Functional roles of human class II antigens in T cell proliferative response. Comparison of the DR and DQ molecules using mouse monoclonal antibodies.

The monoclonal antibody (mAb) NC1 recognizing a monomorphic DR determinant and the anti-DQ mAbs PLM2, PLM9, and PLM12 recognizing polymorphic DQ determinants were produced. NC1 reacted with the H1L1 (DR) and H1L2(DRw52, DRw53 and allelic products) complexes of a DR family, while the specificity of PLM12 was DQw3 (previously called TB21). Both PLM2 and PLM9 reacted with the allodeterminant of TA10 (a subtype of DQw3). These three mAbs immunoprecipitated the same H2L3 complex of a DQ family. With the aid of these mAbs, DR molecules but not DQw3 molecules were detected on monocytes in significant quantities. The functional roles of these class II molecules in the T cell proliferative responses to three soluble antigens (PPD, mite, Candida) and alloantigens were examined by blocking assays in vitro. NC1 almost completely inhibited T cell proliferative responses against both soluble antigens and allogeneic antigens. In contrast, PLM2, PLM9 and PLM12 showed no significant inhibitory effects at all. These results indicate that DR and DQ antigens are different in their functional roles as well as their serological and immunochemical characteristics and tissue distributions; these three soluble antigens and the alloantigen were presented in association with determinants residing predominantly on two forms of NC1-reactive molecule, namely the H1L1 and/or H1L2 complex of a DR family but not in association with the DQw3 allodeterminant on the H2L3 complex of a DQ family.

A dual approach to self-stimulation and locomotor trace affected by chronic methamphetamine treatment for an animal model of schizophrenia.

The effect of long-term methamphetamine (MAP) treatment on intracranial self-stimulation of the lateral hypotholamus and locomotor traces was assessed. An attempt was made to provide a useful animal model for understanding anhedonia, stereotypy, and reoccurrence of liability, which are analogous to symptoms of schizophrenia. The frequency of intracranial self-stimulation (ICSS) as used as a measure of the animals' "hedonic-anhedonic" state. Following long-term MAP treatment (3 mg/kg), rats gradually showed stereotyped behavior, and became inactive and unresponsive to ICSS. These behavioral changes and decreased ICSS lasted several weeks after cessation of chronic MAP treatment and seemed to suggest post-MAP chronic psychosis and (or) anhedonia, two of the negative symptoms of schizophrenia. The traces of rat behavior affected by chronic MAP treatment were classified into three types, peripheral, mixed, and fixed, occurring in a dose-dependent manner. Reverse tolerance, similar to the reoccurrence of schizophrenic symptoms, was observed as a fixed stereotypy associated with loss of ICSS. These abnormal phenomena were suppressed by pretreatment with haloperidol. In the present study, the combination of ICSS and locomotor trace affected by chronic MAP treatment was proposed as an animal model of schizophrenia and as a useful technique for gauging the effect of neuroleptics.

Response characteristics of amygdaloid neurons provoked by emotionally significant environmental stimuli in cats, with special reference to response durations.

Extracellular single or multiple neuronal activities were recorded from the basolateral portion of the amygdala of wild adult cats under an unanesthetized, freely moving condition, and neuronal responsiveness to neutral, aversive, and appetitive stimuli was studied. Of 71 units, 47 (66%) responded to some of the stimuli. The patterns of neuronal responses were classified into three types on the basis of response duration. Of the responses sampled, 9% rapidly attenuated and disappeared before termination of stimulus presentation (pattern A), 58% of responses were maintained during the period of the stimulus presentation but disappeared abruptly after termination (pattern B), and 33% of responses markedly outlasted the stimulus presentation period (pattern C). Pattern A responses habituated readily and were most prominent when neutral stimuli were presented, so this type of response was considered to underlie altering or orienting responses. Pattern B responses were observed equally for the three kinds of stimuli, and were suggested to be predominantly involved in perception of the environmental stimuli. Pattern C responses habituated least and tended to be elicited more frequently by aversive stimuli. This type of response was interpreted to reflect emotional arousal. These findings were considered to be compatible with the hypothesis that the amygdala plays an important role in converting environmental stimuli into emotions such as rage or fear.

How antenatal ultrasound diagnosis of congenital malformations has contributed to fetal outcome: a 22-year review.

We herein reviewed 630 malformed fetuses delivered from 24 weeks of gestation onwards in our institute over the past 22 years. These fetuses were divided into 2 groups: 210 from 1970 to 1982 (group 1) and 420 from 1983 to 1991 (group 2). Twenty-two varieties of congenital malformations were diagnosed antenatally in group 1, whereas 55 additional malformations became diagnosable in group 2. Thirty-four varieties of congenital malformations still remained undiagnosed throughout the 22-year study period. Cases receiving 'fetal therapy' and 'close obstetric care' increased, with statistical significance, from 1.8 to 15.7%, and from 22.9 to 55.7% in groups 1 and 2, respectively. A significant increase was also noted in the survival rate from group 1 (63.3%: 105/166) to group 2 (75.5%: 259/343). This study has revealed that the steady advancement in antenatal diagnosis of congenital malformations, along with great efforts in terms of intensive care, has contributed to improved fetal outcome.

Enhanced anti-tumour effect of cisplatin with low-voltage electrochemotherapy in hamster oral fibrosarcoma.

The aim of this study was to determine the effects of low-voltage electrochemotherapy with intraperitoneal cisplatin on hamster oral fibrosarcoma. Oral fibrosarcoma was transplanted submucosally into the cheek pouch mucosa of 100 hamsters. After transplantation, the hamsters were randomly divided into four equal groups. These groups received no treatment (D-E-); 2 mg/kg body weight cisplatin treatment without electroporation (D+E-); electroporation without cisplatin treatment (D-E+); or 2 mg/kg body weight cisplatin treatment followed by electroporation (D+E+). Electrical pulse treatment together with cisplatin injection markedly reduced the size of the tumour, whereas cisplatin injection or electrical pulse treatment alone did not. These results clearly indicate that the anti-tumour effect of cisplatin on hamster oral fibrosarcoma was considerably potentiated or enhanced by the administration of local electrical pulses at low voltages.

DNA vaccine against hamster oral papillomavirus-associated oral cancer.

Previously we developed a carcinogenesis model involving the combination of 9,10-dimethyl-1,2-benzanthracene (DMBA) application with physical wounding of hamster lingual mucosa. The presence of a novel hamster oral papillomavirus (HOPV) was demonstrated and its genome sequenced. In the present study, this HOPV hamster model was used to test whether vaccination with the L1 gene could prevent the development of oral carcinoma. DNA plasmids encoding the L1 gene or the vector alone were injected intramuscularly into 20 vaccinated and 20 control hamsters, respectively. The lingual tips of the hamsters were painted with DMBA for 8 weeks. A portion of the lingual tips was excised, and the tips were then painted daily with DMBA until the animals were killed 13 days later. All control hamsters developed lingual carcinoma, whereas 12 of the L1-vaccinated hamsters showed no lesions. These results suggest that immunization with L1 DNA vaccines may prevent the development of papillomavirus-associated oral cancer.

Usefulness of multi-detector row computed tomography for accurate preoperative assessment of pancreatic adenocarcinoma: report of a case.

A 74-year-old female underwent surgical treatment for adenocarcinoma of the pancreatic head. Preoperative multi-detector row computed tomography (MD-CT) demonstrated tumor invasion into the accessory right colic vein and the branch of the middle colic artery (MCA), which was not detected by digital subtraction angiography. MD-CT showed anatomical variants in the left hepatic artery arising from the left gastric artery, and the right posterior hepatic artery arising from the superior mesenteric artery. Three-dimensional reconstruction CT generated a clear picture of the anatomy of the region concerned, which is essential for a safe operation. The MD-CT findings were highly consistent with the intraoperative findings. We have demonstrated that MD-CT is an important and highly accurate modality for pancreatic surgery.

Antitumor activity of orally administered SMANCS, a polymer-conjugated protein drug, in mice bearing various murine tumors.

The antitumor activity of an oily formulation of SMANCS (oily SMANCS), which is a product of conjugation between a proteinaceous antitumor antibiotic neocarzinostatin and poly (styrene-co-maleic acid), after oral administration to mice inoculated with various murine tumors was investigated. BALB/c mice, inoculated either s.c. or i.p. with allogeneic (sarcoma-180) or syngeneic (RL male 1 leukemia and Meth A fibrosarcoma) tumors, were treated with oily SMANCS orally or with an aqueous formulation of SMANCS (aqueous SMANCS) i.v. or i.p. Oral administrations of oily SMANCS or i.v. administrations of aqueous SMANCS to mice bearing three types of tumors in the ascites form resulted in a weak inhibition of tumor growth as compared to the complete inhibition of these tumors by aqueous SMANCS administered i.p. In mice bearing solid tumors, tumor growth was inhibited by 63-82% when a 10 mg/kg dose of oily SMANCS was administered orally to these mice. The antitumor potential of oily SMANCS administered orally was comparable to that obtained from solid tumor-bearing mice receiving i.v. doses of aqueous SMANCS. These results suggest that the oral administration of oily SMANCS to mice bearing various solid tumors inhibits the tumor growth as effectively as aqueous SMANCS administered i.v.

Minor changes in regional cerebral blood flow revealed by the early distribution of I-123 IMP in brain.

To evaluate the early distribution of I-123 IMP in the brain, 10 dynamic images were obtained in the first 10 minutes after injection using a ring-type SPECT system with a high-sensitivity collimator. In cases of chronic carotid occlusion without brain CT abnormalities, areas of low perfusion were more clearly demonstrated in dynamic images than in static images obtained beginning 20 minutes after injection and continuing for 15 minutes using a high-resolution collimator. In cases of hyperperfusion following infarct or surgery, there was a difference between dynamic and static images in the visualization of hyperemic lesions. The distribution of I-123 IMP in the brain changes gradually, even in the early period after injection, and evaluation of early accumulation is useful for the detection of minor changes in regional cerebral blood flow.

Improvement in Fourier analysis of gated blood-pool studies using single photon emission computed tomography: methodology and clinical feasibility.

We have developed a new method for obtaining reconstructed left ventricular (LV) planar images acquired from multidirectional views using gated blood-pool single photon emission computed tomography (SPECT), and have applied Fourier analysis to these planar images. In this paper, we describe the methodology of our new technique and discuss its feasibility for the detection of LV wall motion abnormalities (WMA) in patients with coronary artery disease (CAD). Sagittal long-axis, horizontal long-axis and short-axis sections of the LV were generated from the reconstructed transaxial tomograms. Sections covering only the LV at the same phase of the cardiac cycle were added to reconstruct planar images. Fourier analysis was then performed to construct phase and amplitude images. In 42 patients with significant WMA, a comparative study concerning the detection of WMA was performed between conventional and SPECT methods. The SPECT method had greater sensitivity in all segments of the LV, with no significant loss in specificity. In particular, the sensitivity of the SPECT method was significantly greater than that of the conventional method for inferior and posterior WMA (91% vs 57%; and 76% vs 35%, respectively). The method described here shows greater clinical effectiveness for the detection of LV WMA in patients with CAD.

Interferon induction by SMANCS: a polymer-conjugated derivative of neocarzinostatin.

Poly (styrene-co-maleic acid)-conjugated neocarzinostatin, SMANCS, induced antiviral activities in the circulation of mice. The maximum titer of the activity (240 U/ml) was observed 20 h after administration an 8 mg/kg iv dose of SMANCS. Various experiments showed that this antiviral substance induced by SMANCS was an interferon (IFN). Sixty percent of the IFN response stimulated by SMANCS was impaired in mice pretreated with anti-IFN gamma antiserum. This suggests that the serum IFN induced by the agent contained about 60% of IFN gamma. When Thy 1+ or Lyt 2+ T-cells were depleted from mice by administration of anti-Thy 1.2 or anti-Lyt 2.2 monoclonal antibody (mAb), this IFN response was eliminated. However, when natural killer cells were depleted from mice by treatment with antiasialo GM1 antiserum, no alteration in the IFN response was observed. The SMANCS-stimulated IFN response was also abrogated in mice treated with macrophage blockers (carrageenan and trypan blue), whereas administration of anti-Lyt 1.2 mAb had no effect on the IFN response. These results suggest that macrophage and T-cells w Lyt 1-2+ phenotype may have an important role in the IFN response stimulated by SMANCS.

Tongue cancer: correlation of MR imaging and sonography with pathology.

Ten patients with tongue cancer underwent both MR imaging and sonography. In seven of these patients, pathologic findings from glossectomies were correlated with MR and sonographic results. MR images of resected specimens also were obtained in two patients, and relaxation time was calculated in one of these patients. MR images (5- to 7-mm thick slices) were obtained by using a 0.1-T resistive magnet with a 128 x 256 acquisition matrix. MR and sonography had almost the same sensitivity for detecting primary-site tongue cancer. However, in the three patients with extraorgan spread of tumor, MR was superior, showing three of three cases, compared with sonography, which showed extraorgan spread in only one of the three cases. Although MR failed in one patient to differentiate postradiation scar tissue from tumor, because of similar relaxation time of both, this imaging technique proved to be an important adjunct to the physical examination in the staging of tongue cancer.

Cytotoxicity of smancs in comparison with other anticancer agents against various cells in culture.

The cytoxicity of neocarzinostatin (NCS) and smancs [copoly(styrene maleic acid)-conjugated NCS] to various cultured cells was compared with that of several other antitumor agens in clinical use on various malignant and non-malignant cells as regards to their effect on colony formation of cells. Both NCS and smancs showed the most potent cytotoxicity against all tumor cell lines tested; the IC50s (colony inhibitory concentration 50%) of these drugs were 3.2-20 nM, 10-1000 times lower than those of other drugs. In contrast, NCS and smancs exhibited relatively lower toxicity to normal cells such as human skin fibroblasts and chick embryonic fibroblasts (IC50, about 50 and 100 nM, respectively). Normal rat hepatocytes were found to be very resistant to NCS and smancs (both IC50s were about 500 nM). Moreover, the minimum exposure time of smancs to cultured tumor cells required to achieve effective cytotoxic activity was much shorter than that of NCS and other drugs. Namely, at 30 nM more than 80% cells were killed by exposure to smancs for only a few minutes, whereas with NCS more than 80 min of exposure time was required. It was also found that smancs inhibited the uptake of 3H-thymidine into DNA as expected. These results clearly indicate that smancs is an unique antitumor agent with a broad antitumor spectrum which exhibits some characteristics similar to, but also some very different from NCS.

Radionuclide equilibrium ventriculography in infants and children using converging collimator.

In infants and young children, the radionuclide cardiac blood-pool images obtained with a general-purpose (GP) parallel-hole collimator are often too small to provide accurate delineation of the left ventricle (LV), especially the interventricular border. In order to obtain magnified cardiac images, a radionuclide gated equilibrium study was performed with a converging (CV) collimator. We compared these two methods in terms of image quality and estimation of LV ejection fraction (EF). Twenty-five children (age, 10 months to 6 years, mean 2.5 years; weight, 5.5 to 20 kg, mean 12 kg) with various heart diseases were studied with administration of 3-7 mCi 99mTc human serum albumin. The GP collimator was used in 15 subjects and the CV collimator in 16. The CV collimator provided optimal magnification and better resolution than the GP type. Cardiac images obtained with the CV collimator facilitated precise delineation of the entire interventricular border in all 16 cases. Those obtained with the GP collimator showed poor delineation of the interventricular border in four subjects. Both GP and CV collimations enabled LVEFs to be calculated, and the values correlated well with those obtained by cine-angiography. The correlation coefficient, however, was higher with the CV collimator (r = 0.787 vs. 0.861, p less than 0.001). Radionuclide cardiac blood-pool study with CV collimator has clinical significance because it provides sufficiently magnified cardiac images with high resolution, enabling accurate estimation of LVEF in infants and young children.

Paravertebral arteriovenous malformation with venous connection to left iliac vein. Report of a case.

An unusual systemic arteriovenous malformation arising from the left 9th intercostal artery, taking a caudal course through the left paravertebral veins, eventually drained into the left ascending lumbar vein, as illustrated at computed tomography and angiography. An embryologic maldevelopment of the azygos venous system is not the only possible explanation for venous drainage in the caudal direction. We believe that predominant venous drainage can take place in either direction because of the potentially bidirectional flow in the vertebral venous systems.

Primary choriocarcinoma of the stomach presenting as gastrointestinal hemorrhage: report of a case.

A case of primary choriocarcinoma of the stomach presenting as gastrointestinal hemorrhage in a 58-year-old postmenopausal woman is presented. Arteriographic features included a large hypervascular gastric mass and multiple liver masses with vascular poolings, mimicking those of cavernous hemangioma. Postcontrast CT including delayed scan showed a large central nonenhanced area with marginal peripheral contrast enhancement. Close observation of the angiographic findings revealed peripheral vascular poolings and central avascularity, which are characteristic of uterine choriocarcinoma. Human chorionic gonadotrophin (HCG) titer was significantly elevated, and extragenital choriocarcinoma of the stomach was confirmed by autopsy.

Clinical value of deconvolution analysis in radionuclide renal study.

Deconvolution analysis was applied to radionuclide renal study. The purpose of this study was to clarify the underlying renal physiology that determines the shape and values of the transfer function. Analysis of the renal models revealed that the initial height of the transfer function is the ratio of the radioactivity extracted by the kidney to that in the input function. In addition, the shape of the transfer function displayed the distribution of the transit times and enabled us to evaluate the percentage of nephrons having a particular transit time. Based on the relation between the input and output function, a new method for measuring GFR or ERPF was developed in which the input function was corrected by the plasma radioactivity at 5 min after injection of the tracer. As one of the advantages of the transfer function, it was proved clinically that the contribution of the blood background is automatically eliminated from the amplitude of the transfer function. From these findings, our method for the measurement of GFR was considered superior to that proposed by Gates and highly useful for routine radionuclide renal study.

Tumor targeting by arterial administration of lipids: rabbit model with VX2 carcinoma in the liver.

We previously found that a lipid contrast medium, Lipiodol, remained selectively in liver tumors for a prolonged time after injection via the hepatic artery. We now extend this technique to other lipids including linoleic acid, olive oil, tea seed oil, and medium-chain triglyceride (MCT). MCT is a semisynthetic triglyceride with a lower viscosity than that of the other lipids. Each lipid was mixed with 14C linoleic acid, and the mixture with or without anticancer agent smancs was injected via the proper hepatic artery of rabbits with VX2 carcinoma in the liver. The 14C count in the tumor tissue was always more than 100 times that of the plasma or kidney, and the radioactivity 15 min after injection of MCT was more than 2500 times greater than that of blood plasma. At 24 hr after injection, the radioactivity recovered in the tumor was 4.9-37.0% (average 19.6%) of the dose, which indicated the greatest retention in the tumor tissue; there was rapid clearance from the rest of the body primarily via the bile. Advantages of the administration of lipid-solubilized drugs are: (i) remarkable tumor-selective targeting (a decisive anticancer effect with fewer side effects); (ii) prolonged drug retention (resulting in infrequent administration); (iii) various choices of different fatty acids with various pharmacokinetic characteristics as the carriers of the anticancer agents; and (iv) arterial administration is practicable in most hospitals.
