[Postmenopausal virilization in thecosis of the ovaries].

We describe the case of a 74-year-old woman with a fifteen-year old history of virilism and with a Type-II-diabetes mellitus treated with insulin. After the removal of the ovaries, the formerly elevated serum-testosterone levels normalised and the signs of virilism gradually abated. The histological finding of stromal thecosis in the ovaries is discussed in relation to hyperinsulinaemia and other potential stimuli for ovarian stromal hyperplasia.

[Ovarian cancer in ovarian remnant syndrome?].

We report on a malignant pelvic tumour in a 69-year old woman, whose uterus and adnexa had been removed 22 years ago because of endometriosis. Clinical and pathological findings pointed to an ovarian cancer in an ovarian remnant syndrome. In a case of a carcinoma, it is difficult to furnish proof of an ovarian remnant syndrome. Differential diagnoses, especially the ureteral adenocarcinoma, are discussed.

Neurological deterioration under isovolemic hemodilution with hydroxyethyl starch in acute cerebral ischemia.

In a prospective study, we randomly allocated 70 patients with acute ischemic stroke to two therapy groups. Up to interim analysis, 33 patients underwent bloodletting, with simultaneous infusion of an identical volume of hydroxyethyl starch (10%, 200/0.5). The target hematocrit was 35%. A control group of 37 patients did not receive hemodilution. Apart from an unequal sex distribution, the two groups were comparable with regard to age, cardiovascular risk factors, and medical history. In the hemodilution group, the mean hematocrit fell from 44.4% to 37.7%. After 14 days, improvement on the neurological score scale was 3.3 points in the hemodilution group compared with 6.5 points in the control group (p = 0.12). Subgroups with early inclusion (less than 12 hours) or pronounced lowering of hematocrit (greater than 15% of initial hematocrit) also did not profit from hemodilution. Clinical deterioration observed in eight hemodilution group patients (p less than 0.01) led to discontinuation of the study for ethical reasons.

[Bilateral fetal hydrothorax].

A bilateral fetal hydrothorax was recognized in the second pregnancy of a 30-year-old woman. Because of increasing pleural effusions, fetal hydrops and hydramnion a cesarean section was performed in the 31st week of pregnancy. The child died of pulmonary hypoplasia. The ante- and postnatal diagnosis and therapy and the poor prognosis of bilateral congenital hydrothorax are discussed.

[Metastasizing malignant melanoma of the vagina].

We report on a 54-year-old patient with multiple cutaneous, lung and liver metastases. The primary tumour is a malignant melanoma of the vagina, who developed within one year after the last gynaecological examination. Our case report is another example for the poor prognosis of this rare vaginal tumour.

[The effect of blood glucose on the course of cerebral ischemia].

Experimental und clinical data suggest a negative influence of diabetes mellitus and hyperglycemia on the outcome of cerebral ischemia. In a retrospective analysis of 135 consecutive patients with cerebral ischemia we found more severe clinical defects in diabetics and patients with initial hyperglycemia but without previously known diabetes mellitus than in patients with initial normoglycemia. This was especially true for blood glucose above 10 mmol/l. Before drawing therapeutic conclusions, further prospective studies are necessary.

Hemoglobin quantitation in subarachnoid hemorrhage.

The amount of hemoglobin in cerebrospinal fluid was quantitated in 15 patients with subarachnoid hemorrhage and in 35 subjects without subarachnoid hemorrhage (controls). The photometric determination of the hemoglobin content in cerebrospinal fluid indicated that patients with subarachnoid hemorrhage in contrast to controls showed a significant increase (P greater than 0.01) of hemoglobin (Mann-Whitney test). In 5 of the patients with subarachnoid hemorrhage it was possible to determine the amount of hemoglobin in cerebrospinal fluid over a period of time. Possible correlations between clinical course and hemoglobin concentration as well as the use of hemoglobin quantitation in late diagnosis of subarachnoid hemorrhage are discussed.

[Abdominal pregnancy. Case report and review of the literature].

Two cases of abdominal pregnancy are presented. Since this type of pregnancy has an incidence rate of 1.6% of all ectopic pregnancies only, it must be classified as a rare occurrence. Even more so, an advanced abdominal pregnancy with a viable foetus is an absolute rarity. The most frequent signs are abdominal pain, nausea and vomiting, unclear blood loss, and painful foetal movements. If the diagnosis is clear in an early stage of pregnancy, rapid laparotomy is mandatory. In advanced pregnancy with viable foetus, the viability must be improved under clinical control. After delivery, management of the placenta is the greatest problem. The best results are obtained if the placenta can be removed safely. If it must be left in situ, the effectivity of methotrexate therapy has not been established.
