A method for sheep scab control by applying selective treatment based on flock serology.

Sheep scab, caused by Psoroptes ovis, is a severe and debilitating disease that can be treated and controlled by the use of acaricidal dips or the use of broad-spectrum avermectins. In Switzerland, control measures are state regulated. In particular, sheep should be routinely treated with ectocide dips or avermectin injections before they are moved onto common alpine pasture in late spring. However, a substantial part of the sheep population remains untreated and represents a potential reservoir for the mite population. Untreated sheep that are not moved to alpine pasture may infest treated sheep when flocks are reassembled in autumn. In an attempt to identify infested sheep, all flocks in the Canton of Schwyz (Switzerland) were serologically tested in 2001 and in 2002 (587 and 565 flocks, respectively). In 2003, a representative number (182 of the 531 flocks) was again investigated. Seropositive flocks were treated with doramectin (0.3 mg kg(-1) body weight, intramuscularly) from 2001 to 2003. In spring 2002, no chemo-methaphylaxis was given to seronegative flocks before movement onto common alpine pastures. Of the 587 flocks surveyed in spring 2001, 34 were seropositive (5.8%). These consisted of 21 infested with P. ovis, 1 with P. cuniculi, 4 with Chorioptes spp. and 8 of seropositivity of unknown origin; there was a decrease of seropositive flocks in spring 2002 (4.4%) with 15, 0, 2 and 8, respectively. Of the 182 flocks surveyed in spring 2003, just 4 flocks (2.2%) were seropositive. All the seropositive reactions in these flocks were the result of Chorioptes spp. infestations. There was a corresponding decrease in the proportion of seropositive animals from 6.3% in spring 2001 to 2.1% in spring 2003. These results corroborate the concept that it may be possible to target chemo-metaphylaxis and hence decrease the use of endectocides as well as of ectocides to control sheep scab. This would be of great benefit in reducing the likelihood of development of anthelmintic resistance against avermectins, decreasing the extent of environmental and human contamination with potentially toxic products and diminishing potential drug residues in meat and milk.

[Autochthonous infections with Angiostrongylus vasorum in dogs in Switzerland and Germany].

Angiostrongylus vasorum is endemic in foxes an other carnivores in the South and south East of France, Denmark and Great Britain. The reddish nematode is present in the Arteria pulmonalis and the right side of the heart and causes respiratory and cardiovascular symptoms. From 1999 to 2004, A. vasorum was diagnosed in 5 dogs from northern Switzerland, in 1 dog from southern Germany and in 3 dogs from south Ticino. Clinical signs in the affected dogs varied and ranged from cough, tachypnoea and dyspnoea to neurological symptoms in 2 of the dogs. Four dogs died and in 3 of the 4 dogs adults and larvae were found after digestion of formalin-fixed tissue. Diagnosis in the other 5 dogs was achieved by detecting the larvae coproscopically. Based on anamnestic data, these 9 dogs are probably autochthonous cases.

Cognitive and motor deficits associated with HIV-2(287) infection in infant pigtailed macaques: a nonhuman primate model of pediatric neuro-AIDS.

Lentivirus-infected nonhuman primates exhibit behavioral and neurological pathology similar to human immunodeficiency virus (HIV)-infected humans and offer a means to examine the effects of lentivirus infection while controlling for confounding factors inherent in human populations. The purpose of this study was to examine cognitive and motor development in infant macaques vertically infected with HIV-2287. Subjects were 20 infant pigtail macaques (Macaca nemestrina); 8 controls born to uninfected dams, and 12 infants whose dams had been inoculated and infected with HIV-2287 in the third trimester of pregnancy. Eight of these pregnancies had undergone surgical procedures in the form of maternal amniotic catheters or maternal amniotic and fetal carotid artery and jugular vein catheters. Data indicated that catheterization had little or no impact on behavioral development. Seven infants were vertically infected (as measured by polymerase chain reaction (PCR) at birth) and five were not infected (as measured by PCR and coculture on repeated testing). Infected infants attained cognitive and motor milestones at significantly later ages than controls. Uninfected infants, born to infected dams, attained developmental milestones at later ages than controls on all tasks, but this reached statistical significance only for the Fine Motor Task. Attainment of milestones was not correlated with viral dose, maternal CD4+ levels at parturition or infant viral RNA levels at birth. Attainment of milestones was negatively correlated with infants' proportions of CD4+ lymphocytes at birth and significantly correlated with proportions of CD4+ lymphocytes 2 weeks after birth, indicating poorer performance in those infants with a more rapid CD4+ depletion. These cognitive and motor deficits closely resemble those observed in human infants and children infected with HIV and indicate that HIV-2287-infected infant macaques represent an excellent model of pediatric neuro-acquired immunodeficiency syndrome (neuroAIDS).

Clinical development and serological antibody responses in sheep and rabbits experimentally infested with Psoroptes ovis and Psoroptes cuniculi.

Psoroptes ovis of sheep origin, and Psoroptes cuniculi of rabbit origin were used in experimental infestations. In experiment I, groups of four rabbits and four sheep were infested with 50-100 mites of each isolate on the skin of the back (skin infestation, SI) or in the external auditory canal (aural infestation, AI). In rabbits, SI and AI with P. cuniculi and AI with P. ovis induced in all animals typical ear lesions and pronounced antibody reactions to P. cuniculi antigens in ELISA. After SI of rabbits with P. ovis no clinical signs were detected, no mites could be reisolated and no specific antibodies were detected. In sheep, P. ovis SI induced mange whereas AI did not induce typical clinical signs and mites could not be reisolated. In both these animal groups, ELISA revealed pronounced and comparable specific antibody reactions. After SI and AI with P. cuniculi no clinical symptoms were observed and no mites could be reisolated. Nevertheless, low levels of specific antibody were detected. In experiment II, clinical progression and antibody reactions to P. ovis SI in naive sheep were compared with sheep previously exposed to P. ovis or P. cuniculi. In both pre-exposed groups of animals, clinical signs appeared within 2 days after challenge infestation and three days earlier than in primarily infested sheep. Subsequently, no obvious difference in the clinical progression was observed between the three groups of animals. The results of this study document antigenetic crossreactivity of the two morphologically and genetically distinguishable Psoroptes species but differences in their biological behaviour and virulence which both are of epidemiological and taxonomic relevance.

[Outbreak of sheep scab on two adjacent common alpine pastures in the canton of Schwyz (Switzerland)].

Sheep scab, which is caused by the mite Psoroptes ovis, is a notifiable disease in Switzerland. In the framework of an epidemiological study in the canton of Schwyz, a sheep scab outbreak on 2 adjacent alpine pastures with 62 flocks (1770 sheep) was followed up clinically, parasitologically and serologically. No mass treatment of the flocks with organophosphates or avermectins had been undertaken before the flocks were taken up to the alpine pastures, but they were treated according to their serological status. Supervised treatment of seropositive judged flocks (at least one seropositive or at least 2 serologically equivocal animals per flock) with Doramectin was undertaken, whilst seronegative judged flocks remained untreated. Sheep returned from the summer pastures in early September 2001 and within three months scab infestation was diagnosed in 53 flocks (85.5%). These infested flocks were scattered all over the canton. Furthermore, the transfer of sheep from 6 infested flocks resulted in the transmission of the disease to 10 new flocks. Infested flocks were kept in quarantine and sanitized. The complexity of the traditional use of shared alpine pastures and the intense, uncontrolled trading with animals render the control of sheep scab difficult.

Serological cross-sectional survey of psoroptic sheep scab in Switzerland.

Sheep scab, caused by the mite Psoroptes ovis, is a notifiable disease in Switzerland. Mandatory prophylactic treatments are carried out in spring for all sheep grazing on common alpine pastures. Despite these strict control measures the number of outbreaks has been increasing in recent years, raising questions on the spread of this epizootic disease. To improve the traditional microscopic diagnosis, we developed an ELISA for the specific determination of antibodies against psoroptic antigens with sensitivity and specificity of 93.7 and 96.5%, respectively. Using this test, a serum bank, representative of the sheep population in Switzerland (approximately 15% of registered sheep flocks, randomly chosen, collected between February and May 1998), was screened. The prevalence using 16,404 sera was 10.4, and 11.9% of the 2083 flocks were diagnosed as positive. Additionally, there was a significant increase found for mean seropositivity relative to the flock size (P<0.001, linear regression). Therefore, larger flocks had a higher risk of sheep scab, which may be caused by more frequent animal movement. Spatial analysis of the results revealed a region with significantly higher risk (relative risk = 3.5, P<0.001) of seropositivity, where no cases have officially been notified in the period between 1997 and 1999. The next goal is to establish an efficient control strategy based on serological monitoring of the total sheep population.

Diagnosis of psoroptic sheep scab with an improved enzyme-linked immunosorbent assay.

An enzyme-linked immunosorbent assay (ELISA) was developed for the detection of specific antibodies against crude Psoroptes antigen. The diagnostic sensitivity was 93.7% in 191 sheep with clinical signs associated with mange. These animals originated from 29 flocks in which psoroptic mites were detected. All of 59 sheep infested with Psoroptes ovis were seropositive. Additionally, in 49% of 70 clinically unaffected sheep originating from P. ovis-infested flocks, specific antibodies could be detected, suggesting that asymptomatic infestations can be diagnosed by serology. The specificity of the ELISA was 96.5% as determined with 254 sheep originating from 44 flocks without clinical mange. Cross-reactivity in a low range was detected with selected sera of sheep with clinical chorioptic or forage mite infestations. Four sheep seroconverted 2 weeks after experimental P. ovis infestation, i.e. 2 weeks before clinical signs became obvious. After successful doramectin treatment of 14 sheep with naturally acquired P. ovis infestation, the ELISA values declined slowly but remained positive in seven cases beyond 17 weeks.

V(D)J recombination defects in lymphocytes due to RAG mutations: severe immunodeficiency with a spectrum of clinical presentations.

Severe combined immunodeficiency (SCID) comprises a heterogeneous group of primary immunodeficiencies, a proportion of which are due to mutations in either of the 2 recombination activating genes (RAG)-1 and -2, which mediate the process of V(D)J recombination leading to the assembly of antigen receptor genes. It is reported here that the clinical and immunologic phenotypes of patients bearing mutations in RAGs are more diverse than previously thought and that this variability is related, in part, to the specific type of RAG mutation. By analyzing 44 such patients from 41 families, the following conclusions were reached: (1) null mutations on both alleles lead to the T-B-SCID phenotype; (2) patients manifesting classic Omenn syndrome (OS) have missense mutations on at least one allele and maintain partial V(D)J recombination activity, which accounts for the generation of residual, oligoclonal T-lymphocytes; (3) in a third group of patients, findings were only partially compatible with OS, and these patients, who also carried at least one missense mutation, may be considered to have atypical SCID/OS; (4) patients with engraftment of maternal T cells as a complication of a transplacental transfusion represented a fourth group, and these patients, who often presented with a clinical phenotype mimicking OS, may be observed regardless of the type of RAG gene mutation. Analysis of the RAG genes by direct sequencing is an effective way to provide accurate diagnosis of RAG-deficient as opposed to RAG-independent V(D)J recombination defects, a distinction that cannot be made based on clinical and immunologic phenotype alone.

Characterization of a maternal-fetal HIV transmission model using pregnant macaques infected with HIV-2(287).

To study mechanisms involved in mother-to-fetus transmission of human immunodeficiency virus (HIV) in utero, we have developed a chronically catheterized pregnant macaque model that permits simultaneous and sequential determination of virus in maternal and fetal blood and amniotic fluid during pregnancy. In this report, we have characterized this model using three groups of pregnant macaques designed to sample: (1) maternal blood, fetal blood, and amniotic fluid (n = 6); (2) maternal blood and amniotic fluid (n = 6); or (3) maternal blood only (n = 2). After inoculation with the highly pathogenic HIV-2(287), all pregnant macaques developed brief but intense viremias followed by precipitous CD4+ T-cell declines within 2-3 weeks. While all the infants born to dams of the three groups were HIV positive, the degree of infection and outcome of HIV infection varied. All infants were shown to be HIV-RNA-positive by reverse transcriptase-polymerase chain reaction (RT-PCR). However, HIV-infected cells were detected only in the blood of those born to dams enrolled in groups 1 and 2: most of these infants progressed to CD4+ T-cell depletion. The infants in group 3 exhibited HIV-RNA in plasma, although neither HIV-infected cells nor CD4+ T-cell depletion was detectable. However, all infants developed HIV-2-specific antibody at various levels by 2 months of age. Together, the data suggest that, while the degree of instrumentation may modulate intensity of virus transmission to fetus, the highly pathogenic HIV-2(287) exhibited a high frequency of virus transmission from the mother to fetus.

[Effect of a single injection of doramectin on gastrointestinal nematode infections of sheep grazing on alpine pastures].

The persistent effect of doramectin injectable against gastrointestinal nematodes was investigated in a controlled field study with 70 sheep kept on alpine pastures in Switzerland. After grazing on home pastures for four weeks 50 lambs and 20 ewes were allocated to two equal groups according to age and body weight. At turnout to alpine pasture in June doramectin (0.3 mg/kg) was administered by intramuscular injection to 25 lambs and 10 ewes (Group D), whereas control sheep (Group K) remained untreated. Animals of both groups were kept on separate pastures (altitude: 1100 m) and were rotated between three paddocks during a total grazing period of 13 weeks. After doramectin treatment faecal examinations of Group D sheep showed a marked reduction of the trichostrongyle egg output which remained close to zero for eight weeks. During this period serum pepsinogen levels did not indicate the presence of a substantial immature worm burden in the abomasal mucosa. In the last five weeks of the alpine grazing period the trichostrongyle egg counts increased markedly in the lambs of Group D. Lambs of the control group developed moderate-to-high Haemonchus-infections, and eight animals of this group had to be treated with anthelmintics. Lambs of Group D had gained significantly (P < 0.05) more weight eight weeks after turnout whereas the mean bodyweight stagnated during the last five weeks of the grazing period. The results indicate, that the single administration of 0.3 mg/kg doramectin to lambs and ewes provided good protection against severe infections with gastrointestinal nematodes for a period of approximately eight weeks.

Chronic granulomatous disease. Report on a national registry of 368 patients.

A registry of United States residents with chronic granulomatous disease (CGD) was established in 1993 in order to estimate the minimum incidence of this uncommon primary immunodeficiency disease and characterize its epidemiologic and clinical features. To date, 368 patients have been registered; 259 have the X-linked recessive form of CGD, 81 have 1 of the autosomal recessive forms, and in 28 the mode of inheritance is unknown. The minimum estimate of birth rate is between 1/200,000 and 1/250,000 live births for the period 1980-1989. Pneumonia was the most prevalent infection (79% of patients; Aspergillus most prevalent cause), followed by suppurative adenitis (53% of patients; Staphylococcus most prevalent cause), subcutaneous abscess (42% of patients; Staphylococcus most prevalent cause), liver abscess (27% of patients; Staphylococcus most prevalent cause), osteomyelitis (25% of patients; Serratia most prevalent cause), and sepsis (18% of patients; Salmonella most prevalent cause). Fifteen percent of patients had gastric outlet obstruction, 10% urinary tract obstruction, and 17% colitis/enteritis. Ten percent of X-linked recessive kindreds and 3% of autosomal recessive kindreds had family members with lupus. Eighteen percent of patients either were deceased when registered or died after being registered. The most common causes of death were pneumonia and/or sepsis due to Aspergillus (23 patients) or Burkholderia cepacia (12 patients). Patients with the X-linked recessive form of the disease appear to have a more serious clinical phenotype than patients with the autosomal recessive forms of the disease, based on the fact that they are diagnosed significantly earlier (mean, 3.01 years of age versus 7.81 years of age, respectively), have a significantly higher prevalence of perirectal abscess (17% versus 7%), suppurative adenitis (59% versus 32%), bacteremia/fungemia (21% versus 10%), gastric obstruction (19% versus 5%), and urinary tract obstruction (11% versus 3%), and a higher mortality (21.2% versus 8.6%).

Defective self-reactive antibody repertoire of serum IgM in patients with hyper-IgM syndrome.

We have analyzed the self-reactive repertoires of IgM and IgG Abs in the serum of 19 patients with hyper-IgM syndrome (HIM) by means of a quantitative immunoblotting technique that allows for a quantitative comparison of Ab repertoires in health and disease by multiparametric statistical analysis. Normal tissue extracts of liver, lung, stomach, and kidney were used as sources of self Ags. Extracts of Pseudomonas aeruginosa and Staphylococcus epidermidis were used as sources of nonself Ags. We demonstrate a significant bias in repertoires of reactivities of IgM of patients with HIM with self Ags. Ab repertoires of IgM toward nonself Ags did not differ, however, between patients and controls. No difference was found between IgM repertoires of untreated patients and those of patients receiving substitutive treatment with i.v. IgG. IgG in the serum of HIM patients lacked reactivity with self Ags, although it exhibited a pattern of reactivity with nonself Ags that was similar to that of IgG of healthy controls. The data demonstrate that functional CD40-CD40 ligand interactions are essential for the selection of natural self-reactive B cell repertoires.

Single nucleotide variation in rDNA ITS-2 differentiates Psoroptes isolates from sheep and rabbits from the same geographical area.

Psoroptic sheep scab and psoroptic otoacariasis of domestic rabbits occur in the same geographical regions of Switzerland. To address the question as to whether Psoroptes mites are naturally transmitted between sheep and rabbits, we determined the sequences of the rDNA second internal transcribed spacer (ITS-2) from psoroptic mites from sheep (5 field isolates from Switzerland and 2 laboratory isolates from Ireland and the UK) and from rabbits (8 field isolates from Switzerland). The ITS-2 sequences for all Psoroptes mites originating from sheep were identical and differed at 1 nucleotide position from all the sequences of the rabbit-derived isolates. A statistically significant difference between a rabbit-derived isolate and isolates originating from sheep was also obtained by morphometric analysis of the lengths of the outer opisthosomal setae. For comparative purposes, the ITS-2 sequences from Chorioptes and Sarcoptes collected in Switzerland were also determined. No intraspecies variation was found in 6 sarcoptic isolates from red foxes, with a sequence identity of 41% as compared to Psoroptes. The ITS-2 sequences of 3 chorioptic isolates differed by 24-29% from the Psoroptes sequence. Identical sequences were found for the Chorioptes isolates from sheep and a camel, which differed by 18% from the sequence of an isolate from a cow. These genetic data of psoroptic mites originating from sheep and rabbits from the same geographical area suggest the existence of epidemiologically separated populations.

Newly synthesized L-enantiomers of 3'-fluoro-modified beta-2'-deoxyribonucleoside 5'-triphosphates inhibit hepatitis B DNA polymerases but not the five cellular DNA polymerases alpha, beta, gamma, delta, and epsilon nor HIV-1 reverse transcriptase.

Novel beta-L-2',3'-dideoxy-3'-fluoro nucleosides were synthesized and further converted to their 5'-triphosphates. Their inhibitory activities against hepatitis B virus (HBV) and duck hepatitis B virus (DHBV) DNA polymerases, human immunodeficiency virus (HIV) reverse transcriptase (RT), and the cellular DNA polymerases alpha, beta, gamma, delta, and epsilon were investigated and compared with those of the corresponding 3'-fluoro-modified beta-d-analogues. The 5'-triphosphates of 3'-deoxy-3'-fluoro-beta-L-thymidine (beta-L-FTTP), 2',3'-dideoxy-3'-fluoro-beta-L-cytidine (beta-L-FdCTP), and 2',3'-dideoxy-3'-fluoro-beta-l-5-methylcytidine (beta-L-FMetdCTP) emerged as effective inhibitors of HBV/DHBV DNA polymerases (IC50 = 0.25-10.4 microM). They were either equally (FTTP) or less (FMetdCTP, FdCTP) effective than their beta-d-counterparts. Also the 5'-triphosphate of beta-L-thymidine (beta-L-TTP) was shown to be a strong inhibitor of these two viral enzymes (IC50 = 0.46/1.0 microM). However, all beta-L-FdNTPs (also beta-L-TTP) were inactive against HIV-RT, a result which contrasts sharply with the high efficiency of the beta-D- FdNTPs against this polymerase. Between the cellular DNA polymerases only the beta and gamma enzymes displayed a critical susceptibility to beta-D-FdNTPs which is largely abolished by the beta-L-enantiomers. These results recommend beta-L-FTdR, beta-L-FCdR, and beta-L-FMetCdR for further evaluation as selective inhibitors of HBV replication at the cellular level.

[Prevention of gastrointestinal and lungworm infections in alpine calves: use of doramectin pour-on in before and after the alpine grazing season].

The effect of doramectin pour-on (Dectomax 0.5% pour-on, Pfizer) against gastrointestinal nematodes (GIN) and lungworms was investigated in calves grazed on alpine pastures in Switzerland. Doramectin pour-on (0.5 mg/kg body weight) was administered topically to calves (n = 57, Group D) before and after the alpine grazing season (mid-June to end of September) when the calves were kept on altitudes between 1800 and 2600 m together with 20 untreated calves (Group K). Calves of both groups were grazed together during the alpine season and were subsequently moved to prealpine and home pastures where they were kept until stabling (end of November). Faecal examinations, performed 4 to 5 weeks after anthelmintic treatment of Group D calves showed a marked reduction of the trichostrongyle egg output, which was also low in the untreated calves. Low numbers of Dictyocaulus-larvae were excreted by the calves at turnout to the alpine pastures. At the end of September the mean excretion of Dictyocaulus-larvae had increased markedly in both groups. After treatment of Group D calves the larval output remained negative until stabling. In contrast, the larval excretion of the untreated calves (Group K) reached highest levels at the end of October and 4 calves exhibited clinical signs of dictyocaulosis. The results indicate that the strategic use of doramectin pour-on in June and in late September provided good protection against severe infections with D. viviparus. The infection level with GIN confirmed that these parasites are of minor relevance under the extensive grazing conditions on high alpine pastures.

A randomized, comparative trial of triptorelin depot (D-Trp6-LHRH) and danazol in the treatment of endometriosis.

To compare treatment efficacy and safety parameters a total of 55 premenopausal women with histologically proven endometriosis (stage II-IV) were randomized to receive the LHRH-analogue depot triptorelin (n = 30) or the steroid danazol (n = 25) for a total of 24 weeks. Immediately after cessation of the endocrine therapy a second-look operation was performed. Four as well as 24 weeks after the end of treatment patients were seen for re-evaluation of clinical symptoms and safety parameters. Estradiol suppression was significantly more pronounced with triptorelin, while the free androgenic index rose with danazol. Both substances were equally effective in reducing endometriotic implants (58% and 51%, respectively). Dysmenorrhea was absent at the end of medical therapy in both treatment groups. Dyspareunia and pelvic pain decreased at least by 50%. Red blood count, thrombocytes, liver enzymes and the atherogenic index rose with danazol, while the urinary calcium/creatinine ratio showed a marked elevation with triptorelin. Adverse effects were mainly due to the hypoestrogenism of the LHRH analogue and the androgenic/anabolic properties of the steroid. Triptorelin and danazol are equally effective in treating endometriosis. Therefore, choice of treatment should be based on the patient's medical history and the pharmacological profile of each substance.
