Electroretinographic responses in patients with pulseless disease vary with head and body positions.

We examined bright-flash electroretinograms obtained in two Japanese women with pulseless disease. One 48-year-old woman (case 1) who has had the disease for 17 years showed no oscillatory potentials. When she sat and tilted her head to the right, a- and b-wave amplitudes diminished in the left eye. A second 48-year-old woman (case 2) who has had the disease for 10 years also demonstrated absent oscillatory potentials. When she sat and raised her chin, a- and b-wave amplitudes diminished in both eyes. These amplitudes were normal when the patients were in a supine position.

Immunological comparison between prostate-specific antigen and gamma-seminoprotein.

Prostate-specific antigen (PA) and gamma-seminoprotein (gamma-Sm) were compared by immunocytochemical, immunodiffusion and immunoblotting methods using rabbit anti-PA antibody and rabbit anti-gamma-Sm antibody. Enzyme immunoassays (EIAs) were developed for measurements of PA and gamma-Sm to determine a correlation between serum PA and gamma-Sm levels in patients with prostate cancer. The patterns of localization and distribution of PA and gamma-Sm were identical in prostate tissue sections, including benign and cancerous human prostates. The immunodiffusion study showed that the antigens with which anti-PA antibody and anti-gamma-Sm antibody reacted in seminal plasma and prostate tissue homogenates were identical to each other. In the immunoblotting study, anti-PA antibody and anti-gamma-Sm antibody recognized a single antigen corresponding to a molecular weight of approximately 33,000 both in seminal plasma and prostate tissue homogenates. The EIAs developed in this study were sensitive, specific, and reproducible, and the correlation between serum PA and gamma-Sm values determined by these EIAs was highly significant (r = 0.99, P less than 0.001). These results indicated that PA and gamma-Sm were immunologically identical and that serum PA and gamma-Sm determined by immunoassays using anti-PA antibody and anti-gamma-Sm antibody should be evaluated as identical tumor markers for serodiagnosis of prostate cancer.

Decreased cerebral glucose metabolism associated with mental deterioration in multi-infarct dementia.

Cerebral glucose metabolism of 18 patients with multi-infarct dementia (MID) and 10 age-matched normal subjects were examined with positron emission tomography and the 18-F-fluoro-deoxy-glucose technique. MID patients had significantly lower glucose metabolism in all the grey matter regions measured and were also characterized by more individuality in metabolic pattern. MID patients were also evaluated as to intelligence quotient (IQ). A positive correlation between IQ as shown by the Tanaka-Binet test and glucose metabolism for the entire grey matter was found. The clinical applicability of this test for predicting cerebral metabolism is discussed.

Inhibitory effects of phenyltin compounds on stimulus-induced changes in cytosolic free calcium and plasma membrane potential of human neutrophils.

To clarify the inhibitory mechanisms by triphenyltin chloride (TPTCl) on superoxide anion formation in neutrophils, the effects of phenyltin compounds [TPTCl, diphenyltin dichloride (DPTCl2) and phenyltin trichloride (MPTCl3) on the increase of cytosolic free calcium and the changes in membrane potential in neutrophils stimulated by n-formyl-methionyl-leucyl-phenylalanine (FMLP) were examined. TPTCl and DPTCl2 concentration dependently inhibited the increase of fluorescence intensity of the dye 3,3'-dipropyl-thiodicarbocyanine iodide [diS-C3-(5)] (membrane potential probe) in neutrophils induced by 0.1 microM FMLP in the presence or absence of extracellular calcium (1.26 mM). TPTCl had a greater inhibitory effect on FMLP-mediated membrane potential change than that of DPTCl2. In the presence of extracellular calcium, TPTCl and DPTCl2 increased intracellular free calcium [( Ca2+]i) of unstimulated fura-2-loaded neutrophils at concentrations from 1.0 to 10 microM TPTCl and from 2.5 to 10 microM DPTCl2. TPTCl and DPTCl2 also increased slightly, in the absence of extracellular calcium, [Ca2+]i without stimulation of FMLP in neutrophils. However, TPTCl and DPTCl2 significantly inhibited the rise of [Ca2+]i in neutrophils stimulated by FMLP at concentrations from 2.5 microM to 10 microM TPTCl and at a concentration of 10 microM DPTCl2 in the absence of extracellular calcium. TPTCl and DPTCl2 significantly inhibited the superoxide anion production by FMLP at concentrations over 2.5 microM in the presence of extracellular calcium. In the absence of extracellular calcium, TPTCl and DPTCl2 also inhibited the superoxide anion production by FMLP at concentrations over 1.5 microM TPTCl and over 5.0 microM DPTCl2.(ABSTRACT TRUNCATED AT 250 WORDS)

Endoscopic bipolar electrocoagulation in upper gastrointestinal bleeding.

A total of 213 cases of upper gastrointestinal bleeding treated by bipolar electrocoagulation (BPEC) were reviewed and its efficacy in the management of massive bleeding was evaluated. Initial hemostasis was achieved by BPEC in 97.6% cases with an overall rebleeding rate of 17.1%. The majority of rebleeding occurred in patients with acute mucosal lesions who had serious underlying medical problems. There were no complications related to the procedure. Endoscopic BPEC safely arrested oozing to spurting bleeding and contributed greatly to the control of upper gastrointestinal bleeding.

Positive regulator for the expression of Mba protein of the virulence plasmid, pKDSC50, of Salmonella choleraesuis.

A positive regulator was identified within a 2.3 kb fragment of the 6.4 kb mouse bacteremia region (mba region) of the virulence pKDSC50 plasmid of Salmonella choleraesuis. Sodium dodecyl sulphate polyacrylamide gel electrophoresis showed that Escherichia coli K-12 carrying the recombinant plasmids of the 2.3 kb fragment produced Mba1 protein with a molecular mass of 32 kDa. The recombinant plasmids carrying a 4.1 kb fragment, the other part of 6.4 kb region, produced Mba2 (32 kDa), Mba3 (70 kDa) and Mba4 (29 kDa) proteins. All three proteins were expressed by using the lacZ promoter under isopropyl thiogalactoside induction. In contrast to this, Mba3 protein was overexpressed independently of the lacZ promoter when the 2.3 kb fragment coexisted either in cis or trans. These results suggest that Mba1 is a trans-acting positive regulator for the expression of the Mba3 protein of mba region of pKDSC50.

Effects of ethane-1-hydroxy-1, 1-diphosphonate on cell differentiation, and proteoglycan and calcium metabolism, in the proximal tibia of young rats.

The effects of ethane-1-hydroxy-1, 1-diphosphonate(EHDP) on cell differentiation, and on the metabolism of proteoglycan and calcium in the epiphyseal plate and metaphysis of rats were investigated through histology and autoradiograms of [35S]-sulfate, 45Ca, and [3H]-thymidine. Suppression of bone resorption in the metaphysis due to low dose EHDP administration was associated with a proliferation of osteoclasts with an increased number of nuclei. High dose EHDP induced enlargement of the hypertrophic zone of the epiphyseal plate and suppression of calcification of the cartilage matrix. This change had a significant association not only with the suppression of chondroitin sulfate synthesis and the degradation in the cartilage matrix, but also with the suppression of growth and differentiation of chondrocytes. Calcification was also inhibited in the metaphysis, and growth and differentiation from undifferentiated mesenchymal cells to osteoblasts, osteocytes, and osteoclasts were also suppressed by high dose EHDP.

Cell culture of the intervertebral disc of rats: factors influencing culture, proteoglycan, collagen, and deoxyribonucleic acid synthesis.

To establish cell culture of the nucleus pulposus and anulus fibrosus of rat intervertebral disc, the effects of culture conditions on the growth of cells and the synthesis of DNA, proteoglycan, and collagen were studied. For cell culture of the nucleus pulposus, the use of 3-week-old rats and a medium adjusted to pH 7.0 was optimal. There was almost no difference in growth between cells in Ham's F12 medium and those in Dulbecco's Modified Eagle Medium. In cells isolated from the anulus fibrosus, a medium adjusted to pH 7.0-7.6 was preferable, but irrespective of rat age. Culture cells of the nucleus pulposus were composed of large cells with vacuoles and small polygonal cells. These cells had a slight growth activity and a fair capability of proteoglycan and collagen synthesis. Culture cells of the anulus fibrosus were composed of polygonal and spindle-shape cells, and the growth was more vigorous with the potentials for proteoglycan and collagen synthesis than the nucleus cells.

Myocardial contractility and contraction duration are unrelated in the left ventricle with moderate pressure-overload hypertrophy.

Both myocardial contractility and contraction duration were assessed load-independently in isolated hypertrophic and control rat hearts. Hemodynamic parameters for isolated isovolumically contracting hearts were compared for 7 Wistar rats with DOCA-salt induced hypertrophy (LVH) and 7 controls. Loads were altered by changing the volume of an intraventricular balloon. Myocardial contractility was assessed in terms of the shortening-afterload (Sm-sigma aft), and the ejection fraction-afterload (EF-sigma aft) relations at common preloads, based upon the systolic myocardial stiffness concept. Indices of contraction duration (time to maximum stiffness, pressure time integral divided by peak pressure and the time required for left ventricular (LV) peak pressure to be reduced by a half) were prolonged in LVH. Multiple regression analyses indicated that the degree of hypertrophy was an independent determinant of the contraction duration. The Sm-sigma aft and EF-sigma aft relations at common preloads demonstrated normal contractility in the LVH group. The time constant of the fall in LV pressure and the diastolic myocardial stiffness remained normal in the LVH group. Thus, we introduced the new approach to load-independently assess both the myocardial contractility and the contraction duration in a whole ventricle. A prolonged contraction duration in the presence of normal myocardial contractility was observed in a ventricle with moderate pressure-overload hypertrophy.

Untreated hypophosphatemic vitamin D-resistant rickets with symptomatic ossification of the ligamentum flavum.

An adult case of untreated hypophosphatemic vitamin D-resistant rickets with symptomatic ossification of the ligamentum flavum in the thoracic region is reported and discussed with regard to the calcium metabolism and the ligamentous ossification. Factors influencing ossification of the ligamentum flavum may be mechanical stress on the spine and an increase in calcium retention.

Identification of two calcineurin alpha isoforms in bovine brain by two different monoclonal antibodies.

Calcineurin (calcium- and calmodulin-stimulated phosphatase) alpha subunit purified from bovine brain was found to be composed of two polypeptides, 61 KDa (alpha 1) and 59 KDa (alpha 2). The two peptides were separated and extracted from polyacrylamide gel. The immuno-peptide mapping of the purified peptides by partial proteolysis showed that the 59-KDa polypeptide was not a degradative product of the 61-KDa polypeptide. The interaction of the enzyme with two monoclonal antibodies, Vj6 and Vd3, raised against bovine brain calcineurin revealed that the 61-KDa polypeptide was recognized by both Vj6 and Vd3, whereas the 59-KDa one was recognized only by Vj6. These results indicate that there are at least two isoforms of calcineurin alpha subunits in bovine brain.

[A case of chest wall rhabdomyosarcoma].

A 56-year-old man was admitted to our hospital with right chest pain. Chest X-ray, CT scan and MRI revealed a chest wall tumor and enlarged mediastinal lymph nodes. Percutaneous lung biopsy was performed, and the pathological diagnosis of pleomorphic rhabdomyosarcoma was obtained. The only significant abnormal laboratory finding was elevation of serum NSE (24.5 ng/ml). Although chemotherapy (VAC-ADM) and radiation therapy were performed, the patient died about 7 months after admission. To our knowledge, only 17 cases of chest wall rhabdomyosarcoma have been reported in Japan.

[Analysis of prognostic factors in high risk gestational trophoblastic disease with respect to remission and recurrence].

Sixty-nine patients with high risk gestational trophoblastic disease (excluding invasive mole) were treated at Chiba University from 1973 to 1988. The clinical features and results of treatment which influenced remission and recurrence in these high risk patients were analyzed statistically. The factors analyzed were (1) the height of pretreatment urinary hCG titer, (2) the type of antecedent pregnancy, (3) the time interval from pregnancy to treatment, (4) the metastatic site, (5) age, (6) the type of chemotherapy, (7) the time of operation and (8) the number of additional chemotherapy courses. With respect to the remission rate and the metastatic site, the type of chemotherapy was statistically significant. In addition, the factors related to the time of operation were significant in relation to recurrence.

Interaction of sodium and potassium ions with Na+,K(+)-ATPase. IV. Affinity change for K+ and Na+ of Na+,K(+)-ATPase in the cycle of the ATP hydrolysis reaction.

The Kd for ouabain-sensitive K+ or Rb+ binding to Na+,K(+)-ATPase was determined by the centrifugation method with radioactive K+ and Rb+ in the presence of various combinations of Na+, ATP, adenylylimidodiphosphate (AMPPNP), adenylyl-(beta,gamma-methylene)diphosphonate (AMPPCP), Pi, and Mg2+. From the results of the K+ binding experiments, Kd for Na+ was estimated by using an equation describing the competitive inhibition between the K+ and Na+ binding. 1) The Kd for K+ binding was 1.9 microM when no ligand was present. Addition of 2 mM Mg2+ increased the Kd to 15-17 microM. In the presence of 2 mM Mg2+, addition of 3 mM AMPPCP with or without 3 mM Na+ increased the Kd to 1,000 or 26 microM, respectively. These Kds correspond to those for K+ of Na.E1.AMPPCPMg or E1.AMPPCPMg, respectively. 2) Addition of 4 mM ATP with or without 3 mM Na+ decreased the Kd from 15-17 microM to 5 or 0.8 microM, respectively. Because the phosphorylated intermediate was observed but ATPase activity was scarcely observed in the K+ binding medium containing 3 mM ATP and 2 mM Mg2+ in the absence of Na+ as well as in the presence of Na+ at 0 degrees C, it is suggested that K+ binds to E2-P.Mg under these ligand conditions. 3) The Kd for Na+ of the enzyme in the presence of 3 mM AMPPCP or 4 mM ATP with Mg2+ was estimated to be 80 or 570 microM, respectively.(ABSTRACT TRUNCATED AT 250 WORDS)

Mechanism of transient increase in intracellular concentration of free calcium ions in HL-60 cell differentiation induced by vitamin D3 and phorbol ester.

A promyelocytic leukemia cell line, HL-60, was induced to differentiate into monocyte-macrophage lineage cells by treatment with active vitamin D3 and phorbol esters, and into granulocyte lineage ones by retinoic acid and dimethylsulfoxide. The changes in intracellular concentration of free calcium ions ([Ca2+]i) were measured and analyzed by calcium-imaging analysis with Fura 2-AM. A significant and transient increase in [Ca2+]i was observed in active vitamin D3 and phorbol ester systems; however, no change was detected with retinoic acid and dimethylsulfoxide. This increase was due to the influx of calcium ions from outside of the cells, and L-type calcium channels were shown to mainly contribute to this influx. Protein kinase C was also shown to be involved in the increase in [Ca2+]i.

[Liver disorders in patients with Mycoplasma pneumoniae pneumonia].

Liver disorders were reviewed in 76 patients with mycoplasma pneumoniae pneumonia (MP) admitted to Kamo Hospital from 1979 to 1989. Liver dysfunction was detected in 16 patients (21%). Liver disorder were detected in 10% of MP from 1979 to 1980, but in 20% from 1984 to 1985, and in 36% from 1988 to 1989. By multiple regression analysis CRP was found to be the most significant factor among other factors such as prevalence, age, sex, history of smoking, history of drinking, and days after onset of fever.
