The use of intra-operative endo-anal ultrasound in perianal disease.

INTRODUCTION: Endoanal ultrasound (EAUS) has demonstrated high sensitivity and specificity for the structural imaging of anorectal pathology. This study prospectively assessed the impact of intra-operative EAUS on the surgical management of perianal disease. METHODS: EAUS was performed prior to and after examination under anaesthesia (EUA) in a consecutive series of patients with perianal disease. The impact of EAUS on the surgery performed was identified. RESULTS: Forty-three procedures have been performed in 38 patients (21 male, 17 female; mean age 42.7 years, range 6-76 years) over a three year period. Pathologies encountered were fistula-in-ano (42%), fissure-in-ano (26%), complicated perianal sepsis (16%) and carcinoma (5%). No specific abnormality was identified in 5 symptomatic patients (12%). Four patients with fissures had undergone previous sphincterotomy. In 22 cases (51.2%) the EAUS findings affected the surgical management (extent of muscle above a fistula 9 cases, extent of sphincterotomy 7 cases, site of sepsis identified 2 cases, exclusion of sepsis 2 cases, assessment of cancer resectability 1 case, biopsy of intersphincteric lesion 1 case). CONCLUSION: Intra-operative EAUS accurately identifies perianal disease and influences the surgical procedure performed. While not essential, it is a useful adjunct especially in recurrent perianal sepsis, undiagnosed anorectal pain and anal fissure.

Use of helical CT in assessment of crossing vessels in pelviureteric junction obstruction.

AIM: The purpose of this study is to confirm the accuracy of non-invasive helical computed tomography (CT) with multiplanar reformatting in the diagnosis of crossing vessels in patients with pelviureteric junction (PUJ) obstruction. MATERIALS AND METHODS: Nineteen patients with confirmed PUJ obstruction underwent CT of the renal area with intravenous contrast medium. Patients shown to have crossing vessels had to undergo a pyeloplasty by open surgery or laparoscopy. Patients with no vessels could have their PUJ obstruction safely treated with less invasive techniques such as balloon dilatation. RESULTS: Fourteen patients were shown to have crossing vessels. Of these, 10 had their relationship to the PUJ confirmed at a subsequent laparotomy. Of the remaining four patients, one was lost to follow-up and two were unwell due to unrelated disease. The other patient had already had a vessel moved at a previous laparotomy. Of the five patients without obstructing vessels, two were confirmed not to have a related vessel at laparotomy, one has undergone balloon dilatation and the other two were lost to follow-up. CONCLUSION: Helical CT is an accurate and non-invasive method of demonstrating crossing vessels in PUJ obstruction. Diagnosis of these vessels has a major role in the choice of therapeutic treatment.

Pictorial review: the sonographic diagnosis of pathology in the Achilles tendon.

This pictorial essay details Achilles tendon pathology using real-time sonography. Characteristic diagnostic images for tenosynovitis, acute and chronic tendinitis, peritendinitis, nodular tendinitis and complete and partial tendon ruptures are described. The anatomy of the Achilles tendon in relation to these diagnoses is discussed. Ultrasound is recommended as a useful first-line approach in imaging the Achilles tendon.

Panoramic sialography: an alternative technique.

Pantomographic sialography, referred to in this paper as panoramic sialography, was performed on 30 out of 51 patients. In 21 patients, it proved to be a good alternative technique to conventional sialography whilst in the other nine patients it was the principal method used to study the salivary glands. The techniques employed in this study for both conventional and panoramic methods are described. The radiation dose delivered to the patients was similar for both techniques. Water soluble contrast medium, Conray (325), was used in all cases.
