The treatment of fractures and osteotomies of the mandible and the midface show very different problems. The mandible must be treated in a stable and the midface in a static way. Due to the different bio-mechanics and topography of these bony structures, the advantages and disadvantages of treating a fracture in the maxillo-facial-region by the means of osteosynthesis will be discussed.

The application of traction strapping with a single cortical screw in the form of lag screw osteosynthesis has been suggested for the treatment of angular factures of the mandible. This is also possible in those cases where there are other mandibular fractures, whether the molar teeth are present or not. The original occlusion is thereby easily reconstructed. The rehabilitation of the patient is generally possible 8 days postoperatively when the intra-oral stitches are removed.

A standardized reproducible technique is reported for inducing traumatic edema in the rat paw showing considerable swelling volume and low tissue damage. The edema is measured volumetrically subsequent to squashing the rat paw under standardized conditions in a wringing-machine with two wooden rollers. The technique is compared with other methods.
This study reports on the first time three-dimensional presentation of odontogenic cysts demonstrated by means of a simple histological method. Histological sections of biopsy material having been embedded in paraffin were projected on cardboard. After the projections had been cut out, the single parts were arranged in a way to form a three-dimensional model. Radicular cysts are monolocular and exhibit a smooth surface. The individual cyst is surrounded unilaterally by epithelial cell remnants of Malassez opposite to the adhering tooth. The bone shows a reactive-inflammatory transformation. Keratocysts occur in connection with the Gorlin-Goltz-syndrome and reveal a primary multiple structure. Smaller cysts have a round and smooth appearance, whereas the larger ones show an irregular surface. Satellite cysts of various sizes which develop in the adjacent areas are connected with the primary cyst. We are therefore dealing with two different types of cysts: with monolocular cysts which show a complex network and are connected with satellite cysts on the one hand, and, on the other, with additional multiple cast systems including monolocular and multilocular cysts of various sizes. The surrounding bone shows the characteristics of compression atrophy rather than signs of an inflammation.

First experiences concerning reconstructive vascular surgery in malignant tumor surgery are presented. In order to improve total tumor excision, the resection of great veins or nutritive arteries may become necessary. In some cases the reconstruction of vascular defects together with tumor resection may be indicated. Only in the case of younger patients vascular reconstruction of essential vessels seems to be indicated.
The result of the inadvoidable removal of bone in the canine fossa during antral fenestration is a large bone defect. Scar tissue grows across this defect and sometimes obliterates and interferes with the antral physiology. This paper presents our experience with the reconstruction of the defect by means of the osteotomised canine fossa bone with adhering soft tissues. In this way the antral volume remains unchanged and irritation of the infraorbital nerve often caused by the scar tissue of the facial fenestration as well as other pathological sequelae after the Caldwell-Luc operation are avoided.
Over the past few years a number of methods, including the miniplate system for the osteosynthesis of facial fractures have been developed. These and the instruments used are described with examples.
In recent years close consideration of the biomechanical principles of treatment of mandibular fractures has led to the use of operative as well as conservative methods. Since this combined method of treatment appeared cumbersome, a new kind of osteosynthesis plate was developed for functionally stable internal fixation of mandibular fractures, without the necessity of additional fixation elements. After consideration of the biomechanics of the mandible, an even greater indication for the use of the lag-screw osteosynthesis is found. Use of both the tension band plate and the lag screw makes it possible to perform the operation intraorally. This is a further indication for the use of the operative internal fixation of fractures in the treatment of mandibular fractures, since it has many advantages for the patient.
