[Technique and results of arthrodesis of the ankle joint with autocompression plates (author's transl)].

The article describes the technique of arthrodesis of the ankle joint, introduced in 1973 by Mittelmeier, with suitable preparation (freshening) of the joint, resection of syndesmoses and internal osteosynthesis with autocompression angle plates, as well as additional fastening by screwing the fibula against tibia and talus. This method imparts high biomechanical stability to the joint thanks to the internal bracing of the frame; compared with external fixation, this method offers the advantage of avoiding the risk of subsequent infection of the borehole, as well as the possibility of early impaired wearing of trousers and of being able to bathe after superficial wound healing. Treatment results achieved in 14 patients showed (despite 1 reinfection following osteomyelitis caused by a fracture due to a bullet wound) regular, rapid and stable osseous reconstruction of the joint in which arthrodesis had been performed. Hence, this technique seems to be superior to external compression arthrodesis in aseptical cases.

[Results of treatment of habitual laxation of the shoulder with modified surgical technique (author's transl)].

The article describes a modification of the original surgical technique employed by M. Lange in the treatment of habitual laxation of the shoulder. This consists mainly of opening the joint with bone chip implantation under visual control, and turning the spongy side towards the head of the humerus and interpositioning the capsula while tightening the same. In addition, however, the point of insertion of the subscapular tendon is shifted while drawing it tighter also. The article describes the advantages, as seen from the case histories of 57 operations, about one-half being subjected to follow-up examinations over a period of at least 3 years, on the average 9 years. In all patients, excellent to good stabilisation of the shoulder was achieved.

[Malignant T-cell lymphoma with osteomyelitis-like bone infiltration].

After a short review on the late literature, existing about various forms of acute lymphoblastic leucemias, it is reported on a rare case of malignant T-cell-Lymphoma with ostemyelitis-like, painfull bone infiltration. The clinical symptoms, as well as differential-diagnostic criterias to other leucemias are described.

[Psoriasis-arthritis in early childhood].

It is reported on a seldom case of psoriasis-arthritis in early childhood. The onset of skinlesion firstly occurred at the age of one month. The clinical symptoms of psoriasis advantage are firstly described and compared with the symptoms in our case. It was an unusual early beginning.

[Carcinoma in scar tissue after frost inquiry to the foot (author's transl)].

Carcinomas of the extremities are relatively rare neoplasms when compared to Sarcomas. After a review of the literature about carcinomas in chronic fistulae and other bone lesions, two cases are reported in which a carcinoma originated in scar-tissue, 31 years after a frost-bite lesion of the foot. The problems of etiology, pathogenesis and therapy are pointed out.

[Desmoid fibromas of the extremities (author's transl)].

Two cases of rare desmoid fibromas are being reported, localised in the extremities. After a literature review of these tumors, the clinical characteristics are pointed out and difficulties in therapy discussed. The tumors occasionally appear on the forearm or the thigh of juvenile patients and show a similar development. In the first case, 7 operations with resection of the n. medianus and n. ulnaris were necessary to prevent a recurrence. In the second case, we saw a solitary and rapidly growing tumor, surrounding the sciatic nerve with a great tendency to recur.

[The biomechanical significance of the vastus medialis and lateralis muscles (author's transl)].

Anatomic considerations and electromyographic investigations show that the Mm. vasti medialis and lateralis both have a synergistic function with regard to the extension of the knee and an antagonistic function in stabilizing the rotation of the femur against the tibia. The rotational stabilisation for the femur by the Mm. vasti is very important in preventing knee injuries, especially ligamentous ruptures. On the other hand, extreme reflectoric muscle contractions, caused by a rotational trauma can cause chondromalacic lesions in the femoro-patellar joint, broadening the syndrome of the "unhappy triad" to an "unhappy tetrad".
