[Acute retinal necrosis syndrome. Retrospective study apropos of 14 eyes in 11 patients].

PURPOSE: The authors present a retrospective study of fourteen eyes of eleven patients suffering from acute retinal necrosis syndrome (ARN) and compare the results with previously published data. MATERIAL AND METHODS: All patients were referred to us between 1985 and 1993. The results of initial examination, the epidemiological and aetiologic data, as well as the clinical course and treatment are summarized in two tables. RESULTS: First clinical examination found a prominent ocular inflammatory reaction, several sites of peripheral retinal necrosis and one retinal vasculopathy. Bilateralisation occurred in 28% of the patients. An optic neuropathy developed in 57% of the cases, and a retinal detachment in 78%. Initial examination and data on the clinical course are discussed and compared with those described in the literature. CONCLUSION: The results of the current study are representative of the various clinical and biological profiles of acute retinal necrosis and are similar to those described in previous publications. Earlier care in specialized departments of ophthalmology should allow a better anatomical and functional prognosis in the future.

[Williams-Beuren syndrome: diagnosis and ocular manifestations].

A 24-year-old woman presented with a Williams-Beuren Syndrome which included a characteristic facies, cardiovascular dysfunction and neurologic troubles. Ophthalmic examination showed a stellate pattern of the iris and tortuosities of the retinal vessels. Knowledge of these ocular manifestations could help early diagnosis.

[Primary conjunctival-palpebral lymphedema and Milroy disease].

An 18-year-old girl had swelling of both upper and lower extremities due to chronic hereditary lymphoedema. She had bilateral eyelid and conjunctival lymphoedema. This is an ocular manifestation of Milroy's disease: primary and secondary forms of lymphoedema may occur and are discussed.

[Madarosis and alopecia areata of eyelashes].

We presented the clinical features of a 9-year-old girl and a 52-year-old man with alopecia areata of eyelashes. Alopecia areata refers to the idiopathic lesions of the eyelashes and is generally classified as being partial, total or universal.

[Achromial pityriasis versicolor of the eyelid].

A 40-year-old man developed a white lesion of the eye, lid with fail of the eyelashes. The lesions are caused by Pityrosporum Orbiculare, a very common commensal of hair follicules. The hyphal forms are seen among the keratin.

[Treatment of herpetic endogenous uveitis with acyclovir by the systemic route].

Seventeen patients with presumed chronic herpetic uveitis of all anatomic types underwent systemic treatment with oral and/or intravenous, acyclovir. The diagnosis of herpes had been established on data from serological tests of the aqueous humor. Results were judged according to corticodependance and visual acuity. Results were all the more positive as treatment was more prolonged and also correlated with the dose of acyclovir injected intravenously or taken orally every day. Follow-up ranged from 6 months to several years.

[Malignant melanoma and melanosis of the conjunctiva. Limited surgical excision and cryotherapy].

A 92 year old woman was noted to have three black conjunctival nodules and a black-brown pigmentation of the temporal conjunctiva. Extensive diffuse intraepithelial conjunctival melanomar was developed with invasive nodules. We treated this disease by judicious excision of the invasive nodules combined with cryotherapy delivered to the surrounding flat intraepithelial areas of involvement.

[Disciform keratitis and Kawasaki's disease].

Kawasaki's disease is a mucocutaneous lymph node syndrome associated with an acute febrile illness affecting children. Ocular signs are a principal presenting feature with hyperhemic indurated limbal sparing inflammation (88 to 96%), and some mild anterior uveitis. We report a case of disciform keratitis affecting a 10 year old child. Ocular features of Kawasaki's Disease resolved completely within a few weeks.

[Orbital invasiveness of Merkel cell tumor of the eyelid].

A new case of neuroendocrine carcinoma (Merkel cell tumor) of the eyelid is reported. Local recurrence, spread to the orbit, led to exenteration and death of the patient. This trabecular carcinoma can be identified by histology, immuno-histochemistry and electron structural microscopy.

[Granuloma of the conjunctiva caused by synthetic fibers].

A five year old girl with pseudo-tumoral lesion of the inferior right fornix underwent surgical excision. Histology of the lesion showed a granulomatous giant cell inflammation surrounding foreign material, probably related to synthetic fabric fibers.

The possible role of herpes viruses in multifocal choroiditis and panuveitis.

7 cases of multifocal choroiditis and panuveitis are reported here (6 females, 1 male). All clinical data were carefully considered. In all cases an aqueous sampling was made for the detection of anti-herpes virus antibodies in aqueous and serum. 3 specificities were tested: herpes simplex (HSV), herpes zoster (HVZ) and cytomegalovirus (CMV). An intraocular synthesis of specific antibodies was found against VZV in 2 cases and against HSV in 1 case. There was another presumptive case for HSV.

Nonspecific immunoglobulin E in aqueous humor: evaluation in uveitis.

The levels of immunoglobulin E (IgE) in aqueous humor (AH) and serum were determined in 74 uveitis patients and 13 control patients with senile cataract. Uveitis patients' levels ranged from less than 0.06 to 460 IU/ml according to the paper radioimmunosorbent test (PRIST; limit of sensitivity, 0.06 IU/ml). Only one control patient had an AH level higher than the sensitivity limit (0.08 IU/ml). Of the five patients with the highest levels in the AH, four had toxocaral retinitis. The ratio of AH/serum to IgE normalized to the albumin concentration was calculated in 25 uveitis patients and was greater than 0.60 in 13 cases, indicating a possible intraocular IgE synthesis.

[Radiation-induced maculopathies].

About two clinical cases with maculopathy resulting from irradiation with so low dosage as to have been previously considered harmless in most of the literature. The authors suggest that early photocoagulation laser treatment as a mean of preserving a good visual prognosis for these eyes.
