Mandibular alveolar ridge extension method using a surgical splint with porous hydroxyapatite (HAP) particles.

The mandibular alveolar ridge extension method is a surgical technique to extend the alveolar ridge up to the required level. Using a surgical splint prepared to meet the clinical requirements establishes the desirable alveolar ridge extension with porous hydroxyapatite (HAP) particles. Before the operation, a working cast of the extremely resorpted mandibular alveolar ridge is remodeled into its desired shape with paraffin wax, and the extended surgical splint is cured by clear acrylic resin. After subperiosteal tunnel dissection, the surgical splint is fixed to the mandible with circummandibular ligatures; then the HAP particles are injected into the tunnel. After healing, a treatment denture is cured between the artificial dentition and the surgical splint with self curing resin. In this method, the HAP particles are injected into the subperiosteal tunnel that is created between the surface of alveolar bone and the periosteum covered by the surgical splint; the migration of HAP particles completes the extension of the alveoral ridge.

The carcinostatic effects of 1-(2-tetrahydrofuryl)-5-fluorouracil and uracil (UFT) on tongue carcinoma induced by 4-nitroquinoline 1-oxide (4NQO) in rats.

UFT is a carcinostatic agent used in adjuvant chemotherapy for head and neck cancer. In the present study. UFT was given orally to treat tongue carcinoma in rats induced by 4-nitroquinoline 1-oxide. The antitumor effects of UFT were studied macroscopically and histologically. In addition, the antitumor effects of UFT were evaluated in relationship to lesions of the clinical and, invasive types, and to there vascular structure. In clinical lesions, the antitumor effect of UFT was higher in extrovert tumor-mass lesions and lower in ulcerous lesions. With regard to vascular structure, the effect was higher in cases demonstrating irregular net-like patterns and branch-like patterns and lower in cases in which the pattern had been destroyed. There was a correlation between antitumor effect and invasive type. As invasive tendency the 3H-thymidine labeling index, and mitotic index increased, antitumor effect and degree of tumor cell degeneration decreased.

Immunohistochemical study of basal cell adenoma in the parotid gland.

Basal cell adenoma of the parotid gland was studied with immunohistochemical methods. We observed cells in the tumor with positive reaction to polyclonal keratin, prekeratin, monoclonal PKK-1, polyclonal S-100 protein, monoclonal S-100 protein (alpha), secretory component, actin and laminin. However, no cells which stained positively with monoclonal KL-1, amylase, carcinoembryonic antigen, or epithelial membrane antigen were recognized. From these immunohistochemical results and our ultrastructural observations reported previously, we conclude that the cells constituting the basal cell adenoma are ductal, myoepithelial, and squamous cells but not secretory ones. It is also suggested that the origins of basal cell ademona as well as those of pleomorphic and clear cell adenoma are undifferentiated cells of intercalated duct.

[A study of dynamics of orthodontic archwire on the dental arch. 1. Distribution of orthodontic forces during labial movement of bilateral incisors].

The study on labial movement of maxillary unilateral incisor had been reported previously. This study was the case of maxillary bilateral incisors. Orthodontic forces during labial movement of maxillary bilateral incisors which were produced by stainless steel archwire with vertical loops and nickel titanium archwire, were measured and were examined the distribution of orthodontic forces to the dental arch in the experimental apparatus with strain gauges. The following results were found: 1. In nickel titanium archwire, orthodontic forces were distributed to the entire dental arch, and the distribution of orthodontic forces could be classified into three types, which were similar to the previous study. 2. In stainless steel archwire with vertical loops, orthodontic forces were distributed only to bilateral incisors and adjacent tooth to bilateral incisors. 3. When bilateral incisors displaced lingually, it showed that there were additional orthodontic forces distributions as if two unilateral distributions were overlapped. 4. For maxillary bilateral incisors were moved in optimum force, some consideration to adjust the loops of stainless steel archwire was needed.

[Clinical evaluation of clarithromycin in treatment of acute dental infections. Comparative double-blind study using josamycin as the control].

In order to objectively evaluate the usefulness of clarithromycin (TE-031, A-56268), a new oral macrolide antibiotic, in the treatment of acute dental infections, a double-blind comparative clinical trail was conducted using josamycin (JM) as the control drug. TE-031 was administered at a daily dosage of 400 mg in 2 divided doses, and JM was given at a daily dosage of 1,200 mg in 3 divided doses. The administration period was, as a rule, 7 days. A total of 302 patients were administered with the test substances (TE-031 and JM), and the clinical efficacy was evaluated by investigators for 284 patients and by a committee using a score method for 273 patients. Efficacy rates as evaluated by the investigators were 77.2% (105/136) in the TE-031 group and 69.6% (103/148) in the JM group. Efficacy rates as evaluated by the committee by the score method were 86.0% (111/129) in the TE-031 group and 80.6% (116/144) in the JM group. The differences between the 2 drug groups were not statistically significant. The investigators' evaluation of the clinical efficacy in the treatment of osteitis of the jaw gave an efficacy rate of 83.0% (44/53) in the TE-031 group and 64.7% (33/51) in the JM group. The efficacy rate in the TE-031 group was statistically higher than that in the JM group. Side effects were recorded in 7 patients (4.8%) in the TE-031 group and 3 patients (2.0%) in the JM group, while abnormal laboratory test values were detected in 3 cases each in the TE-031 and JM groups. None of these differences were statistically significant. The usefulness rates ("satisfactory" plus "very satisfactory" cases) were 73.9% in the TE-031 group and 70.3% in the JM group and were thus almost the same for the 2 drug groups. On the basis of the above results, TE-031 was concluded to be a useful drug in the treatment of acute dental infections and is expected to be able to achieve almost identical clinical efficacy as JM at only one-third of the usual dosage of JM.

[The relations between doses of streptozotocin and pathosis in induced diabetes mellitus].

Tail-vein injections of streptozotocin (STZ) in various doses (20-60 mg/kg body weight) were used to induce diabetes mellitus in male rats of the Sprague-Dawley strain. Resulting pathosis was observed on the basis of various parameters (body weight, blood glucose, plasma insulin, glucose-tolerance tests, uroscopy, hemodiagnosis, and skin strength) Results 1. Diabetes mellitus was induced in all rats of the group in which the dosage was STZ 40 mg/kg body weight (S-40 group). Blood-glucose level in this group was about 340 mg (3 times the quantity in controls). Plasma-insulin level was about 8.9 microU/ml (about 1/3 the quantity in controls). Glucose-tolerance tests and observation of urino-glucose showed reductions in glucose tolerance and plasma-insulin response. 2. In the S-40 group, pathosis of induced diabetes mellitus had stabilized 30 days after and persisted for 90 days after STZ injection. 3. Skin strength decreased for 20 days after STZ injection. The level remained stable at half the strength of control skin from 30 to 90 days after the injection. 4. In groups in which doses were STZ 20 mg/kg body weight or STZ 30 mg/kg body weight, diabetes mellitus was induced in some of the rats. The animals tended to recover from the induced pathosis. 5. Diabetes mellitus was induced in all rats to which doses of STZ 50 mg/kg or STZ60 mg/kg body weight were induced. shortly after injection, the induced-diabetes pathosis changed for the worse; and grave complications (hypo-albuminosis, diabetic ketocacidosis, and diabetic proteinuria) were observed. A large number of the rats in these groups died. 6. The results of this study confirm the opinion that STZ 40 mg/kg body weight is the optimum dose for STZ induction of diabetes mellitus in rats for experimental studies. The suitable term for such studies is from 30 to 90 days after STZ injection.

[Physicochemical characteristics of new reinforcement ceramic implant].

Investigation of the physicochemical characteristics of reinforcement ceramic implant materials which have needle crystal produced the following results. 1. Simple diopside and hydroxyapatite with diopside have enough dynamic intensity of both the implant in point of bending intensity and the breaking toughness. 2. Simple diopside and hydroxyapatite with diopside go well as biomechanics because of their Young's modulus being close to the surrounding bone. 3. Diopside in pseudo-body fluid developed hydroxyapatite on surface like bioglass, and this resulted in high possibility of direct bond with bone. 4. The results of this study indicate that the diopside and hydroxyapatite with diopside have enough dynamic intensity and are bioactive dental implant materials.

Ultrastructure of basal cell adenoma in the parotid gland.

Basal cell adenoma of the parotid gland was studied with electron microscopy. The cells constituting this tumor were divided into three types of epithelial cells; ductal, myoepithelial, and squamous cells. The ductal cells, which were polygonal and cuboidal in shape, formed a sometimes distinct lumen. Glycogen were recognized in the cytoplasm of these cells. The myoepithelial cells appeared as plasmacytoid cells which contained abundant microfilaments. The squamous cells were characterized by the presence of well-developed tonofilaments and desmosomes. However, no secretory cells could be found, although small, electron dense granules were detected in the cytoplasm of the ductal cells. The granules were unlike secretory granules in their size, number and location. In consideration of the presence of secretory and myoepithelial cells, we reviewed previously reported literature and discussed the identification of secretory granules. From our and other reported results, we tentatively concluded that the electron dense granules described as secretory granules are not intrinsic secretory granules. Further, we suggested that the cell types and the histogenesis of basal cell adenoma are analogous to those of both pleomorphic and clear cell adenomas.

[A statistical study of autopsies performed on cases of oral cancer: with reference to distant metastasis in tongue cancer].

1. A statistical survey of oral-cancer (tongue, maxilla, and mandible) patients on which autopsies were performed by the 2nd. Department of Pathology of Tokyo Dental College from 1964 to 1987 was carried out. Of a total of 18 autopsies in cases in which death resulted from tongue carcinoma, 11 subjects were male and 7 female. Average age was 63.4 years. This cancer occurred most frequently in individuals in the age group of 60 to 69 years. The lung was the most common (50%) site of distant organic metastasis; the next most common sites were the liver, the thyroid gland, and the diaphragm (11.1% each). Submandibular and cervical lymph nodes were the most common sites of lymphatic metastasis in cases of tongue cancer. These data were similar to those in previous reports. 2. An autopsy on a 57-year-old man who died of tongue cancer revealed a number of distant metastases in both organs and lymph nodes were reported. 3. The report includes a review of literature on distant metastasis in cases of tongue cancer.
