[Computed tomography for the diagnosis of thrombosed aortic dissection with extravasation].

Four cases of thrombosed aortic dissection with extravasation demonstrated by CT were presented. In CT diagnosis for this type of dissection, cautions should be employed not only in an inhomogenous density area in the mediastinum and pleural cavity but also in the presence of deviation of intimal calcification and relatively high density area of crescent shape in aortic wall on plain CT.

[Transarterial hepatic embolization of metastatic liver cancers with reference to 17 cases of survival for more than 2 years].

The evaluation of TAE was studied for 73 patients with metastatic liver cancers but without metastasis in other organs, who were treated with initial TAE more than 2 years ago. Seventeen patients survived for more than 2 years. The primary lesions consisted of colon cancer in 9 patients, the Langerhans islet cell tumor of the pancreas in 4, gastric cancers in 2, breast cancer in 1 and leiomyosarcoma of the colon in 1. The overall 2-year survival rate calculated by the direct method was 23.3% and mean survival period was 15.8 months. The prognosis was good in H1 and V3. Lp-TAE produced better effects but no statistically significant difference was found between the 2-year survivals by Lp-TAE and by GS-TAE.

[Evaluation of a biliary endoprosthesis using expandable metallic stents].

A biliary endoprosthesis constructed of Gianturco self-expanding metallic stents was placed in twenty-nine patients with obstructive jaundice, 27 malignant and 2 benign. All endoprostheses were placed successfully. External drainage catheters were removed in twenty-seven patients. At the follow-up, ranging from two to 59 weeks, the 30 day mortality rate was 7.4%. Twenty-eight patients had obvious clinical improvement with relief of jaundice. Seven patients experienced recurrent jaundice. In two of these patients, the obstruction was relieved by additional metallic stents. The expandable metallic biliary endoprosthesis is suggested as an effective treatment for biliary obstruction.

Structure of 5-methoxy-2-([4-methoxy-3,5-dimethyl-2-pyridinyl) methyl]sulfinyl)-1H-benzimidazole (omeprazole).

C17H19N3O3S, Mr = 345.42, triclinic, P1, a = 10.686 (5), b = 10.608 (7), c = 9.666 (6) A, alpha = 119.75 (5), beta = 112.02 (5), gamma = 68.33 (4) degrees, V = 859 (1) A3, Z = 2, Dm = 1.332 (2), Dx = 1.335 g cm-3, Cu K alpha, lambda = 1.5418 A, mu = 18.04 cm-1, F(000) = 364, T = 293 K, R = 0.057 for 1962 observed reflections. The methylsulfinyl group, which adopts a trans conformation, links the pyridine and benzimidazole rings in an almost coplanar orientation. Thus the molecule, as a whole, adopts a nearly extended form. Two centrosymmetrically related molecules form a cyclic dimer by intermolecular N-H...O hydrogen bonding, and the dimers are held together by van der Waals contacts between the neighboring aromatic rings in the crystal structure.

[Treatment with expandable metallic stent for arterial occlusive disease].

Gianturco expandable metallic stents were used in 4 cases with ASO of the iliac artery after PTA for the purpose of the prevention of restenosis. The insertion of stent was succeeded in all 4 cases, and the improvement of clinical symptoms and elevation of ankle pressure index were observed. Follow-up over 3 to 8 months revealed no occlusion and migration of stent. Expandable metallic stent was suggested to be contributable to the treatment of arterial occlusive disease.

Mode of binding of E-64-c, a potent thiol protease inhibitor, to papain as determined by X-ray crystal analysis of the complex.

The three-dimensional structure of the E-64-c-papain complex has been determined by X-ray crystal analysis at 2.5 A resolution (conventional R = 26.9%). The structure determined indicates that: (i) the C2 atom of the oxirane ring of E-64-c is covalently bound by the S gamma atom of Cys-25 of papain; (ii) this covalent bond formation results in a configurational conversion of the oxirane C2 atom from the S- to the R-form; and (iii) extensive hydrogen bonding and hydrophobic interactions are responsible for the specific interaction of the E-64-c molecule with papain.

Revised amino acid sequence, crystallization, and preliminary x-ray diffraction analysis of acidic phospholipase A2 from the venom of Agkistrodon halys blomhoffii.

The complete amino acid sequence of acidic Agkistrodon halys blomhoffii phospholipase A2 has been redetermined by a combination of manual and automatic Edman degradations. Acidic A. halys blomhoffi phospholipase is a single polypeptide chain consisting of 122 amino acids and is highly homologous in sequence with corresponding regions of phospholipase A2 from a variety of sources. Prism crystals of acidic A. halys blomhoffii phospholipase have been reproducibly grown from 2-methyl-2,4-pentanediol solution adjusted to pH 8.0 with 50 mM Tris-HCl buffer in the presence of 10 mM CaCl2. The crystals belong to space group P6(1)22 or P6(5)22 with hexagonal unit cell dimensions of a = b = 76.22 A and C = 76.56 A. One molecule occupies the asymmetric unit of the crystal. The diffraction extends to at least 2.5 A.

Transcatheter arterial embolization using iodized oil (lipiodol) mixed with an anticancer drug for the treatment of hepatocellular carcinoma.

The therapeutic results of Lp-TAE (transcatheter arterial embolization with Gelfoam particles preceded by the infusion of a mixture of lipiodol and an anticancer drug via the proper hepatic artery) were evaluated in hepatocellular carcinomas (523 non-resected and 24 resected cases). Excellent therapeutic effects were confirmed not only for the main tumor but also for the daughter nodules by a histological examination of the liver tissues resected after Lp-TAE. The cumulative 1-year, 2-year and 3-year survival rates in the 523 non-resected cases were 60.4%, 42.9% and 28.0% respectively. These survival rates were all higher than those achieved by Gelfoam TAE. The above results suggest the usefulness of Lp-TAE in the treatment of hepatocellular carcinoma.

Polyamine interaction with Z-DNA.

In order to elucidate the detailed Z-DNA interaction with polyamines and also to clarify the mutual molecular recognition between the left-handed helix and the biologically important polyamine molecule, several polyamine-Z-DNA hexamer complexes were crystallized and their crystal structures were determined by X-ray diffraction. The general interaction modes found in these crystal structures were discussed in comparison with those in the complexes between polyamine and the right-handed DNA or RNA.

Depression of hyperthermic potentiation in cell killing of ultraviolet light-irradiated Dictyostelium discoideum by pre-heat treatment.

Heat treatment of 30 degrees C for 15 min immediately after ultraviolet (UV) irradiation enhanced cell killing of Dictyostelium discoideum. However, when the cells were heated at 30 degrees C for 15 min and then cultured for more than 10 min at 23 degrees C prior to UV exposure, the hyperthermic potentiation in cell killing was depressed. When these preheated cells were cultured in the presence of cycloheximide, the depression of the hyperthermic potentiation disappeared. These results suggest that the depression in hyperthermic potentiation may be the result of the induction of some heat-shock-type proteins.

Right upper lobe versus right middle lobe: differentiation with thin-section, high-resolution CT.

A new method was developed to differentiate the upper lobe from the middle lobe of the right lung with thin-section, high-resolution computed tomography. In the upper lobe, a medial subsegmental bronchus of an anterior segmental bronchus is always located lateral to the corresponding artery. In the middle lobe, a medial subsegmental bronchus of a lateral segmental bronchus and a superior and inferior subsegmental bronchus of a medial segmental bronchus are always located medial to the corresponding artery. In other words, the anatomic relationship between the subsegmental bronchi and the corresponding pulmonary arteries in the upper lobe is opposite to that in the middle lobe. One hundred seventeen cases, including 54 cases of lobar volume loss, were reviewed with this method, and in each case it was possible to differentiate the upper lobe from the middle lobe without the contiguous section analysis.

Histogenesis of neoformation in the endocrine pancreas of aging horses.

Pancreatic tissue from 20 horses was examined using immunocytochemical techniques. In aged horses, neogenesis of endocrine cells, neoformation, and hyperplasia of islets occurred closely associated with the pancreatic duct; these changes were regarded as nesidioblastosis. In addition, pancreatic fibrosis accompanied by ductal proliferation and endocrine neogenesis was considered a regenerative change. Thus, the origin of neoformation in the endocrine pancreas was in the ductal system, and it is suggested that the pancreatic endocrine cells were of endodermal origin.

Studies on serum selenium and tocopherol in white muscle disease of foal.

In order to clarify the cause of white muscle disease (W.M.D.) in foals, tocopherol and selenium concentrations in serum and glutathione peroxidase activities in blood were measured. Examination was made on the samples from horses affected with W.M.D., the foal kept with them in the same stable, the foals kept in the stables without affected foals, and respective mares. The heavy-breed horses in Fukuoka prefecture and Tokachi district were also examined for comparison. Serum tocopherol levels of these foals were normal because after intake of colostrum. Mares of affected foals showed lower tocopherol levels than other examined mares (p less than 0.01). Serum selenium levels of all foals were below 65 ppb, showing deficient levels. The mares of affected foals had lower selenium levels than other mares (p less than 0.01). There was a good correlation between serum selenium concentration and blood glutathione peroxidase activity (r = 0.81). Selenium levels in the liver of affected foals were lower than the foals which succumbed with other diseases. Diet supplied in the stables with affected foals showed low alpha-tocopherol and selenium contents. These findings suggest that W.M.D. in foals is attributed to nutritional muscular dystrophy caused by tocopherol and selenium deficiency of their mares in late gestation period.

[Ultrasonic detectability of multiple small hepatic nodules].

We analyzed the ultrasonic detectability of multiple daughter nodules accompanied by 18 hepatocellular carcinomas and multiple small nodules in 11 metastatic liver tumors identified by infusion hepatic angiography. In the cases of hepatocellular carcinoma, ultrasonic detectability of daughter nodules was remarkably limited. If any daughter nodules are detected by US, there is the great possibility that many other daughter nodules are undetected. However, US is effective in detecting small nodules in the metastatic liver tumor.
