[Part I. Theory and computer simulation for cerebrospinal infusion].

A pharmacokinetic one-compartment model for the cerebrospinal infusion for the brain tumor chemotherapy is described together with various parameters used for computer simulation. An antitumor protein, neocarzinostatin (NCS) as a prototype drug, has been utilized since it was found effective against glioblastoma cells at extremely low concentration (less than 5 ng/ml) and it is readily inactivated by serum. A very slow infusion velocity was found necessary for an appropriate dose regimen; for example, 0.25mg of the drug should be infused into CSF for about 40 min to attain a drug level of 8 ng/ml.

[Distribution of cefotiam dihydrochloride to the intraabdominal exudate (author's transl)].

The concentration of serum and intraperitoneal exudate after dripping intravenous administration of CTM were measured. Four male cases of the emergent laparotomy, a Cushing's ulcer, a hemorrhagic gastric ulcer, an adhesive ileus and acute terminal ileitis were investigated on this study. Two grams of CTM dissolving into 100 ml of physiological saline solution were prepared and administered in a period of 1 hour by intravenous drip (i.v.d.) for all cases after operation. The measurement of CTM-concentration in serum and intraperitoneal exudate were made at 30 minutes, 60 minutes following administration by serum and intraperitoneal exudate were made at 30 minutes, 60 minutes following administration by i.v.d. and at 30 minutes, hourly intervals over a 6-hour period after total administration. The intraperitoneal exudate for this study were collected through a drain which provided at the operation. These samples were measured by Agar-well method at Takeda Central Research Laboratory. Takeda Co., Ltd. The mean value of serum CTM at 30 minutes after the beginning of i.v.d. administration was 65.7 micrograms/ml and the maximum value, 79 micrograms/ml was obtained at 60 minutes. The following mean values at 30 minutes, 1 hour, 2 hours, 3 hours, 4 hours, 5 hours and 6 hours after total administration were 39.1, 26.4, 14.6, 8.9, 4.7 and 0.6 microgram/ml. On the other hand, the mean values of CTM intraperitineal exudate at the same time intervals were 4.9, 20.7, 32.0, 34.3, 22.4, 13.9, 11.2, 8.7 and 3.3 micrograms/ml. These results showed that there were much transfer of CTM between serum and intraperitoneal exudate corresponding to the amount of administration.

[Use of a lipid lymphographic agent, lipiodol, as a carrier of high molecular weight antitumor agent, smancs, for hepatocellular carcinoma].

Two advantages of the present therapeutic approach were described. Firstly, a selective deposition of lipiodol in tumor tissue was verified, thus more precise and accurate diagnosis by X-rays was possible either by CT or plain X-ray film. Secondly, pronounced accumulation of smancs in tumor tissue was observed, which established highly effective chemotherapy of unresectable hepatoma of 22 cases and 12 other cases based on (a) decrease in alpha-fetoprotein (86%), (b) tumor size (95%) and histology. Drug was given via the hepatic artery mostly 3-4 mg in 3-4 ml of lipiodol once every 3 to 4 weeks. Most patients have experienced a total dose of 6-8 mg in two cycles, but drug activity lasted more than 3 weeks. Neither hematosuppression nor anaphylaxis was observed. Major side effect was transient fever (38-39 degrees C) in about 50% of the cases which lasted no more than one week. Other minor side effect was abdominal pain during or after arterial infusion which lasted for about 20 min. Liver function was affected very slightly if any. Mild leukocytosis was observed in 65% of the patients.

Evaluation of different decompression schedules by an agarose gel bubble technique.

It is well known that diving decompression schedules vary according to country and that Navy schedules are often different from civilian ones. Furthermore, it is difficult to compare them effectively, because the bends incidence for each schedule is rarely known accurately. We have evaluated various decompression schedules by an agarose gel bubble technique in which the appearance of bubbles in a dry chamber at a controlled temperature was used, and we report this as a useful method of evaluating decompression tables. It is generally accepted that decompression sickness symptoms are due to bubble formation and the body's subsequent physiological reaction. Individual differences and physical conditions are also influential factors. However, bubble formation appears to be the most important factor in initiating symptoms. As the number of bubbles is influenced by the physical decompression ratio, it can be estimated which decompression schedule is more likely to minimize the number of bubbles. Nine decompression schedules were studied in this experiment: U.S. Navy Table, French Navy Table, Japanese Standard Decompression Schedule Numbers 1 and 2, Royal Naval Physiological Laboratory Table, French Ministry of Labor Table, Model 1 Table by Mano et al., Washington State Table, and Blackpool Table.

[Clinical experience of cefoxitin in the treatment of infections complicated by intractable blood disorders].

A total of 19 patients who had local or systemic infections with a variety of difficult-to-treat blood disorders were treated with cefoxitin (CFX). The following findings were obtained. 1. Fifteen out of 19 patients' responses were judged "excellent" or "good" with an overall cure rate of 78.9%. 2. As to the side effects of CFX, transient elevations in S-GOT, S-GPT were observed in 3 patients, urine protein in 2 patients, and hematuria in 1 patient. However, these abnormalities cannot be solely attributed to CFX because these patients were treated with either antitumor agents or aminoglycosides, and some had undergone blood transfusions. 3. CFX should be considered for the treatment of severe infections because it is well tolerated, has a broad spectrum of antibacterial activity against both Gram-positive and Gram-negative pathogens and is highly resistant to beta-lactamases.
