[One-stage bilateral STA-MCA anastomosis in multiple occlusive cerebrovascular disease].

Out of 12 cases of bilateral stenosis of the internal carotid (IC) system underwent unilateral STA-MCA anastomosis on the symptomatic side, no ischemic signs recurred in 8 cases, while, in the remaining 4, symptoms appeared postoperatively, requiring the anastomosis of the contralateral side. Preoperative findings of CT scan, CAG and r-CBF were reviewed and compared. It has, consequently, become evident that, in the latter 4 cases, angiography showed stenotic lesion of over 50% of the ICA lumen, between the mastoid-mandibular line (MML) and the circle of Willis, on the initially asymptomatic side. Furthermore, in the same 4 cases, multiple thrombotic foci were more liable to be demonstrated on the CT scans and r-CBF was lowered in both hemispheres without significant difference in laterality. In additional 7 cases presenting such characteristics as stated above, one-stage bilateral STA-MCA anastomosis was carried out with a successful result.

Reconstruction of scutum defects (scutumplasty) for attic cholesteatoma.

The use of a small plate of autogenous bone to reconstruct a scutum defect--scutumplasty--in combined approach tympanoplasty in 79 patients with attic cholesteatoma has been studied for a period of one or more years. The bone plate obtained from the mastoid cortex and sculptured to the proper size and shape was snugly fitted between two grooves made by a bur at the anterior and posterior bony external canal wall just proximal to the scutum defect. In this series, recurrence of postoperative cholesteatoma in the attic was found in 5.1%, and retraction pockets, which remained dry and self-cleaning, were seen in 3.8% of the patients. Postoperative hearing results were satisfactory in 68.3%, and hearing was unchanged or deteriorated in 31.7%.

Selective local anti-inflammatory activity of THS-201, a new intra-articular steroid, in cotton pellet granuloma and monoarticular arthritis.

A new steroid, THS-201 was administered locally in one of a pair of inflamed sites and its local anti-inflammatory activity was compared with those of triamcinolone acetonide, methylprednisolone acetate and hydrocortisone acetate. In rats, THS-201 (80 micrograms/pellet) completely inhibited the formation of granulation tissue even after 21 days. In antigen-induced bilaterally arthritic rabbits, THS-201 (2 mg/joint) decreased the swelling of the treated knee joint for more than 24 days, but no such effect was seen in the opposite knee joint. The anti-inflammatory activity of the other steroids employed as reference drugs were weaker than that of THS-201. These results indicate that THS-201 is an anti-inflammatory steroids exclusively acting at the site of administration for a long period, and for this reason it might be suitable for intraarticular application in clinical use.

[Associations between ovarian cancer and diet].

With special reference to diet, we epidemiologically assessed the factors which would modify the morbidity of ovarian cancer. As a result of the case-control study, the women with post menopause-occurring ovarian cancer used to eat meat and fish more frequently. This finding is consistent with the results from some correlation studies, which showed the possible association between ovarian cancer and fat consumption. Further, the ovarian cancer patients used to drink milk less frequently. This finding was still significant even if other risk factors were simultaneously taken into consideration. Regarding these results, several etiological inferences were discussed.

[Four cases of cerebral arterial ectasia--its clinical manifestations and surgical treatment].

Four cases of intracranial arterial ectasia are reported. The clinical manifestations of the four cases due to the arterial ectasia were ischemic attack in two patients, subarachnoid hemorrhage in one patient and abducens palsy in one patient. Surgical treatment was performed in three patients. The angiographic findings of the four patients were reported and the pathogenesis and the possibility of the surgical treatment of the arterial ectasia were discussed.

[A case of systemic lupus erythematosus associated with multiple intracranial aneurysms].

The author reports a case of systemic lupus erythematosus (SLE) with multiple intracranial aneurysms and subarachnoid hemorrhage. A 31-year-old woman was admitted to the department of internal medicine of Shizuoka General Hospital for the treatment of nephrotic syndrome due to systemic lupus erythematosus on 1984. She spend an uneventful life until April, 1985 when she suddenly complained of severe headache and nausea. The computed tomographic scan revealed subarachnoid hemorrhage and the cerebral angiography showed multiple intracranial aneurysms and stenotic lesion. A review of the literature was made to clarify the incidence, the pathological change, and the prognosis of systemic lupus erythematosus with subarachnoid hemorrhage. The incidence of subarachnoid hemorrhage in SLE was about two percent in the reported clinical cases. The mechanisms of the subarachnoid hemorrhage and the aneurysmal formation in SLE seemed to be due to angitic changes, which involved the vessels of the whole body systematically. The exact prognosis of SLE with subarachnoid hemorrhage is difficult to say, but it seems to be unfortunately poor due to multiplicity of the lesion and the difficulty of its treatment.

Vanillylmandelic acid, homovanillic acid, and catecholamines in urine of infants with neuroblastoma 6- to 11-month-old.

The amounts of vanillylmandelic acid (VMA), homovanillic acid (HVA), and catecholamines (adrenaline, noradrenaline, and dopamine) in 24-hr urine specimens of 10 infants with neuroblastoma detected through mass screening in Sapporo City (patient group) were compared with those of 24 healthy infants (control group). The ages of the infants of both groups ranged from 6 to 11 months, which agrees with the age when mass screening is performed. For VMA and HVA, the mean value plus two or three times the standard deviation of the control group clearly separated the two groups in the units of mg/day, microgram/mg of creatinine, mg/kg/day, and mg/m2/day. The cut off values of 24 micrograms/mg of creatinine for VMA and 25 micrograms/mg of creatinine for HVA are now being used by us for assaying 24-hr urine specimens. No significant difference was found between the groups in levels of catecholamines, but their data were thought to be useful as normal values, since few published data regarding their normal values are available.

Time dependent effects of selenium on cadmium-induced acute mouse testicular damage.

Twenty-four hr after a single subcutaneous injection of Cd (0.024 mmol/kg), the testis showed severe and widespread degeneration. The damage was accompanied by an increase of lipoperoxide and a decrease of glutathione in the testis. Se (0.048 mmol/kg) injections 2 hr before, at the same time, and 1 hr after the Cd injection, decreased the damage. In these groups, the level of glutathione and lipoperoxides was restored to the control level. With a Se injection 6 hr after the Cd injection, the preventive effect of Se was no longer found. In the testis in which Se injection alleviated the damage caused by Cd, the changes in Ca, Mg and Zn were also restored to the control level. Uptake of Cd into the testis was stimulated by the Se injection. However, the uptake still remained at a low level in the testis in which the Se preventive effect was not found. These results suggest that the preventive effect of Se against testicular Cd toxicity is dependent on the interval between Cd and Se injections.

Reconstruction of the defect of the oropharynx using free jejunum.

Reconstruction of oropharyngeal defect using free jejunum was carried out in two patients with selected situation. One patient had problem of donor site, which was developed breast. Another patient had problem of recipient site, which required the flap with thin subcutaneous tissue. Reconstruction with free jejunal transfer was successful in both cases. The major advantage of the free jejunal transfer for oropharyngeal defects was discussed in comparison with cutaneous flaps. In conclusion this safety procedure is recommended for reconstruction of oropharyngeal defect.

Spasmodic head movements produced by destruction of unilateral ventromedial tegmentum in cats.

In cats, unilateral electrolytic destruction in the dorsomedial parts of the ventral tegmental area (VTA) (Tsai) led to the appearance of torticollic rotatory head postures associated with a marked drop of the corresponding caudate serotonin. Results obtained from fluorescence histochemistry and brain dialysis also revealed that decrease in serotonin shared the leading role. However, since a chemical lesion produced by neurotoxic agents failed to induce such abnormal postures, concomitant interruption of nonaminergic fibers at VMT might also be the essential factor. Among them the interstitial nucleus (Cajal) and its descending tract were the most suspicious.
