Effects of serine protease inhibitors on oxyradical burst from phagocytizing neutrophils--analysis by chemiluminescence counting and its microscopic imaging.

Reaction difference of oxyradical generation and luminol-dependent photoemission of zymosan- and phorbol ester-treated neutrophils were investigated using a conventional photomultiplier and ultrasensitive photonic imaging technique. Zymosan-treated cells released a concentrated photonic burst corresponding to the cellular distribution. In contrast, phorbol ester-treated cells produced a negligible level of photoemission, and the additional application of Ca2+ ionophore enhanced the photonic burst, which was gradually spread out into extracellular space. Serine protease inhibitors did not attenuate PMA-induced chemiluminescence but did attenuate zymosan-induced chemiluminescence. This suggests the involvement of serine protease in the respiratory burst of phagocytizing neutrophils.

Abnormally high values of CA 125 and CA 19-9 in women with benign tumors.

Tumor markers have proved to be generally useful for diagnosing and monitoring malignancy. In contrast, high levels of tumor markers disturb us for differing benign disease from malignancy. We had 5 cases with benign disease who showed high values of tumor markers (CA 125 and CA 19-9). In this report, we illustrated 3 patients with endometriosis who showed high values of CA 125 and 2 patients with dermoid cyst who showed high values of CA 19-9. It was impossible to rule out malignant disease in all cases preoperatively. In diagnosing ovarian tumors, bimanual examination, ultrasound, CT scan and tumor markers should all be considered.

[A study using a food-frequency questionnaire--simultaneous discrimination of dietary patterns of anemic and hyperlipemic women].

A survey investigating the intake frequency of various foods and eating habits was made by a special form of structured questionnaire. Those who answered the questionnaire were 184 women aged from 40 to 59 living in a rural district in Kyoto Prefecture. They were divided into three blood groups according to the contents of hemoglobin (Hb) and serum cholesterol (T-CHO) as follows: Group A (Anemia): Hb less than 12 g/dl and T-CHO less than 220 mg/dl Group B (Normal): Hb greater than or equal to 12 g/dl and T-CHO less than 220 mg/dl Group C (Hyperlipidemia): Hb greater than or equal to 12 g/dl and T-CHO greater than or equal to 220 mg/dl As far as the intake frequency of each food and the eating habits of the three blood groups are concerned, there wasn't any notable difference. However, when the statistical method called "quantification theory II" was applied using the blood groups as the objective variable and the 27 items in the food frequency questionnaire, as well as the obesity index, as explanatory variables, it was found that the dietary patterns were different among the three blood groups. The result was that 68% of women in group A were discriminated correctly, as were 55% in group B, and 83% in group C. The analysis of the data from food frequency questionnaires by the "quantification theory II" statistical methods seems to be useful to reveal dietary patterns.

A histological and experimental study on the fate of an increased number of lymph follicles produced in the mouse popliteal lymph node by exogenous antigen stimulation.

Eight-week-old female C57B1/6 mice were injected with endotoxin LPS and/or other antigens into the left hind footpad, and then the number of lymph follicles in the draining popliteal lymph nodes was examined. In untreated mice each popliteal node contained 10-12 lymph follicles at both 8 and 15 weeks of age. Animals given 50 micrograms of LPS at 8 weeks of age showed an increase in the number of lymph follicles 3 weeks later, but this number returned to normal levels by 15 weeks after the LPS injection. After a 2-micrograms-LPS injection at 23 weeks of age, the number of lymph follicles in the draining lymph node was unchanged, but that in animals given the 2-micrograms-injection 15 weeks after the 50-micrograms-LPS injection was significantly increased. In animals receiving 2 Lf of diphtheria toxoid, instead of the 2-micrograms-LPS, at 15 weeks after a 50-micrograms-LPS injection, the number of lymph follicles per draining node was within the normal range. In one group of mice, the initial injection of 50-micrograms-LPS at 8 weeks of age was followed by injections every third week of several kinds of antigens which had been shown to be ineffective in inducing follicle formation. Here, the number of lymph follicles in the draining popliteal node was kept to significantly increased levels at 25 weeks of age. The present results suggest that, while most lymph follicles normally developing in the lymph node are maintained for a long time under normal conditions, many lymph follicles induced by antigenic challenge have a limited life span and undergo atrophy unless they are periodically activated by additional antigenic stimuli, and that atrophied follicles finally become unable to respond to antigenic stimulation. It is also suggested that antigenic materials which trigger the formation of lymph follicles in the primary challenge can evoke follicle formation more efficiently in the secondary challenge.

[High-voltage electron microscopic observations on mouse olfactory bulb neurons].

The fine structures of Golgi-impregnated mitral, tufted and granule cells in the mouse olfactory bulb were investigated by a high-voltage electron microscope. Observation was focused exclusively on intraglomerular dendritic trees of mitral and tufted cells as well as granule cell spines. These structures could be precisely observed under high-voltage electron microscopy in contrast to the vague observation available by light microscopy. Intraglomerular dendritic tufts of mitral and tufted cells possess a similar structure. Their most characteristic feature is varicose branches, which are morphologically classified into two parts, relatively thick parts and thinner parts. The latter consist of bead-like varicosities and thin segments less than 0.35 microns, combination of which take on a thread and beads appearance. Another common structure is small, spine-like appendages which can be seen on all parts of the tufts. Granule cell spines can be individually observed with the aid of stereoscopy, even where they are closely clustered. As a result, many more spines could be recognized by high-voltage electron microscopy than by light microscopy. The width of terminal swellings of spines in the granule cell layer are, in most spines, shorter than 0.4 microns. On the other hand, in the mitral body and external plexiform layer, sizes of spines are much more variable; larger ones with a width of between 1.0 and 1.5 microns as well as smaller ones between 0.4 and 0.5 microns are usually found. Consequently, these findings described above proved to be useful for attaining a deeper understanding of both light and thin section electron microscopic findings of these structures.

[Clinical evaluation of serum basic fetoprotein--comparative study of serum immunosuppressive acid protein].

The clinical significance of serum basic fetoprotein (BFP) in renal cell carcinoma was investigated in comparison with serum immunosuppressive acid protein (IAP). Investigated in this study were 46 patients with renal cell carcinoma, 31 males and 15 females ranging in age from 40 to 85 years (mean age: 56 years). According to Robson's system of staging, 10 cases (22%) had a stage I disease, 14 cases (30%) a stage II disease, and 18 (39%) a stage IV disease. The positivity rate for and levels of serum BFP were found higher as disease stage advanced, with 54% of the entire cases and 72% of stage IV cases being positive for serum BFP. On the other hand, 76% of 34 cases for whom assay of serum IAP was feasible and 79% of stage IV cases were positive for serum IAP, thus here again the positivity rate for and serum levels of the marker became increasingly higher with advancing disease stage. Simultaneous assay of serum BFP and IAP demonstrated that 85% of stage IV renal cell carcinoma cases were positive for at least one of the two markers. A study of changes in these tumor markers before and after curative resection of the tumor showed that BFP decreased below the preoperative level within 1 week postoperatively in 92% and even returned to normal in 75% of cases who were positive for the tumor markers preoperatively, while serum IAP reduced below the preoperative level in 82% and returned to normal in 27% of the cases preoperatively.

Correctional effect of Cotrel-Dubousset instrumentation for idiopathic scoliosis. An early follow-up study.

We reviewed the results of posterior spinal fusion with Cotrel-Dubousset Instrumentation for idiopathic scoliosis from fifty-six patients (sixty-five curves). The postoperative follow-up on them ranged six months to three years and seven months with an average of one year and nine months. From overall results, scoliotic curves were corrected by 36.5 from 60.3 degrees before operation to 23.8 degrees after operation on average. The rate of correction was 60.5%. On a sagittal plane, thoracic kyphosis improved from 22 to 35 degrees in the Cobb angle of T1-T12, to be more physiologically aligned. In addition, the derotational effect was elucidated by the measurement on pre- and post-operative CAT scan images in the apical vertebra, and on moiré topography. A reduction of the rib hump and the trunk inclination was also noted, but did not necessarily reflect the correctional gain of the scoliotic curvature. From the biomechanical point of view, the correction of rotational deformity by CD instrumentation is derived from a couple of the following movements: one is rotation of the apical vertebra around its axis and the other is revolutional displacement of that vertebra on a transverse plane (R-R movement).

[Retention in the conic telescope crown system. The influence of the inner crown form].

In a study of retention of the conic telescope crown system, the influence of the morphological condition of the inner crown on retention was examined. The usefulness of a milling machine with a polishing disk, a newly developed polishing system, was also evaluated. The results were as follows. 1) In polishing the axial surface of the inner crown of the conic telescope crown system, the milling machine with a polishing disk facilitated specular finishing without causing undercutting in the region from the occlusal surface to the dental cervix. 2) In producing the outer crown of the conic telescope crown system using Deguvest HFG, a phosphate-investing material, the retentive force of the crown tended to increase as the concentration of the solution for specific use was decreased to 50%, 30% and 10%, and reached appropriate levels of 500-1,000 g with the 50% solution. 3) When the height of the inner crown was changed to 2 mm, 4 mm and 6 mm, no significant difference was found in the retentive force, the mean values being 516 g, 836 g and 1,003 g under loading of 10 kg, 20 kg and 30 kg, respectively. 4) The retentive force tended to increase with increased width of the dental cervix of the inner crown: it was 745 g, 1,067 g and 1,376 g at 6 mm, 8 mm and 10 mm of cervical diameter, respectively, under a load of 30 kg. 5) The retentive force was significantly changed with varying loads (99% reliability), and tended to increase with increased load. 6) There was a highly significant correlation between retentive force and sinking of the inner crown. 7) When a tensile load was placed 3 mm lateral to the central axis of a test piece, the retentive force tended to increase when the vertical diameter of the inner crown cervix was larger than the horizontal diameter, while it decreased when the vertical diameter was shorter than the horizontal diameter, as compared with central axis loading.

[Analysis of platelet surface conformation in thrombin-induced aggregation].

We used flow cytometry to investigate the change of platelet membrane glycoproteins (GPIb and GP IIb/IIIa) and the distributions of fibrinogen (Fbg), thrombospondin (TSP) and fibronectin (Fn) on the surface of thrombin-stimulated platelets. The binding of a monoclonal antibody directed at the von Willebrand factor binding site on GPIb decreased in thrombin-stimulated platelets. This antibody caused a reactive delay in thrombin-induced aggregation, but had little influence on aggregability. Slight thrombin-induced aggregation was observed even after blocking the binding of Fbg to GP II b/IIIa. The new expression of GP II b/IIIa was detected on the surface of thrombin-stimulated platelets, whereas there was little increase of Fbg dependent on this GP II b/IIIa. An increase of TSP after thrombin stimulation was observed on the surface of platelets of healthy controls and patients with Glanzmann's thrombasthenia (Type I). The level of on platelet surface was slightly increased by thrombin stimulation. The mechanism involved in thrombin-induced aggregation appears to differ from that in ADP-induced aggregation.

[Effect of blockage of the afferent lymphatic vessels on the development of popliteal lymph nodes in the rat].

The effect of blockage of the afferent lymphatic vessels on the development of popliteal lymph nodes in the rat was studied. The afferent lymphatic vessels to each popliteal node were surgically interrupted at the lowest edge of the popliteal fossa at 3, 7 or 28 days after birth and the popliteal nodes were obtained from treated animals at 4, 8 or 16 weeks after the operation. At 7 days after birth, each popliteal node was small and weighed 0.2 mg. Its parenchyma consisted of reticular cells and a small number of dispersed lymphoid elements. Four weeks after birth, each node weighed 4-5 mg, and its parenchyma comprised two layers, an outer continuous layer of peripheral cortex containing 40-50 lymph follicles and an inner discontinuous layer of deep cortex made up of 4-5 deep cortical units. At 10-12 weeks after birth, each node weighed about 10 mg and showed full structural development; the peripheral cortex contained 100-130 lymph follicles and the deep cortex consisted of 4-6 well developed units. The popliteal node was drained by 4-6 afferent lymphatic vessels, which opened into the subcapsular sinus of the node. Each lymphatic opening was topographically associated with a respective deep cortical unit, as previously described by Bélisle and Sainte-Marie (1982). In animals treated at 3 or 7 days after birth, the development of the popliteal nodes was considerably inhibited. Four weeks after surgery, each node showed 10-30 lymph follicles in the peripheral cortex and 1-3 small units in the deep cortex. Sixteen weeks after surgery, the node weighed about 4 mg and its cortex exhibited about 50 lymph follicles in the peripheral cortex and only 2 units in the deep cortex. The popliteal nodes of the treated animals generally received 2 afferent lymphatic vessels. In animals treated at 4 weeks after birth, the popliteal nodes showed no gain in weight for following 16 weeks. Four weeks after surgery, each node usually had 4-5 deep cortical units and 50-60 lymph follicles. Thereafter, some units and their overlaying peripheral cortex underwent atrophy, while others persisted. Sixteen weeks after surgery, the popliteal node showed only 2 deep cortical units and 50-60 lymph follicles, and was drained by 2 afferent lymphatic vessels. Surgical interruption of the afferent lymphatic vessels to the popliteal node at the lowest edge of the popliteal fossa did not obliterate all the draining lymphatic vessels , but reduced the number of vessels opening into the node.(ABSTRACT TRUNCATED AT 400 WORDS)

[Thrombolytic therapy of pulmonary thromboembolism--result of 18 patients].

In Japan, pulmonary thromboembolism (PTE) is rare and the therapy of this disease, especially the thrombolytic therapy, has not been established yet. We studied the effect of thrombolytic therapy by urokinase (UK) in eighteen patients with PTE. We also compared the results with those in Europe and the United States. Eighteen patients (male 7, female 11) were 27 yo to 77 yo (mean 55 yo). Seven cases were mild and 11 cases were moderate to severe. The initial dosage of UK administration was 2.4 x 10(5) to 7.2 x 10(5) IU with the mean of 4.1 x 10(5) IU, which was approximately the double of those in Europe and the US. The duration of UK therapy was 1 to 9 days with the mean of 4.8 days. It was longer than in Europe and the US (12-24 hours). Total amount of UK in each case ranged 4.8 x 10(5) to 42 x 10(5) IU (mean 19.3 x 10(5) IU) with the daily dosage of 4.0 x 10(5) IU in average. They were almost half of those in Europe and the US. Clinical signs and laboratory findings subsided in 1-7 day by UK administration in 14 cases (87%) with the reduction of defects on perfusion lung scans and all of these cases survived. No severe complications were found in any cases. We conclude that the thrombolytic therapy with UK in Japan might be acceptable from the results of this present study. However, the prospective study of thrombolytic therapy on a larger scale remains to be done in the future.

[Analysis of anti-platelet antibody and platelet volume in idiopathic thrombocytopenic purpura].

We investigated platelets and plasma from patients with idiopathic thrombocytopenic purpura (ITP) to elucidate the antigenic determinants at which their autoantibodies are directed, and studied the relationship between anti-platelet antibody and platelet volume. We used flow cytometry to detect platelet-associated IgG (PAIgG), C3 (PAC3), IgM (PAIgM) and platelet volume, and also to determine the binding rate of monoclonal anti-platelet antibodies in patients with ITP. The following results were obtained. 1. Both anti-GPIIb/IIIa autoantibodies (21 of 71 patients) and anti-GPIb autoantibodies (3 of 71 patients) were found in ITP. 2. The decrease in platelet count in patients without anti-GPIIb/IIIa autoantibodies was significant. 3. The increase in platelet volume was found more frequently in patients with a platelet count less than 50,000 and in untreated patients. 4. There was a positive correlation between the platelet volume and PAIgM in patients with a platelet count less than 30,000 and high levels of PAIgM.

High voltage electron microscopic studies on mitral, tufted, and granule cells in the mouse olfactory bulb.

High-voltage electron microscopic observations have been performed on intraglomerular dendritic branches of mitral and tufted cells and spines of granule cells in the Golgi-impregnated mouse olfactory bulb. The observations revealed that intraglomerular tufts of mitral and tufted cells have a very similar structure consisting of four morphologically distinct parts: (i) thick, smoothly outlined proximal parts; (ii) relatively thick varicose parts; (iii) very thin varicose parts, which form terminal branches; and (iv) spine-like small appendages. Granule cell spines in the external plexiform and mitral body layer have terminal swellings of various shapes and sizes, whose width is between 0.4 and 1.5 microns. Spines in the granule cell layer usually have swellings smaller than 0.4 micron in width. In addition, the density of spines per proximal 10 microns length of a deep dendritic trunk was estimated in 10 granule cells and proved to vary greatly from cell to cell--from 7 to 28 spines.

CA-125 is an effective marker for patients with external endometriosis and on danazol: case reports.

Serum levels of CA-125 in women with external endometriosis and on danazol were assayed for an extended period, and changes in the CA-125 pattern were analyzed. Whereas the prognosis for the patients in case 1 was good, those for cases 2, 3, and 4 were poor. We propose that the Ca-125 levels should be carefully determined for individual patients, because a relapse can be predicted.
