Intraspecific variation of unusual phospholipids from Corynebacterium spp. containing a novel fatty acid.

The novel fatty acid trans-9-methyl-10-octadecenoic acid was isolated from the coryneform bacterial strain LMG 3820 (previously misidentified as Arthrobacter globiformis) and identified by spectroscopic methods and chemical derivatization. This fatty acid is attached to the unusual lipid acyl phosphatidylglycerol. Five different species of this lipid type were identified; their structures were elucidated by tandem mass spectrometry and are reported here for the first time. Additionally, we identified three different cardiolipins, two bearing the novel fatty acid. The characteristic 10-methyl-octadecanoic acid was present only in phosphatidylinositol. Because of the unusual fatty acid pattern of strain LMG 3820, the 16S rDNA sequence was determined and showed regions of identity to sequences of Corynebacterium variabilis DSM 20132(T) and DSM 20536. All three strains possessed the novel fatty acid, identifying trans-9-methyl-10-octadecenoic acid as a potential biomarker characteristic for this taxon. Surprisingly, the fatty acid and relative abundances of phospholipids of Corynebacterium sp. strain LMG 3820 were similar to those of the type strain but different from those of Corynebacterium variabilis DSM 20536, although all three strains possessed identical 16S rDNA sequences and strains DSM 20132(T) and DSM 20536 have 90.5% DNA-DNA homology. This is one of the rare cases wherein different organisms with identical 16S rDNA sequences have been observed to present recognizably different fatty acid and lipid compositions. Since methylation of a fatty acid considerably lowers the transition temperature of the corresponding lipid resulting in a more flexible cell membrane, the intraspecific variation in the lipid composition, coinciding with the morphological and Gram stain reaction variability of this species, probably offers an advantage for this species to inhabit different environmental niches.

Percutaneous closure of secundum atrial septal defect with a new self centering device ("angel wings").

OBJECTIVE: To investigate the safety, efficacy, and clinical application of a new self centering device ("angel wings") for closure of secundum atrial septal defects (ASD II) and persistent foramen ovale in all age groups. DESIGN: Multicentre, prospective, nonrandomised study. PATIENTS INCLUSION CRITERIA: defects with an occlusive diameter of < or = 20 mm and a surrounding rim of > 4 mm; body weight > 10 kg; and an indication for surgical closure of secundum atrial septal defect. Additionally, there were compassionate indications for closure in patients with persistent foramen ovale. INTERVENTIONS: Defects were closed by a transcatheter device consisting of two square frames made of superelastic nitinol wire. The frames are covered by elastic polyester fabric, which is sewn together at a central circle. All procedures, except for three interventions that were carried out under sedation, were performed under general anaesthesia using transoesophageal echocardiography and fluoroscopy to monitor intervention. RESULTS: Closure was attempted in 75 (71%) of 105 patients. An ASD II was present in 35 children and 15 adults. A persistent foramen ovale was present in 25 adults with suspected paradoxical embolism. Transcatheter closure was unsuccessful in three children and crossover to surgery was required. Residual shunts were found in 20 patients (27%) immediately after the procedure. A transient atrioventricular third degree block occurred in three patients (4%) and the right atrial disk was not fully deployed in three. A minor shunt (< 3 mm) was present in only three (4%) of 72 patients during follow up of 1-17 months. Blood clots on the right atrial disks in two patients (one required lysis) were seen during follow up transoesophageal echocardiography. Serious complications demanding surgical removal of the device occurred in three patients. One patient had haemopericardial tamponade because of an aortic lesion. Left atrial thrombus formation due to an unfolded right atrial disk was found in a second patient and dislodgement of the left atrial disk resulted in a large residual shunt in a third. CONCLUSIONS: Percutaneous closure of a central ASD with a diameter < or = 20 mm in paediatric and adult patients is feasible and effective with this new device. It is a promising alternative to surgical closure. Modifications of the design, however, seem to be mandatory as 4% of patients developed serious complications.

[Psychosocial risk factors in vulvovaginal mycosis. A multivariate long-term study].

The results of a multivariate logit-analysis of n = 9098 (female) patients of a gynecologic practice (from 1991 to 1994) confirm the relevance of psycho-social factors like employment, being married, smoking etc. on the incidence of the vulvovaginal mycosis. By the statistical results the studied factors can be rank-ordered from "smoking" as the most relevant to "virginity" as the most irrelevant factor. Surprisingly sexual contact and using oral contraception had no relevance. A check-list using factor-combinations enables to discriminate 48 types of female with different risk of incidence. The results of the study are in accordance with the following hypothesis: In most cases stress, induced by the psycho-social situation of the female and weakening the immune system, is the only responsible factor for the incidence of vulvovaginal mycosis.

Improvement of acral circulation in a patient with systemic sclerosis with stellate blocks.

We report on a 77-year-old male patient with systemic sclerosis. He suffered from secondary Raynaud's phenomenon on the basis of systemic sclerosis. Medical treatment in the past, including the administration of calcium-channel blockers, pentoxifylline and intravenous prostaglandin therapy, was unsuccessful and the clinical situation became worse. In a final effort stellate blocks were performed over a period of several weeks and, for the first time, there was lasting clinical benefit. Complaints and Raynaud's attacks abated significantly, as documented by local cold exposure tests. Therapeutic benefit from stellate blocks in a patient with systemic sclerosis is described here for the first time.

[Veterinary surgeon Gottfried Carl Haubner (1806-1882)--promoter of scientific animal nutrition].

The veterinary surgeon G. C. HAUBNER (1806-1882) was teacher for veterinary science at the Royal prussian academia for agriculture in Eldena near Greifswald (1836-1852) and at the royal school of veterinary science in Dresden (1853-1879). He stimulated animal nutrition in different ways. First of all he disclosed the theoretical gaps in the field of animal nutrition during the first half of the 19th century, furthermore he transferred the knowledge in agriculture chemistry and animal physiology to feeding praxis and altogether he favoured the transition from the empirical to the scientific based animal nutrition, last not least by starting exactly planned and practiced feeding experiments (s. Table 2). Most remarkable are his very early work on digestion physiology in ruminants (1837), his proof (1854), that fiber is digestible in ruminants or horses as well as his investigations on nutrition related diseases in cattle (smelting-house-smoke- and deficiency-diseases).

Mechanical circulatory support after orthotopic heart transplantation.

Frequently the only therapy for primary graft- and right heart failure, as well as low output syndrome from acute of chronic rejection, is implantation of a mechanical circulatory support system, until recompensation or retransplantation. At our institution, mechanical assist devices were implanted in 25 heart recipients for a cute rejection (n=9), primary graft failure (n=7), acute right heart failure (n=7), and chronic rejection with low output syndrome (n=2). Patients (pts) with primary graft failure (n=3) received an intraaortic balloon pump (IABP), one pt an IABP plus Abiomed-System for left ventricular support, one pt the Thoratec-System for biventricular support. Patients with right heart failure (RHF) received the Biomedicus centrifugal pump for right ventricular support. Nine pts suffered from acute rejection. Six pts received an IABP, one the Biomedicus as femoro-femoral bypass, one the Abiomed-System for biventricular support, two the Thoratec-System for biventricular support and two within this group switched from the Biomedicus pump to the Thoratec-System for biventricular support. Patients with chronic graft failure (n=2) received the Novacor-System (LVAD) for left ventricular support, one received a Tojobo-System and an oxygenator for biventricular support post coronary artery bypass surgery. Support time ranged from 0.5-h to 73 days. Five pts were weaned. Two (8%) of 25 pts were retransplanted, 18 (72%) died in spite of mechanical support from multiple organ failure. The use of a mechanical assist device after heart transplantation is encouraging only in the case of early right heart failure, as well as primary and chronic graft failure. In view of the poor results, the use of mechanical assist devices should not be recommended in the case of heart failure caused by acute rejection.

[Analysis of the acceptance of the internet presentation of the 115th Congress of the German Society of Surgery].

The World Wide Web presentation of the 115th Annual Meeting of the German Society of Surgery was examined with regard to the frequency of hits per day and the use made of the online offers. The high frequency of the hits clearly shows the high acceptance of this medium. The temporal distribution of the hits imply that the group that was aimed at, namely "surgeons", have indeed been reached.

The Medicare mall. Will Washington like what it built?

"Shopping spree" doesn't begin to describe it. Next year, 38 million seniors will begin choosing their health care coverage from a menu crammed with options. Medicare Plus Choice means just that: HMOs, provider-sponsored plans, private-pay options, medical savings accounts, and more. It also means a bevy of new rules--and risks--for everyone.

Medicaid: states serve up a real turkey.

Policymakers promised a season of plenty by shifting Medicaid recipients to managed care--enough to expand coverage to the uninsured. But many states have sliced their rates severely. Big HMOs bolted from some markets and threaten to leave others. That leaves managed care rollout plans in trouble. And hardly anyone is talking about expanded coverage.
