[Effect of suloctidil on cerebral venous outflow in the dog (author's transl)].

The effects of intravenous administration of suloctidil [erythro-1-(4-isopropylthiophenyl)-2-n-octylamino-propanol], (MY 103) on cerebral venous outflow (VOF), blood gases, cerebral metabolic rate of oxygen (CMRO2) and other hemodynamic parameters were studied and effects compared to those of cinnarizine and papaverine, in gallamine immobilized dogs. MY103 and papaverine at doses 0.1-1.0 mg/kg, i.v. caused significant increases in VOF by 6.1-38.3% and 12.1-26.1% respectively. The changes in VOF persisted for 1.4 min and 0.5 min in mean following 1.0 mg/kg, i.v. of MY103 and papaverine, respectively. Cinnarizine also increased VOF by 10.0 and 18.7% at doses 0.3 and 1.0 mg/kg i.v. The duration of VOF changes by cinnarizine 1.0 mg/kg, i.v., 2.4 min (mean), was no longer than the duration seen with the same dose of MY103 or papaverine. Mean blood pressure (MBP) decreased dose dependently with these drugs and consequently the cerebral vascular resistance (CVR) decreased 8.6-40.2% with MY103, 13.6-25.0% with cinnarizine and 16.1-39.5% with papaverine administration, respectively. MY103 and papaverine at a dose of 1.0 mg/kg i.v. did not affect pH, Pco2 and Po2 in arterial blood, but these two drugs did show a tendency to lower Pco2 and elevate Po2 in cerebral venous blood. CMRO2 was not significantly affected by MY103 or papaverine. It is suggested that cerebral vasodilation may be the mechanism for VOF increase following MY103 administration.

[Studies of solid ovarian tumors in Tokai District (author's transl)].

The prognosis of patients with ovarian carcinoma is believed as worst among malignant diseases in the field of gynecology. Multiple factors including clinical stage, histologic type and grade may play an important role in the prognosis of patients. Because of these factors and the few cases of ovarian carcinoma seen in any one center over a short period, the ability to make valid evaluation of these factors has been limited. We organized ovarian tumor studying group in Tokai district with eleven member hospital and analyzed 453 cases of solid ovarian tumor retrospectively. 1) Cumulative survival rate was calculated according to the clinical stages. Five year survival rate of stage 1 was about 80%, that of stage 2 was about 30% and that of stage 3 and stage 4 were around 10% respectively. 2) Cumulative survival rate was calculated according to histologic types. There was minor variation of survival by histologic types. When we compared with different histologic types by same clinical stages such as stage 1 and stage 3 there was no definite differences of survival rate except slightly poor prognosis in case of mucinous cystadnocarcinoma. 3) Cumulative survival rate was calculated according to histologic grading. Histologic grade affects the prognosis of patients with ovarian carcinoma.

[Effectiveness of acetylspiramycin for Mycoplasma pneumonia in children (author's transl)].

The clinical efficacy and safety of acetylspiramycin for Mycoplasma pneumonia in children were studied at a dose level of 50 mg/kg/day q.i.d. and the following results were obtained. Acetylspiramycin showed effect in 24 patients out of 25 (96.0%). The level of transminase was increased in 3 cases on admission. One of them was an uneffective case, and eruption was seen after administrating the drug. Diarrhea and poor appetite were observed in one case as side effects, and they were controlled easily. It may be concluded from these results acetylspiramycin was relatively free from side effects and considered to be useful for Mycoplasma pneumonia.

[Laboratory and clinical studies cefroxadine in pediatric field (author's transl)].

Cefroxadine (CGP-9000, CXD) is an orally active cephalosporin which has been newly developed by Ciba-Geigy. Its antibacterial activity against clinically isolated organisms, absorption and excretion after oral administration and clinical responses of patients with infections were studied. Its antibacterial activity against S. aureus and K. pneumoniae was comparable to that of CEX. Against E. coli, it showed slightly stronger activity than CEX. The serum concentrations of CXD was measured in 6 patients given orally 10 mg/kg of it after meals. The peak mean serum level attained 1 hour after administration was 11.66 mcg/ml. Forty-three children with various infections (upper respiratory infections, lower respiratory infections, urinary tract infections, etc.) were given 17 approximately 46 mg/kg (averaged 28.7 mg/kg) of CXD daily. The result was excellent in 26 cases, good in 12 cases, fair in 3 cases, fair in 3 cases and poor in 2 cases, showing an effective rate of 88.4%. Side effect of transient leukopenia and elevation of S-GOT were noticed in 2 cases. However, there were no serious adverse reactions in our study. It is concluded that CXD might be effective in rather mild cases of respiratory or urinary tract infections.

Blood group Rh-D factor in human trophoblast determined by immunofluorescent method.

The chorionic villi of human placentas, 12 to 40 weeks' gestation, were examined for Rh-D blood group antigens with a direct immunofluorescent (IF) technique. Specific IF-standing was shown on the plasma membrane and cytoplasm of the villous trophoblasts. This staining decreased in intensity as the placenta matured. In addition, the trophoblasts of hydatidiform and destructive moles were also examined. The intensity of the IF-staining in these trophoblastic tumors was about the same as that in the young trophoblasts of the first trimester of pregnancy.

[Protective effect of suloctidil against cerebral hypoxia (author's transl)].

The protective effect of suloctidil [MY 103, erythro-1-(4-isopropylthiophenyl)-2-n-octylaminopropanol] against cerebral hypoxia was investigated in terms of survival time under normobaric hypoxia (96% N2 + 4% O2 gas mixture) in mice and evoked potential in prepyriform cortex of guinea pig in vitro. In a hypoxic condition, the pretreatment with MY 103 (3.0-12.5 mg/kg, i.p.) resulted in a marked and dose-dependent prolongation of survival time of mice without any adverse effect. The prolongation of survival time was statistically significant at doses over 6.3 mg/kg. The amplitude of evoked potential to electrical stimulation of olfactory tract in the brain slice was consistently and reversibly suppressed by the perfusion with low oxygen Ringer's solution containing about 45% O2 or normal Ringer's solution. This amplitude reduction after 6 minutes exposure to hypoxic perfusate, was consistently reproducible in the same preparation. Preincubation of brain slices in normal Ringer's solution containing 10(-7)-10(-6) g/ml MY 103 for 15 min, however, effectively prevented the hypoxic reduction of the amplitude of evoked potentials to 30-50% of the control experiments. The above data indicate that MY 103 exerts its anti-anoxic effect presumably through modified metabolic processes such as active oxygen or glucose uptake by the brain.

Gastric motility after selective proximal vagotomy.

Effects of vagotomy on gastric motor activity were studied on 39 patients after vagotomy for gastric or duodenal ulcer by fluoroscopic examination and by monitoring of intragastric pressure using a balloon introduced into the stomach. These patients were divided into three groups according to types of vagotomy; selective proximal vagotomy (SPV), selective vagotomy (SV) and atypical selective proximal vagotomy. In atypical SPV, dorsal antral branches of vagal gastric nerves were sacrificed in addition to conventional SPV because of local conditions around the ulcer. In patients after SPV or atypical SPV, no gastric stasis was observed on fluoroscopic examination. However, marked gastric stasis with delayed emptying was a constant finding in patients after SV, especially at the early postoperative period. In recording of intragastric pressure, basic peristaltic waves of a 20-second rhythm were observed following inflation of the balloon in all patients after any type of vagotomy. In addition, an alternation of active and resting periods of motor activity and confluence of large waves at the end of active period were observed as characteristoc findings in patients after both SPV and atypical SPV. The large waves were considered to coincide with active propulsive movements seen on fluoroscopy in these patients. The results in this study confirmed that good gastric motility could be preserved in patients after SPV and even after atypical SPV.

Criteria of complete remission from trophoblastic neoplasia with the use of human chorionic gonadotropin (HCG) excretion pattern as a parameter.

The excretion pattern of human chorionic gonadotropin (hCG) or luteinizing hormone (LH) was observed in the urine of 77 patients with trophoblastic neoplasia, 109 with complete remission from trophoblastic neoplasia, and 94 with no trophoblastic neoplasia, when the anti-beta-subunit hCG system radioimmunoassay (anti-beta-subunit RIA), which specifically measured hCG was used. The sensitivity of anti-beta-subunit RIA was limited to urinary hCG 16 mIL/ml from the specificity of the anti-beta-subunit serum. Luteinizing hormone in the urine of patients with complete remission and in normal menstrual, postmenopausal, and castrated women was less than 16 mIU/ml in most cases. The excretion pattern of urinary hCG in the patients undergoing treatment for trophoblastic neoplasia was more clearly comprehended with the anti-beta-subunit RIA as compared with the anti-hCH RIA. The criteria for judgment of complete remission were that the hCG value dropped to less than 16 mIU/ml and that cellular response was not observed at least in the last two courses.

Characterization of two different factors chemotactic for cancer cells from tumor tissue.

Two different factors chemotactic for cancer cells were extracted in pseudoglobulin fraction of rat ascites hepatoma transplanted tissue. After chromatography on Sephadex G-50 and CM-sephadex, these factors were separated by gel filtration on Sephadex G-100. The factor a was further fractionated by immunoadsorbent chromatography with goat antirat gamma-globulin antibody and then with rabbit antirat hemoglobin antibody; it was a protein with a molecular weight of about 78,000, resembling a chemotactic factor previously reported, and its activity was thermolabile. The previously undescribed factor b was also a protein with a molecular weight of about 14,000 and its activity was thermostable. Intradermal injection of these factors at low concentrations induced an extravascular migration of circulating tumor cells and formation of metastatic secondary tumors; and little difference in the in vivo effect between these factors was observed. It was thus assumed that the combined action of these two factors may be involved in malignant invasion.
