Inhibitory effect of fatty acids on glucose and insulin uptake in the perfused rat hindquarter.

To elucidate the effects of fatty acids on the uptake of glucose and insulin in the muscle, the effects of oleic and palmitic acids on the uptake of glucose and insulin were investigated in perfused hind-quarters of rats. In the absence of insulin, glucose uptake in the hindquarter was slightly, but not significantly, decreased by the addition of oleic and palmitic acids. In the presence of 100 or 500 microU/ml insulin, glucose uptake in the hindquarter (243 +/- 58 or 282 +/- 65 mumol/30 min) was significantly decreased by the addition of 1000 microM oleic acid and 500 microM palmitic acid (175 +/- 50 or 207 +/- 47 mumol/30 min). The decrease in insulin uptake rate, although not significant at 500 microU/ml insulin, is of similar magnitude to the decrease in glucose uptake. In the presence of 1000 microU/ml insulin, glucose and insulin uptake was not significantly altered by the addition of fatty acids. These results indicate that fatty acids directly inhibit the muscular glucose uptake via the decrease in muscular insulin uptake at a physiological concentration of insulin.

Osteopenia in primary biliary cirrhosis and cirrhosis of the liver in women, evaluated by dual-energy X-ray absorptiometry.

To study bone involvement in primary biliary cirrhosis (PBC), we used dual-energy X-ray absorptiometry to measure bone mineral density (BMD) in Japanese women with PBC and with cirrhosis of the liver. In both groups, in each decade up to 60 years of age, the mean BMD of the lumbar spine was not significantly different from that in healthy Japanese women; however, in patients aged 60 years or more, the level was significantly lower both in the patients with PBC (P < 0.001) and in those with cirrhosis of the liver (P < 0.01). Patients with PBC were also examined by single-photon absorptiometry. The BMD of the radius in the patients with PBC was less changed than that of the lumbar vertebrae; thus, the bone changes in PBC seem to be greater in spongy than in cortical bone.

Total body and regional bone mineral content in hemodialysis patients.

Bone mineral content (BMC) in the total body and lumbar spine was evaluated in 126 hemodialysis patients (60 males, 66 females) by dual photon absorptiometry with the Norland DBD 2600. Measurements of: 1) total body BMC divided by lean body mass (BMCTB/LBM), 2) bone mineral density (BMD) of total body, 3) BMD of four regional sections (head, trunk, pelvis, and legs), and 4) BMD of lumbar spine, generally showed a significant decrease in the hemodialysis patients compared to the reference population. However, arm BMD did not show a significant difference between patients and control populations. The z-score of BMCTB/LBM declined significantly throughout the duration of hemodialysis, although that of the lumbar spine BMD did not. It should be noted that the degree of decrease in BMC was more prominent in the total body measurement than in the lumbar spine measurement. There was preferential osteopenia of the total body in the hemodialysis patients. Although the lumbar spine BMD showed a lower value than the control population, the lumbar spine is not the recommended region to monitor the BMD change in hemodialysis patients.

[Usefulness of 99mTc-PMT SPECT and 18F-FDG PET in diagnosing orbital metastasis of hepatocellular carcinoma].

We reported a case in which 99mTc-PMT scintigraphy was useful in diagnosing orbital metastasis of HCC. The case involved a 70 y.o. male, who had undergone nine transcatheter arterial embolizations over two years because of HCC and who had a past history of gastric cancer. The patient had complained of headache and visual disturbance for two months. Cranial CT and MRI studies showed a large homogeneous mass with remarkable bone destruction in the right lateral orbital wall. Because AFP was elevated, orbital metastasis of HCC was suspected, and 99mTc-PMT scanning was performed. On the planar and SPECT images, very high uptake was found in the right orbital tumor. The FDG-PET study showed remarkable hypermetabolism in the medial portion of tumor and follow-up MRI revealed that the tumor had expanded and invaded to the medial side of the orbit. 99mTc-PMT scanning was critical in diagnosing this case of orbital metastasis, and FDG-PET imaging was useful in determining the most active portion of the tumor.

[A case of mesenteric panniculitis: comparing the findings of 67Ga scintigraphy with pathological results].

We report a case of mesenteric panniculitis where we compared the findings of 67Ga-citrate (Ga) scintigraphy with pathological results. The patients, a 75-year-old man, was hospitalized for examination of low abdominal mass. After hospitalization he developed high CRP levels. A Ba enema revealed serrated intestinal walls and a thick fat density, the margins of which were distinct, as judged from abdominal CT scans. Therefore we suspected a mesenteric panniculitis. Since the patient did not improve under conservative treatment for three months, surgery was performed a day after Ga scintigraphy which showed the abnormal uptake in the abdominal mass, which was removed. The radioactivity of several tissue slices from the resected specimen containing the affected part which had been described inflammatory in the pathological examination showed very high radioactivity. This disease is usually diagnosed from the typical findings of Ba enema and CT scan. But, when an extraordinary abdominal accumulation presents, one should think of performing a Ga scan as an additional diagnostic parameter.

[Evaluation of renal function by factor analysis using 99mTc-MAG3].

The kinetics of 99mTc-mercapto-acetyltriglycine (MAG3), a new renal scintigraphic agent, was studied by factor analysis in patients with chronic renal disorders, and its application to assessment of the renal function was evaluated. Data were collected for 30 minutes from immediately after intravenous injection of MAG3 at 370 MBq, the first pass image and serial renal scintigrams were obtained, and factor analysis was attempted simultaneously. Since most of MAG3 is excreted via the kidney during the first pass, factor analysis was performed. Two factors reflecting renal parenchymal components and pelvic components could be separated in 8 (Cre < or = 2.9 mg/dl) of 10 patients, and they corresponded with respective segmental renograms. In the remaining 2 (Cre > or = 3.0 mg/dl), separation was impossible by 2-factor analysis, and 3-factor analysis was needed. Factor analysis is considered to allow objective evaluation of renal function without setting the ROI.

Measurement of portal systemic shunting by oral and per-rectal administration of [123I]iodoamphetamine, and clinical use of the results.

UNLABELLED: OBJECTIVE. We used a method by which the portal shunt index via the superior mesenteric vein (SI-S) and that via the inferior mesenteric vein (SI-I) could be evaluated simultaneously. The clinical usefulness of the method was studied. METHODS: Scintigraphy was done 3 h after subjects took an enteric-coated capsule containing [123I]iodoamphetamine. At that time, the radionuclide was injected into the rectum. Counts for the lungs and the liver were used to calculate the two shunt indices. RESULTS: The mean SI-S and SI-I tended to be higher for disorders that were more severe, increasing in the order of chronic persistent hepatitis, chronic aggressive hepatitis, and cirrhosis. In cirrhotic patients, the SI-I was higher than the SI-S (p < 0.001). The SI-S and SI-I were both higher in cirrhotic patients with esophageal varices than in such patients without varices (p < 0.05 and < 0.001). The SI-S and SI-I were higher in cirrhotic patients with ascites than in such patients without ascites (p < 0.001 and < 0.01). Correlation between either of these indices and classical indicators of functional reserve was high. The correlation of SI-S with the Child-Turcotte classification and total bilirubin level was higher than the correlation of the SI-I and these indicators of functional reserve. CONCLUSIONS: This method permits assessment of the portal hemodynamics in a relatively noninvasive way, and is clinically meaningful.

Partial characterization of mechanism(s) by which sulphobromophthalein reduces biliary lipid secretion.

This study was performed to explore the mechanisms by which sulphobromophthalein (BSP) reduces the secretion of biliary lipid using Sprague-Dawley rats (SDR) and mutant rats with congenital conjugated hyperbilirubinaemia bred from SDR (EHBR). We infused the bile-salt-pool-depleted rats with sodium taurocholate at a constant rate of 160 nmol/min per 100 g body wt. with BSP (12.5, 25 and 50 nmol/min per 100 g body wt.) or BSP-GSH (12.5, 25 and 50 nmol/min per 100 g body wt.). The biliary secretion of BSP and BSP-GSH was markedly impaired in EHBR as compared with that in SDR. BSP reduced the biliary secretion of cholesterol and phospholipids in a dose-dependent manner without affecting the secretion of bile salts and composition of fatty acids in phospholipids in SDR, but had no effect on lipid secretion in EHBR. In contrast, BSP-GSH had no such effect on biliary lipids, either in the SDR or EHBR. In addition, the amount of BSP in the liver of EHBR was in the same range as that of SDR. Therefore it is unlikely that an intracellular mechanism is involved in the phenomenon of uncoupling by BSP. We conclude that the uncoupling of biliary lipids from bile-salt secretion by BSP occurs at the level of the bile canaliculus following the secretion of unconjugated BSP.

Possible role of the adrenergic mechanism in gastric inhibitory polypeptide- and glucagon-like peptide-1 (7-36) amide-induced insulin release in the rat.

Gastric inhibitory polypeptide (GIP) and glucagon-like peptide-1(7-36) amide (GLP-1) are thought to be the most probable candidates for incretin. However, the precise mechanism of incretin effect is unclear. In the present study, to elucidate the possible role of the autonomic nervous system in incretin effect, the effects of atropine, propranolol, metoprolol, and phentolamine on GIP- or GLP-1-induced insulin release were investigated in the rat. The GIP-induced (2 or 20 micrograms) insulin release was partly inhibited by propranolol pretreatment (0.5 mg/kg subcutaneously [SC]), and GLP-1-induced (2 or 20 micrograms) insulin release was partly inhibited by propranolol or metoprolol (35 mg/kg SC). These results suggest that a beta-adrenergic mechanism may be involved in the incretin effect, probably through a modulating effect on GIP- or GLP-1-induced insulin secretion.

Absence of hepatic uptake of Tc-99m phytate in a man with chronic toluene hepatotoxicity.

The case reported here is of a 28-year-old man diagnosed as having toluene hepatotoxicity. He had a 5-year history of exposure to toluene and was admitted to hospital complaining of two episodes of loss of consciousness. The high total bilirubin and the low prothrombin time suggested serious liver dysfunction. Based on histologic examination of a liver biopsy specimen, the diagnosis of toxic liver dysfunction caused by toluene poisoning was made. Hepatic images were not present in Tc-99m phytate scintigrams, but they were present in hepatobiliary scintigrams done with the Tc-99m N-pyridoxyl-5-methyltryptophan. The diagnosis of hepatic reticuloendothelial failure was made.

Finding of right hepatic vein focal defect in liver scan of a patient with veno-occlusive disease.

A 38-year-old woman hospitalized for delivery after treatment for infertility with lymphocyte transfusions from her husband developed severe hepatopathy. On the basis of findings in a hepatic specimen obtained by biopsy, the authors diagnosed hepatic veno-occlusive disease. In scintigrams that used Tc-99m phytate, a long, narrow defect was found that extended from the outer part of the right lobe of the liver to the junction of the hepatic veins and the inferior vena cava. In SPECT of the liver performed with Tc-99m phytate, the defect corresponded to the region of the right hepatic vein.

Correlation between blood pressure and plasma insulin in acromegaly.

OBJECTIVES: The aims of this study were to determine the possible role of hyperinsulinaemia in the increase in blood pressure in acromegalic patients. DESIGN: After an overnight fast, blood samples were obtained at 0. 30, 60, 120 and 180 min after 75 g glucose ingestion, and plasma growth hormone (GH) and plasma insulin (IRI) were measured by radioimmunoassay. The blood pressure was measured at 10.00 hours for 3 days in a supine position. SUBJECTS: Nineteen (nine women and 10 men) untreated acromegalic patients, aged 25-69 years. RESULTS: There were no significant correlations between mean blood pressure (MBP; diastolic+one-third pulse pressure) and basal GH, sum of plasma GH at 0, 30, 60, 120 and 180 min or basal IRI. However, significant correlation was observed between MBP and IRI at 120 min (r = 0.57, P < 0.02) or sum of plasma IRI at 0, 30, 60, 120 and 180 min) (r = 0.58, P < 0.02). CONCLUSIONS: We conclude that hyperinsulinaemia may be involved in an increase in blood pressure in active acromegalics.

Hepatic extraction and hepatic action of insulin, glucagon, and epinephrine in bivascularly perfused rat liver.

Using a bivascularly perfused rat liver, we investigated the hepatic extraction and hepatic action on glucose output of insulin, glucagon, and epinephrine. The liver was perfused for 70 min without recirculation via the portal vein and hepatic artery (3.0 ml/(min.g liver) from portal vein and 1.0 ml/(min.g liver) from celiac artery). Low and high concentration of insulin, glucagon, or epinephrine (450 and 4,500 pM, 21.5 and 215 pM, or 410 and 4,100 pM, respectively) was added to the perfusing medium via portal vein (P-liver), via celiac artery (H-liver), or via portal vein and celiac artery (H-P-liver). Hepatic extraction of insulin, glucagon, or epinephrine was not significantly different among H- (56 +/- 9 and 21 +/- 5%, 31 +/- 7 and 24 +/- 6%, or 22 +/- 8 and 17 +/- 5%), P- (50 +/- 10 and 23 +/- 6%, 32 +/- 9 and 23 +/- 7%, or 22 +/- 9 and 18 +/- 6%), and H-P-liver (55 +/- 11 and 24 +/- 6%, 30 +/- 9 and 26 +/- 8%, or 20 +/- 9 and 17 +/- 6%). Glucagon- or epinephrine-induced increase in glucose output was also similar in H- (245 +/- 47 and 498 +/- 82 mumol/30 min, or 112 +/- 21 and 215 +/- 38 mumol/30 min), P- (258 +/- 51 and 512 +/- 95 mumol/30 min, or 128 +/- 20 and 220 +/- 36 mumol/30 min), and H-P-liver (263 +/- 58 and 515 +/- 94 mumol/30 min, or 129 +/- 22 and 225 +/- 39 mumol/30 min).(ABSTRACT TRUNCATED AT 250 WORDS)

Uptake of ketone bodies in perfused hindquarter and kidney of starved, thyrotoxic, and diabetic rats.

To elucidate the peripheral disposal of ketone bodies in hyperketonemia accompanied with starvation, hyperthyroidism, or diabetes mellitus, the uptake of acetoacetic acid (AcAc) and beta-hydroxybutyrate (BOHB) was investigated in the perfused hindquarter and kidney of starved, thyrotoxic, and diabetic rats. Thyrotoxicity was induced by daily subcutaneous injection of thyroxine (100 micrograms/kg/day) for 7 days, and diabetes mellitus was induced by intraperitoneal streptozotocin (50 mg/kg) injection. Plasma concentration of AcAc and BOHB was significantly higher in starved, thyrotoxic, and diabetic rats than in controls. The hindquarter or kidney of rats was perfused with synthetic medium containing 0.2 or 1.5 mM AcAc or BOHB for 30 min at a flow rate of 0.5 ml/g muscle wt/min or 3.2-3.5 ml/kidney/min, respectively. In perfused hindquarter, the uptake of AcAc and BOHB was significantly reduced in starved and diabetic rats, but not in thyrotoxic rats. In perfused kidney, the uptake of AcAc and BOHB was not significantly different between the experimental and control rats. These results suggest that in starvation or diabetes mellitus, disposal of AcAc and BOHB was impaired in the hindquarter but not in the kidney, and that disposal of AcAc and BOHB by hindquarter and kidney was not impaired in thyrotoxic rats.

[Doppler flow velocity waveforms in uteroplacental and fetal circulation in normal pregnancy and pregnancy-induced hypertension].

The present study was conducted to elucidate circulation dynamics in uteroplacental and fetal circulation in 130 normal pregnancies and in 40 patients with pregnancy-induced hypertension (PIH). A sector and pulsed Doppler system was employed to measure the uterine artery pulsatility index (UtA-PI), umbilical artery pulsatility index (UAPI), fetal middle cerebral artery pulsatility index (MCPI) and fetal descending aorta peak velocity (AoPV). Furthermore, peak velocity during early diastolic (E) and atrial contraction (A) periods through the mitral (M) and tricuspid (T) valves was measured to determine their ratios (M-E/A and T-E/A). Twelve of the 40 PIH women had UtA-PIs of 95% or higher in normal pregnant women. Ten of the 12 fetuses in women with high UtA-PIs had high UAPIs, 5 of whom demonstrated an MCPI decrease. Two of them represented an increase in M-E/A and a decrease in T-E/A and AoPV, and the prognosis was poor. Only 2 of the PIH women with normal UtA-PIs had high UAPIs. The fact that both M-E/A and T-E/A were inversely correlated with MCPI and UAPI, respectively, suggests that these parameters could be used as indexes demonstrating cardiac afterload in fetuses. UtA-PI measurement in combination with fetal circulation indexes could predict fetal and neonatal well-being.

[Evaluation of clinical usefulness of 99mTc-GSA, a functional hepatic imaging agent--correlation with per-rectal portal scintigraphy].

Liver scintigraphy using 99mTc-GSA, which binds specifically to the asialoglycoprotein receptors on the surface of the hepatocytes, was performed in 41 patients with chronic liver disease (11 chronic hepatitis cases and 30 liver cirrhosis) to investigate the correlations between the images and the various liver function tests. Two liver functional indexes, HH15 for plasma clearance of 99mTc-GSA and LHL15 for liver accumulation of 99mTc-GSA, showed significant differences between the chronic hepatitis group and the decompensated liver cirrhosis group. These indexes also showed significant correlations with the liver function tests such as serum albumin level and prothrombin time. HH15 and LHL15 correlated with the per-rectal portal shunt index obtained in 10 patients by 99mTcO4- per-rectal portal scintigraphy. These results suggest that HH15 and LHL15 have value as new liver functional indexes.

Results of surgery for portacaval shunt in a patient with situs inversus diagnosed and evaluated by per rectal portal scintigraphy.

A 70-yr-old man diagnosed with situs inversus was hospitalized because of hepatic encephalopathy. Per rectal portal scintigraphy showed portal hypertension and a region with high radionuclide activity in the right lateral region of the abdomen. In percutaneous transhepatic portograms, a giant portacaval shunt was seen in the region with high radionuclide activity. After the portacaval shunt was obstructed surgically, the hepatic encephalopathy disappeared. In per rectal portal scintigraphy done 2 wk after surgery, the pattern was normal and the region with high radionuclide activity had disappeared.

Mechanism for endothelial cell injury induced by 15-hydroperoxyeicosatetraenoic acid, an arachidonate lipoxygenase product.

The mechanisms for endothelial cell injury induced by the lipid hydroperoxide 15-hydroperoxyeicosatetraenoic acid (15-HPETE), an arachidonate lipoxygenase product, were explored in cultured bovine endothelial cells. In serum-free medium, there was significant incorporation of [3H]-15-HPETE into the phospholipids of endothelial monolayers, and 15-HPETE induced severe endothelial cell injury, which was determined by the 51Cr-release assay. In contrast, in serum containing medium, there was little incorporation of [3H]-15-HPETE into the cells, and no cellular injury occurred. In the serum free condition, [3H]-15-HPETE was mainly incorporated into the phospholipids. The incorporated 15-HPETE produced lipid peroxidation, which was determined by the accumulation of malondialdehyde in the cells. The 15-HPETE-induced lipid peroxidation was suppressed by radical scavengers (MK-447, MCI-186), anti-oxidants (alpha-tocopherol, butylated hydroxytoluene) and iron chelators (desferrioxamine,2,2'-bipyridine). Furthermore, these agents also suppressed the 15-HPETE-induced cytotoxicity. These results indicate that 15-HPETE-induced endothelial cell injury depends on iron-mediated lipid peroxidation.
