Lugol staining pattern and histology of esophageal lesions.

To analyze the relationship between Lugol unstained areas and their histologic features, we applied the Lugol test to 24 specimens of resected esophagus. The staining patterns were graded into four types: grade I, hyperstaining; grade II, normal greenish brown staining; grade III, less intense staining; and grade IV, unstained. Most of the grade IV lesions were invasive carcinomas, carcinomas in situ, or severe dysplasia. The carcinomas in situ and the intraepithelial extension of the carcinomas, which were difficult to detect, were clearly shown as grade IV. On the other hand, moderate to mild dysplasia or atrophy showed grade III staining. Grade IV lesions showed well-demarcated sharp margins, whereas grade III lesions showed ill-demarcated dull margins. The grade III carcinomas, however, by the Lugol test, showed well-demarcated margins. Histologic evaluation disclosed that the staining intensity reflected well the thickness of the glycogen-containing cell layer in the lesion. The sharpness of the margin reflected the abrupt or gradual change from the glycogen-containing to non-containing cell layers. These findings suggest 1) the usefulness of the staining pattern of the Lugol test for the diagnosis of esophageal lesions such as squamous cell carcinoma and severe dysplasia, and 2) the usefulness of the Lugol test for precise delineation of the proximal resection line during surgery of esophageal carcinomas with unexpected wide extension.

Hyperthermo-chemo-radiotherapy for patients with early carcinoma of the esophagus.

Hyperthermia combined with irradiation and chemotherapy (hyperthermo-chemo-radiotherapy) was prescribed pre-operatively for 21 patients with submucosal carcinoma of the esophagus, and its effectiveness was evaluated by a comparison with patients who also had massive submucosal carcinoma, but received no pre-operative treatment (n = 12). There was no difference in the survival rates of these groups (hyperthermo-chemo-radiotherapy group: 3-year survival in 56.6%), but the patients who responded well to the HCR therapy (n = 6) demonstrated better survival rates (3-year survival in 83.3%) than did the patients without any pre-operative treatment (3-year survival in 33.9%). An HCR sensitivity test using the succinate dehydrogenase inhibition test was devised and showed good correlation with the clinical remedial values (the diagnostic specificity was 0.89). An accurate diagnosis of the esophageal lesion and appropriate selection of potentially treatable cases are considered essential for the optimal use of HCR therapy in patients with submucosal carcinoma.

Efficacy of sulthiame (Ospolot) on motor partial seizure status during sleep in a patient with motor cortex epilepsy.

An 18-year-old female patient with a particular form of intractable motor cortex epilepsy, in which motor partial seizure status occurred only during sleep every night, was reported focusing on the drastic efficacy of sulthiame on the seizure status. Moreover, the present study demonstrated that single photon emission computed tomography (SPECT) and pulse oximetry examinations, both of which were performed in the ictal state, were useful for the regional diagnosis of the epileptic focus and observations of the seizure frequency, respectively. The therapeutic effect, epileptic picture and clinical examinations described here appear to be relatively rare in the literature, and therefore, this case report may be of clinical significance.

[Clinical study of S-1108 fine granule in the pediatric field].

S-1108 in a fine granular form was administered in 14 children and its safety and efficacy in bacterial infections were evaluated. Among them, 2 cases of cystitis and 1 case of pneumonia were considered unevaluable for the efficacy. The results obtained are summarized as follows. 1. The overall clinical efficacy rate was 81.8% in the eleven evaluable cases treated with S-1108 fine granules including 5 cases of pharyngitis, 2 cases each of tonsillitis, pertussis and cystitis. 2. Bacteriological efficacy of 100% was achieved against pathogens identified in 5 children including 1 case each of Staphylococcus aureus, Streptococcus pyogenes and Haemophilus influenzae and 2 cases of Escherichia coli. 3. The only abnormal laboratory test results observed were eosinophilia and leukocytopenia in one case each. Diarrhea was recorded in 1 case. Judging from the above results, it appears that S-1108 in the fine granular form is an effective, useful and safe antibiotic of first choice for the treatment of infections in the pediatric field.

The role of prenatal diagnosis in congenital diaphragmatic hernia.

Prenatal diagnosis of surgically correctable fetal anomalies is increasing with the improvement of ultrasonographic techniques. In spite of theoretical advantages, the mortality rate in cases of CDH presenting in the early hours of life has remained high. We have experience of two consecutive cases of CDH, diagnosed prenatally. With maternal transport, planned delivery by Cesarean section, immediate resuscitation of the babies after birth, and prompt surgical intervention by pediatric surgeons, the babies were successfully managed even though they had severe hypoplasia of the left lung. Other reported cases of CDH, have died in the neonatal period mainly from associated lethal anomalies and contralateral lung hypoplasia and in spite of prenatal diagnosis and early surgical intervention. Most of these cases belong to group 3 in Ein's classification. Cases belonging to group 3 are virtually impossible to save with current medical techniques. The effects of prenatal diagnosis on the survival rate in CDH have been discouraging in those series which include these types of cases. The advantage of prenatal diagnosis and early surgical intervention are obvious in groups 1 and 2. Our cases show the importance of cooperation between obstetricians, neonatologists and pediatric surgeons in the prenatal and postnatal management of babies with CDH.

Pravastatin administration to hyperlipidemia in patients with nephrotic syndrome.

We treated hyperlipidemia in patients with nephrotic syndrome (NS) using pravastatin for more than one year. There were 7 cases consisting of 3 with minimal change nephrotic syndrome (MCNS), 1 with focal glomerulosclerosis (FGS), 1 with proliferative glomerulonephritis (PGN), 2 with membranous glomerulonephritis (MGN) and 1 of unknown origin. Three cases consisted of frequent relapsers, and 4 were steroid-resistant. Six randomly selected age- and sex-matched nephrotic patients were used as controls. The daily excretion of proteinuria was not decreased after pravastatin treatment, but, the serum albumin rose from 3.1 +/- 1.1 to 3.4 +/- 1.0 mg/dl. The serum total cholesterol level was significantly reduced from 401 +/- 174 mg/dl to 331 +/- 103 mg/dl in spite of an absence of marked change in the control group. However, there were no significant changes in the triglyceride and lipoprotein levels. The atherogenic index was 7.1 +/- 3.7 before and 2.8 +/- 1.7 after pravastatin treatment, respectively. Improvement of renal function defined by delta decrement of renal function per year (0.08 +/- 0.06 vs. 0.12 +/- 0.26) was observed after the discontinuation of pravastatin administration in 3 out of 4 intractable cases. We conclude that pravastatin has a potent effect in reducing the serum level of total cholesterol, but not triglyceride in NS. Furthermore, pravastatin can induce renal dysfunction especially in patients with intractable nephrotic syndrome.

Impaired sensory conduction in a mixed nerve induced by high frequency stimulation of the muscle fibres. An experimental study in cats.

To see if symptoms such as sensory disturbance or numbness worsen during exercise as is thought to occur in entrapment neuropathy, we studied the interaction between sensory and muscle nerve fibres in a mixed nerve in cats. Stimulation electrodes were placed on both the deep (muscle) and superficial (sensory) branch of the radial nerve at the elbow. High frequency stimulation at 100 Hz was applied to one branch for five minutes and the nerve action potential of the other branch was recorded at the axilla where the radial nerve is mixed. The stimulation decreased the amplitude of the nerve action potential and prolonged its latency. Recovery from these changes was gradual, and changes were the same whichever branch was stimulated. When a cat's leg was made ischaemic, the changes were more pronounced. The changes in the nerve action potential may have arisen from ischaemia in the nerve, possibly caused by high frequency stimulation.

[Reconstruction of the pulmonary outflow tract without prosthetic conduit for correction of tetralogy of Fallot with pulmonary atresia].

We reconstructed right ventricular outflow tract without prosthetic conduit for 2 year 11 month and 59 day old Tetralogy of Fallot with pulmonary atresia patients. Left appendage anastomosed between pulmonary trunk and right ventricle was utilized as the posterior wall of the tract. Pericardial patch covered the tract the beneficial methods for patients with Tetralogy of Fallot with pulmonary atresia to avoid late postoperative deleterious complications of prosthetic conduit.

[Anatomic correction without extracardiac conduit for Taussig-Bing malformation (REV procedure)].

A 2-year-old boy with Taussig-Bing malformation with mild pulmonary stenosis underwent anatomical repair without extracardiac conduit (REV procedure). He had pulmonary arterial banding at 4 months of age, and REV procedure was performed at 2 years and 4 months of age. Under cardiopulmonary bypass, enlargement of ventricular septal defect were performed with an internal conduit. Pulmonary arterial trunk was divided and the proximal end was closed. Direct anastomosis of the posterior wall of the distal pulmonary arterial trunk to the right ventriculotomy was carried out without transection of aorta or translocation of pulmonary artery. Right ventricular outflow tract (RVOT) was reconstructed with a patch bearing a monocusp. The postoperative course was very smooth and postoperative cardiac catheterization revealed no stenosis in both left and right ventricular outlfow tract and good cardiac performance.

[Treatment for cardiac rupture following acute myocardial infarction].

We experienced thirty-three patients with cardiac free wall rupture following acute myocardial infarction (AMI). Twelve patients received reperfusion therapy. Surgical closure of cardiac rupture was performed in twelve patients under cardiopulmonary bypass (CPB). Two of these patients were placed on percutaneous cardiopulmonary support system (PCPS) immediately after the onset of blow out rupture. Cardiac rupture occurred in 19 patients within 3 days from the onset of AMI (early rupture) and in 14 patients 4 days after the onset (delayed rupture). All five patients with successful reperfusion were in the early rupture group, while there were no patients with successful reperfusion in the delayed rupture group. In 12 surgical patients, 7 patients had blow out rupture and 5 patients had oozing rupture. In 7 patients with blow out rupture, four patients could not be disconnected from CPB and 2 patients had cerebral death. One patient received PCPS support showed no mental disturbance after surgery, but he died of renal failure 6 days after surgery. Three out of 5 patients with oozing rupture survived. The interval from the onset of hypotension to establishment of cardiopulmonary support was 5 minutes and 60 minutes in two patients using PCPS. However it averaged 94 minutes in 5 patients using usual CPB. It is suggested that early reperfusion therapy may prevent delayed cardiac rupture following AMI, and surgical therapy using PCPS may be useful for the treatment of cardiac blow out rupture.

[Adrenocorticotropin receptor in familial glucocorticoid deficiency].

Recent studies have revealed the presence of four subtypes for the melanocortin receptor (MC-R). Among these MC-Rs, MC2-R is considered to be an adrenocorticotropin (ACTH) receptor because its expression is almost localized in the adrenal cortex. Five Japanese patients with ACTH unresponsiveness were examined as to whether they have mutations in the putative ACTH receptor. Among these patients, there are two groups of siblings, each of which consists of two individuals. The coding region of the ACTH receptor gene was amplified by polymerase chain reaction and directly sequenced on both strands, however, no point mutation was found in any of the five patients, suggesting that familial glucocorticoid deficiency, caused by the mutated ACTH receptor, may be rare.

Evaluation of various mechanical heart values in the mitral and aortic positions.

To evaluate the function of various types of mechanical heart valves (Björk-Shiley = BS, Medtronic Hall = MH, St. Jude Medical = SJM, Duromedics = DM), hemodynamic studies were performed in 236 patients with valve replacement in addition to the evaluation of the clinical results. In 144 patients with mitral valve replacement including patients with double valve replacement, the degree of the postoperative improvements of clinical symptoms and hemodynamics were similar in patients with BS, DM and MH valves. There was a tendency for the effective orifice area calculated from pressure-half time obtained by continuous-wave Doppler (CWD) echocardiography to be larger in MH or DM valves than in BS valve of the same size on the average. In 115 patients with aortic valve replacement including patients with double valve replacement, there was no significant difference in the degree of postoperative reduction of the left ventricular hypertrophy among the patients with BS, MH and SJM valves. The mean value of the pressure gradient (PG) across the valve at rest obtained by CWD was less than 20 mmHg in each type of valve without significant difference among the valves. PG well correlated with cardiac output (CO) in each type and size of valve, and the rate of increase in PG with the increase in CO was smaller in the larger-sized valve of the same type and also smaller in MH valve than in BS valve of the same size. The frequency of valve-related complications was extremely low in each type of valve except for a higher incidence of occult hemolysis in SJM and DM valves. Overall, there were no significant differences in the clinical results among the patients with valvular prostheses examined. However, MH valve had advantages in hemodynamics and a lower incidence of hemolysis.

[Experimental study on heparin-free cardiopulmonary bypass using nafamostat mesilate (FUT-175) and heparin-coated circuit].

We investigated whether the combination of a new synthetic protease inhibitor, nafamostat mesilate (FUT-175), and heparin-coated cardiopulmonary bypass (CPB) circuit can achieve heparin-free CPB in rabbits. Nine rabbits underwent 2 hours of CPB with heparin-coated circuits. In 4 rabbits (FUT-Group), FUT was administered before (2 mg/kg in bolus) and during (5 mg/kg/hr continuously) CPB. In 5 rabbits (Heparin-Group), heparin was given before (4 mg/kg) and at 1 hour (2 mg/kg) of CPB. Activated clotting time, activated partial thromboplastin time and fibrin degradation products showed no significant differences between the two groups during CPB. However, prothrombin time was significantly (p < 0.05) shorter in FUT-Group than in Heparin-Group. Platelet counts in FUT-Group was significantly lower than Heparin-Group. Massive clots were observed in all of the venous reservoirs of FUT-Group after CPB, whereas there were no major clots in Heparin-Group. The occlusion rate of hollow fibers of the oxygenators was significantly higher in FUT-Group (55 +/- 25%, mean +/- SD) than in Heparin-Group (14 +/- 12%). We concluded that heparin-free CPB with FUT and heparin-coated circuit is difficult in rabbits because activation of the extrinsic coagulation cascade cannot be sufficiently suppressed by FUT.

[Primary mediastinal tumors in children--comparison with mediastinal tumors in adults].

During the past 43 years, 628 patients with a primary mediastinal tumor underwent surgical operation in our institution. Of those patients, 106 patients (16.9%) were children of 15 yr of age or less and 522 patients (83.1%) were adults. 47 (44.3%) of the 106 children had neurogenic tumors, while 21 patients (19.8%) had a teratoma. 189 (36.2%) of the 522 adults had thymoma and 89 patients (17.1%) had teratoma. In the 106 children there were 71 benign tumor patients (66.9%) and 35 malignant tumor patients (33.1%). There were 277 (53.1%) benign and 245 (46.95) malignant tumors in the 522 adults. The adults had significantly more malignant tumors than the children (p < 0.01). In our series of malignant mediastinal tumors in children, 14 patients (13.3%) had a lymphoma and 11 patients (10.5%) had a neuroblastoma. On the other hand, in the adults, 127 patients (24.3%) had a thymoma and 66 patients (12.6%) had a lymphoma. Regarding benign mediastinal tumors, in children, 30 patients (28.5%) had a ganglioneuroma and 19 patients (18.0%) had a teratoma. In the adults, 68 patients (13.0%) had a teratoma and 62 patients (11.9%) had a thymoma. In the clinical manifestation, 51 child patients (48.1%) and 191 adult patients (36.6%) were asymptomatic. As symptoms due to compression or direct invasion to adjacent structures, dyspnea was seen in 16 child patients (15.1%), and chest pain occurred in 59 adult patients (11.3%). Dyspnea was significantly more common in children than in in adults (p < 0.01). Emergency operations were performed in children more than in adults.

[Response to exercise in patients after repair of tetralogy of Fallot].

Cardiac catheterization and submaximal exercise testing was performed in 38 patients after repair of tetralogy of Fallot (TF), and compared to 6 control patients who had functional murmurs. Cardiac index, heart rate, and stroke volume index were significantly lower in the TF group than in the control group. Right and left ventricular end-diastolic pressure increased significantly during exercise, which was not found in the control group. Total pulmonary vascular resistance (TPVR), which decreased significantly with exercise in the control group, did not change remarkably during exercise. TPVR was significantly higher in the TF group than in the control group both at rest and during exercise. Several factors were compared between patients with good cardiac index (> 5.0 l/min/m2; Group 1) and poor cardiac index (< 5.0 l/min/m2; Group 2) during exercise. Stroke volume index, right ventricular ejection fraction at rest were significantly higher in Group 1 than Group 2. TPVR, right and left ventricular end-diastolic and end-systolic volume index were significantly lower in Group 1 than in Group 2. There was no significant difference in heart rate, left ventricular ejection fraction, residual pulmonary stenosis, right to left ventricular systolic pressure ratio, and severity of pulmonary regurgitation between two groups. These findings indicate that abnormalities of exercise tolerance in patients after repair of TF were related to poor response of heart rate, pulmonary vascular resistance, and systolic and diastolic ventricular function.

Long-term results of esophagectomy for early esophageal carcinoma.

In this paper we report on the long-term results achieved in 75 patients with superficial carcinoma of the thoracic esophagus who have been treated since 1965 with esophagectomy at our institute. In three of these patients, the carcinoma was limited to the epithelium (intra-epithelial carcinoma), while in 13 cases it was confined to the mucosa, and in 59 cases to the submucosa. There was no lymph node metastasis in patients with intra-epithelial or mucosal carcinoma, but lymph node involvement was found in 13 (22.0%) of the 59 cases of submucosal carcinoma. The five-year survival rate of patients with intra-epithelial carcinoma and mucosal carcinoma was 100%, which was significantly better than that for carcinoma invading the submucosa (49%). The five-year survival rate for patients with lymph node involvement was only 46%, which was significantly worse than that for patients without lymph node involvement (60%). No recurrence occurred in patients with intra-epithelial carcinoma or mucosal carcinoma, while recurrent disease occurred in 19 (32.2%) of the 59 patients with submucosal carcinoma. Lymph node recurrence and hematogenic recurrence such as lung and bone metastasis were the most frequent developments in patients with submucosal carcinoma of the esophagus.

How to detect early carcinoma of the esophagus.

In order to identify the most effective approaches for detecting early carcinoma of the esophagus, 46 patients with such lesions, including three epithelial, 13 mucosal and 30 submucosal carcinomas, were studied. Among 16 patients with epithelial and mucosal carcinomas, five (31%) were symptomatic, and in 15 (94%), the lesions were detected by endoscopy. Among 30 submucosal carcinomas, 17 (57%) were associated with symptoms such as mild dysphagia and a sharp pain, and 22 (73%) were detected by endoscopy. With respect to the gross appearance of early esophageal carcinoma, a protruded type frequently caused symptoms (70%) and was often detected by radiology (50%). Although endoscopy always permitted a correct diagnosis of the lesion (100%), radiology often failed to detect it (47%), especially when the lesion was of the superficial type. In asymptomatic patients, most early esophageal carcinomas were detected during the follow-up of gastric diseases, mass-screening or medical examination for gastric cancer, and in connection with abdominal pain due to other diseases. These results indicate that, in order to detect early carcinoma of the esophagus, it is important to perform Lugol-combined endoscopy with biopsy rather than radiology.
