Microwave surgical treatment of the prostate: clinical application of microwave surgery as a tool for improved prostatic electroresection.

Microwave surgery as a tool for improved prostatic electroresection is introduced. Prior to electroresection the prostate was coagulated with 2,450 MHz of microwave which was emitted from the bipolar electrode of a specially designed probe. We conducted a comparative study of 35 patients with bladder neck obstruction. There was a microwave coagulation group treated with subsequent transurethral resection (TUR) and a conventional TUR group. Both groups were analyzed for the amount of blood loss, irrigant absorption and the frequency of complications. Both during and following surgery, the former group had a significant reduction in blood loss and had no complications. We conclude that the combination procedure of microwave coagulation and TUR can minimize the disadvantage of formal TUR and may be of value in the treatment of patients with both prostatic obstruction and of hemorrhagic diathesis as well as in high risk patients.

Pontine urine storage center in the dog.

Electrical stimulation of the presumptive pontine urine storage center, areas ventral to and/or lateral to the pontine micturition center, elicited an increase in threshold volume at which reflex micturition occurred in response to bladder filling with physiological saline, suggesting a facilitatory effect on the urine storage function. The electrolytic lesions made in the stimulated areas were histologically identified to distribute at the nucleus reticularis pontis oralis, Kölliker-Fuse nucleus and subcoeruleus. These nucleus seemed to work as a pontine urine storage center and to perform a reciprocal action with the pontine micturition center.

[Effect of bunazosin on lower urinary tract function in the decerebrated dog].

The effect of bunazosin on the lower urinary tract function was studied by combined recording of cystometry and sphincter electromyogram (EMG) in 8 decerebrated dogs. Micturitions were induced by bladder filling and microelectrical stimulation of the pons before and after the administration of bunazosin. Bunazosin (0.03 mg/kg, i.v.) produced overflow incontinence in 7 of the 8 dogs and a significant decrease in the contractile response pressure to microelectrical stimulation of the pons in 6 dogs. Bunazosin appears to produce a decrease in urethral resistance and to be useful for the treatment of the failure of bladder emptying due to high urethral resistance.

Human chorionic gonadotropin in primary liver carcinoma in adults. An immunohistochemical study.

Production of human chorionic gonadotropin (hCG) by extragonadal tumours is not a rare phenomenon. In the liver, similar results have been reported in hepatoblastomas. The present study was attempted to survey hCG level in serum and hCG-immunoreactivity in primary liver carcinoma in adults. Although hCG was elevated in serum in 2 (22.2%) of 9 autopsied cases with hepatocellular carcinoma (HCC), the hCG-reactivity of carcinoma cells was found in 2 (2.1%) of 95 HCC cases. Carcinoma cells positive for immunoreactive hCG was found in 2 (15.4%) of 13 cases with cholangiocarcinoma (CC). The patients with hCG-immunoreactivity in carcinoma and/or elevated serum level of hCG failed to reveal distinct clinical and endocrinological disturbance due to excess hCG. The hCG-positive cells were focal within the carcinoma and showed poor histological differentiation in both HCC and CC, and there were no trophoblastic cells. It is suggested that hCG is one of the hormones produced by primary liver carcinoma in adults and can be localised immunohistochemically in a small number of poorly differentiated carcinoma cells.

[PMU therapy of recurrent gastric cancer. A case report].

A 56-year-old woman with recurrent gastric cancer treated with PMU therapy, combined CDDP 75 mg/m2 i.v. MMC 10 mg/body i.v. and UFT 400mg/body/2 alpha/day p.o., was reported. She was admitted because of cervical lymph node swelling and abdominal tumor (para-aorta lymph node swelling). She was treated two times with this therapy and induced into complete remission. The serum CEA level, more than 500 ng/ml before the treatment, was reduced to 6.7 ng/ml after treatment. She has currently been free of disease for more than four weeks. We conclude that this PMU therapy is extremely effective for indurable gastric cancer.

[A case of advanced esophageal cancer resected curatively by preoperative chemotherapy with cisplatin].

A case of highly advanced esophageal cancer with bilateral multiple cervical lymph-node metastasis is reported. The largest size of lymph node was 5.0 cm in diameter, and intramural metastasis to the cardia of the stomach and provable invasion to the aorta were also seen. After treatment, with 100 mg/m2 cisplatin on two occasions these lesions were resected curatively. This preoperative chemotherapy would be expected to increase the resectability rate and possibly the survival rate.

Microwave surgical treatment of diseases of prostate.

A new transurethral probe for microwave radiation of the prostate has been developed. As a preliminary experiment, sliced ham was radiated with microwaves using this probe in order to evaluate the extent of thermal effect. Using mongrel male dogs, microwave coagulation of the prostate was examined. These animal experiments showed marked destruction of the prostate gland. Furthermore, the safety of this method was confirmed on the basis of results from the experiments. Prostatic bladder neck obstruction also has been treated in 6 patients with benign prostatic hypertrophy or carcinoma of the prostate by this technique. There has been no mortality and also no complications. The results of this preliminary clinical trial have been excellent.

Prevention of perinatal transmission of hepatitis B virus (HBV) to children of e antigen-positive HBV carrier mothers by hepatitis B immune globulin and HBV vaccine.

The efficacy of hepatitis B immune globulin (HBIG) with two hepatitis B virus (HBV) vaccines was investigated for prevention of vertical transmission of HBV to infants born to e antigen-positive HBV carrier mothers. Infants received 150 mg of the F(ab')2 fraction of HBIG intravenously within 2 hr of delivery, and serum titers of antibody to hepatitis B surface antigen (anti-HBs) were kept greater than 1:4 (passive hemagglutination) by subcutaneous injection of HBIG thereafter. HBV vaccine was started three or six months after delivery. Of 50 infants older than nine months, 49 were actively immunized against hepatitis B surface antigen. Vaccine containing alum adjuvant immunized the infants in a shorter period than did vaccine without adjuvant. Serum titers of anti-HBs of actively immunized infants were well maintained afterward. None of the infants treated with HBIG and HBV vaccine was positive for serum antibody to hepatitis B core antigen at 12 months of age. No side effects were observed.

The estimation of urine bolus volume for patients with congenital hydronephrosis.

A new examination (bolusmetry) to evaluate the urine transport function of the ureteropelvic system was performed in 7 adult patients with congenital unilateral hydronephrosis. Whistle-tipped Fr. 5 catheters were introduced to each ureter about 5 cm proximal from the ureteral orifice by transurethral endoscopic technique. Bolus volume and frequency, and changes in them caused by furosemide injection, were estimated by using a drop counter which was connected to the terminal end of the ureteral catheter. Bolusmetry was performed comparing the hydronephrotic side and the healthy side, pre and postoperatively, and these results were then compared with a conventional examination. We obtained the following results: Bolus volume of the hydronephrotic side was 0.05 +/- 0.02 (mean +/- S.D.) ml at oliguric state, and it was significantly lower than the value of the healthy side which was 0.19 +/- 0.07 ml. Injection of diuretics increased the bolus volume of the healthy side ten times or more. On the other hand, the bolus volume of the hydronephrotic side was increased only slightly by the injection of furosemide, it being approximately one-fourth of the value of the healthy side. The value of bolus frequency was similar to peristaltic frequency which was measured by the electromyogram. The tendency of a decrease was noticed in bolus frequency of the hydronephrotic side but it was not significant. Of patients with severe hydronephrosis, the bolus volume of the hydronephrotic side was decreased and the response to the diuretics was not so significant. By bolusmetry, functional or organic obstruction of the ureteropelvic junction was detected. Of 3 patients who had nephrectomy or nephrostomy, the kidney function had deteriorated severely. The bolus volume was lower than 0.25 ml at the diuretic state. Bolus volume was increased postoperatively in 3 of 4 patients who had received pelvioplasty. One patient did not show the formation of bolus by the injection of diuretics, and the cause of the hydronephrosis was functional obstruction of the pelvioureteric junction. We concluded that bolusmetry is a valuable method for evaluation of the function of urine transport in the ureteropelvic system. Especially in hydronephrosis, bolusmetry is a useful in the choice of operative procedure, and for postoperative evaluation.

Noninvasive screening procedure for evaluation of bladder emptying function.

Bladder emptying function has been initially evaluated by uroflowmetry and post void residual urine measurement. Post void residual urine volume was commonly measured by a transurethral catheter which was not desirable to some patients. In the present study 7 patents with a complaint of dysuria were subjected to combined uroflowmetry and detection of post void residual urine using sector transabdominal ultrasound scanning. The volume estimation with assumption of bladder as an ellipsoid was not so accurate, nevertheless detection of post void residual urine itself was possible in all cases. Thus, the combined method appears to be very useful for evaluating bladder emptying function.
