A clearance method for ceftizoxime (CZX), a new cephalosporin antibiotic, was designed and utilized to assess the mechanism of renal excretion and evaluate kidney distribution under steady-state serum level. Renal clearance ratio of CZX to inulin, when corrected for protein binding, was 1.6 at serum level of 13 micrograms/ml and was decreased to around unity with a serum level of 700 micrograms/ml or higher or by infusing p-aminohippuric acid or probenecid. The clearance ratio appeared to be independent of urinary pH or changes in urinary flow. The kidney/serum concentration ratio for CZX decreased from 2.5 to 1.5 in proportion to the extent of reduction of the clearance ratio. p-Aminohippuric acid and probenecid further reduced the kidney/serum ratio to about 1.0 with concomitant inhibition of tubular transport of CZX. Cortex/serum and medulla/serum ratios were also reduced similarly to that of the whole kidney. It is concluded that the tubular transport of CZX is similar to that for p-aminohippuric acid transport system and that passive reabsorption from the tubular lumen may not necessarily be present.

It is clear that the prognosis of uterine cervical cancer depends on the spread of cancer, especially lymphnodes metastases and parametrial infiltration. The correct diagnosis of parametrial infiltration before treatment is, however, difficult and there are many over- and under-diagnosis. In this paper, we discuss the details of parametrial infiltration based on the radically hysterectomized specimens in Shinshu University Hospital from 1964 to 1978. In these period, 284 cases of uterine cervical cancer were operated by Okabayashi's radical hysterectomy and our postoperative classification were carried out. The results were as follows; 1) The incidence of parametrial infiltration were 16.4 in Stage Ib, 36.6% in Stage IIa, 57.5% in Stage IIb and 35.6% in all cases. 2) When divided the parametrial tissue into three parts, we could find carcinomatous infiltration into uterine part in 77.3% of positive cases, middle part in 16.2% and pelvic part in 16.2%. 3) The agreement between clinical diagnosis and histological findings were 72.2% in all cases. 4) The CPL classification had close relation with parametrial infiltration (C type; 0%, P type; 17.9%, L type; 61.8%). 5) The 5-year survival rate of patients with infiltration showed poor outcome (63.0% in 81 cases), but that with no infiltration had good prognosis (98.4% in 128 cases). In conclusion, parametrial infiltration is very important for treating the uterine cervical cancer patients.

Two types of mutants highly sensitive to beta-lactam antibiotics were obtained from Pseudomonas aeruginosa PAO 2142 by treatment with N-methyl-N'-nitro-N-nitrosoguanidine. One type of mutant showed over 30 times higher sensitivity to mecillinam, carbenicillin and sulbenicillin than did the parent, but not to most other beta-lactam antibiotics tested. In contrast, the other type mutant was about 30 times more sensitive to ampicillin, cephaloridine, cefoxitin and cefmetazole, but resistant to mecillinam, carbenicillin and sulbenicillin at the same level as the parent. Beta-lactamase activity of these mutants ws not different from that of the parent. Defect in either of penicillin-binding proteins 1A/1B or 5 was observed in some mutants of P. aeruginosa highly sensitive to beta-lactam antibiotics.
