Necrotizing sarcoid granulomatosis: computed tomography and pathologic findings.

OBJECTIVE: To describe the CT findings and corresponding pathological findings of necrotizing sarcoid granulomatosis (NSG). MATERIALS AND METHODS: We reviewed the high-resolution CT and macroscopic and histologic findings in three pathologically proven cases of NSG. RESULTS: The CT findings consisted of one case each with single nodule, multiple nodules, and parenchymal opacification. The abnormalities were peribronchovascular or subpleural in distribution. Histologically, all patients had sarcoid-like granulomas with central necrosis and granulomatous vasculitis. The CT findings correlated closely with the gross pathologic findings. CONCLUSION: We conclude that the CT manifestations of NSG reflect the variable macroscopic pathologic findings.

In situ expression of ICAM-1 and its mRNA in the lung tissue of asthmatic rats.

In situ expression of ICAM-1 and ICAM-1 mRNA on the lung tissue of asthmatic rats was studied by immunohistochemical staining and in situ hybridization, respectively. The results showed that in normal rats ICAM-1 expression was rare on the endothelium of pulmonary artery and vein, and on the bronchial and alveolar epithelium. The distribution of ICAM-1 expression on the different part of lung tissue of asthmatic rats was similar to that of normal rats, but the level of ICAM-1 expression was significantly increased on the endothelium of pulmonary artery (EPA) and vein (EPV), bronchial epithelium (BEP) and alveolar epithelium (AEP) compared with those of normal and sensitized controls. The distribution and expression of ICAM-1 mRNA on the different part of lung tissue of normal and asthmatic rats were similar to that of ICAM-1 expression. In asthmatic rats, the expression of ICAM-1 mRNA on AEP and EPV was increased significantly compared with those of normal and sensitized controls. It is concluded that the increase of ICAM-1 expression on endothelium of pulmonary vessels, epithelium of broncheoli and alveoli may play an important role of inflammatory cell infiltration in asthmatic rats, and the increased expression of ICAM-1 in asthmatic rats was caused by the increase of expression of ICAM-1 mRNA.

Lower leg ischemia associated with aortic dissection.

Lower leg ischemia associated with aortic dissection is a potentially life-threatening condition requiring immediate treatment. To better understand the diagnostic factors and improve the treatment strategy of this serious complication, we analyzed our experience regarding the radiographic findings, treatment, and outcome in eight patients (aged 28-72 years, six men and two women). CT revealed type A aortic dissection in seven patients and type B in one. The obstructed site was in the iliac artery in five patients and in the abdominal aorta below the renal arteries in three. Surgical procedures included five ascending aortic graft replacements, three femoro-femoral bypasses, and one each of surgical fenestration, aorto-iliac bypass, and axillo-femoral bypass with thrombectomy. Endovascular treatment was performed in two patients, iliac stent placement in one, and thrombolysis of the iliac artery in one. Five patients survived and three died due to myonephrotic metabolic syndrome in two and postoperative bleeding in one. Treatment strategy depends on several issues regarding aortic dissection including ascending aortic involvement, patent false lumen, entry site, renal artery involvement, and thrombosis in a true or false lumen. CT and angiography are the most important methods for deciding upon appropriate therapy in each individual.
