[Serological diagnosis of pulmonary aspergillosis--measurement of IgG-, IgM- and IgA- antibodies against Aspergillus fumigatus by means of ELISA].

Serological diagnosis plays an important role in the diagnosis of pulmonary aspergillosis, however, precipitation-in-gel test is neither sensitive nor quantitative. Recently, several investigators have used the ELISA technique for the detection of antibodies against Aspergillus fumigatus and reported the usefulness of this method. In this report, we measured IgG-, IgM-, and IgA- antibody titers against Aspergillus fumigatus by means of ELISA in sera from patients with several different lung diseases including pulmonary aspergillosis. The results obtained were as follows: 1) Measurement of IgG-antibody titers was most useful for the diagnosis of pulmonary aspergillosis. 2) Measurement of IgG-antibody titers was more sensitive than precipitation-in-gel test. 3) IgG-antibody titer was quantitative and reflected the clinical course of pulmonary aspergillosis.

[Ocular symptoms in mitochondrial myopathy].

The corneal endothelial cells in 6 cases of mitochondrial myopathies were examined by specular microscope, and electron microscopic studies of the cornea were made in one autopsy case. The cornea was clear and the thickness was within the normal range. The endothelial cell mean size was larger in 3 out of 10 eyes than among controls and the variance of endothelial cell size was larger in all of 10 eyes than among controls. The difference was found to be statistically significant. Electron microscopical findings of the cornea revealed mitochondrial abnormalities. Inclusions were observed in the stromal keratocyte.

[Optic neuropathy induced by thinner sniffing].

A case of optic neuropathy induced by thinner sniffing is reported. A 17-year-old girl, who had been sniffing a lacquer thinner for three months, suffered acute blindness followed by optic atrophy. Brain computed tomography revealed symmetrical low attenuation areas in the bilateral putamen. An analysis of the thinner by gas chromatography showed that its major components in a vaporized state were methyl alcohol and metyhl acetate. The optic neuropathy was induced by these solvents. In the diagnosis of the intoxication of mixed organic solvents, the measurement of each solvent in its vapor phase is considered to be quite important.

Effect of in vivo exposure to hypoxia on muscarinic cholinergic receptor-coupled phosphoinositide turnover in the rat brain.

We examined the effects of a transient exposure of rats to severe (5% O2, 95% N2 for 30 min) or mild (10% O2, 90% N2 for 30 min) hypoxia and a chronic exposure to mild hypoxia (10% O2, 90% N2 up to 48 h) on carbachol-stimulated phosphoinositide (PI) turnover and [3H]quinuclidinyl benzilate (QNB) binding in 4 regions of the brain (cerebral cortex, striatum, hippocampus and cerebellum). When the rats were exposed to hypoxia transiently, significant changes were caused only by severe hypoxia. In all 4 regions examined, basal incorporation of [3H]inositol into inositol phosphates (IPs) in the absence of carbachol was significantly increased following transient severe hypoxia and remained high for 48 h. Carbachol stimulation of PI turnover (% of basal) was significantly enhanced only in the hippocampus. The increase in carbachol stimulation in the hippocampus was observed only after a 6-h exposure to room air following the severe hypoxia and lasted for at least 48 h. In Ca2+-free incubation medium, the increase in basal incorporation of [3H]inositol into IPs elicited by severe hypoxia was not observed but carbachol still had an enhanced stimulatory effect. Binding studies showed that maximum binding capacity (Bmax values) of [3H]QNB binding was significantly increased only in the hippocampus and only after a 6-h exposure to room air following the severe hypoxic exposure and remained increased for 48 h. Kd values showed no significant change. While a transient exposure of rats to mild hypoxia caused no significant change either in carbachol-stimulated PI turnover or in [3H]QNB binding parameters, a chronic exposure to mild hypoxia for more than 6 h caused a significant increase in [3H]QNB binding capacity localized in the hippocampus, which was accompanied by an enhanced stimulatory effect of carbachol on PI turnover. These changes observed with mild hypoxia, however, were transient and tended to return to control levels following a 48-h exposure. These results suggest an up-regulation of muscarinic cholinergic receptor-coupled PI turnover in the rat brain caused either by transient severe hypoxia or by chronic mild hypoxia.

Visuo-motor reaction times of normal subjects and schizophrenics using inverting prisms.

The visuo-motor reaction times of normal subjects and schizophrenics were measured to investigate adaptation of eye-hand coordination in schizophrenics under modified vision. Reaction time was measured in 32 normal subjects and 32 schizophrenics by using a rectangular board with 24 illuminated buttons. Subjects' task was to press an illuminated button as quickly as possible under normal and modified vision. Time elapsed from when the light was switched on to when the button was pressed was measured as the visuo-motor reaction time in msec. Three conditions were set up: control in normal vision; inverted using an inverting goggle; reversed using a reversing goggle. In general the visuo-motor reaction times of schizophrenics were slower than those of normal subjects. But in the reversed condition, there was no significant difference between normal subjects and schizophrenics. The visuo-motor reaction times of schizophrenics in the reversed condition showed a large dispersion with the fastest being faster than that of any normal subjects. It is suggested that there may be a group among schizophrenics whose basic paradigm of visuo-motor association is different from that of normal subjects.

Bilateral waveform dissimilarity of auditory evoked potentials may reveal a lateralized motor response process: a correlation method.

A modified correlation method was applied to AEPs under a choice reaction task to distinguish hemispheric influences on motor versus stimulus processing. The subjects were 16 right-handed males (age: 19-30). The task was to press the micro-switch with the right index finger as quickly as possible to 2,000 Hz tone pips (GO) but not to press to 1,500 Hz (NO GO). Active electrodes were placed bilaterally on C3 and C4, according to the 10-20 system and linked-ear electrodes were used as a reference. EEGs triggered by the auditory stimuli were summated 100 times and the obtained AEPs were mainly analyzed by a modified correlation method. The results were as follows; 1. the mean choice reaction time was 332 +/- 83 msec, 2. examined by a modified correlation method, there were significant differences between GO and NO GO at the latencies of 200-300 msec. There is a possibility that these intervals when significant differences were seen by a modified correlation method may relate to a motor response process.

Effects of oxygen depletion on norepinephrine- and carbachol-stimulated phosphoinositide turnover in rat brain slices.

We examined the effects of in vitro anoxia and in vivo hypoxia (8% O2/92% N2) on norepinephrine (NE)- and carbachol-stimulated phosphoinositide (PI) turnover in rat brain slices. The formation of 3H-labeled polyPI in cortical slices was impaired by in vitro anoxia and fully restored by reoxygenation. Accumulation of 3H-labeled myo-inositol phosphates (3H-IPs) stimulated by 10(-5) M NE was significantly reduced by anoxia (control at 60 min, 1,217 +/- 86 cpm/mg of protein; anoxia for 60 min, 651 +/- 82 cpm/mg; mean +/- SEM; n = 5; p less than 0.01), and reoxygenation following anoxia resulted in overshooting of the accumulation (control at 120 min, 1,302 +/- 70 cpm/mg; anoxia for 50 min plus oxygenation for 70 min, 1,790 +/- 126 cpm/mg; n = 5; p less than 0.01). The underlying mechanisms for the two phenomena--the decrease caused by anoxia and the overshooting caused by reoxygenation following anoxia--seemed to be completely different because of the following observations. (a) Although the suppression of NE-stimulated accumulation at low O2 tensions was also observed in Ca2+-free medium, the overshooting in response to reoxygenation was not. (b) Carbachol-stimulated accumulation was significantly reduced by anoxia and was restored by reoxygenation only to control levels. Thus, the postanoxic overshooting in accumulation of 3H-IPs seems to be a specific response to NE. (c) The decrease observed at low O2 tensions was due to a decrease in Emax value, whereas the postanoxic overshooting was due to a decrease in EC50 value.(ABSTRACT TRUNCATED AT 250 WORDS)

Penile vascular system of the dog. An injection-corrosion and histological study.

The vascular architecture of the dog penis was studied by resin casts and histological preparations. Morphological evidence was found for penile engorgement. The crus and the bulb were compressed by the ischiocavernosus and the bulbospongiosus muscles, respectively, indicating that these muscles act as a booster pump. The distal part of the bulb was also strangulated by the external anal sphincter, indicating that this muscle prevents blood in the corpus spongiosum penis from going back into the bulb. The common trunk of the dorsal vein of the penis was significantly occluded by the ischiourethralis muscle. The internal pudendal vein was also throttled by the levator ani, the coccygeus, and the internal obturator muscles. The extremely narrow diameter of each vein draining the erectile body was insufficient to drain the increased blood flow and, therefore, formed a sluice channel. It is inferred that these muscle activities and sluice channels facilitate the erection of the penis.

Plasma glucagon response to intravenous alanine in obese and non-obese subjects.

To investigate glucagon (IRG) and insulin (IRI) responses to alanine infusion in obesity and to assess the effect of body weight reduction with respect to hormonal balance, we compared six obese subjects with nine normal weight controls. None of the subjects were diabetic by OGTT criteria. Plasma IRI and IRG were measured following IV alanine at a rate of 0.1 g/kg over a period of 2 min. Our obese subjects had an increase in IRG response to alanine, which was due to decreased suppression of alpha-cell function due to insulin resistance. Weight reduction via calorie restriction reduced insulin demand, resulting in reduced plasma IRI by restoring beta-cell function, and the IRG response was paradoxically decreased as compared with that before weight loss. It is conceivable that improvements in insulin sensitivity after body weight reduction may re-establish the normalization of pancreatic beta-cell function and the insulin-induced inhibition of IRG secretion. Our obese subjects were characterized by decreased IRG secretion which was reflected in a change in body weight reduction.

[Dysfunction and deficiency of decay-accelerating factor (DAF) demonstrated in the lymphocytes from a patient with paroxysmal nocturnal hemoglobinuria (PNH)].

A defective cell-mediated immunity was seen in a 62-year-old female with paroxysmal nocturnal hemoglobinuria (PNH). We studied the functional defect of the patient's lymphocytes and its relation to the deficiency of decay-accelerating factor (DAF) on the lymphocytes. T cells (CD 5+) and B cells (CD 20+) were obtained by cell-sorting using fluorescence-activated cell sorter (FACS-IV). These two types of cells from the patient were demonstrated to be deficient in DAF by the fluorometric measurement of DAF content using monoclonal anti-DAF antibodies. These cells were shown to be more susceptible to complement-mediated lysis than normal human lymphocytes by a complement-mediated lysis study. It was carried out by treatment of the lymphocytes with either anti-CD 5 or anti-CD 20 antibody plus rabbit complement. The lymphocytes became more susceptible to complement-mediated lysis by an additional treatment with an anti-DAF antibody both in PNH and in normal controls. From these results, we suggest that DAF plays an inhibitory role against complement activation on human lymphocytes. The mononuclear cells of the patient responded poorly to phytohemagglutinin (PHA), concanavalin A (Con A) and pokeweed mitogen (PWM). Skin tests both for PPD and for DNCB showed negative. From these findings, we suggest T cell function in the patient is impaired. Causative relations of the deficiency of DAF to the poor responses of the lymphocytes to lectins and to negative skin tests were discussed.

[Myocardial free wall rupture and thrombolytic therapy in acute myocardial infarction].

We investigated the relation between myocardial free wall rupture and thrombolytic therapy in 200 patients with acute myocardial infarction (AMI). Ten of 200 patients (5.0%) were complicated with cardiac rupture, and all of them died within 70 hours after the onset (29% of the deceased after AMI). The pathophysiologic study of 5 patients undergoing autopsy after cardiac rupture was performed. In 4 patients receiving thrombolysis, autopsy revealed massive hemorrhagic infarction and teared lesion near the center of infarcted area. We assessed that the location of teared lesion might be influenced by broad hemorrhagic infarcted area following thrombolytic therapy. The incidence of cardiac rupture was slightly higher in the group receiving thrombolysis in the early stage of AMI than conventional treatment group. Among the patients receiving thrombolytic therapy, some cases revealed markedly increased fibrinolytic activity. This suggested that such elevated fibrinolytic activity might induce massive hemorrhagic infarction and might be an important factor contributing to the cardiac rupture. Thrombolytic therapy has been frequently reported to improve cardiac function and prognosis, but our study suggests that thrombolytic therapy must be evaluated moreover as one of the risk factors of cardiac rupture.

p-nitrophenyl butyrate hydrolyzing activity of hormone-sensitive lipase from bovine adipose tissue.

The "esterase" activity of hormone-sensitive lipase (HSL) was studied using water-soluble p-nitrophenyl butyrate (PNPB) as a substrate. Bovine adipose tissue HSL was purified to near homogeneity by precipitation at pH 5.0, followed by chromatography on DEAE-cellulose, phenyl-Sepharose, and high performance ion-exchange columns on Mono Q and Mono S. The purified preparation hydrolyzed emulsified triolein and cholesteryl oleate (CO), and water-soluble PNPB. In the two last steps of purification, the elution profile of the CO-hydrolyzing activity coincided with that of PNPB-hydrolyzing activity. The HSL was adsorbed to heparin-Sepharose and the CO- and PNPB-hydrolyzing activities were eluted together in the same peak. Diisopropylfluorophosphate (DFP) strongly inhibited the HSL activities and the inhibition profiles of the triolein-; CO-, and PNPB-hydrolyzing activities were essentially identical. Only one polypeptide of Mr 84,000 in partial purified HSL fraction was labeled by affinity labeling with [3H]DFP. On digestion of the enzyme with trypsin, the triolein- and CO-hydrolyzing activities were lost more rapidly than the PNPB-hydrolyzing activity. Phosphorylation increased the triolein-hydrolyzing activity to 40% more than that of the control, but did not affect the CO- and PNPB-hydrolyzing activities.

[Appearance of Sweet's syndrome in a patient with myelodysplastic syndrome (MDS) without relation to the hematological findings of MDS].

Sweet's syndrome is known often to associate with non-lymphocytic leukemia (ANLL); however, there have been very few reports of Sweet's syndrome associated with myelodysplastic syndrome (MDS). It was reported that improvement and exacerbation of these two syndromes occurred simultaneously. We present here a 49-year-old male with Sweet's syndrome developed in RAEB in T. He complained of fever and infiltrative eruptions on the trunk and legs. At the time of admission to Tsukuba University Hospital, the peripheral blood showed leukocytopenia (WBC 2,000/microliter: Blast 9%, PMN 51%) and anemia (Hb 6.5 g/dl). Pseudo-Pelger anomaly of neutrophils was found on the blood smear. From the hematological findings and the result of skin biopsy, the patient was diagnosed as having MDS (RAEB in T) complicated by Sweet's syndrome. Prednisolone was effective to improve his fever and eruptions. However, when treated with low-dose Ara-C and when transformed into acute myelogenous leukemia, there was no correlation between the condition of Sweet's syndrome and the percentages of blasts in the marrow. We suggest that eruptions of Sweet's syndrome associated with MDS are not always a good index of exacerbation of MDS.

[Clinico-immunological studies of pulmonary tuberculosis in the elderly].

We studied tuberculin reactivity and clinical course after starting chemotherapy in patients with active pulmonary tuberculosis divided by four age-groups less than 29, 30-49, 50-69 and 70 years and more. The skin test to tuberculin purified protein derivative (PPD) was examined in 178 cases of active pulmonary tuberculosis, 120 cases of lung cancer, 25 cases of atypical mycobacteriosis and 466 cases of the other respiratory diseases. The average size of tuberculin reaction in pulmonary tuberculosis decreased with age, but significantly higher than that in patients with other nontuberculous pulmonary diseases of the same age-group. The size of PPD skin test in the group of 70 years and more was significantly lower than other age-groups in pulmonary tuberculosis. We compared the time required for negative conversion of sputum by culture after primary chemotherapy among the different age-groups in pulmonary tuberculosis. It revealed that the time for negative conversion tended to be longer with age, and the time in the group 70 years and more was significantly longer than that of the group less than 29 years of age, although no significant differences in the radiographic severity and conditions of chemotherapy were observed. Finally, the PPD-induced lymphocyte proliferation test in vitro was done in newly diagnosed patients with pulmonary tuberculosis. The patients were divided into two groups by the size of PPD skin test (high responder more than 16 mm and low responder less than 15 mm of erythema induced by PPD).(ABSTRACT TRUNCATED AT 250 WORDS)

Oxymetholone therapy in patients with familial antithrombin III deficiency.

Three patients with familial antithrombin III (ATIII) deficiency, who also have histories of thromboembolism, were treated with oxymetholone in combination with warfarin. Thrombolysis was observed in one patient with acute thrombosis of inferior vena cava during the oxymetholone and warfarin therapy. No further thromboembolic episodes occurred in these patients after initiation of warfarin with or without oxymetholone. The levels of plasma ATIII, alpha 1-antitrypsin, plasminogen and Cl-inactivator were significantly increased in all patients after the introduction of oxymetholone therapy. This suggests that oxymetholone augments anticoagulant and fibinolytic activity. Hence we consider that oxymetholone in combination with warfarin may be possible thrombolytic therapy in patients with familial ATIII deficiency.

Nicotinic acetylcholine receptors in the rat stomach: I. (-)-[3H]nicotine binding.

The nicotinic acetylcholine receptors in the rat stomach were characterized by means of a radioligand binding assay with (-)-[3H]nicotine as ligand. Saturation binding studies on the gastric fundus membranes revealed the presence of two binding sites with dissociation constant (KD) values of 3.1 and 289 nM, and maximum binding capacity (Bmax) values of 3.6 and 76 fmol/mg protein, respectively. The Bmax of the high affinity binding site was greatest in the cardia, followed by fundal mucosa, fundal muscle, and, finally antrum. The IC50 values of cholinergic drugs to inhibit (-)-[3H]nicotine binding in fundus membranes were as follows: (-)nicotine, 0.12 nM; cytosine, 9.3 nM; acetylcholine, 17.7 nM; carbachol, 700 nM; hexamethonium, 2270 nM. The IC50 values of alpha-bungarotoxin, d-tubocurarine and atropine were greater than 100 microM. The muscarinic acetylcholine receptors were also characterized with [3H]quinuclidinyl benzilate and the choline acetyltransferase activity was measured. These results suggest that nicotinic acetylcholine receptors as well as muscarinic acetylcholine receptors are present in the rat stomach and that the regional distribution of these receptors is uneven.
