Distribution of neuropeptide-containing nerve fibers in the salamander taste organs.

Salamander taste organs were recognized as oval cell clusters within the dorsal surface of the tongue. A moderate number of SP, CGRP, VIP, NPY, and GAL immunoreactive nerve fibers terminated in the cell clusters, and some of them penetrated into the basal half of the cell clusters. Around the glands, VIP, NPY, and GAL fibers were numerous, and SP and CGRP fibers were less numerous. Immunoreactivity of SOM and FMRF was not detected either in the nerve fibers associated with the cell clusters or those around the glands. These findings suggest that the chemosensory mechanisms of the salamander gustatory organs are under the control of peptidergic innervation. In addition, the present study indicates that the caudate taste organs are structurally primitive but functionally mature.

Distribution of VIP receptors in the human submandibular gland: an immunohistochemical study.

Distribution of vasoactive intestinal polypeptide (VIP)-immunoreactive nerve fibers and VIP-receptor (VIP-R)-immunoreactive sites in the human submandibular gland were examined by the peroxidase-antiperoxidase method using the specimens taken from patients that had not received radiotherapy. VIP-immunoreactive fibers were found around both serous and mucous acini, the duct system, and those around the mucous acini were more numerous than those around the serous acini. VIP-R immunoreactivity was restricted to the mucous acini and the intercalated duct segment. The serous acini, striated duct, and excretory ducts lacked VIP-R immunoreactivity. These findings suggest that the mucous acinar cells contain VIP-R, and that VIP-R-mediated VIP action is involved in regulating synthesis of viscous saliva and its release.

Point mutation in exon 12 of the receptor tyrosine kinase proto-oncogene RET in Ondine-Hirschsprung syndrome.

A 5-year-old girl with congenital central hypoventilation syndrome associated with Hirschsprung's disease (Ondine-Hirschsprung syndrome) representing a missense mutation in exon 12 of the receptor tyrosine kinase (RET) proto-oncogene is reported. Using a direct sequencing technique, genomic DNA obtained from the patient's peripheral leukocytes was analyzed for its nucleotide sequences in all 20 exons of the RET proto-oncogene, seven regions of the 1st to the 7th exon of the endothelin-B receptor gene and endothelin 3 gene, including sequences corresponding to proteolytic cleavage sites. The analysis revealed that adenine at the 2116th base in the 12th exon in the RET proto-oncogene was substituted by guanine, supposedly resulting in a mutation of Thr 706 to Ala. No other mutational change was observed in the gene examined in this case. Mutation analysis has not been described previously on the gene in this disease complex. Mutation in this case might impair the maturation of the tyrosine kinase protein and subsequently cause neurocristopathy supposedly originating from the neural crest.

[Characterization of phenolic glycolipid-I (PGL-I) like antigen in renal cell carcinoma].

BACKGROUND: We investigated whether phenolic glycolipid-I (PGL-I) produced by Mycobacterium leprae (M. leprae), can be used as a marker of renal tumors. METHODS: Using a preparted anti-PGL-I monoclonal antibody (SF-I), anti-PGL-I group antibodies and PGL-I group antigens were detected and PGL-I immunohistologic staining were performed. RESULTS: The titer of anti-PGL-I antibody in patients with renal cell carcinoma was 0.283 +/- 0.103, showing a trend of rising titer of anti-PGL-I group antibody with higher cytologic atypia. When compared by the extent of tumor infiltration, a high antibody titer was observed in stage 4 infiltration group. Regarding the PGL-I group antigen that was detected using SF-1 in the urine of patients with renal cell carcinoma, no relationships between the positive rte of the antigens with common antigenicity to PGL-I and the grade of carcinoma were observed. However, in terms of the extent of tumor infiltration, a trend that the positive rate for PGL-I group antigen increased with the progression of cancer stage was seen. The Western blot assay of the urine of patients with renal cell carcinoma following electrophoresis revealed a change around 40 KD. The immunohistologic stains with SF-1 disclosed a predominance in the proximal kidney tubule. CONCLUSION: Using PGL-I group antigen and PGL-I group antibody as markers of renal cell carcinoma, a rapid and precise measurement may be provided.

[Minimally invasive cardiac surgery--the efficacy of right parasterna approach].

The recent concepts of minimally invasive surgery have affected even cardiovascular surgery. Especially, the desire to lessen incisional pain and hospital stay has made minimally invasive cardiac surgery (MICS) desirable. However, its efficacy is still controversial. To investigate this goal, we assessed the efficacy of avoidance of median sternotomy through right parasternal approach in view of the postoperative bleeding, % transfusion, postoperative intubation period, degree of incisional pain and serum level of cytokines. Patients with mitral valve disease or atrial septal defects were divided into the MICS (M) group and the control (C) group. In the M group, operations were performed through right parasternal incision under cardiopulmonary bypass (CPB) instituted by placing a venous cannula directly into superior vena cava and arterial and the other venous cannulae into femoral artery and vein. On the other hand, in the C group, operations were performed through median sternotomy under conventional CPB. There were no significant differences in CPB and AXC time between two groups. The M group showed significantly lower value in the postoperative bleeding volume, % transfusion, postoperative intubation time. Patients in the M group showed higher satisfaction of small incision as compared with those in the C group. Serum level of IL-8 after CPB was significantly lower in the M group than in the C group. These results suggested that MICS for mitral disease or ASD appears to be less invasive when median sternotomy is avoided. This suggest that MICS is a promising and contributed approach for open heart surgery to improve the QOL of the patients.

[Clinical manifestations and diagnosis of patients with sarcoidosis].

We reviewed the records of new patients with uveitis who visited the Hokkaido University Hospital between 1991 and 1994. For the diagnosis of sarcoidosis, we used the criteria revised by the Diffuse Pulmonary Disease Research Committee of Japan in 1989. The total number of patients was 374, and 61 cases were diagnosed as sarcoidosis. Of 61 patients with sarcoidosis, 58 cases were diagnosed histologically. Transbronchial lung biopsy was the basis for 70% of the histological diagnosis. After the revision of the criteria of sarcoidosis, the percentage of histological diagnosis increased. The Diffuse Pulmonary Disease Research Committee of Japan also proposed guidelines for the diagnosis of ocular sarcoidosis in 1990. We applied these guidelines to our uveitis patients. Most of the patients (77.1%) with sarcoidosis were included in the group of probable sarcoidosis, but 20% of patients with uveitis who were not diagnosed as having sarcoidosis were also included in this group. These guidelines are not yet adequate.

Expression of c-kit and stem cell factor mRNA in liver specimens from healthy adult dogs.

OBJECTIVE: To determine expression of c-kit and stem cell factor (SCF) mRNA in liver specimens from healthy adult dogs and to investigate whether differentiation of mast cells in the liver of dogs is supported by the c-kit receptor tyrosine kinase/SCF system. ANIMALS: 3 healthy adult Beagles. PROCEDURE: The nucleic acid sequence of the canine c-kit fragment of the intracellular tyrosine kinase domain was determined. Magnitudes of c-kit and SCF mRNA expression in liver samples was determined by means of northern blot analysis, using probes based on canine sequences. To determine which cells were expressing c-kit and SCF mRNA, in situ hybridization was performed. RESULTS: Expression of c-kit and SCF mRNA in liver samples was weak but appreciable. In situ hybridization revealed that c-kit and SCF mRNA expression was restricted to mast cells and plasma cells, respectively. CONCLUSION: Expression of SCF mRNA was detected in liver from healthy adult dogs. The c-kit receptor tyrosine kinase/SCF system may, possibly in combination with other growth factor and receptor systems, be involved in proliferation and differentiation of liver mast cells in dogs.

Ocular tilt reaction with vertical eye movement palsy caused by localized unilateral midbrain lesion.

A 60-year-old man developed diplopia and experienced difficulty moving his eyes. Vertical movement of each eye, including vestibulo-ocular reflex and smooth pursuit, was extremely limited. Horizontal eye movements were normal. His head position was tilted toward his left. There was 10 prism diopters of exotropia and 10 prism diopters of right hypertropia. Fundus photographs revealed a clockwise torsion of both eyes. These signs indicate leftward ocular tilt reaction. Magnetic resonance imaging showed a small area of an increased signal intensity localized in the midbrain dorsomedial to the red nucleus on the right side. Based on recent experimental evidence, it may be assumed that the unilateral lesion involving the right interstitial nucleus of Cajal most probably caused leftward ocular tilt reaction in our patient.

Isolation of macrophage-like cell mutants resistant to the cytotoxic effect of oxidized low density lipoprotein.

A high concentration of oxidized low density lipoprotein (Ox-LDL) showed a cytotoxic effect on mouse macrophage-derived J774 cells. Mutant cells were selected from these cells that were resistant to the cytotoxic effect of Ox-LDL. One mutant form, named JO21b cells, was characterized in the present study. In spite of a marked resistance to the cytotoxic effect of Ox-LDL, JO21b cells were apparently as sensitive as the parent cells not only to toxic moieties of Ox-LDL, such as 7-ketocholesterol and lysophosphatidylcholine, but also to t-butyl hydroperoxide, an artificial lipid hydroperoxide analog. However, the cellular association of 125I-labeled Ox-LDL with, and subsequent endocytic degradation by JO21b cells was reduced by 70-80% compared with J774 cells. Similarly, accumulation of cholesteryl esters in JO21b cell by Ox-LDL was also reduced by 70%. Northern blot analyses of type I and type II macrophage scavenger receptors (type I and type II MSR) demonstrated that the mRNA levels of JO21b cells were lower than those of J774 cells. Moreover, peritoneal macrophages obtained from MSR-knockout mice showed a higher resistance to the cytotoxic effect of Ox-LDL than those from their wild-type littermates. Our results suggest, therefore, that macrophage scavenger receptor-mediated endocytic uptake of oxidized low density lipoproteins (Ox-LDL) may play an enhancing role in Ox-LDL cytotoxicity to macrophages or macrophage-derived cells.

[Aqueous levels of eosinophil cationic protein and major basic protein in patients with retinal detachment associated with atopic dermatitis].

We measured the aqueous levels of eosinophil cationic protein (ECP) and major basic protein (MBP) to investigate a possible relationship between cytotoxic eosinophil granule proteins and retinal detachment (RD) in patients with atopic dermatitis. While aqueous ECP was detected in only 1 of 15 cases of RD associated with atopic dermatitis, aqueous MBP was detected in 6 of 14 cases (42.9%). 11 cases of RD without atopic dermatitis were also studied, but neither ECP nor MBP was detected. Although the role of aqueous ECP and MBP was not elucidated, the results indicate that cytotoxic eosinophil granule proteins, especially MBP, are present in aqueous humor in certain cases of RD associated with atopic dermatitis.

[Relationship between imaging and pathological features and clinical factors in surgical cases of temporal lobe epilepsy].

The relationships between imaging, pathology and presumed causes in surgical cases of temporal lobe epilepsy (TLE) was studied. The subject was 62 patients (33 males and 29 females) who had had no attacks for more than one year after surgery. Average age at surgery was 28.2 +/- 9.9 years. Obvious neurological abnormalities were not found in these cases. MRI, PET and SPECT were performed. Hematoxylin and eosin was used for pathological judgement. Their medical charts were used to investigate their clinical factors. Although patients suspected of having encephalitis/meningitis had been hospitalized for 2 days to three months during childhood due to disturbance of consciousness with high fever and convulsion for several days; they were not diagnosed with encephalitis/meningitis at that time, and they suffered almost no handicaps other than epilepsy for several years following their comatose episodes. [Result] (1) On MRI, mesial temporal sclerosis (MTS) was detected in 48 of 52 patients (92%); 32 (62%) had high-signal intensity on T 2-weighted images; 31 (60%) had atrophy ¿23 (44%) had high-signal intensity on T 2 + atrophy¿; 5 (10%) had calcified lesions; and 2 (4%) had cystic lesions. On PET and SPECT, abnormal cerebral blood flow was noted in 33 of 36 (92%). (2) On pathological findings (61 cases), Ammon's horn sclerosis (AHS), tumors, gliosis in lateral temporal and meningeal inflammatory finding were detected in 42 (69%), 10 (23%) and 8 (13%) cases, respectively, whereas 2 showed no abnormalities. The 2 patients with normal pathology showed both high-signal intensity and atrophy on MRI. (3) The presumed causes of TLE were encephalitis/meningitis and/or suspected of these diseases in 15 patients (24%), injuries at birth in 5 (8%), and none in 42 (68%). The presumed causes in the 43 patients with AHS were encephalitis/meningitis in 11, injuries at birth in 3, and none in 29. Of the 15 patients in whom encephalitis/meningitis was estimated as the causes of TLE, only 6 (40%) had pathological evidence of meningeal inflammatory finding. Of the 42 patients in whom cause could not be determined, 2 had pathological evidence of meningeal inflammatory finding.

[Reoperation after the Fontan operation in single ventricle and tricuspid atresia].

The outcome of the Fontan-type operation for complex heart disease has been significantly improved, and low morbidity and mortality rates are reported. However, some problems still occur in late follow-up, including ventricular failure, hepatic congestion, supraventricular arrhythmia, cerebrovascular events, exercise intolerance, and residual or new cardiac lesions. Reinterventions after the Fontan-type operation for complex cardiac defects in 43 patients are reviewed. In addition to catheter interventions, the main reasons for reoperation were subaortic stenosis, pulmonary artery and vein obstructions, and interatrial shunt. Early intervention, particularly for subaortic obstruction, is recommended, and the Damus-Kay--Stansel anastomosis appears to be the procedure of choice. Reoperation was required in 6 of the 43 patients, with one operative death. The total event-free survival rate after 10 years of follow-up was 53%. The need for reoperation appears to be reduced after performing the total cavopulmonary shunt procedure compared to that after atriopulmonary connection. However, late arrhythmia remains a significant problem in this group of patients, and further assessment of the results of reintervention for arrhythmia is required.

[Regional cerebral blood flow measurements by a noninvasive microsphere method using 123I-IMP: comparison with the modified fractional uptake method and the continuous arterial blood sampling method].

A noninvasive microsphere method using N-isopropyl-p-[123I]iodoamphetamine (123I-IMP), developed by Yonekura et al., was performed in 10 patients with neurological diseases to quantify regional cerebral blood flow (rCBF). Regional CBF values by this method were compared with rCBF values simultaneously estimated from both the modified fractional uptake (FU) method using cardiac output developed by Miyazaki et al. and the conventional method with continuous arterial blood sampling. In comparison, we designated the factor which converted raw SPECT voxel counts to rCBF values as a CBF factor. A highly significant correlation (r = 0.962, p < 0.001) was obtained in the CBF factors between the present method and the continuous arterial blood sampling method. The CBF factors by the present method were only 2.7% higher on the average than those by the continuous arterial blood sampling method. There were significant correlation (r = 0.811 and r = 0.798, p < 0.001) in the CBF factor between modified FU method (threshold for estimating total brain SPECT counts; 10% and 30% respectively) and the continuous arterial blood sampling method. However, the CBF factors of the modified FU method showed 31.4% and 62.3% higher on the average (threshold; 10% and 30% respectively) than those by the continuous arterial blood sampling method. In conclusion, this newly developed method for rCBF measurements was considered to be useful for routine clinical studies without any blood sampling.

A chicken anti-conoid monoclonal antibody identifies a common epitope which is present on motile stages of Eimeria, Neospora, and Toxoplasma.

The chicken monoclonal antibody (mAb) 6D12-G10, raised against Eimeria acervulina sporozoites, has previously been shown to recognize the conoid of E. acervulina sporozoites and inhibit sporozoite invasion of lymphocytes in vitro. In indirect immunofluorescent assay, the mAb 6D12-G10 also reacted with merozoites from E. acervulina and identified a 21-kDa merozoite protein on western blots. By confocal laser scanning microscopy, the conoid of sporozoites from 6 different avian Eimeria species (E. brunetti, E. maxima, E. mitis, E. necatrix, E. praecox, and E. tenella) were reactive with 6D12-G10 mAb. Furthermore, the 6D12-G10 mAb also showed cross-reactivity with motile stages of 2 closely related apicomplexans, Neospora, and Toxoplasma. These results indicate that the mAb 6D12-G10 identifies a conserved epitope on the conoid that is important in host cell invasion by the apicomplexan parasites.

Resting and acetazolamide-challenged technetium-99m-ECD SPECT in transient global amnesia.

Regional resting cerebral blood flow and vascular reserve in a patient with transient global amnesia (TGA) were evaluated during and after a TGA episode using 99mTc-ethyl cysteinate dimer (ECD). The patient had consecutive SPECT studies before and after acetazolamide (ACZ) administration with adjunctive radionuclide angiography using equal-volume-split 99mTc-ECD. SPECT study during TGA episode showed poor vasodilatory reactivity to ACZ in the left medial temporal region involving the hippocampus and resting hypoperfusion in the regions bilaterally. The resting hypoperfusion with reserved vasodilatory reactivity to ACZ also was seen in the bilateral thalami. Abnormal findings in these regions disappeared on the follow-up SPECT study 15 days after the onset. No previous SPECT evaluation of regional abnormalities of both hemodynamic reserve and resting perfusion during and after an episode of TGA has been reported.

[Postoperative complications after Mainz pouch construction--with emphasis on measures to deal with narrowing of the duct aperture].

Thirty patients underwent Mainz pouch construction between December 1992 and January 1998. The post-operative observation period varied from 6 to 57 months (average, 38.5 months). A mechanism to ensure urinary continence was provided in 24 cases, while formation of a nipple valve by intussusception of the ileum with seromuscular stripping and submucosally embedded in situ appendix was performed in 6 cases. An umbilical stoma was adopted in all cases. Late-phase complications included 3 cases of hydronephrosis requiring treatment. Efferent limb of the stoma was observed in 8 cases, 6 of which were attributable to mild constriction of the stoma at the site of anastomosis of the navel cavity. In those cases in which stomal stricture occurred, difficulty in withdrawing urine was eliminated by providing a fistula stopper. Although few clinical methods to deal with stomal stricture have been reported, we found the fistula stopper to be useful providing good external appearance and patients' quality of life.

[Clinical experience of transurethral collagen injections for urinary stress incontinence--analysis of subjective symptoms].

We evaluated the results of transurethral collagen injections for urinary stress incontinence. Twenty five women (mean age was 61.3 years) with urinary incontinence were treated with transurethral collagen injections using local or spinal anesthesia. The mean follow-up was 11.7 months (range 2 to 30). We examined the results based on subjective symptoms for incontinence. We could judge convalescence efficacy to some degree 1 month after operation, but patient age, type of stress incontinence, pad test and volume of collagen were not significantly different between patients who were cured and those not cured. Of the patients who needed injections more then 2 times, treatment was effective in type III patients. The patients whose symptoms were improved 3 months after operation wanted a re-operation when their incontinence recurred. Injection of transurethral collagen appears to be a safe and effective method for treating urinary incontinence. This procedure is a first choice for urinary incontinence.
