Recent trends in research on occupational cervicobrachial disorder.

Recent rapid development of mechanization and automatization in many industries in Japan have increased simple and repetitive tasks that have resulted in some new types of occupational health hazards, in particular among young female workers. One of them is occupational cervicobrachial disorder, the concept of which was established by the committee organized by the Japan Association of Industrial Health in 1972. The committee demonstrated that the disorder resulted from repetitive upper limb manipulation in punching and operating various kinds of business machines, and concluded that the disorder was closely related to chronic fatigue in the shoulders, neck, lower back and other parts of the body. In 1975 a study team was organized with a grant from the Ministry of Education. The team was divided into four working groups. Topics of these groups were 1) etiology of the disorder, 2) natural history of the disorder in various kinds of jobs, 3) mass examination techniques for the disorder, and 4) a health care system to be provided for the workers. The persent report summarizes the results of group discussions with a view to indicating the current problems of occupational cervicobrachial disorder and the necessary future steps for its care treatment and prevention.

Secretin secretion in patients with duodenal ulcer, chronic pancreatitis and diabetes mellitus.

Secretin releasing response to intraduodenal acid infusion was investigated in 15 cases of diseased control, 7 cases of duodenal ulcer, 5 cases of chronic pancreatitis, and 6 cases of diabetes mellitus. Plasma secretin levels in response to duodenal acidification were less in duodenal ulcer and the appearance of the maximal peak was delayed compared with that found in control. It is suggested that the secretin release was impaired in duodenal ulcer in spite of hypersecretion of gastric acids. In chronic pancreatitis, secretin releasing response to acidification was markedly impaired, in addition, inhibition of secretin release by bicarbonate was diminished due to a lack of bicarbonate flow from the pancreas. On the other hand, although the response of secretin release in diabetes mellitus was also lower compared with that in control group, the capacity of secretin response showed values in-between control subjects and chronic pancreatitis. This research was supported in part by grant from the Ministry of Education, Science and Culture in Japan.

Brain tumor in newborn babies.

5 cases of brain tumor in newborn babies under 2 months are presented. 4 of them were supratentorial teratoma and originated from the midline, and 1 was a glioma at the cerebelloponitine angle. 2 cases died before surgery and 2 cases after surgery. In our 5th case a benign teratoma of 150 g was removed from the third ventricle. He was discharged and enjoyed rather good health for 2 years and 1 month before dying of recurrence of tumor. Internal hydrocephalus associated with the huge tumor was responsible for the sudden deterioration of general condition. Therefore, shunting procedures should be performed as the first aid. However, in our experiences, the conditions at admission were the critical and/or too late to be beneficial. Thus early diagnosis is absolutely necessary for surgical treatment. A summary and discussion of the data of 103 previously reported cases are included.

Persistent left superior vena cava draining into the left atrium associated with the atrial septal defect and partial anomalous pulmonary venous drainage: surgical correction with sepcially designed prosthetic material.

Anomalous drainage of the persistent left superior vena cava into the left atrium with partial anomalous pulmonary venous return and the atrial septal defect is relatively rare condition, and there reported various kinds of surgical procedures in correction for this lesion. This report describes the original and simple surgical procedure in total correction of these lesions performed in our institute, using a large Dacron patch connected with a knitted Dacron vascular graft in its center. Surgical correction was accomplished by anastomosis of a Dacron draft with the orfice of the persistent left superior vena cava into the left atrium, widening of the atrial septal defect, and reconstruction of a new atrial septum with a Dacron patch. Postoperative cardiac evaluation revealed good condition of blood stream through a Dacron draft from the left superior vena cava to the right atrium, with corrected pulmonary venous drainage into the left atrium without pulmonary venous congestion and superior vena cava syndrome.
