Development of bioactive functions in Hydrangeae dulcis folium. VI. Syntheses of thunberginols A and F and their 3'-deoxy-derivatives using regiospecific lactonization of stilbene carboxylic acid: structures and inhibitory activity on histamine release of hydramacrophyllols A and B.

Lactonization reaction of 2-carboxystilbene mediated by copper(II) chloride proceeded regiospecifically to give the five-membered lactone, while the bromolactonizations using N-bromosuccinimide and anodic oxidation were found to furnish the six-membered lactone. Using these regiospecific lactonization reactions as a key step, antiallergic and antimicrobial isocoumarins and the benzylidenephthalides thunberginols A and F and their 3'-deoxyanalogs were synthesized from phyllodulcin and hydrangenol. Two phthalides called hydramacrophyllols A and B were isolated from Hydrangeae Dulcis Folium and their stereostructures were determined on the basis of physicochemical and chemical evidence, which included the syntheses of hydramacrophyllols A and B from hydrangenol by the application of the lactonization method using copper(II) chloride. In addition, hydramacrophyllols A and B were found to exhibit an inhibitory effect on the histamine release from rat peritoneal exudate cells induced by antigen-antibody reaction.

Bioactive saponins and glycosides. V. Acylated polyhydroxyolean-12-ene triterpene oligoglycosides, camelliasaponins A1, A2, B1, B2, C1, and C2, from the seeds of Camellia japonica L.: structures and inhibitory activity on alcohol absorption.

Acylated polyhydroxyolean-12-ene triterpene oligoglycosides, camelliasaponins A1, A2, B1, B2, C1, and C2, were isolated from the seeds of Camellia japonica L. The structures of six camelliasaponins were elucidated on the basis of chemical and physicochemical evidence. Camelliasaponins B1, B2, C1, and C2 were found to exhibit inhibitory activity on ethanol absorption. By comparison of the inhibitory activities for camelliasaponins with those for desacyl-camelliasaponins, acyl groups such as the angeloyl or tigloyl group were found to be essential to exerting the activity.

Bioactive saponins and glycosides. VI. Elatosides A and B, potent inhibitors of ethanol absorption, from the bark of Aralia elata SEEM. (Araliaceae): the structure-requirement in oleanolic acid glucuronide-saponins for the inhibitory activity.

Potent inhibitors of ethanol absorption, elatosides A and B, were isolated from the bark of Aralia elata SEEM, through bioassay-guided separation together with elatosides C and D and four known oleanolic acid glucuronide-saponins, spinasaponin A, spinasaponin A 28-O-glucoside, and stipuleanosides R1 and R2. The structures of elatosides A, B, C, and D were determined on the basis of chemical and physicochemical evidence as oleanolic acid 3-O-¿[beta-D-xylopyranosyl (1-->2)] [beta-D-galactopyranosyl (1-->3)]¿-beta-D-glucopyranosiduronic acid, oleanolic acid 3-O-¿[beta-D-galactopyranosyl (1-->2)] [beta-D-galactopyranosyl (1-->3)]¿-beta-D-glucopyranosiduronic acid, and their 28-O-glucopyranosyl esters, respectively. The inhibitory effect of various oleanolic acid 3, 28-O-bisdesmosides, oleanolic acid 3-O-monodesmosides, and oleanolic acid on ethanol absorption was examined and it was found that the 3-O-glucuronide moiety and the 28-carboxyl group in oleanolic acid glucuronide-saponin were required to exert the inhibitory activity.

Bioactive saponins and glycosides. VII. On the hypoglycemic principles from the root cortex of Aralia elata Seem.: structure related hypoglycemic activity of oleanolic acid oligoglycoside.

The hypoglycemic component, elatoside E, was isolated from the root cortex of Aralia elata SEEM. (Araliaceae) together with elatoside F and eight known oleanolic acid glycosides, elatosides A and C, oleanolic acid 3-O-[alpha-L-arabinofuranosyl (1-->4)]-beta-D-glucopyranosiduronic acid, oleanolic acid 3-O-beta-D-glucopyranosiduronic acid, stipuleanosides R1 and R2, and chikusetsusaponins IV and IVa. The structures of elatosides E and F were determined on the basis of chemical and physicochemical evidence as oleanolic acid 3-O-[beta-D-xylopyranosyl (1-->2)][beta-D-glucopyranosyl (1-->3)]-alpha-L-arabinopyranoside and its 28-O-beta-D-glucopyranosyl ester, respectively. The hypoglycemic activity of oleanolic acid and nine oleanolic acid oligoglycosides from the root cortex of Aralia elata was determined by monitoring inhibition effect on the elevation of plasma glucose level by oral sucrose tolerance test in rats, and some structure-activity relationships of oleanolic acid glycoside were obtained.

[Studies on Rehmanniae radix. III. The relation between changes of constituents and improvable effects on hemorheology with the processing of roots of Rehmannia glutinosa].

The relation between constituents and effects on hemorheology with processing of root of Rehmannia glutinosa was investigated. With the processing to dried or steamed root from crude root, the content of stachyose, which was the highest component in crude root, was decreased and the content of manninotriose was increased. Iridoid glycoside, catalpol, was gradually decreased with the processing. From these results, the content of carbohydrates and catalpol made it possible to estimate the quality of Rehmanniae Radix by processings. On the other hand, the change of pharmacological activities with the processing was investigated. The 50% ethanolic extract (BJ-ext) from steamed root of R. glutinosa increased erythrocyte deformability and erythrocyte ATP contents, inhibited polybrene-induced erythrocyte aggregation, and promoted an activity of fibrinolytic system. The extracts from crude or dried root had weak or no effect. The crude or dried root and steamed root of R. glutinosa showed the different pharmacological activities, and the quality of Rehamanniae Radix by processings may be estimated by investigation of correlation between the changes of constituents and improvable effects of hemorheology.

Recurrent left atrial myxoma in a patient with a twin fetus pregnancy.

We experienced a case of a recurrent left atrial myxoma in a pregnant woman with twin fetuses. Normal healthy twins weighing 1430 and 1190 gm were delivered by cesarean section performed under epidural anesthesia at 32 weeks gestation. Neither cardiac failure nor embolisms were observed during pregnancy or delivery. The left atrial myxoma was removed surgically one week after delivery. The infants and mother remain in good health. According to previous reports and our present observations, the decision for cardiac surgery during pregnancy should be based on the maternal and fetal status.

Genetic analysis of the sam mutations, which induce sexual development with no requirement for nutritional starvation in fission yeast.

The cAMP pathway and the Ras pathway are the two major pathways to sexual development in the fission yeast Schizosaccharomyces pombe. To understand the cAMP pathway or the related pathway, we analyzed mutants that display a phenotype similar to cyr1-, that is, hyper-sporulation. Nine mutants termed sam (sporulation abnormal mutant), which are highly inclined to sexual development despite the presence of nitrogen sources, were partially characterized. Cyclic AMP was detected in all nine sam mutant cells, and over-expression of the adenylyl cyclase gene (cyr1) failed to suppress the hyper-sporulation phenotype of these sam mutants, suggesting that none of the sam mutants were likely to be allelic to cyr1. Epistatic tests of sam mutants showed that they were divided into two dominant and seven recessive mutants. Dominants were able to make spores in sam/sam+ heterodiploid cells upon abundant nutrients. Both two dominant mutants bypassed the inability to make spores in ras1 deficient diploid cells, suppressed the deficiency to execute sporulation in byr2 deficient diploid cells, but failed to suppress the byr1 deficiency. Two dominant mutations seem not to occur within the byr2 gene.

Two chicken B cell lines resistant to ouabain for the production of chicken monoclonal antibodies.

Two chicken B cell lines, termed MuH1 and MuH4, which are resistant to ouabain, were established as fusion partners for production of chicken monoclonal antibody from the parental cell lines R27H1 and R27H4 which were deficient in thymidine kinase activity. The MuH1 synthesized, but did not secrete mu chain, whereas the MuH4 secreted non-specific IgM. Fusions of the MuH1 and MuH4 with spleen cells from chickens immunized with human IgG resulted in successful preparations of hybridomas secreting antibodies specific to human IgG. Transformed cells insensitive to HAT appeared in many culture wells after cell hybridizations with R27H4, but on hybridization with MuH1 or MuH4 the appearance of such unfavorable cells could be avoided by the inclusion of ouabain in HAT medium. Thus, in comparison with the R27H4 line, the MuH1 and MuH4 lines did not provide higher fusion efficiencies, but gave increased opportunities to obtain hybridomas producing specific antibodies. Among a total of eight hybridomas, four derived from HuM1 and four derived from MuH4, seven secreted both IgG and IgM, and the remaining one derived from MuH1 secreted only IgG. In all eight hybridoma lines IgG was specifically reactive to human IgG.

Central alveolar hypoventilation syndrome (Ondine's curse) with gastroesophageal reflux.

Congenital central hypoventilation syndrome (Ondine's curse) is a rare disorder with lack of automatic control of ventilation during sleep. We have reported a case of Ondine's curse in a patient who underwent Nissen's fundoplication for gastroesophageal reflux (GER) at age 5 months. Ventilatory challenge test during sleep was done to confirm central alveolar hypoventilation. This female patient, without cor pulmonale, was a good candidate for diaphragm pacing. Thus, the patient underwent implantation of a diaphragm pacer at age 3 years; she had required mechanical ventilation since birth. Diagnosis, pathogenesis, and problems in the setting of diaphragm pacing for an infant are discussed.

Williams syndrome associated with chronic renal failure and various endocrinological abnormalities.

A 31-year-old man who had been under regular hemodialysis for 6 months was diagnosed as Williams syndrome (WS) by fluorescence in situ hybridization (FISH) chromosomal analysis. The association of WS and chronic renal failure (CRF) is only rarely encountered. Endocrinological examinations revealed hypergonadotropic hypogonadism. Prolonged and exaggerated responses of adrenocorticotropin (ACTH) to insulin-induced hypoglycemia and corticotropin releasing hormone (CRH) were also noted. While most of the endocrinological abnormalities observed in this patient could be attributed to altered endocrine circumstances in CRF, some findings stand in contrast. Furthermore, the testicular biopsy specimen showed severe hypospermatogenesis. Endocrine disorders observed in this patient may be at least in part, responsible for various clinical features underlying WS.

Nonsurgical management of patients with blunt splenic injury: efficacy of transcatheter arterial embolization.

OBJECTIVE: We evaluated the efficacy of nonsurgical management of patients with blunt splenic injury using detailed angiographic examinations and transcatheter arterial embolization. SUBJECTS AND METHODS: We prospectively studied 228 patients who had blunt abdominal injury and for whom CT was performed. When splenic injury was revealed by CT, angiography was performed in all patients except those requiring emergency surgery. Transcatheter arterial embolization was performed when patients had the following angiographic criteria: (1) extravasation of contrast material extending beyond or within the splenic parenchyma, (2) arterial disruption or major arteriovenous fistula, or (3) both. Splenic function was subsequently estimated by 99mTc-sulfur colloid scintigraphy and repeat angiography. RESULTS: Of 228 patients with blunt trauma, 31 patients had CT evidence of splenic injury. In three of these 31 patients, emergency laparotomy was performed before angiography because of an associated injury or unstable circulatory status. In 13 of the 28 remaining patients, transcatheter arterial embolization was not required as these patients did not meet the necessary criteria. They were treated with bed rest. Transcatheter arterial embolization was performed in the remaining 15 patients and was completely successful in 13. Because one of these 13 patients died of a brain contusion, follow-up angiography and scintigraphy were performed in the remaining 12 patients and showed preservation of splenic function. Nonsurgical treatment of splenic injury with angiography was successful in 93% of patients. CONCLUSION: Our success rate for nonsurgical management of patients with blunt splenic injury should encourage more extensive evaluation and use of angiography for splenic injury and the subsequent management of splenic injury without surgery.

Cytokeratin and proliferative cell nuclear antigen expression in superior limbic keratoconjunctivitis.

PURPOSE: Superior limbic keratoconjunctivitis (SLK) is a disease of unknown etiology which showed various degrees of keratinization. The cytokeratins (CKs) are known to be the hallmark of epithelial differentiations and each CK has a characteristic distribution dependent on the type and the differentiation status of epithelium. The authors have studied the expression of cytokeratins (CKs) as well as proliferating cell nuclear antigen (PCNA) in the conjunctiva of SLK. METHODS: Three superior limbic conjunctivae of patients with SLK and two normal conjunctivae were examined immunohistochemically using monoclonal antibodies against CKs and PCNA. RESULTS: In SLK conjunctivae, increased expression of a basal cell marker (CK14) was observed throughout the epithelium. Sporadic positive staining with CK10, which is a specific marker for keratinization, was correlated to the severity of the disease. The disappearance of CK13 staining, the marker for nonkeratinized stratified epithelia, at the basal cell layer of SLK was noted. Moreover, increased expression of PCNA in SLK was observed. CONCLUSIONS: The altered expression of CKs in SLK suggests an abnormality of differentiation in the conjunctival epithelium of the disease. Upregulated proliferation of conjunctival epithelial cells may be another feature of the disease.

Effect of beraprost sodium (Procylin), a stable prostaglandin I2 analogue, on a dorsal skin flap model in rats.

Beraprost sodium, a chemically stable prostaglandin I2 analogue with antiplatelet vasodilator actions, was tested by comparing the extent of viability of random pattern skin flaps in control rats and rats treated with beraprost sodium. An isolated tail based 2 x 7 cm flap was constructed in 50 Sprague-Dawley rats. In group 1 (beraprost sodium treated group, n = 25) 0.015 mg beraprost sodium were injected intraperitoneally immediately after the flap had been raised. In group 2 (control group, n = 25) the animals were injected intraperitoneally with the same volume of saline immediately after the flap had been raised. At the end of the seventh day the flap survival area was documented using digital planimetry. The difference between the results showed that beraprost sodium had significantly beneficial effect on skin flap survival (p < 0.01, Welch's test).

[Neuroendocrine carcinomas of the head and neck].

Neuroendocrine carcinomas (NECs) of the head and neck are rare. Between 1980 and 1995, only 163 cases of laryngeal and 30 cases of sinonasal NEC had been reported worldwide. We report two cases of sinonasal NEC. Case 1: A 49-year-old man complaining of left nasal obstruction was admitted to our hospital. A tumor with a tendency to bleed was seen in the left nasal cavity. CT scanning revealed an enhanced mass in the left nasal cavity and both maxillary and ethmoid sinuses. An open biopsy was performed to determine the diagnosis. The diagnosis of NEC was based on light microscopic and immunohistochemical findings. Chemotherapy and radiation were performed, but bone, liver and brain metastasis occurred 15 months after diagnosis and the patient died. Case 2: A 59-year-old man, complaining of right forehead swelling visited our department. CT scanning and MRI revealed that the right ethmoid sinus was filled by a tumor which had invaded the base of the skull. An open biopsy was performed. From the results of light microscopic and immunohistochemical investigations, NEC was highly suspected. Chemotherapy and radiation were performed, but the patient died 6 months after diagnosis. This study shows that there are aggressive types of nasal NEC, although these sinonasal carcinomas were reported in the literature to be not very lethal. The diagnosis, histopathological classification, treatment, and prognosis of sinonasal and laryngeal NECs were reviewed.
