The whole gene deletion CYP2A6*4, the defect of the main nicotine oxidase, contributes to limiting lifelong and daily cigarette consumption. However, the effects on smoking habits of CYP2A6*7 and *9, two major functional polymorphisms common in Asian populations, have not been reported. The present study examined the relationship between polymorphisms *4, *7 and *9 with the smoking habits of 200 Japanese smokers who visited the Keio University Hospital (Tokyo, Japan). The allele frequencies of *1 (wild type), *4, *7 and *9 were 52, 17, 11 and 20%, respectively. When the three polymorphisms were considered simultaneously, the percentages of homozygous wild type, heterozygote, and homozygous mutants and compound heterozygotes were 26.0, 52.5 and 21.5%, respectively. Homozygous mutants and compound heterozygotes (n = 43) smoked fewer cigarettes daily than heterozygotes (n = 105) and homozygous wild-type individuals (n = 52). Smokers with *7/*7, *9/*9 or *7/*9 had lower daily cigarette consumption than smokers with *1/*1. In conclusion, polymorphisms *4, *7 and *9 of CYP2A6 were detected in approximately three out of four Japanese smokers, and their daily cigarette consumption was genetically modulated by these functional polymorphisms.
A 32-year-old female patient with systemic lupus erythematosus was admitted to our hospital with fever and cytopenia, and diagnosed as haemophagocytic syndrome (HPS) by bone marrow aspiration study showing haemophagocytosis. Since the serologic activity of lupus was not increased at that time and HPS was refractory to the conventional therapies, an additional aetiological factor was suspected. Real-time PCR analysis identified reactivation of Epstein-Barr virus (EBV). A combination therapy targetting EBV-associated HPS, consisting of intravenous administration of cyclosporine A as well as immunoglobulin with a high titre of anti-EBV antibody, significantly suppressed EBV viraemia and led to the remission of HPS until the time of writing.
We studied 21 patients with a spontaneous palsy of the anterior interosseous nerve. There were 11 men and 10 women with a mean age at onset of 39 years (17 to 65). Pain around the elbow or another region (forearm, shoulder, upper arm, systemic arthralgia) was present in 17 patients and typically lasted for two to three weeks. It had settled within six weeks in every case. In ten cases the palsy developed as the pain settled. A complete palsy of flexor pollicis longus and flexor digitorum profundus to the index finger was seen in 13 cases and an isolated palsy of flexor pollicis longus in five. All patients were treated without operation. The mean time to initial muscle contraction was nine months (2 to 18) in palsy of the flexor digitorum profundus to the index finger, and ten months (1 to 24) for a complete palsy of flexor pollicis longus. An improvement in muscle strength to British Medical Research Council grade 4 or better was seen in all 15 patients with a complete palsy of the flexor digitorum profundus and in 16 of 18 with a complete palsy of flexor pollicis longus. There was no significant correlation between the duration of pain and either the time to initial muscle contraction or final muscle strength. Prolonged pain was not always associated with a poor outcome but the age of the patient when the palsy developed was strongly correlated. Recovery occurred within 12 months in patients under the age of 40 years who achieved a final British Medical Research Council grade of 4 or better. Surgical decompression does not appear to be indicated for young patients with this condition.
BACKGROUND AND PURPOSE: We evaluated the effects of varying tube voltage, current per rotation, and section thickness on detectability of 2- and 4-Hounsfield unit (HU) differences on brain CT between normal and ischemic gray matter within 6 hours of ischemia onset, by using a low-contrast phantom. METHODS: The phantom with an attenuation of 36 HU corresponding to normal gray matter contained 2 sets of spheres (34 HU and 32 HU) corresponding to the early CT signs of ischemic brain and complete infarction, respectively. The reproducibility of the CT numbers and the contrast-to-noise ratio (CNR), defined as the CT number difference between the background (36 HU) and the spheres (34 HU or 32 HU) divided by the SD of the background CT number were measured. Five radiologists rated the phantom images for detection of the low-contrast spheres by visual inspection. RESULTS: The CT numbers were reproducible within 1 HU with a tube current of > or =150 mAs at 120 kVp. The CNRs for the 34- and 32-HU spheres were positively correlated with the tube voltage, tube current per rotation, and the section thickness. A CNR of 1.0 was obtained for the 34-HU sphere when scanning was conducted with a section thickness of 10 mm at 120 kVp and 700 mAs, or 135kVp and 450 mAs, respectively. A significant improvement of the accuracy of detection was found with increasing tube current, tube voltage per rotation, and section thickness. CONCLUSION: Our study indicated that the 2-HU hypoattenuation corresponding to the early CT sign of acute ischemic stroke can be detected by using appropriate parameter settings.
OBJECTIVE: To examine patterns of high resolution computed tomography (HRCT) of lungs of adults with disseminated tuberculosis (TB). DESIGN: Disseminated TB was defined as radiological involvement of all lung lobes. RESULTS: The case series illustrated wide variation in HRCT of disseminated TB. Patterns identified on HRCT included (1) miliary TB (haematogenous dissemination), (2) miliary TB with exudative reaction, (3) bronchogenic spread, (4) miliary TB mixed with bronchogenic spread, and (5) bronchogenic spread with multiple cavity formation. CONCLUSION: The HRCT patterns described allow classification of disseminated TB according to the mechanism of spread (haematogenous and/or bronchogenic) and the degree of local lung involvement (reaction or cavitation).
BACKGROUND AND PURPOSE: Cellulose porous beads (CPBs) are a new, exceptionally uniformly sized, nonabsorbable embolic agent. We evaluated their efficacy in the preoperative embolization of meningiomas. METHODS: In 141 consecutive patients, we used CPBs (200-microm diameter) for the preoperative embolization of meningiomas. We selected patients whose tumors were > or =4 cm with 50% of blood to the tumor supplied by the external carotid artery (ECA). All patients underwent a provocation test before embolization. The percentage of blood supplied to the tumor by the internal carotid artery and ECA was determined angiographically. Nonenhanced areas on postembolization MR imaging were calculated. Intraoperative blood loss, units of blood transfusion, and hemostasis at the time of surgery were recorded for each patient. The interval between embolization and surgery was intentionally longer than 7 days. RESULTS: Of the 141 patients, 128 underwent CBP embolization. Eleven patients had positive provocation test results, and 2 had vasospasm; they were not CBP embolized. In 72% of the patients CBP embolization achieved reduction in the flow of the feeding artery by more than 50%. The nonenhanced area on MR imaging was not significantly correlated with the degree of ECA supply or devascularization. The interval between embolization and surgery was 8-26 days (mean, 9.9 days). The longer this interval, the greater was the tumor-softening effect and the rate of tumor removal. CONCLUSIONS: CPBs may be useful for the preoperative embolization of meningiomas. To increase the efficacy of CPB embolization, the interval to surgery should be at least 7 days.
A 78-year-old man with old tuberculosis was pointed out the high CEA (347 ng/ml) and a mass shadow in right side of middle mediastinum. Examination by bronchoscopy showed no abnormal finding, cytological diagnosis of sputum was negative. In computed tomography (CT), there was tumor-like shadow in the right upper lobe and mass shadow in the right side of middle mediastinum, and we suspected of lung cancer. We performed a wedge resection in the right upper lobe with regional lymph node dissection. In the histological diagnosis, there were no malignant finding in the right lung, and mediastinal lymph nodes showed poorly differentiated adenocarcinoma. In immunohistochemistry staining, CEA was positive, surfactant apoprotein and thyroid transcription factor-1 (TTF 1) were negative. Postoperative course was uneventful and 50 Gy radiotherapy were given. CEA was decreased to 2.1 ng/ml 6 months after the operation. This case was highly suspected to be a rare case of T0N2M0 lung cancer.
BACKGROUND AND PURPOSE: On diffusion-weighted imaging (DWI), metastatic tumors of the brain may exhibit different signal intensities (SI) depending on their histology and cellularity. The purpose of our study was to verify the hypotheses (1) that SI on DWI predict the histology of metastases and (2) that apparent diffusion coefficient (ADC) values reflect tumor cellularity. MATERIALS AND METHODS: We assessed conventional MR images, DWI, and ADC maps of 26 metastatic brain lesions from 26 patients, 13 of whom underwent surgery after the MR examination. Two radiologists performed qualitative assessment by consensus of the SI on DWI in areas corresponding to their enhancing portions. We measured the contrast-to-noise ratio (CNR) on T2-weighted images and normalized ADC (nADC) values, and compared them with tumor cellularity. RESULTS: The mean SI on DWI and the CNR on T2-weighted images were significantly lower in well differentiated than in poorly differentiated adenocarcinomas and lesions other than adenocarcinoma. The mean nADC value was significantly higher in well differentiated than poorly differentiated adenocarcinomas and lesions other than adenocarcinoma. All 3 small-cell carcinomas and 1 large-cell neuroendocrine carcinoma exhibited high SI on DWI. The nADC value showed a significant inverse correlation with tumor cellularity. There was no significant correlation between the CNR and tumor cellularity. CONCLUSION: The SI on DWI may predict the histology of metastases; well differentiated adenocarcinomas tended to be hypointense, and small- and large-cell neuroendocrine carcinomas showed hyperintensity. Their ADC values reflect tumor cellularity.
BACKGROUND AND PURPOSE: Volume flow rates in the feeding arteries of the brain are measured to evaluate blood flow dynamics in vascular disease. Although these flow values are thought to be effected by anatomic variations in the circle of Willis, few reports have described the effect. This study reports on the relationship between variations in the circle of Willis and volume flow rates in the bilateral internal carotid and basilar arteries of normal volunteers. METHODS: We prospectively examined 125 healthy volunteers by MR imaging. Variations in the circle of Willis were classified as "textbook" type, hypoplasia of the precommunicating segment of the anterior cerebral artery (A1), hypoplasia of the precommunicating segment of the posterior cerebral artery (P1), or "other." Volume flow rates were measured by 2D cine phase-contrast MR imaging. Lumen boundaries and volume flow rates were semiautomatically determined by pulsatility-based segmentation. RESULTS: Of the 117 subjects (61 men, 56 women; mean age, 23.6 years) considered suitable for flow measurement, 105 showed textbook type, and 6 each showed A1 hypoplasia and P1 hypoplasia. Total flow rates for the 3 variations were 781 +/- 151 mL/min (mean +/- SD), 744 +/- 119, and 763 +/- 129, respectively. Relative contributions by flow rates of the internal carotid arteries and the basilar artery for the 3 variations were 39.8%:38.9%:21.3%, 31.8%:49.1%:19.0%, and 46.6%:41.6%:11.7%, respectively, showing statistically significant differences. CONCLUSIONS: Variations in the circle of Willis correlate significantly with relative contributions by the flow rates of the bilateral internal carotid and basilar arteries.
We report a case of a 64-year-old woman with an anterior mediastinal tumor on a chest computed tomography (CT) before operation of uterine cancer. After the radical surgery and a chemotherapy for uterine cancer, surgical resection of mediastinal tumor was performed in July 2005 because of gradually progression in tumor size. Under the median sternotomy, the tumor revealed intensive adhesion to the right lung and the left brachiocephalic vein. We performed a complete resection of the tumor with combined resection of adherent parts of them. The tumor was 9 x 8 cm in size, containing with yellow cream-like fluid and hair. Histologically, the tumor was diagnosed as a mature teratoma with the tissue of bone, digestive tract epithelium, bronchial epithelium and so on. In benign teratoma, it is not rare to perforate to the adjacent structures. So, we concluded if the mediastinal teratoma was suspected, long-term observation had the risk for adhesion and perforation to adjacent tissue, and in case of operation, we should keep in mind the possibility of combined resection of involved organs.
BACKGROUND: It is thought that overexpression of epidermal growth factor receptor (EGFR) in non-small cell lung cancer (NSCLC) might compromise patient survival, presumably by promoting tumour growth by an autocrine mechanism. However, conflicting results have been reported from various laboratories, and the clinical importance of EGFR overexpression remains unsettled. METHODS: A meta-analysis of previous studies was performed to quantitatively review the effects of EGFR overexpression on survival in patients with NSCLC using a DerSimonian-Laird random effects model. Eighteen studies including 2972 patients were subjected to final analysis. RESULTS: Overall, positivity for EGFR overexpression differed between histological types: 39% in adenocarcinomas, 58% in squamous cell carcinomas, 38% in large cell carcinomas, and 32% in cancers in a miscellaneous category (p<0.0001). The combined hazard ratio (HR) was 1.14 (95% CI 0.97 to 1.34; p = 0.103), indicating that EGFR overexpression has no significant impact on survival. When only the 15 immunohistochemistry based studies were considered, the combined HR was 1.08 (95% CI 0.92 to 1.28; p = 0.356), again suggesting that EGFR overexpression has no impact on survival. Heterogeneity testing indicated that there was heterogeneity between studies but publication bias was absent, which suggests that the summary statistics obtained may approximate the actual average. CONCLUSIONS: EGFR overexpression was not associated with poorer survival in patients with NSCLC. Specific mutations of the EGFR gene will need further study in terms of survival implications.
The previously developed instanton theory [G. V. Mil'nikov and H. Nakamura, J. Chem. Phys. 122, 124311 (2005)] is applied to the calculation of vibrationally assisted tunneling splitting of the deuterated formic acid dimer (DCOOH)2 with all the degrees of freedom taken into account. The ground-state tunnel splitting is determined by the density-functional theory combined with coupled cluster level of quantum chemistry to be 0.0038 cm(-1) which is comparable to the experimental value of 0.0029 cm(-1). Further, the tunnel splittings of fundamental excitations are estimated for frequencies below 300 cm(-1). In this energy range it is found that the excitation modes may either enhance or suppress tunneling as compared to the ground state. For the higher-frequency modes a rapid growth of the tunnel splitting is observed. At frequencies above 1000 cm(-1) the semiclassical solution becomes unstable and no reliable tunneling splittings can be obtained. This is in vast contrast to the adiabatic approximation to the instanton theory in which the tunnel splittings can be retrieved up to 3000 cm(-1). We discuss this disparity from the viewpoint of the multidimensional character of tunneling in hydrogen bonds and the adiabatic approximation is concluded to be inaccurate.
We present visualization and measurement of the convection of water under a high magnetic field applied vertically to the fluid. The convection was either suppressed or enhanced depending on the direction of the magnetic force. The magnetic field effect was evaluated quantitatively by measuring the onset of convection, and discussed in terms of the Rayleigh number which includes the magnetic term. The results clearly show that the convection in a diamagnetic fluid such as water can be controlled using a common 10 T class magnet.
Extent of resection needed to treat lung cancer has long been an issue. The sole randomised controlled trial, reported by the Lung Cancer Study Group, advised against limited resection as standard surgery even for small peripheral non-small-cell lung cancers (< or =3 cm), because of frequent local recurrences. Elsewhere, conflicting results have been reported from different institutions. We therefore conducted a meta-analysis of reported studies to compare survival of stage I patients between limited resection and standard lobectomy. A MEDLINE web search for computer-archived bibliographic data yielded 14 articles suitable for analysis. Combined survival differences (survival rate with lobectomy minus that with limited resection) at 1, 3, and 5 years after resection according to the DerSimonian-Laird random effects model were 0.7% (95% CI, -0.8 to 2.1; P=0.3659), 1.9% (95% CI, -3.7 to 7.4; P=0.5088), and 3.6% (95% CI, -0.4 to 10.5; P=0.3603), respectively. None of these survival differences were significant, indicating that survival after limited resection for stage I lung cancer was comparable to that after lobectomy. However, since interstudy heterogeneity was detected, caution is required in interpretation of the results.
Energetic electrons and protons are observed when a target consisting of a reentrant cone with a disk at the tip is irradiated by a petawatt (PW) laser at an intensity of approximately 10(19) W cm(-2). The angular distribution of the electrons and protons, dependent on the open angle of the reentrant cone, is found to differ from that in the case when a target with planar geometry is used. Two jet beams are observed, in directions parallel to the cone axis and normal to the cone-shaped wall. The number and cutoff energies of the generated protons are also related to the open angle of the cone. The efficiency of the generation of energetic electrons from the cone target is 2-3 times higher than that from a simple plane target. These results indicate a guiding of the PW laser beam in the cone geometry.
Gastrointestinal stromal tumour rarely develops in the duodenal ampulla region. We report here a case of gastrointestinal stromal tumour of the ampulla of Vater found in a 44-year-old Japanese man presenting with biliary obstruction. He died of hepatic failure with diffuse liver metastasis. The postmortem examination showed a large Borrman type III-like tumour in the duodenal ampullary region with direct invasion of the pancreas and extrahepatic bile duct as well as metastases to the liver and regional lymph nodes. The duct orifice was located at the centre of the tumour. Microscopically, the tumour consisted of anaplastic spindle cells with high mitotic activity (90 mitoses per 50 high-power fields). Immunohistochemically, the spindle cells were positive for KIT and CD34. The final diagnosis was high-grade malignant gastrointestinal stromal tumour of the ampulla of Vater. Considering the recent advances in the diagnosis and treatment of gastrointestinal stromal tumour, this neoplasm should be included in the differential diagnosis of the tumours appearing in the duodenal ampulla region.
In order to elucidate mutual interrelationship between neurological and systemic dysfunctions in patients with subarachnoid hemorrhage (SAH) at acute stage, neurological condition, systemic complications and plasma catecholamine (CA) level were studied in 1431 consecutive cases admitted within 72 hours after the onset. Five hundred and twenty-four cases with Glasgow Coma Scale (GCS) score 8 or less were assigned to the group of severely ill cases (G-ill), 907 cases with GCS score 9 or more to that of the less ill group (G-well). Plasma CA level was extremely high at super-acute stage within an hour after bleeding and lowered fairly quickly within 24 hours to the normal range. Assuming the value obtained from a formula of [blood sugar level (mg/dl)/serum potassium concentration (mEq/L)] as stress index (SI), SI correlates well (r = 0.4-0.6) with serum catecholamine level at acute stage. Thus, sympathetic hyperactivity after SAH can be grossly estimated with SI. SI over 40 means that patients might have considerable neurological insults as well as systemic ones. For patients in G-well, SI over 50 means that there may be risks for systemic complications even in cases with good neurological condition.
