Gastric vagus mediates immobilization-induced hypocalcemia in rats.

The involvement of the parasympathetic nervous system in the etiology of stress-induced hypocalcemia was investigated in the rat. Atropine methyl bromide (0.1 and 0.6 mg/kg ip) given 20 min before immobilization (IMB) was observed to suppress the induction of hypocalcemia in a dose-dependent manner. A vagotomy of the bilateral cervical trunks also abolished the IMB-induced hypocalcemia. A vagotomy on either the thyroid/parathyroid branches or the celiac branches had no effect on the IMB-induced hypocalcemia, but a vagotomy on the gastric branches completely abolished it. Pretreatment with either secretin (2 and 6 micrograms/kg ip), an inhibitor of gastrin release, or cimetidine (5 and 10 mg/kg ip), a histamine H2-receptor antagonist, diminished the IMB-induced hypocalcemia. The concentration of serum gastrin increased significantly during IMB. It is thus concluded that the decreased levels of plasma calcium caused by IMB are due to the activation of the vagus innervating the stomach. Gastrin and histamine are also involved as a consequence of the activation of the vagus.

Exogenous basic fibroblast growth factor and nerve growth factor enhance sprouting of acetylcholinesterase positive fibers in denervated rat hippocampus.

Basic fibroblast growth factor (bFGF) and nerve growth factor (NGF) were administered into the rat brain following unilateral fimbria-fornix transection. Both bFGF and NGF stimulated the sprouting of acetylcholinesterase (AChE) positive fibers in the hippocampus on the lesioned side. Furthermore, a small number of AChE-positive fibers were regenerated even when only the vehicle was administered. Rats treated with NGF as well as control group had only thin fibers, whereas those treated with bFGF had not only thin fibers but also thick fibers. These results indicate that intrinsic NGF is released and acts on damaged neurons directly, while bFGF acts them on directly and/or indirectly after brain injury.

[An aspiration pneumonia in acute airway damage model induced by HCl and/or LPS].

The purpose of this study is to evaluate chemical and pathological changes of the lung and to elucidate the role of TNF alpha and elastase in acute lung injury induced by HCl or lipopolysaccharide (LPS). Anesthetized rats were injected with pH 1.4 0.7 ml/kg body weight of HCl and 0.5 mg/kg body weight (BW) of LPS (E. coli) into the lung. Acute tracheal injury model (Mendelson Syndrome) were made. Control animals received only saline. Animals were sacrificed 1, 6, or 12 hours after the HCl or LPS or HCl and LPS injection, bronchoalveolar-lavage (BAL) was performed in the same way in control and experimental groups. The other animals which were treated as well were excised by histology. There was neither increase in TNF alpha-production nor increase in neutrophils resulting from HCl injection only. Elastase-like activity was not detected in animals treated only with HCl. However, 1 hour after LPS injection, the production of TNF alpha (37.0 +/- 8.0 Units/ml) was significantly greater than that of the control group (12.1 +/- 4.2 Units/ml) in BALF. Six hours after HCl and LPS injection, the concentration of elastase-like activity (0.023 +/- 0.002 nM) was significantly greater than that of the LPS group (0.011 +/- 0.001 nM). Only patches of intraalveolar hemorrhage and elevation of fibrin was observed in the HCl injected rats at 1 hour after injection. Six hours after LPS injection, the alveolar spaces were filled with large amounts of neutrophils. These findings suggest that TNF alpha and elastase play a significant role in HCl and LPS-induced acute lung injury.

[Quantitative histopathological study on the adriamycin testicular toxicity in rats].

Testicular toxicity induced by adriamycin (ADR) was studied using quantitative histopathological methods. Nine-week old male rats were received a single dose of 8 mg/kg ADR. At 3, 7, 14 and 21 days after the treatment, animals were sacrificed for histopathological evaluation. Numbers of seminiferous epithelia were counted in seminiferous tubule of spermatogenic stages I, II, V, VII and XII. A decrease in spermatogonia was observed in seminiferous tubules of stages I, II and V at 3 days after the treatment. Thereafter, numbers of other types of seminiferous epithelia decreased following progress of spermatogenic cycle. Analysis of spermatogenic cycle movement and germ cell differentiation suggested that ADR was highly effective in killing spermatogonia type A, and the severity of ADR damage would be different depending on subtypes of spermatogonia type A and spermatogenic stages.

[Torsion of the middle lobe after right upper lobectomy: a case report].

Following right upper lobectomy for a pulmonary carcinoma, torsion of right middle lobe occurred. There have been few reports in the literature of pulmonary torsion after lobectomy. A 68-year-old man had a right upper lobectomy for pulmonary carcinoma. On the seventh day after right upper lobectomy, he got high fever. At the same time a roentgenogram of the chest showed complete opacification like ground-glass in the upper lung field. Gradually the opacification increased and hemosputum also appeared. Bronchoscopy revealed 90-degree torsion of the middle lobe. In the treatment with respirator (CV 2000), his clinical conditions improved.

Congenital cataract in the right eye and primary clinical anophthalmos of the left eye in a patient with cerebellar hypoplasia.

We examined a 26-year-old man who had an opaque lens OD and clinical anophthalmos OS. Low densities in the left cerebellar hemisphere and vermis and dilated ventricles were noted on a computed tomographic scan of his brain. We believe that the association of developmental cataract OD, clinical anophthalmos OS, and cerebellar hypoplasia in our patient may be rare.

[A study of first and second line chemotherapies in gestational choriocarcinoma].

Twenty-eight patients with choriocarcinoma have received the three kinds of combination chemotherapy since 1983 at our department, i.e., MOA consisting of moderate dose methotrexate (MTX), actinomycin-D (Act-D) and vincristine, MEA (moderate dose MTX, Act-D and etoposide) and FA (high dose 5-Fluorouracil and Act-D). The clinical and laboratory data obtained in the 28 patients were summarized as follows; 1. The MOA regimen was administered to 4 patients primarily and to 2 secondarily. All of the 6 patients attained remission, but finally two (33.3%) developed relapse. 2. The MEA regimen was administered to 12 patients primarily and to 12 secondarily. Of the 24 patients, five (20.8%) were found to be resistant to the MEA regimen. Nineteen patients (79.2%) attained remission, but 2 (10.5%) developed recurrence. 3. The FA regimen was attempted in one patient primarily and in 6 secondarily. Although one patient died, the remaining 6 achieved remission and one relapse has been observed in the 6 cases. 4. By applying the above mentioned 3 combination chemotherapy regimens, the overall survival rate was pushed up from 64% to 90% in choriocarcinoma patients. 5. Three patients finally died of the disease but not from the side effects of the combination chemotherapies. The major adverse effects were alopecia, nausea, vomiting and myelosuppression. In particular, serious myelosuppression was caused by the MEA or FA regimen in 5-7% of all chemotherapy courses.

Morphometric analysis of subchondral bone of the tibial condyle in osteoarthrosis.

Tibial condyles in 11 patients with knee joint osteoarthrosis were analyzed by subchondral bone morphometry using 11 static parameters. The results were evaluated by comparison with the grade of joint cartilage degeneration in relation to the external, intermediate, and internal areas of the tibial condyle. The bone morphometric data also were analyzed according to the depth of layers, from the surface to the depth of 2000 microns of the bone. A parallel relationship between the bone volume/bone formation activity and the Mankin grade of cartilage degeneration was noted in both medial and lateral condyles. Bone resorption activity in the medial condyle was found only in the external and intermediate areas; it was extremely low in the lateral condyle. Throughout the observation areas, the values of bone volume in relation to depth were highest in the superficial layer with a gradual decrease as the depth increased. The bone formation activity was highest in the superficial layer in the lateral condyle, whereas in the medial condyle, it was highest in the layer between 1000 and 1500 microns in depth. The bone resorption activity showed no variations according to the depth. The progression of joint cartilage degeneration was associated with remodeling of the underlying subchondral bone.

[Cavernous lymphangioma of mediastinum--a case report].

A 2-year-old girl with mediastinal cavernous lymphangioma was successfully treated in 1991. She had a cough and an abnormal shadow in the right mediastinum was shown on her chest X-ray. The tumor, which spread from the right upper mediastinum to the pre-aorta, was demonstrated on CT and MRI. On August 8, 1991, the operation was performed through the right thoracotomy. The tumor was located in the right-upper and middle mediastinum and involved N. phrenics and vagus, but didn't invade and was completely resected. It was 4.2 x 3.4 x 3.2 cm in size and the cut surface of the tumor had an appearance consisting of the soft solid tissue, but containing partial cystic spaces. Histological diagnosis was cavernous lymphangioma. The mediastinal cavernous lymphangioma has been reported in only 6 cases in the Japanese medical literature.

[The effect of etoposide on ovarian function in patients with gestational trophoblastic disease].

Thirty-four patients with low risk gestational trophoblastic disease were treated with etoposide alone. Within 60 days after treatment, synthetic luteinizing hormone releasing hormone (LH-RH: 100 micrograms) was injected intravenously. The serum concentrations of LH, follicle-stimulating hormone (FSH) before and 30, 60, 120 minutes after LH-RH administration, estradiol (E2) and progesterone were measured. In addition, the basal body temperature (BBT) was taken continuously to evaluate ovulation. (1) The patients aged 40 or more in whom serum LH and FSH levels were higher before LH-RH administration, showed excessive response to LH-RH. These excessive responses were 45.5% and 7.1% in patients in their thirties and twenties, respectively. (2) Serum E2 and progesterone levels decreased with advanced age, which was not affected by total doses of etoposide. (3) Seven (77.8%) patients over 40 years were found to have anovulatory cycles, based on their BBT, and ovulation was confirmed in all the patients under 40 years. (4) Eleven pregnancies were confirmed. Nine delivered apparently healthy infants. The influence of etoposide on ovarian function was transient except for patients over 40 years old.

[The relation between the anaerobic threshold and exercise-induced myocardial ischemia in patients with ischemic heart disease].

The aim of this study was to clarify whether the anaerobic threshold (AT) in patients with ischemic heart disease is determined by exercise-induced myocardial ischemia. A) The reproducibility of the VO2 at the AT (AT VO2) were studied. In 13 patients with exercise-induced myocardial ischemia, submaximal Treadmill exercise tests were performed twice using a cardiopulmonary monitoring system. The reproducibility of the AT VO2 was good (r = 0.92), and the mean +/- SD was -1.3 +/- 8.2%. B) The change of the AT VO2 after percutaneous transluminal coronary angioplasty (PTCA) was studied. In 30 patients who underwent successful PTCA, submaximal Treadmill exercise tests were performed before and after PTCA using a cardiopulmonary monitoring system. After PTCA both the exercise duration and the peak VO2 increased significantly (569.0 +/- 200.8 sec vs 681.9 +/- 206.9 sec, p < 0.001: 19.5 +/- 3. 4ml/min/kg vs 21.3 +/- 3.7ml/min/kg, p < 0.001). On the other hand, the AT VO2 did not increase (13.7 +/- 3.0 ml/min/kg vs 13.9 +/- 3.2ml/min/kg, NS). The significant increase of the AT VO2, more than 15.1%, was recognized only in 5 patients. Neither did the AT VO2 increase even in patients without hibernating myocardium. In conclusion, there are many cases in which AT is not determined by exercise-induced myocardial ischemia.

[A case of gastric cancer with multiple liver metastases showing marked efficacy of preoperative and postoperative chemotherapy with CDDP, MMC and UFT(PMU), or CDDP, MMC, UFT and etoposide (PMUE)].

The patient, a 65-year-old male with far advanced gastric cancer of H3N4 (Stage 4), was assumed inoperable on admission and chemotherapy using CDDP, MMC and UFT(PMU) was carried out. As a result, the levels of AFP and CEA were reduced notably, and PR effects were recognized in liver and lymphnode metastatic lesion. Thus, 2 months later, reduction surgery was performed, during which primary lesion was resected and a reservoir tube for chemotherapy was placed in the common hepatic artery. Subsequently, the chemotherapy with Etoposide added to PMU(PMUE) was continued by utilization of a reservoir, so that liver metastasis decreased more than 90% from the maximum. However, metastasis lesions of left lobe of the liver had enlarged with reincrease of AFP and CEA since 6 months after the operation. A month later left lobectomy of the liver was performed. Residual metastases of the liver were then enlarged. PMUE with Ca antagonist was used with little effect then. The patient died of liver failure 15 months after initial admission.

The evidence for post-meiotic expression of a testis-specific isoform of a regulatory subunit of calcineurin using a monoclonal antibody.

The expression of a regulatory subunit of calcineurin (CaN beta) during rat spermatogenesis was examined in rat testes using a monoclonal antibody Va1. Results showed that a testis-specific isoform of CaN beta was expressed only 3 weeks after birth, when meiosis begins, and increased in amount depending on the maturation of spermatogenesis. The matured sperm, which consists of only post-meiotic cells, is most likely to have only the testis-specific isoform of CaN beta. The brain type isoform of CaN beta was not detected in rat sperm. Immunoblot analysis of testes from different rodent species by a monoclonal antibody Va1 showed that all rodent species examined had their own homologues corresponding to a testis-specific isoform of CaN beta in rats, although they showed distinctively different molecular weights on SDS-PAGE compared to the testis-specific isoform in rats. Each homologue was shown to be specifically expressed in post-meiotic phase of spermatogenesis, as was seen in rats.

Effect of semi-random mutagenesis at the C-terminal 4 amino acids of human interleukin-6 on its biological activity.

The carboxyl(C)-terminus of human interleukin-6 (hIL-6) has a critical role in the expression of the biological activity of this cytokine. To define the structure-function relationships of this region, semi-random mutagenesis of the C-terminal Leu181-Arg182-Qln183-Met184 sequence of hIL-6 was performed. The mutants were produced in Escherichia coli, renatured, and purified. Alterations of the C-terminal 4 amino acids caused a significant reduction of the proliferative effect of the mutants on MH60.BSF2 and KT-3 cells, and also led to a drastic decrease in receptor binding affinity. These results suggest the importance of a positively charged residue at position 182 or 183 and an alpha-helix at position 181 for the biological activity of hIL-6.

Identification of a src family protein specifically expressed in rat astrocytes by immunohistochemistry and double immunofluorescent study.

Identification of a src-related tyrosine kinase and its regional and cellular distributions were studied in rat brain. The study was performed using a specific antibody raised against a synthetic peptide corresponding to the conserved autophosphorylation site of src-family tyrosine kinases. The antibody (alpha-src antibody) recognized a 43 kDa polypeptide in plasma membrane enriched fraction. The immunohistochemical data revealed that the polypeptide is localized in astrocytes of corpus callosum and fimbria hippocampus. The presence of this src-related protein in astrocytes suggests that it may have some control in their proliferation and differentiation.

In vivo microdialysis of amino acid neurotransmitters in the hippocampus in amygdaloid kindled rat.

Extracellular concentrations of gamma-aminobutyric acid (GABA), glutamate (Glu) and aspartate (Asp) were determined by microdialysis in rat hippocampus during various amygdaloid kindled stages. The values of GABA and Glu were increased 3-4 times in C2-C3 stages in comparison with the values in control animals. After reaching the C5 stages, these values were increased 3-7 times. However, the concentration of Asp decreased depending on the kindling stage, reaching the lowest value of 33% in comparison with the normal value. The observed changes may be related to kindling induced seizures.
