Carbohydrate and lipid metabolism in open-heart surgery.

Carbohydrate and lipid metabolism was studied in 8 patients who underwent open-heart surgery with the aid of extracorporeal circulation. Hyperglycemia was observed during perfusion. Despite the high glucose levels during perfusion, insulin responses were depressed. A rise of insulin levels was observed one hour after perfusion, and at the same time the glucose levels dropped. Suppression of insulin secretion during perfusion may be the result of increased catecholamine secretion, induced hypothermia, or heparin administration. High levels of non-esterified fatty acids (NEFA) and low levels of triglycerides were observed immediately before, during, and after perfusion while heparin was being utilized. This phenomenon was considered to be strongly affected by the use of heparin. The levels of growth hormone were depressed during perfusion but significantly elevated one hour after the end of perfusion. These phenomena may be caused by the fluctuations in glucose and NEFA levels.

Successful closure of ventricular septal defect through a left-sided ventriculotomy in corrected transposition of the great vessels.

Corrected transposition of the great vessels is often associated with other cardiac anomalies. Ventricular septal defect (VSD) is the most common among them. Closure of a VSD is usually performed through a right-sided ventriculotomy. We had previously closed the VSD through the right-sided ventricle in 2 patients with corrected transposition of the great vessels and in 2 others with double-outlet right ventricle with ventricular inversion, which resulted in complete heart block. Recently we have closed the VSD through the left-sided ventricle in 3 patients with corrected transposition, and no block resulted.

Pancreatic carcinoma in infancy. An electron microscopic study.

A case of a malignant epithelial tumor of tail of the pancreas in a 3-year-old Japanese girl is presented. This is the fourth case reported with ultrastructural study. This type of pancreatic carcinoma of the infant has no endocrine granules or endocrine function. The ultrastructural study suggests that the tumor is derived from a primitive epithelial cell with differentiation toward acinar cell.

Intraventricular repair of single ventricle associated with transposition of the great arteries.

Intraventricular repair was performed successfully in a 6-year-old body having a ventricle associated with d-loop and d-transposition in situs solitus of the atria and viscera. A spiral Teflon patch was utilized to partition the ventricle. Complete heart block did not result, and the patient is doing well 8 months after the operation. Details of the pre- and postoperative studies, as well as details of the surgical technique, are presented. A review is presented of the operative procedures that have been designed for the correction of a single ventricle associated with transposition of the great arteries and have been reported previously in the literature. The advantages for use of the techniaue employed in the present case are discussed.

Immunotherapy of human lung cancer with BCG cell-wall skeleton.

Effect of BCG cell-wall skeleton (BCG-CWS) on the recovery of responsiveness of lymphocytes and the survival period was studied in patients with lung cancer. (a) Responsiveness of lymphocytes as demonstrated by proliferative response to phytohemagglutinin (PHA) or microcytotoxicity test with cultured cells of a bronchogenic carcinoma was depressed in the patients at later stages of cancer and it was restored by treatment with BCG-CWS in many of such patients. (b) Survival period of the patients at later stages such as III or IV was prolonged by treatment with BCG-CWS. Such an effect was detected even in the patients with carcinomatous pleuritis. (c) A close correlation was found between the reactivity such as PHA response or cytotoxicity and clinical course of the patients given BCG-CWS. Assay system with PHA response and microcytotoxicity test appears to be reliable in anticipating the prognosis and in following clinical course after the BCG-CWS treatment.

Skin lesions during intravenous hyperalimentation: zinc deficiency.

Of 54 patients who received prolonged intravenous hyperalimentation during the past 4 years, six developed progressive, characteristic skin lesions. These cutaneous manifestations frequently were accompained by abdominal symptoms resembling those of acrodermatitis enteropathica in many respects. In the first four of these six patients, skin eruptions disappeared promptly after the initiation of oral feeding, which led us to assume that these skin manifestations are due to some unknown nutrient deficiency. Our experience in the remaining two patients enabled us to presume the underlying pathology to be zinc deficiency.
