[Cytolysosomes in red blood cells of patients with Behçet's disease at the exacerbation stage].

Peripheral erythrocytes of 5 patients with Behet's disease, not receiving systemic treatment, at the exacerbation stage, and those of 3 healthy controls were investigated with a transmission electron microscope. More than 100 cells were examined per subject. Cytolysosomes (autophagic vacuoles) were frequently observed in cells of the patients. The average frequency of appearance of cytolysosomes was 5.4 +/- 1.5% (mean +/- deviation) in erythrocytes of the 5 patients, and 0.5 +/- 0.4% in those of the 3 control subjects. The difference was statistically significant (p < 0.026). These results suggest that cytolysosomes of erythrocytes may reflect abnormal production or removal involved in the pathophysiology of Behçet's disease.

Optic neuropathy and acute transverse myelopathy in primary Sjögren's syndrome.

A case of Sjögren's syndrome with optic neuropathy and acute transverse myelopathy mimicking multiple sclerosis is described. The patient suddenly lost her visual acuity resulting in a permanent visual deficit in spite of the massive steroid therapy given. During the treatment the patient showed dry eye symptoms and was finally diagnosed as having primary Sjögren's syndrome. It is suggested that primary Sjögren's syndrome should be considered in the differential diagnosis of neurologic disorders that resemble multiple sclerosis.

[Quantitative changes in cerebral blood flow of Shy-Drager syndrome induced by upright position: assessment by 99mTc-HMPAO].

We assessed orthostatic changes of cerebral blood flow (CBF) in 4 patients with Shy-Drager syndrome. Brain perfusion index (BPI) was measured by 99mTc-hexamethylpropylene amine oxime (99mTc-HMPAO). Cerebral angiography using aortic time-activity curve as an input function BPI values in supine and upright positions in Shy-Drager syndrome were compared with those in 2 cerebrovascular disease patients without orthostatic hypotension. BPI values in the upright position were significantly decreased in patients with Shy-Drager syndrome as compared with these in the supine position. No significant decrease in BPI was noted in the patients without orthostatic hypotension. These results suggest that the BPI measurement by 99mTc-HMPAO cerebral angiography is useful for the noninvasive evaluation of the orthostatic changes of the CBF.

The pathogenic role of Bacteroides vulgatus in patients with ulcerative colitis.

To clarify the role played by Bacteroides vulgatus in the pathogenesis of ulcerative colitis, the serum antibody titer to the outer membrane protein of B. vulgatus, obtained from the inflamed rectal mucosa, and measured by enzyme-linked immunosorbent assay, was investigated in patients with ulcerative colitis. High serum antibody titers to the outer membrane protein of B. vulgatus were found in these patients. The specific outer membrane protein of B. vulgatus reactive to serum antibody was recognized as a 26-kDa protein. These findings indicate that B. vulgatus containing a 26-kDa protein in the outer membrane may be implicated in the pathogenesis of ulcerative colitis.

Visual function in patients with optic neuritis associated with acute transverse myelopathy in multiple sclerosis.

The authors reviewed the records of 20 patients with optic neuritis, all of whom were diagnosed as having clinically definite multiple sclerosis (MS). They were classified into two subgroups: Group A, consisting of 9 patients who had shown acute transverse myelopathy (ATM); and Group B, 11 patients without ATM. Four patients (44%) in Group A had complete visual loss, but none in Group B. Six patients (67%) in Group A had less than 0.1 visual acuity in the affected eye, but only 2 patients (18%) in Group B. Four patients in Group A showed evidence of anticardiolipin antibodies. While both groups were diagnosed as having clinically definite MS, there were differences between them in the clinical features. We assume that the patients with ATM may constitute a different subgroup among MS patients.

In vitro antibacterial activity of FA103, a new quinolone derivative of C-7 position with 7-perhydrodiazepinone.

A number of new quinolone antibacterial agents such as ofloxacin, norfloxacin, ciprofloxacin and sparfloxacin have been developed and introduced to the market. They possess a broad spectrum of activity against Gram-positive and Gram-negative bacteria. Ciprofloxacin has the highest activity against Gram-positive is higher than other quinolones. The activity of these quinolones against Gram-negative bacteria is generally higher than against Gram-positive bacteria. The substitution group of quinolones at C-7 position is responsible to show similar antibacterial activity with broad spectrum and similar pharmacokinetic properties, and variety of the substituents have been synthesized in many laboratories. Most of the substituents are piperazinyl of six-membered ring or pyrrolidinyl of five-membered ring, being modified with an alkyl group or another group. The development of potent quinolones against bacteria involved in pneumonia was seemed to be useful, and we investigated structure-activity relationships of new quinolones with a stronger activity against Gram-positive bacteria. A quinolone derivative with a seven-membered ring, perhydrodiazepinone, at the C-7 position was found to be a candidate for further evaluation. No previous attempts have been made to synthesize this type of derivatives. The compound FA103, 5-amino-1-cyclopropyl-6,8-difluoro-7-(2,3,4,5,6,7-hexahydro-1 H-1,4-diazepin-5-on-1-yl)-1,4-dihydro-4-oxoquinoline-3-carboxylic acid, was synthesized and evaluated. Chemical structure of FA103 is shown in Fig. 1. This is a new difluoroquinolone with a broad antibacterial spectrum and improved activity against Gram-positive bacteria. In this paper, we compare the in vitro activity of FA103 with that of ofloxacin, norfloxacin, sparfloxacin against Gram-positive and Gram-negative bacteria.

Participation of non-major histocompatibility antigen complex gene in experimental autoimmune uveitis.

It is known that the susceptibility to experimental autoimmune uveitis (EAU) is controlled by both major histocompatibility antigen complex (MHC) and non-MHC genes. In this report, we studied the role of non-MHC genes in the induction of EAU. LEW, WKAH, (LEW x WKAH)F1, and (LEW x WKAH)F2 rats were examined to determine the incidence and severity of EAU induced by immunization with bovine S-antigen (S-Ag). We found that all the LEW rats developed severe EAU within 2 weeks after immunization, but no WKAH rats did. Most of the F1 rats developed mild inflammation in 3-4 weeks after immunization. A quarter of the F2 rats developed EAU within 2 weeks, half of them, in 3-6 weeks after immunization, and the others did not. These findings suggest that only one non-MHC gene controls the susceptibility to S-Ag-induced EAU in rats.

Poor response of cultured mast cells derived from mi/mi mutant mice to nerve growth factor.

Decreased numbers of mast cells and abnormalities in the phenotype of mast cells are observed in the skin of mi/mi mutant mice. Recently, the mi locus was identified to encode a novel member of the basic-helix-loop-helix-leucine zipper protein family of transcription factors. Since nerve growth factor (NGF) has been reported to influence the proliferation and the phenotype of cultured mast cells (CMCs), we compared the effect of NGF between mi/mi and control normal (+/+) CMCs. Addition of NGF to the suboptimal dose of recombinant murine interleukin-3 (rmIL-3) increased the plating efficiency of +/+ CMCs, but not of mi/mi CMCs. Although +/+ CMCs were berberine sulfate-negative when cultured with rmIL-3 alone, +/+ CMCs became berberine sulfate-positive when cultured in the presence of both rmIL-3 and NGF, which suggests increased heparin content. In contrast, NGF did not influence the phenotype of mi/mi CMCs. +/+ CMCs significantly bound 125I-NGF, but mi/mi CMCs did not, which suggests a defect of NGF receptors in mi/mi CMCs. Both p75 and p140 molecules are known to be involved in the formation of NGF receptors. Although the expression of p140 messenger (m)RNA was comparable between +/+ and mi/mi CMCs, the expression of p75 mRNA was significantly lower in mi/mi CMCs than in +/+ CMCs. Taken together, the poor response of mi/mi CMCs to NGF appeared to be attributable to the impaired transcription of the p75 gene.

Effects of transfected complement regulatory proteins, MCP, DAF, and MCP/DAE hybrid, on complement-mediated swine endothelial cell lysis.

We established several swine endothelial cell (SEC) lines, expressing human MCP (CD46), DAF (CD55), and MCP/DAF hybrid by transfection of cDNA, and assessed the function of these transfectant molecules on complement-mediated cell lysis as an in vitro hyperacute rejection model of swine to human discordant xenograft. Discordant organ xenografts are hyperacutely rejected by complement activation. Amelioration of complement-mediated lysis by these transfectant molecules was tested in each SEC line by lactate dehydrogenase assay. Naive swine endothelial cells were markedly damaged by human complement mainly via the classical pathway, activating only minimally the alternative pathway of human complement. Both MCP and DAF protected SEC from human complement attack in parallel with the expression density, with DAF being more effective than MCP. The MCP/DAF hybrid was more effective than MCP alone, and as effective as DAF in this system. The results suggest that the transfection of DAF or the MCP/DAF hybrid cDNA into organs to be transplanted could protect against hyperacute rejection.

The identification of Escherichia coli ispB (cel) gene encoding the octaprenyl diphosphate synthase.

The gene located upstream of the nlp gene at min 69 on the chromosome of Escherichia coli was cloned from the plasmid pLC7-42 constructed by Clarke and Carbon. The extract of the strain harboring the plasmid containing this gene showed increased activity of the prenyltransferase. The product of the enzyme reaction was analyzed by two systems of the thin-layer chromatography and shown to be the phosphate ester of all-E-octaprenol. These results suggested that the gene was the structural gene for the octaprenyl diphosphate synthase which supplied the precursor of the side chain of the isoprenoid quinones.

Beneficial effects of cyclosporine on reoxygenation injury in hypoxic rat liver.

The effect of CsA on hypoxia-reoxygenation injury was studied in perfused rat livers. CsA did not attenuate hypoxic injury, as assessed by the release of lactate dehydrogenase and mitochondrial aspartate aminotransferase. During reoxygenation, the release of lactate dehydrogenase was also not affected by CsA. However, the release of mitochondrial aspartate aminotransferase into the cytosol, which indicates mitochondrial injury, was significantly reduced by CsA. The effect of CsA on mitochondrial function during hypoxia-reoxygenation was also investigated. CsA administration increased both the respiratory control ratio and the adenine nucleotide content after reoxygenation in both isolated mitochondria and perfused livers. In addition, glucose production by perfused livers after reoxygenation was increased by CsA. We conclude that the beneficial effect of CsA on hypoxia-reoxygenation injury may be partly due to protection of the mitochondria against reoxygenation injury.

Analysis of rejection mechanism in the rat to mouse cardiac xenotransplantation. Role and characteristics of anti-endothelial cell antibodies.

Recent reports have demonstrated that humoral factors, especially antibodies elicited by xenoantigens, play an important role in the rejection of concordant cardiac xenografts. These induced antibodies, however, have not been well characterized. Therefore, we investigated the rejection mechanism, especially the role of humoral immunological responses in the concordant rat to mouse cardiac xenograft model. Lewis rat hearts transplanted into C3H/HeN mice were rejected in 5-6 days. The essential role of humoral factors in the rejection was demonstrated by histological analysis of the rejected hearts showing interstitial hemorrhage, scant cellular infiltration, and the dense deposition of mouse IgG, IgM, and C3 on the graft endothelial cells. In addition, mice that received hyperimmune serum (serum at the 10th day after transplantation) rejected rat hearts hyperacutely. Flow cytometrical analysis using cultured donor rat coronary endothelial cells demonstrated the xenoreactive antibodies of all subclasses, but especially strong reactivity of IgM and IgG2a in the serum at rejection. These xenoreactive antibodies were produced against not only MHC, but also non-MHC antigens on graft endothelial cells. In vivo depletion of L3T4+ T cells led to the suppression of xenoreactive antibody production and the prolongation of graft survival, indicating that antibody production in this model needs L3T4+ T cell help.
