Successful removal of meningioma of the pineal region after embolization.

A 69-year-old female presented with a meningioma of the pineal region manifesting as gait disturbance and mental dysfunction. Magnetic resonance imaging revealed a homogeneously well-enhanced circumscribed round mass of about 5 cm in diameter in the pineal region. Angiography demonstrated that the tumor was fed mainly by the bilateral middle meningeal arteries (MMAs), and preoperative intravascular embolization was performed through the bilateral MMAs using estrogen-alcohol and polyvinyl acetate. The tumor was very soft and easily totally resected via the right occipital transtentorial approach. Preoperative embolization is a very useful technique to facilitate removal of deep-seated tumors.

Occlusive cerebrovasculopathy after internal radiation and bleomycin therapy for craniopharyngioma--case report.

A 34-year-old female presented with occlusive cerebrovasculopathy without definite ischemic symptoms and regrowth of a cystic tumor in the third ventricle. She had been treated for a craniopharyngioma 19 years previously by internal irradiation with 198Au colloid combined with intracavitary administration of bleomycin via an Ommaya reservoir. Cerebral angiography demonstrated complete occlusion of the clinoid portion of the left internal carotid artery and stenosis of the left posterior cerebral artery, and numerous transdural anastomoses which had developed after craniotomy. Endoscopic management achieved collapse of the cystic tumor. No treatment for the occlusive cerebrovasculopathy was necessary because of the collateral blood supply. Careful follow-up examinations to detect occult cerebrovasculopathy after such treatment is recommended.

Correlation between human T cell lymphotropic virus type-1 and Strongyloides stercoralis infections and serum immunoglobulin E responses in residents of Okinawa, Japan.

To clarify the relationship between Strongyloides stercoralis, infection with human T cell lymphotropic virus type-1 (HTLV-1), and serum immunoglobulin E (IgE) levels, epidemiologic investigations of these two infections were conducted in inhabitants of Okinawa, a subtropical zone in Japan. Blood and feces samples were taken from 1,347 healthy inhabitants (554 males and 793 females). Antibody to HTLV-1 was measured by particle agglutination, enzyme-linked immunosorbent assay, and Western blotting. The presence of Strongyloides was determined by direct detection of rhabditiform larvae in fresh stool on agar-plate cultures. Serum IgE levels in 127 inhabitants were measured by a fluoroenzyme immunoassay. Antibody to HTLV-1 was detected in 23.0% of the blood samples and was more frequent in females (25.1%) than in males (20.0%) (P < 0.05). Strongyloides were detected in 21.9% of the feces samples and were more frequent in males (31.9%) than in females (14.9%) (P < 0.001). The prevalence of both infections increased with age, especially in persons 50 years of age and older: The prevalence of Strongyloides infection was significantly higher in HTLV-1 carriers (31.6%) than in those without HTLV-1 infection (P < 0.001). The level of IgE was low in HTLV-1 carriers, and significantly lower in HTLV-1 carriers than in noncarriers among inhabitants with Strongyloides infection. Both HTLV-1 and Strongyloides infections are endemic in the area studied.

[Expression of HGF and c-met/HGF receptor mRNA in brains of normal rats and in phaeochromocytoma cells PC-12 of rats treated with NGF].

Gene expression of HGF and c-met proto-oncogene was examined during rat brain development and in cultured PC-12 cells, using reverse-transcriptase (RT)-polymerase chain reaction (PCR) technique. The both mRNAs of HGF and c-met proto-oncogene were remained at low levels in the middle and late stages of gestation (E-13 and E-18). After birth, and the level of both mRNA expression suddenly increased. During P-1 and P-12, their high level of expression continued and then decreased in P-20 and adult brain. Both HGF mRNA and c-met photo-oncogene mRNA were transiently expressed between day 2 and day 5, and disappeared in cultured PC-12 cells treated with NGF. The neurites of PC-12 cells that were treated with anti-sense oligonucleotides of HGF and c-met proto-oncogene, were shorter and fewer in number than untreated control cells. We conclude that neurite extension of PC-12 cells treated with NGF may ensue by way of c-MET protein activation and signal transduction pathways. Thus, c-MET protein activation and up-regulation of the two mRNAs may also play an important role in neuronal maturation in the developing rat brain.

[Is reduced left ventricular volume related to mechanisms of dynamic mid-ventricular obstruction provoked by dobutamine infusion?].

Forty-seven patients with unexplained chest pain and normal resting echocardiograms were examined to see whether dynamic mid-ventricular obstruction (MVO) is induced by dobutamine infusion. Dynamic MVO was provoked in 17 patients (MVO group), but not in the other 30 patients (Non-MVO group). Before dobutamine infusion, the blood pressure in the MVO group was higher than that in the Non-MVO group (p < 0.05), but end-diastolic volume index (p < 0.001), end-systolic volume index (p < 0.01), stroke volume index (p < 0.001), cardiac index (p < 0.001), end-diastolic volume (p < 0.01) and end-systolic volume (p < 0.05) of the apical territory of the left ventricle in the MVO group were significantly less than those in the Non-MVO group. The left atrial function, left ventricular ejection fraction and ejection fraction of the apical territory of the left ventricle did not differ between the groups. Seven patients in the MVO group were re-examined by dobutamine stress echocardiography after beta-blocker administration, showing that the dynamic MVO was completely suppressed. The end-diastolic volume tended to increase after beta-blocker administration, but no significant difference was found in any other variables except heart rate. The results suggest that a smaller left ventricle and higher blood pressure are important characteristics in patients with dobutamine-induced dynamic MVO, and additionally, the difference in local myocardial contractility may be an important cause of the induction of dynamic MVO.

Long-term follow-up of hemostatic molecular markers during remission induction therapy with all-trans retinoic acid for acute promyelocytic leukemia. Keio Hematology-Oncology Cooperative Study Group (KHOCS).

Hemostatic molecular markers were serially monitored in a prospective fashion during remission induction therapy with all-trans retinoic acid (ATRA) in sixteen patients with acute promyelocytic leukemia (APL). One patient with leukocytosis before treatment and three patients who later developed hyperleukocytosis also received chemotherapy with behenoyl Ara-C and daunorubicin. Plasma levels of E-fragment of fibrin and fibrinogen degradation product (FDP-E), FDP-D dimer (D-D), thrombin-antithrombin complex (TAT), and plasmin-alpha 2 plasmin inhibitor complex (PIC) were markedly elevated in all but one patient before treatment, and these parameters decreased to normal or near normal ranges in most patients within the first 7 days of treatment. Interestingly, we have found that these parameters were again elevated during the later course of ATRA therapy (after day +7) in eleven patients for various reasons including cytotoxic chemotherapy (3 cases), fever (5 cases; 2 cases with apparent infection, 3 cases without known etiology), Caesarean section (1 case), and no apparent etiology (2 cases). Three patients showed bleeding complications during re-elevation of molecular markers, but none developed thrombosis. Plasma elastase-alpha 1 proteinase inhibitor complex (E-alpha 1 PI) was markedly elevated in all patients at diagnosis and did not decrease significantly during ATRA therapy. Plasma tissue factor antigen was mildly elevated in one out of four patients studied, and thrombomodulin was elevated in two out of ten patients tested. These results confirmed the rapid normalization of coagulopathy during the early phase of remission induction therapy with ATRA but suggest that re-elevation of molecular markers occurs frequently during the later course of ATRA therapy.

[Accuracy of three-step diagnosis in discriminating subtypes of acute ischemic stroke].

The aim of this study is to evaluate the accuracy of three-step diagnosis in discriminating subtypes of acute ischemic stroke. A total of 120 consecutive patients with first-ever ischemic stroke, admitted to one general hospital, were prospectively studied. In the first step (within 24 hours of clinical onset), the first diagnosis was made according to clinical symptoms and signs, and patients were subdivided into four groups according to the classification of Oxfordshire Community Stroke Project: lacunar infarcts (LACI), total anterior circulation infarcts (TACI), partial anterior circulation infarcts (PACI), and posterior circulation infarcts (POCI). In the second step (24 hours to 72 hours from the onset), neuroimaging diagnosis was performed by CT and/or MRI. Four lesion sites were classified: 1) small subcortical infarction < or = 1.5 cm in diameter in the perforating artery territory (SSI), 2) supratentorial cortical or striatocapsular infarction (CI), 3) low-flow infarction (LFI) which includes centrum semiovale infarct and internal junctional infarct, and 4) posterior circulation infarction other than SSI (PI). In the third step, etiological diagnosis was made by examination including trans-thoracic echocardiography and MRA (3-D, PC). In accordance with the TOAST Study, the presumed stroke mechanism was categorized as either small-vessel occlusion (lacune), cardioembolism (CE), large-artery atherosclerosis (LAA), or others. The majority of patients with TACI, PACI or POCI showed the corresponding lesions on CT or MRI, while only 69% of LACI patients demonstrated SSI. Seventy-five percent of patients with TACI were categorized as CE in the third diagnosis, while the etiology of the patients with PACI was either CE or LAA in equal numbers. Only 60% of LACI patients were classified as lacune and 21% of them as LAA. Patients with LACI but classified as LAA usually had atypical clinical symptoms (e.g. monoparesis) and lesions other than SSI. The positive predictive value (PPV) of lacune in the combination of LACI and SSI was 0.75. Eighty-two percent of patients with CE had atrial fibrillation (af), which was the most frequent cardioembolic source. When patients with TACI or PACI had af, the PPV of CE was 0.93, but when they did not, the PPV of LAA was only 0.68. The etiology of POCI was variable. In conclusion, the agreement of the three-step diagnosis is considerable, but more rigorous clinical examination is needed for some clinical groups (POCI and LACI) and the etiological diagnosis of LAA.

[A study of the effects of antidepressants on the GABAA receptor and its complex based on the drug actions on the power-spectral changes of rat hippocampal EEG induced by GABA antagonists and inverse agonists].

To analyze the effects of antidepressants on the GABAA receptor, we investigated how the chronic administration of antidepressants (10 mg/kg twice a day for three or seven days, ip) influenced the power-spectral changes induced by pentylenetetrazol (PTZ; a GABA antagonist; 27.5 mg/kg) or beta-carboline-3-carboxylic-acid-methylester (beta-CCM; an inverse agonist; 1 mg/kg) on rat hippocampal EEGs. PTZ and beta-CCM are known to inhibit the chloride ionophore and benzodiazepine receptor (GABAA receptor complex), respectively. After the ip injection of both compounds, the EEG power under 12 Hz increased to about five times that before injection. Between the rats that did not receive any antidepressants and all those injected with the drugs for 3 days or treated with desipramine (DMI) for 7 days, there were no apparent changes in the effect of PTZ or beta-CCM. However, in the rats treated with imipramine, fluoxetine or trazodone for 7 days, the increase in power after the injection of PTZ or beta-CCM was apparently suppressed. In these rats, the power values were less than three times those before the dosing of PTZ or beta-CCM. DMI is known to inhibit the re-uptake of norepinephrine (NE), while the other three antidepressants inhibit that of serotonin (5-HT). Trazodone is also reported to block the 5-HT2 sites. These observations might indicate that the chronic administration of antidepressants prompted the function of the GABAA receptor complex. Moreover, it is also suggested that, to that action, the effect of antidepressants on the 5-HT system or interaction between the 5-HT system and GABA receptors might play some role.

Transient sick sinus syndrome associated with immunoblastic lymphadenopathy.

We describe a very rare case of immunoblastic lymphadenopathy (IBL) accompanied with a sick sinus syndrome (SSS) in a 64-year-old male. SSS presented with progression of the IBL and subsided with improvement of the IBL following high dose prednisolone (PSL) therapy. The clinical course of this patient strongly suggested cardiac involvement in IBL and being the cause of the transient SSS. To our knowledge, this is the first case of IBL demonstrating SSS as a possible complication of the disease. This case may provide further information for the treatment of similar patients with IBL.

Class-switching of the IgM type anti-adenocarcinoma human antibody HB4C5 into an IgG1 type resulted in the loss of the antigen binding ability.

The heavy and light chain genes (mu and lambda) of the human anti-adenocarcinoma monoclonal antibody HB4C5 (MAb-C5) were cloned from the human-human hybridoma cell line HB4C5 and co-expressed in Chinese hamster ovary (CHO) and COS-7 cells. ELISA assay and the RI imaging of the cancer tissue xenografted into nude mice showed that the recombinant MAb-C5 retained the original antigen binding activity. We then replaced the IgM constant region of the MAb-C5 heavy chain gene with the human IgG1 constant region gene and co-expressed it with the light chain gene. This recombination was confirmed by a complete DNA sequencing and Western-blot analysis. Despite the fact that the DNA sequence, the expressed protein size, and the assembly of heavy and light chains were indicated to be normal, the IgG1 type MAb-C5 could not bind to the original antigen. This result suggest that this antibody alters its antigen binding property upon class-switching.

Eosinophilic gastroenteritis presenting with acute pancreatitis.

We present the findings on a 27-year-old male with eosinophilic gastro-enterocolitis accompanied with acute pancreatitis. Acute pancreatitis may be induced by pancreatic duct obstruction caused by marked swelling of the papillary region of the duodenum due to eosinophil infiltration. After prednisolone treatment, clinical manifestations rapidly improved; the serum amylase decreased and the peripheral eosinophilia was recovered. The serum interleukin-5 (IL-5) level was high at diagnosis and decreased by prednisolone therapy; however, IL-5 was detected by enzyme immunoassay even during clinical remission. These results indicate that eosinophilia is mediated by IL-5, and detectable levels of IL-5 indicate the possibility of relapse.

Impact mass chemotherapy with praziquantel on schistosomiasis control in Fanhu village, People's Republic of China.

The paper describes the endemic situation of schistosomiasis japonica in Fanhu village, Poyang Lake region, China and the effect of the strategy of combining annual mass chemotherapy with health education on schistosomiasis control in the community. The results showed that the prevalence of infection with schistosome reduced form 26.0% in 1992 to 10.7% in 1994, the intensity of infection in residents decreased from 1.92 in 1992 to 0.55 in 1994 and the condition of hepatomegaly, splenomegaly and liver fibrosis also improved after chemotherapy in the individuals in the case prospective study. Moreover, the future strategies of schistosomiasis control in this area have been suggested according to the transmission of schistosomiasis in the lake region and the effect of anti-schistosomiasis control indifferent populations.

Ammonia determination as an early indicator in experimental superior mesenteric artery occlusion.

Superior mesenteric artery occlusion (SMAO) is often fatal. An indicator which enables the early diagnosis of SMAO is needed. As we think putrefaction products must appear and increase in the blood and ascites in SMAO, changes in the concentrations of ammonia, one of the putrefaction products, were measured in this study. Thirteen adult mongrel dogs were used for the in vitro experiment. The jejunum, ileum, and ascending colon were resected and incubated in saline. Changes in ammonia concentrations in the saline were examined at various incubation times. In the in vivo experiment, 11 mongrel dogs comprised the SMAO group and another 10 mongrel dogs comprised the control group. Changes in ammonia concentrations in the blood and ascites were examined in both groups. In the in vitro experiment, ammonia concentrations in the saline bath increased in all samples. It was highest in the sample from around the ascending colon, and lowest from around the jejunum. However, at the end of experiment, this difference became insignificant. In the in vivo experiment, ammonia concentrations in samples of the blood increased early and significantly in the SMAO group, compared with the control group. Ammonia concentrations in samples of the ascites also increased significantly. The in vitro experiment showed that ammonia leaked from the ischemic intestines, and secondarily, a large amount of ammonia was produced from intestinal putrefaction. The in vivo experiment revealed that the ammonia level in the blood could be used as a good early indicator of acute mesenteric ischemia.

Serum beta-2-microglobulin in patients with multiple myeloma treated with alpha interferon.

In ten patients with multiple myeloma (MM), serum beta-2-microglobulin (B2M) levels were monitored in order to clarify the influence of alpha interferon (IFN) administration. Despite decreases in M-protein and the absence of renal dysfunction, the levels of serum B2M were sustained above those prior to melphalan-prednisolone and IFN therapy in seven patients with MM for six months. Serum B2M did not increase in ten patients with MM treated only by melphalan-prednisolone. Furthermore, serum B2M levels in a patient who achieved a complete response were sustained above her prior level and returned to normal after cession of IFN therapy. Our study suggests that the serum B2M level is increased by treatment with IFN, and does not prove the condition of the disease.
