Abnormally high diffuse activity on bone scintigram. The importance of exposure time for its recognition.

When the bone scintigram reveals high diffuse skeletal activity, it may be misinterpreted as normal. Some authors have reported such scintigrams in articles entitled "Significance of absent or faint kidney sign on bone scan" and "False negative bone scintigram". Three cases with bone metastasis showing high diffuse skeletal activity are presented in this paper. The recognition of abnormally high diffuse skeletal activity on bone scintigrams is discussed. The exposure time of all three cases was short when compared with normal cases, when the bone image was taken with a preset count. Therefore, exposure time is very important for objective differentiation between the cases showing high diffuse skeletal activity and normal cases.

Uptake and localization of 99mTc-methylene diphosphonate in mouse osteosarcoma.

The localization of 99mTc-methylene diphosphonate (99mTc-MDP) in mouse osteosarcoma was studied using a skeletal scintigraphic method and macro- and microautoradiographic methods. On the skeletal scintigraph, the tumor showed a high uptake of 99mTc-MDP and its concentration was about 20 times as high as the value seen in the muscular tissue. The macroautoradiography showed high activity in the tumor bone region and little activity in the nonossified region. On the microautoradiography, the activity of 99mTc-MDP was localized in the mineralized matrix of tumor bone and was almost nil in the nonmineralized matrix (osteoid) and the tumor cells. These findings indicate that the mineralization of the tumor bone matrix plays an important role in 99mTc-MDP uptake in osteosarcoma.

[Studies on the vibration disease - an attempt of a new classification (author's transl)].

In the first section of this study, a survey was made of 1,215 subjects who had been using mowing madchines. Of them 548 subjects without clinical symptomes were examined closely. Vibration threshold and maximal grasping power showed a normal distribution. Since the values decreased with an advancing age, limit values for each age range were calculated from respective probability vales. Advancing age did not affect skin temperature which showed a skewed distribution: limit values therefore were obtained from cumulative frequency distribution. In the second section, we investigated the actual conditions of the authorized patients with vibration diseases. Few severe cases were encountered, but adequacy of the conventional classification as to the degree of severity was questioned. Finally, an attempt was made to classify the disorder according to our new criteria of RNA system. The new system revealed that the vibration disease is classifiable into 7 types: R, N, A, RN, RA, NA and RNA. Single form such as R and N type appeared relatively earlier in life with shorter duration of exposure to vibrating machines. A few cases of A type were seen. Combination form such as RN, RA, NA and RNA type was the form of more severly deranged disorder, as judged from the age of onset, duration of exposure to vibrating machines, physical signs and also laboratory findings. Therefore, the new classification with RNA system appears more adequate in describing the pathosymptomatology of the vibration disease than the conventional method hitherto used.

[Clinical usefulness of RI angiography using amplitude image and phase image by Fourier analysis for the detection of anteroseptal myocardial infarction (author's transl)].

In 19 patients with anteroseptal myocardial infarction, the wall motion of interventricular septum was analyzed by RI angiography using the amplitude image and the phase image. Both images were obtained from ECG gated blood pool data by Fourier analysis. Namely, from the first harmonic in the Fourier series, the heart motion was detected as the amplitude and phase of time-activity curve in each pixel. The distribution of the amplitudes and phases were displayed in color scale, and then both the amplitude and phase images were acquired. Those images allowed us visualization of the ischemic area, even if the scar was parallel to the detector. Moreover, we were able to assess the wall motion of the entire interventricular septum and to distinguish hypokinetic region and dyskinetic region from the hypokinetic region with shift of phase estimated by conventional method. Abnormal region detected by those images closely corresponded with the region of decreased radioisotope concentration in thallium-201 myocardial scintigram.

A case of congenital hepatic fibrosis: usefulness of hepatobiliary scintigraphy.

A case of operatively proven congenital hepatic fibrosis was studied by scintigraphy with Tc-99m-phytate and Tc-99m-Pl (a hepatobiliary radiopharmaceutical). Two focal defects were recognized in the liver on the colloid liver scintigram. Ultrasound and contrast angiography suggested that the two focal defects were cystic lesions. Radionuclide hepatobiliary imaging was performed with Tc-99m-Pl, since intravenous cholangiography was unsatisfactory. One focal defect was proved to be a dilated intrahepatic bile duct and another was found to be cyst. Radionuclide hepatobiliary imaging with Tc-99m-Pl proved to be very useful in identifying dilated intrahepatic bile ducts in this patient as well as in following the progress of this disease.

Electron paramagnetic resonance studies of the purified cytochrome P-450scc and P-45011beta from bovine adrenocortical mitochondria.

Electron paramagnetic resonance studies have been carried out on two species of cytochrome P-450 (P-450scc and P-45011beta) purified from bovine adrenocortical mitochondria. The g values of the steroid-bound cytochromes in the high spin form were determined at 4.2 degrees K to be 8.07, 3.60 and 1.70 for P-450scc and 8.00, 3.65 and 1.71 for P-45011beta. The E/D values were estimated to be 0.103 for P-450scc and 0.099 for P-45011beta. Either high spin P-450 was converted into the low spin form by the treatment with an NADPH dependent electron donating system and subsequent gel filtration in order to remove the steroid. The g values of the low spin ferric cytochromes were 2.423, 2.247 and 1.914 for P-450scc and 2.430, 2.251 and 1.919 for P-45011beta at 77 degrees K. The values for magnitude of delta/gamma, magnitude of V/gamma and k were 5.69, 5.21 and 1.11 for P-450scc and 5.94, 5.38 and 1.16 for P-45011beta. These studies indicate that there are some differences in the ferric heme environment between P-450scc and P-45011beta.
