate the feasibility of 123I-IMP planar brain scintigraphy and its semiquantitative analysis in 17 cases with ICA occlusion. After intravenous injection of 3 mCi of 123I-IMP, brain scintigrams were obtained from anterior, Towne, vertex, posterior, right lateral and left lateral views, and were recorded on magnetic tape and Poraloid films. Semiquantitative analysis was performed as a percent index of asymmetry (PIA) from the difference of each hemispheric count in the scintigrams by use of on-lined minicomputer system. Clinical manifestations of these 17 cases were, completed stroke in 10, TIAs in 4, and no neurological symptom in 3. 123I-IMP brain scintigrams demonstrated low flow lesion in 10 out of 10 completed stroke, in 2 out of 4 TIAs and normal perfusion in all cases with no neurological symptoms.(ABSTRACT TRUNCATED AT 250 WORDS)

[Morphometric analysis of diameter and density of myelinated and unmyelinated fibers of normal human sural nerves].

Diameter and density of both myelinated and unmyelinated fibers were morphometrically analyzed in 8 sural nerves obtained from 8 normal volunteers by utilizing a TGA-10 particle analyzer (Carl Zeiss Co., West Germany). The density of total myelinated fibers was (7.4 +/- 1.3) X 10(3) (mean +/- standard deviation) per square millimeter. The border diameter between small and large myelinated fibers was 5.4 +/- 0.5 micron. The densities of small and large myelinated fibers in cross sectional areas were (4.5 +/- 0.9) X 10(3) and (2.9 +/- 0.7) X 10(3) per square millimeter, respectively. The density of unmyelinated fibers was (27.9 +/- 5.8) X 10(3) per square millimeter. The peak and the median diameters in the size distribution of unmyelinated fiber diameters were 0.88 +/- 0.15 and 0.86 +/- 0.12 micron, respectively. The mean distribution histograms of the sizes of both myelinated and unmyelinated fibers of 8 sural nerves were also established. The morphometric findings obtained in this study serve as control in histopathologic analysis of the sural nerves from the patients with various peripheral neuropathies.

Surgical treatment of impotence caused by megalourethra.

A twenty-three-year-old impotence patient is presented who had never achieved penile erection and who suffered megalourethra and megaloureter. Through a penothermocurve the cause of impotence was diagnosed as megalourethra and extensively thickened penile skin. Plastic surgery was performed, and sufficient resection of the enlarged urethral wall and penile skin resulted in successful penile erection and satisfactory intercourse. The usefulness of the penothermocurve was demonstrated as a means for the differential diagnosis of impotence.

Augmentation of in vitro antibody response by disulfide compounds. II. T cell-mediated augmentation by oxidized dithiothreitol, an intramolecular disulfide.

In order to investigate the mechanism of action of disulfide compounds in the augmentation of the antibody response in vitro, we attempted to identify the target cells of the action of disulfides using oxidized dithiothreitol (DTTox; an intramolecular disulfide). DTTox markedly augmented the antibody responses not only to sheep erythrocytes, a T cell-dependent antigen, but also to T cell-independent antigens like dinitrophenyl-Ficoll and trinitrophenyl-lipopolysaccharide. The augmenting effect of DTTox in the response to SRBC was markedly abrogated when murine spleen lymphocytes were depleted of T cells and cultured in the presence of concanavalin A-conditioned medium containing the activity of T cell-replacing factor. The augmentation was restored by adding back purified T cells. On the other hand, the augmentation by 2-mercaptoethanol was not affected by these treatments. The antibody responses to dinitrophenyl-Ficoll and trinitrophenyl-lipopolysaccharide, and the polyclonal antibody response induced by lipopolysaccharide were no longer enhanced by DTTox when T cells were depleted. These results suggested that the augmenting effect of DTTox was not due to the direct activation of B cells, as with 2-mercaptoethanol, but was mediated by the stimulation of T cells. This assumption was further supported by the observation that DTTox stimulated the in vitro induction of helper T cell activity in the presence of antigens.

Structural analysis of the dnaA and dnaN genes of Escherichia coli.

The nucleotide sequence of the entire region containing the Escherichia coli dnaA and dnaN genes was determined. Base substitutions by such mutations as dnaA46, dnaA167, dnaN59, and dnaN806 were also identified. Analyses of coding frames, the mutational base substitutions, and other data indicate that dnaN follows dnaA, both have the same orientation, and are separated by only 4 bp. The deduced amino acid sequence specifies Mrs and isoelectric points consistent with those of the previously identified gene products. The transcriptional initiation site of the dnaA gene was assigned by analysis of in vitro RNA products. Examination of the intercistronic sequence and analysis of in vitro transcription supported the notion that the dnaA and dnaN genes constitute a single operon.
