Carbohydrate metabolism of rats with biliary obstruction.

Carbohydrate metabolism of rats with obstructive jaundice caused by bile duct ligation was studied by intravenous glucose tolerance test (IVGTT) and by liver perfusion. The altered levels of carbohydrate-metabolizing enzyme were examined in relation to the glucose metabolism of the cholestatic rats. In the IVGTT, the rate of fractional glucose removal was increased with increases in plasma insulin and glucagon and with a decrease in non-esterified fatty acid. In liver perfusion, neither the glucose uptake nor insulin extraction by the whole liver of icteric rats was different from the control. The increased rate of glucose removal in IVGTT may be due to enhanced glucose utilization by peripheral tissues resulting from hypersecretion of insulin. In liver perfusate supplemented with glucose, a decrease in the glucose uptake per unit liver weight was observed in relation to the lowered glucokinase activity. Formation of glycogen from glucose and of glucose from lactate was also impaired, indicating inhibition of the gluconeogenic system or relative hyperfunction of the glycolytic system, which may further contribute to the reduction in glycogen content. These metabolic disorders correlated well with the changes in activities of key carbohydrate-metabolizing enzymes, which showed a characteristic pattern consistent with the loss of differentiated hepatic functions. Uptake of glucose and its conversion to glycogen were reduced in the cholestatic liver in close association with altered activities of some of related enzymes. However, due to increased utilization by the peripheral tissues, the total amount of glucose utilized in the whole rat was not reduced.

A case of necrosis of hepatocellular carcinoma and tumor thrombus in the portal vein induced by transcatheter arterial lipiodol chemoembolization.

A case of hepatocellular carcinoma is reported in which the main tumor, intrahepatic metastases and a tumor thrombus in the portal vein were necrotized completely after Lipiodol chemoembolization. In this case, the tumor thrombus seemed to act as a portal embolus. This phenomenon is interesting because Lipiodol chemoembolization alone usually can not necrotize intra- or extra-capsular invasion, intrahepatic metastasis or tumor thrombus in the portal vein. This case is considered to be suggestive of a possible therapy for hepatocellular carcinoma.

Assessment of Kupffer cell function in rats with chronic liver injury caused by CCl4.

Kupffer cell function was assessed by using scintigraphy to evaluate the turnover of a metabolizable tracer (99mTc-millimicrosphered albumin). The organ uptake rate of the tracer, and new parameters concerned with the degradative functions of Kupffer cells obtained from analysis of the excretion phase of the hepatic time-uptake rate curve, were measured in rats with two different types of chronic liver damage induced by carbon tetrachloride (fatty liver group and liver cirrhosis group). The hepatic uptake rate in chronic liver injury decreased, while in contrast the splenic and pulmonary uptake rates increased. A particularly high uptake by the lungs was observed. The data demonstrated a reduced phagocytic activity of the Kupffer cells in rats with chronic liver injury. The new parameters concerned with Kupffer cell degradative function; i.e. the excretion rate (K) and the degradation rate in the first 60-min (D60), were markedly decreased even in the early stage of chronic liver injury. The data showed that the impairment of Kupffer cell degradative function occurred even earlier in liver damage than impairment of the phagocytic activity, so that the K value and the D60 value were the more sensitive indicators of Kupffer cell function.

Clinical values for an index predicting postoperative residual liver function by pre-operative liver-scintigraphy in patients with liver disease.

Hepatic resection is essential in treating hepatocellular carcinoma. However, before an operation, it is difficult to predict the functional reserve in the remnant following massive resection. We devised an original method by which effective liver volume was measured by liver scintigraphy. In order to predict the residual liver function before hepatic resection in a preoperative radiocolloid study, we obtained a predictive index by combining the K values with effective liver volumes which seemed to have the estimated residual liver function. Twenty-one patients with liver or biliary tract disease were selected at random for the present study. We divided them into 3 groups in accordance with prognosis after hepatic resection. There were statistically significant difference between the deceased group who died from hepatic failure and the group who died from causes other than hepatic failure; and between the deceased group who died from hepatic failure and the living group in the preictive index (p less than 0.01). Our data suggest that if the predictive index is above 0.45, the probability of hepatic failure after hepatic resection is low. We concluded that our predictive index is useful to use in preoperative prediction of post-hepatectomic residual liver function.

[Physiological properties of motor units in the human anterior digastric muscle].

The activity of the single motor units in the human anterior digastric muscle was recorded in 7 subjects with a monopolar fine wire electrode. The force generated by jaw opening was simultaneously recorded with a mechanical transducer. The results obtained were as follows: (1) Increasing the force gradually, a few motor units were recruited orderly and the firing rate of each unit reached the peak frequency rapidly. These findings suggest that the control of the muscle force caused by jaw opening movements might depend more on the orderly recruitment of the motor units than on the rate modulation. (2) The contraction time, measured by the spike-triggered averaging technique varied from 25.6 to 132.4 ms. It was suggested that the anterior digastric muscle consisted of both the fast twitch motor units and the slow twitch motor units. (3) The motor units that have a higher threshold force tended to have a larger twitch tension and have a shorter twitch contraction time than that of the lower threshold motor units. This means that the anterior digastric muscle has a relatively simple role during the active jaw opening movements. (4) Fatigue can be estimated as the change in the firing rate. The motor units of a higher threshold force tended to be fatigue during the continuous jaw opening movements.

[Studies on AFP-producing capacity and some other properties of human hepatoma cells treated with various anticancer drugs].

The effect of various anticancer drugs on alpha-fetoprotein (AFP) secretion and some other properties of human hepatoma cells was investigated in vitro with the following results. (1) There was a high correlation between AFP secretion and cell number after treatment of human hepatoma cells with anticancer drugs and the amounts of AFP secreted per 10(4) cells per 72 hours (AFP-secreting capacity) were not affected within therapeutically achievable concentrations (TAC). (2) The AFP-secreting capacity was affected with some exceptions in the cells treated with higher concentration of drugs than TAC. Furthermore, chromosomal and morphological aberrations in the similarly treated cells, were also observed, suggesting the relationship between the change of AFP-producing capacity and that of some other properties.

[Orthodontic approach to temporomandibular arthrosis. A case report of medial displacement of the articular disk].

This clinical report presents orthodontic applications of double contrast arthrotomography and arthroscopy of the temporomandibular joint. We examined a case of mandibular shift with stomatognathic dysfunction using these techniques, and found medial displacement of the articular disk. Generally, displacement of the articular disk is examined only from sagittal section. However, this case indicates that frontal section of double contrast arthrotomography is necessary for the three dimensional understanding of the TMJ. Mandibular position with disk reduction was obtained by double contrast arthrogram, and medial displacement of the disk and malocclusion were treated by dynamic positioner. It is suggested that orthodontic treatment is an effective method both for treatment of internal derangement of the TMJ and also for the final reconstruction of occlusion.

[HCFU and 5-fluorouracil levels in the blood and tissue of hepatocellular carcinoma after oral administration of HCFU].

HCFU was orally administered to 14 patients with hepatocellular-carcinoma, (including 11 patients with liver cirrhosis) and evaluated of HCFU and 5 fluorouracil (5-FU) levels. Blood and tissue 6-8 hr. after oral administration. The concentration of 5-FU in tissue was almost in the effective levels. In addition, the 5-FU level in the tissue of hepatocellular carcinoma tended to be higher than in non cancerous portion of the liver. 5-FU tissue concentration was not correlated with various laboratory data for the liver function (K-ICG, T. bil, GOT, GPT, etc.) From these results, it is suggested that HCFU is a useful anticancer agent for hepatocellular carcinoma especially for the cases accompanied liver cirrhosis.

[Effects of Co, Mo and Cu on the fluidity of 80Ni-20Cr alloy].

An attempt was made to determine the castability of Nickel-Chromium alloys which contained an additional element. 80 Ni-20 Cr binary alloy was prepared in the argon atmosphere and saved as control alloy. Then three sets of different components alloys, each set contains different composition (5, 10, 15%) of Co, Mo, or Cu to the control alloy, were prepared and used as test materials. The fluidity was evaluated by casting them into the cylindrical coil shaped mould cavity and examining their ability to flow into the mould cavity before solidification took place. The fluidity of Nickel-Chromium alloy was not changed by addition of Co or Mo up to 5%. However, the fluidity was significantly decreased by addition of Cu, or Mo when its concentration was increased over 5%. Thus, from the viewpoint of castability a large amount of Mo or Cu should not added to the Nickel-Chromium alloy.

[Single photon emission computed tomography (SPECT) using 99mTc-hexamethyl propyleneamine oxime in patients with ischemic cerebral diseases: comparative study of N-isopropyl-p-[123I]-iodoamphetamine and 99mTc-DTPA SPECT].

In order to confirm the diagnosis and clarify the pathophysiology in the ischemic lesions in brain, 99mTc-PAO SPECT was performed in 42 patients with ischemic cerebral diseases, and the findings on 99mTc-PAO SPECT were compared with those on 123I-IMP or 99mTc-DTPA SPECT, and on X-CT. Furthermore, count ratio of ischemic lesion to normal contralateral region (I/N) was used for the evaluation of the detectability in the lesions on 99mTc-PAO or 123I-IMP SPECT, and less than 0.95 of I/N ratio was defined as the decreased accumulation. The detectability of ischemic lesion in brain with 99mTc-PAO SPECT was 80%, which was superior to that (60%) of LDA on X-CT. Especially, 99mTc-PAO SPECT was useful in cases which were examined early after the onset of the attack. On the other hand, 123I-IMP SPECT showed the best result (88%) in the detectability, and furthermore I/N ratio on 123I-IMP SPECT was lower than that on 99mTc-PAO SPECT. In the delayed phase on 123I-IMP SPECT the redistribution was observed in 30%, while on 99mTc-PAO SPECT it was done in none of them. The accumulation of 99mTc-DTPA in brain was found in 6 of 14 cases (43%), and 2 cases among them also showed the accumulation of 99mTc-PAO in the ischemic lesion. In such cases the destruction of BBB might be considered as the cause of the accumulation of 99mTc-PAO. Thus, 99mTc-PAO SPECT was thought to be useful method in clinical studies of the ischemic cerebral diseases.

[Clinical study of prostatic cancer].

Eighty patients with prostatic cancer, who first visited Kyorin University School of Medicine from January 1976 through December 1986, were analyzed. Incidence of prostatic cancer was 3.9% among male inpatients. Age distribution was between 55 and 88, with an average of 72 years old. The most common symptoms were dysuria followed by pollakisuria, hematuria, lumbago and lower extremity pain. Duration from onset of symptom to examination ranged from 6 to 84 months, with an average of 22 months. Clinical stage was A in 7.5%, B in 10%, C in 11.3% and D in 71.3%. According to histological grade, well, moderately, and poorly differentiated adenocarcinomas were observed in 29.9, 29.9 and 40.2%, respectively. According to the General Rules for Clinical and Pathological Studies on Prostatic Cancer, clinical T classification were T0 in 8.7%, T1 in 3.8%, T2 in 47.5%, T3 in 27.5% and T4 in 12.5%. In the correlation between stage and grade, the largest number of poorly differentiated adenocarcinoma cases was in stage D. There was no correlation between stage and T classification. Of the 80 patients, 71.25% were treated with antiandrogen therapy, 16.25% with radiation therapy chiefly, 7.5% by surgery chiefly, and 5% with chemotherapy. Survival rate was calculated by the Kaplan-Meier method. Overall survival rate of the 80 patients was 54.4% at 5 years. Survival rate by stage were 100% in stage A at 4 years, and 100% in B, 87.5% in C and 40.5% in D at 5 years.(ABSTRACT TRUNCATED AT 250 WORDS)

Effects of bile duct ligation and dietary change on hepatic carbohydrate metabolism in rats.

Effects of bile duct ligation and dietary change on hepatic carbohydrate metabolism in rats were studied by analyzing the levels of fructose 2,6-bisphosphate (fructose-2,6-P2) and other metabolites. There was no significant difference in the levels of fructose-2,6-P2 and glycolytic intermediates between rats with and without bile duct ligation when rats were in a fed state. However, the levels of fructose-2,6-P2, hexose monophosphates, and triose phosphates increased and the level of plasma glucose decreased in rats with bile duct ligation in a starved state. The crossover analysis of gluconeogenic intermediates in rats with bile duct ligation showed accumulations of hexose monophosphates together with fructose-2,6-P2 in spite of the starved state of rats, indicating an impaired conversion of glucose 6-phosphate to glucose. Despite the activation of the fructose 6-phosphate-fructose 1,6-bisphosphate futile cycle as indicated by these findings, the energy charge levels were not reduced. The block at the level of glucose production may explain the relative predominance of insulin effect on glycolysis.

Alterations in key carbohydrate-metabolizing enzyme activities in rat livers following bile duct ligation and its release.

Activities of key carbohydrate-metabolizing enzymes in the livers of rats after bile duct ligation were studied during the recovery phase, after releasing the bile duct from ligation as an external biliary drainage. Enzyme activities were analyzed on the 3rd and 7th days after ligation and on the 3rd and 7th days after releasing the ligation. The altered enzyme activities, which resulted from the ligation of the bile duct, recovered after its release, although the recovery was delayed as when compared with those in the serum liver function tests. Glucokinase was the enzyme which was most delayed in recovery. The decreased activity of glucokinase had not returned to normal, even on the 7th day after release of the bile duct from its 7 day-ligation, whereas all the other enzyme activities had returned to normal by this time. It was also suggested that the decreased activity of glucokinase, which regulates the glucose uptake by the liver, was related to the glucose intolerance in obstructive jaundice. Accordingly, glucokinase can be used as a marker for determining the time of operation after biliary drainage.

Immunoelectron microscopic observation of alpha-fetoprotein synthesis in human non-malignant liver tissues using immunoperoxidase methods.

Alpha-fetoprotein (AFP) synthesis in non-malignant liver tissue of 34 patients with chronic hepatitis or liver cirrhosis, some of whom also had hepatocellular carcinoma (HCC), was studied by light and ultrastructural immunohistochemistry using peroxidase-labeled anti-human AFP. Simultaneously, the serum level of AFP was measured in these patients by radioimmunoassay. AFP-positive cells were identified in non-malignant liver tissue of 7 patients with elevation of serum AFP. AFP was demonstrated in several hepatocytes which were clustered in hepatic lobules, and also in some bile ductular cells which were distributed in the periphery of portal tracts. In an immunoelectron microscopic study of AFP-positive hepatocytes, dense reaction products of anti-AFP were localized in the membranes and cisternae of rough endoplasmic reticulum (r-ER), perinuclear space (PNS) and Golgi apparatus. The prominent feature of AFP-positive hepatocytes was abundant r-ER encompassing many mitochondria. As to AFP-positive bile ductular cells, they had scanty cytoplasm and few intracytoplasmic organelles and were surrounded by basement membrane. AFP was focally localized in the r-ER of such bile ductular cells. These observations suggest that AFP can be produced by malignant and non-malignant liver cells and that in non-malignant liver tissues, AFP can be produced by two distinct cell types; bile ductular cells and hepatocytes themselves.

[Combination chemotherapy including cisplatin in lung cancer by bronchial artery infusion].

Chemotherapy with bronchial artery infusion (BAI) was given to 34 patients with primary lung cancer. Treatment regimens usually employed cis-diammine-dichloroplatinum (CDDP) plus peplomycin for squamous cell carcinoma, and CDDP plus vindesine for adenocarcinoma. The provisional therapeutic effects were evaluated roentgenographically with reference to histological type, T factor and degree of vascularization. Out of 10 cases of squamous cell carcinoma, 7 cases (70%) showed tumor regression greater than 50%, in contrast to 4 of 17 cases (23.5%) of adenocarcinoma. The effects in cases of squamous cell carcinoma were correlated with tumor vascularity. Twenty-two surgically treated cases were examined for the histological effects of BAI. Five of 6 cases (83.3%) of squamous cell carcinoma showed IIb effects by Shimosato's criteria. These results showed that the therapeutic effect of BAI was excellent in cases of squamous cell carcinoma in comparison with cases of adenocarcinoma. Serious side effects including esophago-bronchial fistula, massive hemoptysis and esophageal ulcer were observed in 4 cases.

Radical block resection of hepatoduodenal ligament for carcinoma of the bile duct with double catheter bypass for portal circulation.

A new concept of radical block resection of the hepatoduodenal ligament for carcinoma of the bile duct and new double bypass technique of portal circulation for the procedure were introduced. Clinical experiences of six patients were reported. By combined use of radical block resection of the hepatoduodenal ligament and double bypass technique, the radical resectability of carcinoma of the bile duct became a reality, and improvement of postoperative survival rate is expected.
