[Assessment of Japanese patients receiving heart transplants overseas].

No heart transplants have been performed in Japan due to various obstacles since the only operation performed in 1968. Since 1981, a number of patients requiring heart transplants have been accepted by foreign transplantation centers in England, the U.S.A., and Germany. This report describes an investigation of the postoperative course of these Japanese heart transplant patients and discusses the problems regarding transplantation in Japan. Of the 21 transplant patients, the diagnosis was dilated cardiomyopathy in 17, restrictive cardiomyopathy in 1, hypertrophic cardiomyopathy in 2, and congenital muscle dystrophy in 1. All patients survived surgery but three died in the long-term period. The causes of death were acute rejection (after 3 months), chronic rejection (after 50 months) and infection (after 30 months). The actuarial survival curve of these patients was 95.0% for one-year survival and 86.4% for three-year survival. The postoperative functional class was NYHA classification I in all patients (100%). Ninety-three percent of patients returned to work. Immunosuppressive therapies included triple drug therapy in 14 patients (66.7%), double drug therapy in 4 (19.0%), ciclosporin alone in 2 (9.5%) and FK506 in 2 (5.0%). The incidence of acute rejection was 1.56 episodes per patient per year within 3 months and 2.9 episodes per patient per year within 1 year. The postoperative courses of Japanese patients who underwent heart transplantation at foreign transplantation centers were satisfactory. These results will encourage heart transplantation in Japan.

[Microsatellite instability in renal cell carcinoma and bladder tumors].

We investigated the reduction in the accuracy of DNA replication and repair (i.e. genetic instability) in urinary tract malignancy using microsatellite regions. The subjects were 17 patients with renal cell carcinoma and 14 with bladder tumors. After polymerase chain reaction (PCR) was performed with FITC-labeled primers, CA repeats were analyzed. The primers used were D2S123, D3S1067, and TP53. Tissue positive for all 3 primers was considered to show a replication error (RER). Genetic instability was found in 5 out of 17 patients with renal cell carcinoma (29%) and 3 out of 14 patients with bladder tumors (21%). Among the bladder tumor patients, 2 were positive for only TP53 and 1 was considered to have RER. Among the patients with renal cell carcinoma, one was positive only for D3S1067 and the other 4 were considered to have RER.

[A case of hemolytic uremic syndrome after cadaveric renal transplantation].

We report a cadaveric renal transplant recipient suffering from hemolytic uremic syndrome (HUS). A 46-year-old woman received a cadaveric renal transplantation on February 16, 1994. From the 20th postoperative day, anemia, thrombocytopenia and deterioration of graft function occurred. Graft biopsy on the 20th postoperative day indicated HUS by microthrombotic arterioglomerulopathy. Plasma exchange and 15-deoxyspergualin improved her graft function. From these clinicopathological findings, we conclude that she had HUS caused by acute humoral rejection.

[MR imaging of bladder tumors using fluid attenuated inversion recovery (FLAIR) technique].

Magnetic resonance imaging (MRI) has been reported valuable for the diagnosis of bladder tumors, but does not provide adequate staging of these tumors. Therefore, we examined whether the FLAIR technique could be used to improve the diagnostic ability of MRI. FLAIR utilizes inversion recovery and can display a lesion as a heavy T2-WI by decreasing the signal intensity of water and thus altering the contrast at the bladder wall. The inversion time (TI), that gave urine a signal intensity of "zero", was determined by theoretical analysis. In addition, MRI by the FLAIR technique was performed on 29 patients with bladder tumors. With the FLAIR technique, MRI showed a sensitivity of 78.6% and a specificity of 91.7% for pT1 or smaller lesions, while the values were respectively 91.7% and 78.6% for pT2 or worse lesions.

[Surgical-treatment for active infective endocarditis].

The results of surgical treatment of active native infective endocarditis in 24 patients were analyzed. Fifteen of 24 (63%) patients were successfully cured by operation. Three of the 8 patients with severe cardiac failure who underwent emergency operation died of low cardiac output syndrome and bleeding early postoperatively. One of the 6 patients with late death had preoperative cerebral embolism and died of multiple organ failure. Five of them had annular infection. Among 6 patients with annular infection, 3 patients who had Staphylococcus epidermidis and Gram negative bacterium as a causative microorganism had a short duration within 4 weeks from the onset to operation. Two died suddenly in the long-term period after surgery because of prosthetic valve detachment. The second operation were required for other 4 patients because of prosthetic valve endocarditis or perivalvular leakage. Three of them were lost from low cardiac output syndrome. These findings suggested that delayed surgical intervention may be a major causative factor resulting in high surgical mortality. For patients with active native infective endocarditis, an early aggressive operation should be considered prior to congestive heart failure and the destruction of the annulus.

[The earlier detection of myocardial damage in open heart surgery using serum human heart fatty acid-binding protein].

Human heart fatty acid-binding protein (HH-FABP), which is a low molecular weight protein and abundant in the cytoplasm of myocardial cells, is reported to be released into the circulation shortly after the onset of acute myocardial damage. However, the changes in serum HH-FABP levels in open heart surgery have not been elucidated. To determine whether HH-FABP enables the earlier detection of myocardial damage caused by ischemia-reperfusion in open heart surgery, we measured the serial levels of serum HH-FABP, CK-MB and Troponin T (TnT) at every 15 min for 48 hours after reperfusion in 10 adult patients with coronary artery bypass graft. The serum HH-FABP levels reached the peak within 60 min after reperfusion (mean +/- SD; 49 +/- 7 min), and this was significantly (p < 0.001) earlier than CK-MB (212 +/- 108 min) and TnT (244 +/- 150 min). The peak value of serum HH-FABP had a significant correlation to the peak value of serum CK-MB or TnT (r = 0.815, p = 0.02; r = 0.925, p = 0.0001, respectively). These results indicate that serum HH-FABP enables the earlier detection of myocardial damage than the other markers in the patients with open heart surgery.

[Coronary artery bypass grafting under bradycardia induced by ultra-short acting beta blocker].

From Dec. 1993 to May, 1994, coronary artery bypass grafting (CABG) was performed in 7 patients under bradycardia induced by an ultra-short acting beta blocker (esmolol). The ages ranged from 51 to 68 years. There was one patient with low ejection fraction (EF = 31%) and two patients with porcelain aorta. A tepid temperature was maintained during cardiopulmonary bypass (CPB). A high flow rate of 2.2-2.6 liter/min/m2 was applied to control perfusion pressure above 50 mmHg during CPB. After CPB was started, a high dose of esomolol was added (10-30 mg/kg intravenous bolus followed by a continuous infusion of 1-4 mg/kg/min). Severe bradycardia was achieved by the initial loading of esmolol. The mean heart rate was significantly (p < 0.01) decreased from 78 +/- 12 bpm to 49 +/- 7 bpm by the loading. Altogether, 25 anastomoses (11 ITA, 6 GEA, 8 SVG) were performed to LAD (10), Cx (7) and RCA (8), with an average of 3.6 +/- 0.9 anastomoses/patient. IABP was required for 2 patients postoperatively. There was no operative death, but one hospital death due to aspiration pneumonia 3 months later. Postoperative max CPK-MB was low (17.4 +/- 9.7 IU/L) in 6 patients. The postoperative angiography was performed in all patients with a patency rate of 88%. It was considered that esmolol facilitated CABG under beating heart and this technique is suitable for patients with severe atheromatous disease of the ascending aorta or patients with a low ejection fraction to avoid aortic cross-clamping.

[Noninvasive regional cerebral blood flow measurements at pre- and post-acetazolamide test using 99mTc-ECD].

A technique for serial noninvasive cerebral blood flow measurements at pre- and post-acetazolamide (Diamox) test was newly developed using 99mTc-ECD without blood sampling. Baseline mean cerebral blood flow (mCBF) was measured from graphical analysis of time activity curves for brain and aortic arch obtained from radionuclide angiography by injection of 370-555 MBq 99mTc-ECD. The first SPECT study was performed immediately after intravenous administration of 1 g of Diamox, then baseline regional cerebral blood flow (rCBF) was calculated using Lassen's correction algorithm. Immediately after the stop of the first SPECT study, additional 555-740 MBq of 99mTc-ECD was administered, thereafter the second SPECT study was started. Post-Diamox SPECT images were obtained by subtraction of the first baseline images from the second images. Using Lassen's algorithm, post-Diamox mCBF was estimated from the baseline mCBF, the baseline mean SPECT counts, and post-Diamox mean SPECT counts corrected for administered dose and imaging time. Post-Diamox rCBF was obtained from the post-Diamox mCBF and the post-Diamox mean SPECT counts using Lassen's algorithm. Coefficient variation was shown 2.7% and 3.5%: mCBF and rCBF, respectively in test-retest results in six patients without Diamox administration. Nine demented patients without vascular disorders showed significant mCBF increase of 35.7% on the average by post-Diamox. In conclusion, this simplified method is practically useful for measuring CBF at pre- and post-Diamox test within short period of time without any blood sample.

Exercise performance of rats after isogenic left or right lung transplantation followed by contralateral pulmonary artery ligation.

BACKGROUND: After single lung transplantation for pulmonary hypertension, large mismatches in ventilation/perfusion distribution persist late after transplantation. The larger graft volume of the transplanted lung after transplantation, the better the exercise performance. Because the right lung is larger in volume than the left, we compared the left single lung transplant with the right single lung transplant regarding exercise performance with an animal model. METHODS: To simulate significant ventilation/perfusion imbalance observed after single lung transplantation for pulmonary hypertension, we transplanted isogenic left or right pulmonary grafts to normal rats, and the contralateral pulmonary artery was ligated 2 weeks after transplantation. The treadmill test was performed weekly until 6 weeks after transplantation to measure maximum tolerated running speed and maximum oxygen uptake. RESULTS: Graft vital capacity of left and right pulmonary grafts were 4.5 +/- 0.43 ml (37% +/- 3.7% of recipient's predicted vital capacity) and 7.8 +/- 0.34 ml (63% +/- 2.4%), respectively (p < 0.01). Maximum tolerated speeds of left and right single lung transplants were 8 +/- 7.6 and 29 +/- 2.2 m/min, respectively, at 6 weeks after transplantation (p < 0.01). Maximal oxygen uptake values of left and right single lung transplants were 34 +/- 12.0 and 65 +/- 3.8 ml/kg/min, respectively (p < 0.01). CONCLUSIONS: Results suggest that right lung transplantation is superior to left lung transplantation for pulmonary hypertension in terms of exercise performance in this animal model.

Radioiodinated (-)-2-[4-(3-iodophenyl) piperidino] cyclohexanol: a potential radioligand for mapping presynaptic cholinergic neurons.

We synthesized the (-)-enantiomer of radioiodinated 2-[4-(3-iodophenyl) piperidino] cyclohexanol ((-)-[125I]-m-iodovesamicol [(-)-[125I]mIV]) and evaluated its in vivo binding specificity and affinity for the vesamicol receptor in the rat brain. Significant amounts (approximately 3% of the injected dose) of (-)-mIV accumulated and retention was prolonged. The accumulation of (-)-[125I]mIV in the brain was significantly reduced by the pre- and post-administration of unlabelled vesamicol (0.5 mumol kg -1). Blocking and displacement studies in vivo showed that the binding affinity of (-)-[125I]mIV for the vesamicol receptor was very high, but that for other receptors was very low. Autoradiographic studies showed that the regional distribution of radioactivity, as revealed by coronal sections of the rat brain, was similar for both (-)-[125I]mIV and (-)-[3H]vesamicol. The results demonstrated that the characteristics of (-)-[125I]mIV for binding sites in rat brain were similar in vivo and in vitro. Radioiodinated (-)-mIV may potentially be useful for studying presynaptic cholinergic neurons in the brain.

Interactions of FK506 (tacrolimus) with clinically important drugs.

The effect of eight drugs on the metabolism of FK506 (tacrolimus) by human liver microsomes was studied at a substrate concentration of 10 microM. NADPH-dependent oxidative metabolism of FK506 was inhibited 20, 15, and 10% by quinidine, omeprazole, and sulindac, respectively, at 100 microM. Theophylline, diclofenac, indomethacin, phenylbutazone, and cimetidine (at ten times molar excess of FK506) and all eight drugs (at equal molar concentration) had slight effects on metabolism. The effects of these drugs were much weaker than that of cyclosporin A. The effect of FK506 on NADPH-dependent oxidation of prednisolone and theophylline by human liver microsomes were also studied. FK506 inhibited prednisolone metabolism in a concentration-dependent manner but exhibited a negligible effect on theophylline metabolism. The results suggest potential for interactions between FK506 and drugs metabolized by the cytochrome P450 3A subfamily.

[A specific disorder in Kanji writing in a learning disabled child with a visual cognition disorder--a cognitive-neuropsychological and electrophysiological analysis].

We reported a learning disabled child with a specific disorder involving Kanji writing. The patient was a 13-year-old boy. Although he manifested generally normal intellectual ability and his performance on WISC-R indicated that VIQ was 101 and PIQ 84, he could only write approximately 20% of Kanji, which he learned at age 6 to 7. On cognitive-psychological tests, he showed a visual cognition disorder that was different from that in a previously reported case during information processing. Electrophysiologically, he showed a disorder in visual task-relevant potentials. The mechanism underlying this patient appears to be a retrieval disorder for complex graphic forms in addition to a visual cognition disorder because of two reasons. One is that he could distinguish Kanji from similar graphic forms. The other is that he could read Kanji aloud.

Familial occurrence of Behçet's disease.

The familial occurrence of Behçet's disease in the 564 Behçet's patients treated in the Uveitis Survey Clinic of the Hokkaido University Hospital between 1967 and 1992 was investigated. Familial occurrence of Behçet's disease was found in 18 patients from 9 families. The male-to-female ratio was 3.5 in the familial cases, which was significantly higher than in the sporadic cases. Familial occurrence was most frequently seen among siblings. Most patients, including all female patients, had the complete type. They had typical ocular lesions, and exhibited bilateral uveoretinitis with poor visual prognosis, particularly the female patients. Frequency of HLA-B5 was 92% in the familial patients, which was much higher than in both the controls and the sporadic cases. It seems probable that patients with familial occurrence of this disease have a poorer prognosis with more susceptible immunogenetic backgrounds.

[Behçet's disease and streptococcal antigens].

Many studies have been carried out concerning microbial infections and the occurrence of Behçet's disease. Recently, attention has been focused on the streptococci. The serum antibody titer and delayed type hypersensitivity of patients with Behçet's disease against streptococcal antigens were significantly higher than those of controls. We also found that the oral flora of patients with Behçet's disease was dominated by Streptococcus sanguis, whose serotypes were different from controls. Recent studies have revealed that some streptococcal antigens are associated with heat shock protein and that human gamma delta T cell lines recognize streptococcal antigens. These results suggest that an autoimmune response might be implicated in the etiology of Behçet's disease. To make clear autoimmune mechanisms of Behçet's disease, we have analysed immunogenic antigens of S. sanguis.

[Na, K-ATPase--its structure, function and intracellular transport].

Na, K-ATPase is an integral plasma membrane protein and plays essential roles such as maintaining sodium and potassium ion gradients across the plasma membrane. The enzyme consists of the alpha and the beta subunits with the stoichiometry of one to one. Three alpha subunit and two beta subunit isoforms have been detected in animal cells with the tissue-specific expression of both subunits. Recent advances in molecular biological studies on the Na, K-ATPase enable us to understand the structure-function relationships and mechanisms of intracellular transport of the enzyme. In this article we review the findings deduced from these studies, especially on the assembly and transport to the plasma membrane of the alpha and beta subunits.

[Repair of total anomalous pulmonary venous return using an ultra-short acting beta blockade (esmolol)--case reports].

In 2 infants with asplenic syndrome associated with total anomalous pulmonary venous return (TAPVR), a repair of TAPVR was done under mild hypothermic cardiopulmonary bypass (CPB) using esmolol, an ultra short-acting beta-blocker to obtain good operative field without aortic-cross clamping. Both cases were weaned from CPB uneventfully. Pulmonary venous obstruction was not found after surgery. This strategy seems to be applicable in pediatric cardiac surgery which is mainly limited to the atrial level.

[In vitro and in vivo antibacterial activities of a new quinolone derivative, FD501].

FD501 is a newly synthesized quinolone derivative with aminoazepine group at the C-7 position. The in vitro and in vivo antibacterial activities of FD501 were investigated comparing with those of norfloxacin, ofloxacin, ciprofloxacin and sparfloxacin. The in vitro antibacterial activities of FD501 against Gram-positive bacteria including methicillin-resistant Staphylococcus aureus, were equal to or higher than those of other quinolones. But its activities against Gram-negative bacteria were equal to or lower than those of other quinolones. Minimum inhibitory concentrations of FD501 against Gram-positive bacteria were similar to that of sparfloxacin. The bactericidal activity may be due to the inhibition of the DNA super-coiling activity of DNA gyrase. The area under the serum concentration-time curve of FD501 in rats following oral administration was larger than norfloxacin and ciprofloxacin and smaller than those of ofloxacin and sparfloxacin.

[Regional cerebral blood flow measurements using 99mTc-ECD SPECT in patients with Alzheimer's disease (ad) in the early stage].

Regional cerebral blood flow (rCBF) measurements using a Patlak plot method and SPECT with 99mTc-ECD were performed in 10 patients with possible Alzheimer's disease (AD), 14 patients with probable AD, and 11 age-matched normal control subjects. The whole brain CBF for the probable group showed significant reduction to 37.2 ml/100 g/min as compared with the normal value of 42.1 ml/100 g/min, whereas the possible group did not show significant reduction of the whole brain CBF. In the possible and probable groups, rCBF values in the bilateral hippocampi, supramarginal gyri, angular gyri were lower than normal values. Moreover, in the probable group, rCBF values showed significant decrease in the superior, middle and inferior temporal gyri. The whole brain CBF and rCBF, especially in the bilateral hippocampal regions, showed significant positive correlation with the scale of MMSE and HDS-R. These results suggest that noninvasive rCBF measurements using 99mTc-ECD are quite useful for the early diagnosis of AD.
