[The effect of corneal epithelial cells on the collagen gel contraction by keratocytes].

To understand the mechanism of corneal stromal wound contraction, we investigated the effect of corneal epithelial cells on the collagen gel contraction by keratocytes. Subcultured rabbit keratocytes embedded in type I collagen gel were cultured. Rabbit corneal epithelial cells were also cultured and the cultured medium was collected and used as an epithelial cell conditioned medium (ECCM). The collagen gel contraction by keratocytes was estimated by measuring the diameter of the collagen disc once a day for 5 days. The diameter of the collagen gel decreased in proportion to the number of keratocytes in the presence of fetal calf serum (FCS), but it decreased in inverse proportion to the concentration of collagen. The collagen gel contraction by keratocytes was enhanced by the addition of ECCM in a dose dependent manner. This stimulatory activity was found in ECCM obtained from 3 to 7 day cultivation of corneal epithelial cells. These findings demonstrated that keratocytes contracted collagen gel in the presence of FCS. It also appeared that corneal epithelial cells secrete a factor or factors that stimulate the collagen gel contraction by keratocytes.

Scanning electron microscopy and energy dispersive spectroscopy analysis of calciotraumatic lines in rat labial dentin after acute exposure to strontium chloride.

Rats were given strontium chloride (SrCl2) intraperitoneally at a dose of 500 mg/kg. The upper incisors were removed after injection of strontium. These incisors were studied by scanning electron microscopy-energy dispersive spectroscopy analysis (SEM-EDS) and light microscopy to examine the calciotraumatic lines of strontium in the rat incisor labial dentin. At 24 hours after injection of strontium, the calciotraumatic response was observed in the predentin using hematoxylin and eosin (H-E) staining. At 5 days, three layers of calciotraumatic lines were present in the labial dentin using an H-E staining and backscattered electron imaging in the SEM. The external layer consisted of unmineralized dentin, the intermediate layer of relatively unmineralized dentin, ane the internal layer of unmineralized dentin. By SEM-EDS analysis, strontium was detected in these layers. The elemental dot map showed that the external and internal unmineralized layers had a low calcium content. The magnesium concentration was higher in the internal unmineralized layer than the external unmineralized layer.

[Evaluation of prophylactic hepatic arterial infusion chemotherapy after hepatectomy for metastases from colorectal cancer].

The significance of continuous hepatic arterial infusion (HAI) chemotherapy of 5-FU to prevent recurrence in the remnant liver following curative hepatectomy for colorectal metastases was analyzed in the prospective randomized study. Patients of HAI chemotherapy group received 6 weeks of 5-FU 500 mg/day. The one- and two-year overall survival rates of nine patients with HAI therapy were 87.5% and 87.5%, respectively, while the rates of ten cases without the regional chemotherapy were 100% and 66.7%, respectively, indicating no obvious difference. On the other hand, the disease-free survival rates of HAI Group were higher than those of control group. The disease-free survival rates of the former were 75.0% at one-year and 75.0% at two-year, and the rates of the latter were 47.6% and 47.6%, respectively. No serious complications such as severe liver dysfunction, sclerotic cholangitis or hepatic necrosis were observed. Although the follow-up period was not enough long to accurately evaluate the efficacy, local prophylactic chemotherapy by continuous infusion of 5-FU could be a promising method as an adjuvant chemotherapy after hepatic resection for colorectal metastases.

[Neoadjuvant intraarterial chemotherapy with dose intensification in locally advanced breast cancer].

Sixteen patients with advanced breast cancer were treated with neoadjuvant intraarterial chemotherapy with high dose epirubicin. The following results were obtained: 1) Excellent down-staging effects were confirmed. Response rate of the primary tumor was 81.3%. 2) Leukopenia was the dose-limiting factor, and 81.3% of patients had WHO grade 2 or more leucopenia. However, the regimen was completed with supportive therapies. 3) At a median follow-up of 17 months, improved survival rates were noted. The present study showed the efficacy of neoadjuvant intraarterial chemotherapy with high dose epirubicin in the treatment of locally advanced breast cancer.

[Penetrating injury of the head, neck and chest by a nail-gun: a case report].

A very rare case of directly penetrating injury of the head, neck and chest caused by a nail-gun was reported. A 56-year-old male was admitted to our critical care center due to chest injury. On admission, he showed numbness of the hand & leg, left hemiparesis and hypalgesia. Physical examination disclosed three nails on the left anterior chest, but other wounds or nails were not found. Chest films showed three nails penetrating the lung without reaching the heart, but other nails were found by skull and neck films. One nail had penetrated the cervical canal at the C1 level through the posterolateral cervical region. Two other nails were demonstrated at the right temporal and the left frontal region, Computed tomography revealed no massive cerebral hemorrhage and cerebral angiography showed no extravasation and no passage through main vessels. Emergency surgery was performed uneventfully and the nails in the chest, neck and head were totally removed. He was discharged one month after surgery. Some injuries caused by a nail-gun have been reported in the world literature but in Japan this multiple injury case was the first reported. Since nail-gun injuries can cause multiple damage, systemic X-ray examination was very important.

Binding of hyaluronan to plasma fibronectin increases the attachment of corneal epithelial cells to a fibronectin matrix.

We wished to determine whether hyaluronan would affect the attachment of epithelial cells to extracellular matrix proteins. Multiwell tissue culture plates were coated with human plasma fibronectin, laminin, or collagen type IV (0.01-10.0 micrograms/ml). Single-cell suspensions of rabbit corneal epithelial cells were placed in the wells, and after 45 minutes incubation the cells adhering to the matrix proteins were stained and counted. Cells attached to all three types of proteins. Preincubation of the matrix proteins with hyaluronan (0.1-1.0 mg/ml) significantly increased the number of cells attached to the fibronectin matrix, but it did not increase the numbers of cells attached to laminin or collagen type IV. Hyaluronidase inhibited this stimulatory effect. Glycosaminoglcyans other than hyaluronan (chondroitin sulfate, keratan sulfate, or heparan sulfate) failed to increase the numbers of attached cells. Treatment of the fibronectin matrix with monoclonal antibodies against the cell-binding domain of fibronectin (FN12-8 or FN30-8, 0.03-0.3 mg/ml, for 1 hour), before or after hyaluronan treatment, significantly decreased the numbers of attached cells. Monoclonal antibody against the fibrin- and heparin-binding domain at the N-terminal (FN9-1), however, significantly decreased the number of attached cells only when this antibody treatment preceded the hyaluronan treatment. Preincubation of the cells with hyaluronan had no effect; preincubation with GRGDSP (1 mg/ml), a synthetic peptide that blocks the cell surface receptor for fibronectin, significantly decreased cell attachment whether the fibronectin matrix was treated with hyaluronan or not. Further studies demonstrated that monoclonal antibody against the fibrin- and heparin-binding domain at the N-terminal of plasma fibronectin prevented radiolabeled hyaluronan from binding to fibronectin; likewise, the isolated N-terminal fragment, coupled with Sepharose 4B, bound to hyaluronan in columns. We conclude that hyaluronan binds to a fibrin- and heparin-binding domain at the N-terminal of plasma fibronectin and facilitates the attachment of epithelial cells.

Bilateral giant bullae with rapidly increasing fluid in the right bulla following operation: report of a case.

A 33-year-old Japanese male was admitted to our hospital for the treatment of left pneumothorax. He was found to have bilateral giant bullae. Retention of fluid was observed in the right giant bulla. The fluid accumulated rapidly directly after bullectomy and excision of the left giant bullae. This suggested the existence of some mechanisms of fluid retention other than an infection of the bulla alone after operation.

Pleural lipoma: report of a case.

We report herein the unusual case of a 45-year-old Japanese man whose chest X-rays revealed an abnormal shadow, increasing in size. A chest wall tumor was suspected, based on the findings of computed tomography (CT) of the thorax, the CT number of which was -137, chest roentgenogram, and an echograph. An open biopsy was performed to establish the final diagnosis. The resected tumor was a pedunculated pleural mass, yellowish in color, the pathological diagnosis of which confirmed a lipoma. Intrathoracic lipomas are rare, but pleural lipomas are seen even less frequently. CT, echography, and percutaneous needle biopsy have been found useful for diagnosing intrathoracic lipomas, but these examinations are not always adequate for confirming the final diagnosis. Consequently, tumor resection is essential for obtaining a pathological diagnosis.

[Combination with intra-hepatic arterial infusion of low-dose cisplatin and oral administration of high-dose doxyfluridine in patients with liver metastases of gastric cancer].

Two cases of gastric cancer with multiple liver metastases were treated with 5'-DFUR and CDDP after the surgery. The first case, a 79-year-old male, was administered 1,200 mg/day of 5'-DFUR orally, 6 mg/day of CDDP continuously for 2 weeks, then 20 mg once a week for 5 months by intra-hepatic arterial infusion. This case showed 95% tumor reduction by CT. The other case, a 78-year-old female, was administered 1,200 mg/day of 5'-DFUR orally and 20 mg of CDDP once a week through the hepatic artery. Following several intra-hepatic arterial infusions, it was changed to intra-venous infusion of 20 mg of CDDP twice a week because of trouble with the vascular access system. Two months after, CT showed a 46% tumor reduction in her liver. Neither complained of diarrhea nor nausea, but there was a mild degree of appetite loss. Combination with high-dose 5'-DFUR and low-dose intra-hepatic arterial infusion of CDDP is considered a very effective chemotherapy which can be performed ambulatorily while maintaining the quality of life of patients with liver metastases of gastric cancer.

[Evaluation of prophylactic intra-hepatic-arterial infusion chemotherapy after resection of hepatic metastases from colorectal primaries].

To prevent recurrence in the residual liver after surgical treatment for colorectal metastases, the efficacy of intermittent or continuous intra-hepatic-arterial chemotherapy was analyzed. The three- and five-year survival rate of patients with intermittent infusion of ADM or MMC (n = 11) was 36.4% and 36.4%, respectively, while the survival rate of the patients without the regional chemotherapy (n = 32) was 41.9% and 23.3%, respectively, indicating no statistical significance. On the other hand, one patient out of 8 with continuous infusion of 5-FU and 2 patients out of 6 without local chemotherapy developed recurrent disease during the median follow-up time of 12 months. No serious complication such as sclerotic cholangitis or hepatic necrosis was observed. Although the follow-up was not long enough to accurately evaluate the efficacy, local chemotherapy with continuous infusion of 5-FU could be a promising method as an adjuvant chemotherapy after hepatic resection for colorectal metastases.

[Neoadjuvant intra-arterial infusion chemotherapy for the treatment of locally advanced breast cancer with remote metastases].

Intra-arterial infusion chemotherapy (I.A.) before the operation was performed in 7 advanced breast cancers with remote metastases. The locations of remote metastases were bone (6 cases), lung (one case), and brain (one case). The response rate of primary lesions to I.A. was 71% and the responders of remote metastases were two cases, of which one was CR (lung). After the chemotherapy, all cases underwent standard radical mastectomy, and five of seven cases are alive at this writing without local recurrence. We have performed I.A. for locally advanced breast cancer with remote metastases on the grounds that the effect of this treatment is "semi-local and semisystemic," I.A. bring down-staging to the primary lesions and can control local recurrence. The effect of the drug which leaks from I.A. is expected to the metastatic lesions. I.A. was considered to be useful too for the treatment of locally advanced breast cancer with remote metastases in terms of the excellent control effect of the local lesions and the effect on the metastatic lesions.

[A case of advanced gastric cancer successfully treated with resection after FEP (5-FU, epirubicin, CDDP) combined chemotherapy].

We reported an experience with an advanced gastric cancer patient with direct invasion into liver and metastases to liver, who had responded extremely well to an FEP combined chemotherapy. He had curatively undergone total gastrectomy with partial hepatectomy and enlarged lymph node dissection. The patient received three courses of 5-FU (750 mg/m2/day, for four days, continuous infusion), epirubicin (30 mg/m2, on day 1, i.v.), CDDP (17.5 mg/m2/day, on days 1, 2, 3, 4 i.v.) every 3 weeks in our hospital. No remarkable side effect was encountered. Partial response in the primary and invasive lesions was observed by X-P, endoscopy and CTscan. Accordingly, we could perform curative resection of the stomach with lymph nodes and parts of liver. The effect (partial response) of neoadjuvant chemotherapy was confirmed by histological examinations. FEP combined chemotherapy appears useful as a neoadjuvant approach to advanced gastric cancer.

[Ruptured anterior communicating artery aneurysm associated with inter-optic course of anterior cerebral artery: report of a case and review of the literature].

We report a ruptured anterior communicating artery (ACoA) aneurysm in a patient whose anterior cerebral artery (ACA) arose from the left C3 portion and ascended between the optic nerves. A 50 year-old woman was admitted to our department with complaints of headache and vomiting. CT scan showed thick subarachnoid hemorrhage. Left carotid angiography revealed a saccular aneurysm of the anterior communicating artery and an abnormal vessel which arose from the left C3 portion and terminated in the territory of the ACoA. Surgery was performed about eight hours after subarachnoid hemorrhage. The abnormal vessel was identified as it ascended between the optic nerves. There are only 25 reported cases of this abnormality including our own case. The right carotid artery was the most frequent origin of the abnormal artery (18 cases). 13 aneurysms were associated in 11 cases, in which seven were located in the anterior communicating artery. It was the most frequent site of the aneurysm. There were various explanations for this anomalous ACA. Because of the angiographical and operative findings, we prefer to use "inter-optic course of ACA" as the medical terminology better suited to describe this condition.

Role of urokinase type plasminogen activator (u-PA) in corneal epithelial migration.

The role of plasminogen activator (PA) in the migration of corneal reepithelialization was studied. Rabbit corneal blocks were cultured, and both the extent of epithelial migration over the exposed corneal stroma and the activity of PA released into the culture media were measured. A significant, direct correlation between epithelial migration and PA activity in the medium was observed, even when the migration was stimulated by fibronectin or EGF, or was inhibited by cytochalasin B or cycloheximide. Zymography confirmed that the PA released into the culture medium was of the urokinase type (u-PA). Immunohistochemical studies showed that u-PA and plasmin(ogen) were present at the leading edge of the migrating epithelium. Studies of corneal cell cultures indicated that epithelial cells rather than endothelial cells or fibroblasts were the source of the u-PA. The addition of antihuman u-PA IgG or protease inhibitors retarded the migration of the corneal epithelium in a dose-dependent manner, indicating that u-PA activity is essential for the migration of the corneal epithelium. These findings suggest that the migration of corneal epithelial cells requires not only cell attachment to the extracellular matrix through the fibronectin but also degradation of the fibronectin by the release of cellular u-PA.

Effects of antimicrobials on corneal epithelial migration.

The slowed healing rates observed by some investigators may be caused by vehicles or preservatives in the antimicrobials preparations tested. To determine whether antimicrobials directly inhibit corneal epithelial wound healing, we cultured blocks of the rabbit cornea in media containing various concentrations of antibiotics or antimicrobials (at 1, 10, or 100 micrograms/ml); after 24 hours, we measured the distance of epithelium that had migrated down the side of each block. The higher concentrations of fluoroquinolones (ofloxacin; 74 +/- 5.8% of control at 100 micrograms/ml, p < 0.05, ciprofloxacin; 4.4 +/- 1.5% of control at 100 micrograms/ml, p < 0.01, or norfloxacin; 71 +/- 7.0% at 10 mu g/ml, p < 0.01, and 1.5 +/- 0.4% of control at 100 mu g/ml, p < 0.01) and the highest concentrations of peptides (polymyxin B; 64 +/- 3.0% of control at 100 micrograms/ml, p < 0.01, or colistin; 67 +/- 5.7% of control at 100 micrograms/ml, p < 0.01) or fosfomycin (79 +/- 6.2% of control at 100 micrograms/ml, p < 0.05) had an inhibitory effect on corneal epithelial migration. Among aminoglycosides tested, sisomicin (85 +/- 10.0% of control, not significant), dibekacin (76 +/- 11.6% of control, p < 0.05) and streptomycin (77 +/- 9.4% of control, not significant) were inhibitory at 100 micrograms/ml, but tobramycin had no effect. Penicillins (aspoxicillin, sulbenicillin or ampicillin), cephalosporins (cefmenoxime or cefminox), oxytetracycline, erythromycin and chloramphenicol did not affect epithelial migration at all. These results demonstrate that some antimicrobials are inhibitory at high concentrations, but penicillins, cephalosporins, oxytetracycline, erythromycin or chloramphenicol has no inhibitory effect on corneal epithelial migration.

Calcium phosphate in aspergillosis of the maxillary sinus.

The unique appearance of apatite in fungus balls of aspergillosis in the maxillary sinus was investigated using scanning electron microscopy (SEM), energy-dispersive spectroscopy (EDS), X-ray microdiffraction and Fourier transform infrared spectroscopy (FT-IR). Rod-shaped fragments with tubular structures, and globular vesicles covered with conidia were observed in the fungus balls. Massive fragments of a solid substance were demonstrated inside the fungus balls. Calcium and phosphate were detected in necrotic areas of the fungus balls by EDS. X-ray microdiffraction and FT-IR showed the presence of an apatite-like substance, but failed to demonstrate the presence of calcium oxalate crystals usually found in such fungus balls.

Calcospherites in rabbit incisor predentin.

Calcospherites from the lower incisor dentin of rabbits were investigated by scanning and transmission electron microscopy (TEM), energy dispersive spectroscopy (EDS) and electron diffraction analyses. In the labial predentin, globular calcospherites of 8-31 microns were present at the root apex, decreasing in size toward the incisal region. The calcospherites at the intermediate region were of mulberry- as well as of spindle-shape of 1.5-4 microns diameter. The incisal pulp horn contained micro-calcospherites of 0.3-0.6 micron in diameter. In the lingual predentin, small granular calcospherites of 1.8-3 microns were present at the root apex, increasing in size toward the intermediate region. Ultrathin sections of globular calcospherites showed bundles of collagen fibrils at the root apex of the labial predentin. The diameters of individual bundles ranged from 1.2-3.4 microns. The width of the fibrils in the bundles was approximately 120-170 nm. Bundles of collagen fibrils were not found in the lingual predentin. Crystals of calcospherites were identified as apatite by electron diffraction. Those at the intermediate region showed preferred orientation of the c-axis. TEM-EDS analyses indicated that Ca and P were the major elements, with small amounts of Mg. The Mg/Ca molar ratios decreased from the root apex to the incisal pulp horn. Ca peak intensities increased from the root apex to the incisal region.

[A case of radiation cataract resulting from accidental irradiation of radioisotope].

We studied light and electron microscopic findings in one case of cataract resulting from accidental radioisotope irradiation. A 53-year-old male, a radiographer, was irradiated by Ga-citrate which dropped accidentally into his left eye during a radioisotope procedure. Subsequently opacity appeared in the posterior and anterior subcapsular area of the left lens and increased in intensity. Intracapsular cataract extraction was performed one and a half years after the accident. The extracted lens was studied by light and transmission electron microscopy. Epithelial cells showed frequent stratification in some sites and disappeared in others in the equatorial region. Wedl cells and vacuoles were also seen in the posterior subcapsular area. Degeneration of lens fibers, including multilayered whorls, were observed in the cortex.
