[Cardiac and selective vascular effects of canrenoate-potassium (aldactone pro injectione) in cardiosurgical patients (author's transl)].

Hemodynamic and inotropic effects of 200 mg and 400 mg Aldactone pro injectione (Canrenoate-Potassium, Aldadiene) were measured in three different series (n=60) during and after cardiac surgery in neuroleptanalgesia; results were compared to a control group (n=35). The first investigation was performed after sternotomy and pericardiotomy. During a measuring period of ten minutes no significant changes in inotropic parameter dp/dt max were found in comparison to individual control values. The second investigation during "steady state" extracorporeal circulation revealed a small decrease in arterial perfusion perfusion pressure after both dosages, which can be interpreted as an arteriolar vasodilatation. There was, however, no significant difference in comparison to the control group. Additional measurements were done after cardiac surgery in a total of 20 patients (200 mg and 400 mg Aldactone pro injectione) during an investigation period of 60 min. In comparison to a control group (n=15) no significant differences in arterial pressure, heart rate, cardiac index, stroke index and pulmonary arterial pressure were found. There were no acute and direct positive inotropic effects of Canrenoate-Potassium after administration of 200 mg and 400 mg respectively in cardiosurgical patients.

[Observations in two cases of primary fibrosarcoma of the lung (author's transl)].

Two cases of primary fibrosarcoma of the lung are reported. The different clinical findings as well as the different courses in both patients reveal problems in diagnosis and treatment of this rare pulmonary lesion. The diagnostic aspects are given and discussed with regards to bronchial carcinoma. It is believed that lobectomy is the rational treatment of primary fibrosarcoma although pneumonectomy must be considered in the indifferentiated typ. The doubtful prognosis requires an exact postoperative follow up in order to have the change of early surgical treatment or irradiation if metastasis or recurrence of the tumor occurs.

[Respiratory care in infants following cardiac surgery (author's transl)].

Report on results in postoperative intensive care of 73 infants. 37 underwent corrective surgery on extracorporeal circulation (mortality 24,3%), 36 were operated on without ECC (mortality 16.7%). All infants had been intubated by the nasotracheal route, 39 were treated with CPPV (mean duration 32.5 h), 23 with IPPV (mean duration 28 h). Almost 50% were treated with +7 cm H2O CPAP (mean duration 32 h). Advantages and potential hazards of CPPV and CPAP are discussed.

Complications of permanent transvenous cardiac pacing.

Clinical experience with permanent transvenous pacing during a 6 year period at Hannover Medical School is presented. A total of 1,376 pacemaker operations were performed in 799 patients, with a mortality rate of 1.1 per cent. The most common complications were premature battery failure, dislocation of endocardial electrodes, infections of the generator and/or electrodes, and skin ulcerations. In our cumulative follow-up period of 1,225 years, a complication necessitating a reoperation is to be expected after an average function-time of 31 months. Including normal battery exhaustion in this calculation will make reoperation necessary every 21.9 months.
