[Apposition acetabular roof surgery with screw osteosynthesis and use of wedge osteotomy in intertrochanteric femur osteotomy].

After introductory presentation of the different methods of roofplasty of the hipsocket (including the osteotomies of the pelvis) relative to indication, advantages and disadvantages we describe a new technique of roofplasty for older children, adolescents and grownups, in which the wedge of the intertrochanteric femur osteotomy--up to now a by-product is used for a roofplasty similar to the combined operation with children. We give a presentation of the vertical and transverse apposition technique and especially of the advantage of the compression screw osteosynthesis. In particular we make a comparative evaluation to the concurrent pelvic osteotomy after Chiari. Above all we give a report of the experiences with 62 patients with 67 operations within a period of 7 years (1976-1982): We report the subjective and objective results. In general we achieved very satisfying results with a very low rate of complications.

[An ultrasound hip screening program--middle term results of flexion-abduction orthosis therapy].

INTRODUCTION: Today the hip sonography is an established method to detect congenital hip maturation disturbances in the first days of life. Short-term follow-ups have showed that mature hip joints can be performed with an flexion-abduction bandage within a few months. METHODS: This prospective study reviews in a middle-term follow up 57 dysplastic hips at birth of 42 children after an average period of 7.8 years, who took part in an ultrasonic hip screening and were treated successfully with an abduction pant (33 hips type IIg, 16 hips type D, 8 hips type III according to Graf). RESULTS: There were only 3 cases (5%) who had slight limitations of motion clinically according to the classification of Tönnis. The radiographic control showed an average AC-angle of 11 degree, an average CE-angle of 28 degree and an average hip-score according to Tönnis of 8.5. Slight radiological deviations were found in 9% of the cases. Severe pathologic radiographic appearance or avascular necrosis of the femoral head were not observed. CONCLUSIONS: If DDH is detected in first day of life and immediately treated with abduction pants is physiological development of the hip probably.

[Clinical experience with the implantation of collagen-apatite for local bone regeneration].

After a short review of the problematic of bone substitution the possibility of bone regeneration induced by implantation of synthetic Apatite and reaffined heterogenic Collagen is displayed especially with an industrial manufactured mixed preparation called "Collapat". Firstly it is referred especially to the comparing animal experience and histological results by Nizard. According to this research fine dispersed Apatite powder has obviously a remarkable osteoinductive effect, which is inhenced by the Collagen component, used especially as a distribution medium. The tract mainly concerns the clinical experiences in human patients with Collagen-Apatite-implantation since October 1979 with the indication of larger bone defects. In 85 patients with totally 94 Collagen-Apatite-implantations in different age groups no incompatibility was seen. In X-ray controls a rapid reestablishment of natural bone structures was observed. The Collagen-Apatite product seems particularly to be apt for the reconstruction of defect cavities as well as for an additive to autologous bone transplantation which by this method can be limited as well in number as in quantity.

[Surgical treatment of chronic recurrent patella dislocations. Surgical procedure and results].

This is a report about a simplified operative technique of discission of the lateral retinaculum and vastus and medial drawing tight of the capsule. After the end of growth there will be always a medial transfer of the tuberositas tibiae at the same time by an undermining technique of osteotomy and osteosynthesis with a traction screw. Evaluation of our own 135 cases (1964-1981) with 27 only soft-tissue-operations and 108 combined operations by longterm-observations or recent control (115 cases with average observation period of 5,9 years). The number of reluxations of the patients, who had only a soft-tissue-operation, was as high as other reports; the primary number of reluxations after the combined operation was much lower (6,7%). By further conservative treatment or sometimes second operation none of our patients had any longer reluxations of the patella.
