Two cases of synovial cysts of the hip are reported beside any rheumatoid context. One presented as a pseudoaneurysmal inguinal mass, the other one as a venous compression of the lower limb, both of them were operative discovery. Through a review of the literature, pathogenic, diagnosis and therapeutic aspects of this rare disease are developed.

In spite of its ingenuity, the Sparks-mandril concept has failed to generate a satisfactory arterial substitute. Pathological analysis of prostheses retrieved from humans and explanted from dogs as part of a controlled laboratory study has demonstrated that the tissue laid down during the autogenous generation stage contains neither elastin fibers nor smooth muscle cells capable of providing the required dimensional stability of a blood vessel. The supporting polyester mesh tube has been found to stretch in vivo, resulting in dilation and lengthening of the prosthesis and the formation of aneurysm and thrombosis. It is recommended that the use of devices relying on the Sparks-mandril concept not to be reintroduced until such time as the mechanism of growth promoting factors for angiogenesis are fully understood.

The response of arterial pressure to an infusion of saralasin was compared to the effect of surgical correction of renal vascular lesions (3 to 6 months after surgery) in eleven patients whose hypertension was associated with uni or bilateral stenosis of renal artery. Saralasin was infused after four days of dietary sodium restriction (10--40 mEq/day). An excellent correlation (r = 0.83, p less than 0.005) between the effects of saralasin and surgery was obtained. There was no correlation between the response to saralasin or to surgery and the ratio of renal vein renin activities. It is suggested that saralasin may be a good tool for predicting the effect of surgery in renovascular hypertension, when infused in moderately sodium depleted patients.
