[Recurrences after surgical treatment of achalasia of the cardia (author's transl)].
We observed 17 cases of so-called recurrences of achalasia of the cardia (10 times after preceding dilatation and 7 times after Heller's operation). For surgical treatment 16 of these patients underwent cardiomyotomy, using the transthoracic approach. In one patient who had cardiomyotomy plus fundoplication at the first operation an oesophago-gastrostomy became necessary. Three quarter of these patients had a satisfactory result when followed up. I case of recurrence the authors recommend to give priority to cardiomyotomy, rather than to dilatation or bougienage.
[Contact-free temperature measurement in the heart by means of infrared telepyrometry].
The actual temperature of the myocardium requires the surgeon for the assessment of the ischemia time under hypothermic conditions in operations on the heart. The problem of the contact free temperature measurement on heart was solved by means of the infra-red telepyrometer. Preponderant technical aspects of the application in the cardiosurgery were described. On the basis of the physics the function of an infra-red contactlesspyrometer was discussed and the first results of own investigations about the contact free measurement of the epicardial temperature.
[The value of the symptom of dysphagia from the surgeon's viewpoint].
Dysphagia is the leading symptom in the most frequent and surgically most important disturbances of the passage of the oesophagus. Possibilities and results of the treatment are analysed on the basis of clinical material from the surgical clinic of Halle university. Nevertheless insufficient results in malignant tumours are opposite to favourable results of the operation in benign diseases (diverticles, achalasia, strictures). Measures for the improvement of the situation of treatment are discussed.

[Studies on blood traumatization following heart-valve prosthesis].
In 47 patients blood traumatisation was checked after interventions on cardiac valves. The cases in question were: 34 implantations of valve prostheses, 10 valve reconstructions and 3 re-operations because of paravalvular leakages. The tested valve prostheses according to Starr-Edwards, Björk-Shiley and Beall revealed subclinical haemolyses of various degrees. In aortic position the values for Björk-Shirley prostheses showed advantages as compared to the Starr-Edwards valve. Of all tested valve types the Beall prostheses caused the highest blood traumatisation. Manifest haemolytic anaemia was found only in 3 cases, two of them showing paravalvular leakage. In the cases of double valve replacement performed with combined Starr-Edwards and Beall prostheses the haemolysis was only slightly higher than in cases of mitral valve replacement with Beall valves only. In three cases with paravalvular leakage the parameters of haemolysis increased very much, returning to normal in 2 patients after successful correction. The assessment of the parameters of blood traumatisation is indispensible for the choice and improvement of the different types of valves and essential for the diagnosis of paravalvular leakage as well as for the control after leakage repair.
[Blood traumatization after heart valve replacement (author's transl)].
Blood traumatization after heart valve reconstruction or replacement was checked in 47 patients. Among the types of prostheses tested the Beall-valve caused the highest subclinical hemolysis, the Björk-Shiley-valve the lowest. In the aortic position the values for the Björk-valve were better than those for the Starr-valve. In cases with double valve replacement (Starr in aortic, Beall in mitral position) hemolysis was not much higher than in cases with single Beall mitral valve replacement. Clinical hemolytic anemia was only observed in 3 patients, 2 of them being affected with paravalvular leakage. In cases of paravalvular leakage parameters of hemolysis was very high and became normalized after successful repair in 2 patients.
[Bleeding complications after lung surgery (author's transl)].
Bleeding complications after lung resection are mostly due to surgical causes, seldom to coagulation disorders. Own experiences in 8 cases (= 0,59% of all lung operations) are reported. The early rethoracotomy prevents additional coagulation disorders which may arise following large infusions of plasmaexpanders and blood. An analytical program for quick detection of coagulation disorders is given. Treatment of consumption coagulopathy and hyperfibrinolysis is discussed.
[Experimental coronary stenoses and occlusions].
A constrictor for experimentally inducing stenoses and obstructions of the coronary artery is described. Smallness, good adaptability to every vascular diameter, atraumatic insertion, and slow swelling are its advantages. Within 12 months a coronary artery could be constricted by two thirds. In one third of the cases the lumen was narrowed by more than 75 per cent. Just 50 per cent of the cases showed chronic infarction after coronary obstruction, the other half developed differently marked myocardiac fibrosis. At high-degree narrowing of the arteries, 75 per cent of the cases showed disseminated myocardiac fibrosis. The occlusion of the vessel is caused by constrictor, by fibroplastic alteration of the vascular wall, and by thrombosis. Different degrees of myocardiac ischemia are sequelae of different development of collaterals. The constrictor may be used for studies on the development of collaterals as well as on therapeutic measures in chronic ischemia of the myocardium.

[Dye-marking and hydrogen clearance in demonstrating acute ischemic myocardial areas].
After acute myocardial infarction (produced by ligation in the dog), the authors marked the borders of infarction with disulphine blue or methylene blue and determined, by means of hydrogen clearance, the blood supply in the centre and at the margin of infarction as well as at different distances from the latter. At the margin of infarction, the value for blood flow amounted to almost 17% of the value measured prior to ligation. Only at a distance of 1 cm. from the visible margin of infarction, 50 to 80% of the initial value were obtained. The extension of the area of infarction beyond the marked visible margin must be considered in surgical interventions.

[Experiences from early detection and surgical treatment of mediastinal neoplasms (author's transl)].
87 mediastinal tumors were operated at the Halle Universitätsklinik between 1951-1973. 42.5% were diagnosed occasionally by the yearly X-ray mass screening. 47.5% were benign, 34.4% malignant. The lower proportion of malignancies is attributed to the rapid growth of the tumor and the long interval between the screenings (1 year). Results, localizations and treatment are reported.
