[Hospital admissions for respiratory diseases in the aftermath of the great Hanshin earthquake].

A questionnaire was sent to 30 hospitals and medical institutes in and around the Hanshin district, where the Great Hanshin Earthquake, occurred on January 17th, 1995. The questions concerned patients who were admitted from the day of the earthquake until 10 weeks later. Answers were obtained from 18 of the 30 facilities and the total number of patients admitted with respiratory diseases was 148. Patients with lobar pneumonia or focal pneumonia, or both, accounted for 58.8% of the total, and those with upper respiratory tract infection made up 19.6%. Patients with acute-on-chronic respiratory failure made up 6.8% of the total, as did those with exacerbation of bronchial asthma. Some patients had underlying respiratory diseases, which included bronchial asthma, sequelae of tuberculosis, and chronic obstructive pulmonary disease, and many patients were more than 65 years old. The week with the highest number of admissions was the second week after the earthquake, and 69.6% of the patients were admitted within the first 3 weeks. At time of admission, most patients had been living in Temporary shelters, or with relatives of friends. Most patients (68.9%) were discharged after their conditions had improved, but 9.5% died. All but one of those who died were more than 65 years old. These data indicate that in most cases admission to the hospital resulted not from the earthquake itself but from secondary factors such as unsatisfactory facilities in temporary shelters, psychological stress, the common cold, influenza, overwork, and the disruption of medical services. These data should be useful in establishing medical and political strategies to reduce admissions of patients with respiratory diseases after natural disasters.

[Anterior uveitis with ankylosing spondylitis and HLA].

We investigated the relationship between anterior uveitis with ankylosing spondylitis (AS) and human leukocyte antigen (HLA) alleles. Forty patients were studied, 19 with anterior uveitis and 21 without it. No deviation in the frequencies of HLA-B27 antigen and related alleles were observed between the two groups of patients. Twelve cases (63.2%) with anterior uveitis had expressed HLA-DR8 (DRB1 0803). On the other hand, only 1 case had expressed the same antigen and this difference was statistically significant (relative risk: 34.3 Pc < 0.007) as we reported previously. We found HLA-DR8 was a candidate for susceptibility to anterior uveitis in AS.

[Evaluation of the clinical usefulness of super dynamic 99mTc-HM-PAO SPECT in ischemic cerebrovascular disease--detection of hypo- and hyperperfusion area].

The purpose of this study is to assess the clinical usefulness of super dynamic SPECT of 99mTc-HM-PAO. Six patients with unilateral occlusion of middle cerebral artery (MCA) or internal carotid artery (ICA) in the chronic phase, and 5 patients with subacute cerebral infarction were studied. We used a ring-type SPECT "HEADTOME." Two types of collimator were used: a high sensitivity (HS) collimator for super dynamic scan and a high resolution (HR) collimator for static scan. First, the intravenous constant infusion of 99mTc-HM-PAO (925-1480 MBq) for 1 minute was started. After 30 seconds from the beginning of the injection, we performed the 12 seconds/frame super dynamic SPECT for 2 minutes. Then, the static SPECT for 10 minutes was done. For semiquantitative analyses, differential percentage of regional activity between affected and non-affected hemispheres was calculated in the 6th frame image of super dynamic SPECT and static SPECT image. In all 6 patients with unilateral occlusion of MCA and ICA, super dynamic SPECT images showed the better contrast of low perfusion areas in comparison with the static SPECT images. In 5 patients with subacute cerebral infarction who showed focal hyperactivities on static SPECT, focal hyperactivities (hyperperfusion or hyperemia) were displayed in 3 cases, whereas, focal hypo- or isoactivities (hypo- or isoperfusion) were shown in 2 cases on super dynamic SPECT. However, all patients with subacute cerebral infarction showed hyperfixation on static SPECT as compared with super dynamic SPECT. Although the image quality on super dynamic SPECT is not as high as that on static SPECT, cerebral hemodynamics would be detected with less backdiffusion of 99mTc-HM-PAO from the brain to blood, and with less accumulation of hydrophilic components in subacute infarct region. In conclusion, super dynamic SPECT in early distribution of 99mTc-HM-PAO using dedicated SPECT device might be helpful to detect cerebral perfusion close to true cerebral blood flow distribution.

[Shunt patency after creation of a transjugular intrahepatic portosystemic shunt: evaluation with Doppler ultrasound].

TIPS (transjugular intrahepatic portosystemic shunt) is a new interventional radiologic procedure that is useful in the treatment of variceal bleeding and ascites due to portal hypertension. It is most important that the shunt remains patent for a long time after placement. The potential role of ultrasound in evaluating the patency of the shunt after TIPS has been studied. Duplex and color doppler sonography were performed in 20 patients with portal hypertension after TIPS. Imaging was done in 38 case. The findings were compared with those of follow-up angiography. Shunt patency was evaluated with Doppler US in 28 case. In 27 of them, the equivalent patent shunt was confirmed by angiography; the exception was one case of occlusion in follow-up angiography. Doppler signals in the shunt could not be detected in 10 cases. Follow-up angiography showed occlusion in 7 cases and patency in the other 3. Doppler US sensitivity was 90%, and its specificity was 88%. These results indicate that Doppler US is one of the most useful methods for evaluating shunt patency after TIPS.

[Relationship between diaphragm weight and body composition].

To study the relationship between diaphragm weight and body composition, we measured fat body mass and lean body mass by dual-energy X-ray absorptiometry (XR-26, Norland), and weighed the diaphragms of patients at autopsy. The subjects were 22 patients with various diseases (13 men and 9 women, aged 72.6 +/- 9.5 years). In 8 of the patients, respiratory muscle strength was assessed with maximal pressure at the mouth. The diaphragm weight was 185.2 +/- 58.2 g and it correlated significantly with lean body mass (r = 0.77, r = 0.73, p < 0.01). Maximal inspiratory pressure at TLC correlated significantly with diaphragm weight. These results show that respiratory muscle strength is related to diaphragm weight and that analysis of body composition may be useful in assessing changes in the mass of the diaphragm.

[Quantification of brain metabolites by 1H spectroscopy using cyclohexane as an external reference].

We tested the reliability of quantifying brain metabolite concentrations by MR spectroscopy. We employed a PRESS sequence and used a cyclohexane sample as an external standard. The signal intensity ratios of water and cyclohexane were measured by changing the geometrical arrangement of the cyclohexane sample and water phantom with various loading factors of the coil. Choline (Cho) concentrations were calculated from the signal intensity ratios of Cho and water and compared with actual Cho concentrations (1.0 to 2.6 mmol/L). The brain metabolites Cho, creatine/phosphocreatine (Cr/PCr) and N-acetylaspartate (NAA) were evaluated in five normal volunteers and six patients with metastatic brain cancer before (0 Gy), after 20 Gy and after 40 Gy of whole brain radiotherapy. Variation in the ratio of spectrum intensity between water and cyclohexane was less than 1.5% for various geometrical arrangements and loading factors. The error in quantification of Cho concentrations in the phantom study was 0.05 +/- 0.10 mmol/L. The concentrations measured in the volunteers were: Cho 1.6 +/- 0.4 mmol/L, Cr/PCr 6.4 +/- 1.1 mmol/L and NAA 8.2 +/- 0.1 mmol/L. There was no significant difference (p > 0.05) in these concentrations between the control group and the patients before, during, or after radiotherapy.

An autopsy case of extremely immature newborn covered with fetal membrane.

We report an autopsy case of extremely immature newborn totally covered with fetal membrane. The infant was a female, 32 cm in length, 662 g in weight, accompanied with the intact umbilical cord and placenta. No remarkable pathological findings for disease or anomalies were observed. Floating tests of the lungs and gastro-intestinal tract were completely negative. In histological examination, the alveoli in the lungs were partially slightly open, filled with amniotic fluid. Pulmonary surfactant was negative immunohistochemically. No evidence was found to confirm a live-born. It was diagnosed that the immaturity has caused the delayed rupture of the fetal membrane and the death during the delivery.

Pharmacological advantages of conjugation of Cu, Zn-superoxide dismutase with succinylated keratin fragment: improvement of biological properties and resistance to oxidative damage.

To improve the in vivo pharmacological potential of Cu, Zn-superoxide dismutase (Cu,Zn-SOD), human recombinant Cu,Zn-SOD was conjugated with succinylated keratin fragment (Suc-ker). Suc-ker-conjugated Cu,Zn-SOD (Suc-ker-SOD) was formed by attachment of about 3.7 mol of Suc-ker to 1 mol of Cu,Zn-SOD and exhibited an apparent mean molecular weight of 107 kDa. Suc-ker-SOD exhibited 74.1% SOD activity on a molar basis. When Suc-ker-SOD was administered i.v. into mice, its plasma half-life was prolonged to 20.5 min compared with 4.7 min for native SOD. After i.v. administration of 51Cr-labeled proteins into mice, native SOD was excreted rapidly into urine and no significant accumulation was observed in organs other than the kidneys. On the other had, Suc-ker-SOD was taken up rapidly by the liver. Furthermore, Suc-ker-SOD exhibited much lower immunogenicity and much better therapeutic effects than native and poly(styrene-co-maleic acid)-conjugated SOD against paraquat toxicity in mice. Moreover, the inactivation rate of Suc-ker-SOD by H2O2 was less than those of native SOD and polyethylene glycol-conjugated SOD. The effect of H2O2 on liganded Cu++ in the active site of Suc-ker-SOD was much less than those of other SOD derivatives. These results indicate that Suc-ker-SOD has advantages over the other SOD derivatives tested.

[Evaluation of regional cerebral blood flow in hippocampus by 99mTc-ECD SPECT: comparison with 123I-IMP SPECT].

We evaluated regional cerebral blood flow in the hippocampus in 21 patients with neurological disorders by 99mTc-ethyl cysteinate dimer (ECD) SPECT and compared the findings with those by 123I-N-isopropyl-p-iodoamphetamine (IMP) SPECT. In all subjects, SPECT was taken using ECD (740 MBq) and IMP (167 MBq) using high performance SPECT (3-head SPECT, Toshiba GCA-9300A/HG) within 3 weeks interval. Regional uptake in the hippocampus was examined in the coronal sections and images parallel to the longitudinal axis of hippocampus. ECD and IMP showed similar hippocampal uptake in 9 cases, but ECD showed less uptake than IMP in 12 cases (57%). Regional counts in the hippocampus were related to the counts in the lateral temporal cortices and the mean count ratio was lower in ECD by 8.8% than that in IMP (0.73 +/- 0.11 vs. 0.80 +/- 0.11, p < 0.001). In 6 patients suspected to have hippocampal lesion (4 cases of memory disturbance and 2 cases of epilepsy), all cases showed decreased uptake of ECD in hippocampus, but only 3 cases showed decreased uptake of IMP. This is suggestive to be relevant to the difference in the retention mechanism of ECD and IMP, but further investigation should be performed.

Scleroderma-polymyositis overlap syndrome associated with anti-Ku antibody and rimmed vacuole formation.

Scleroderma-polymyositis overlap syndrome is strongly associated with certain autoantibodies, such as anti-PM-Scl antibody and anti-Ku antibody. We describe a case of scleroderma-polymyositis overlap syndrome associated with anti-Ku antibody, which revealed rimmed vacuole formation in addition to intranuclear and intracytoplasmic filamentous inclusions in muscle biopsy specimens.

[A case of amotivational syndrome as a residual symptom after methamphetamine abuse].

We had a case of psychiatric evidence who was homeless and exhibited severe abulia and autism on detention for assault and battery. It was thought that his past history of chronic methamphetamine abuse and his familial history played some part in his showing such symptoms. His mother was alcohol dependent. He was an ACOA (adult child of alcoholics), which might have led to his chronic abuse of methamphetamine. On the other hand, it is well-known fact that the amotivational syndrome induced by marijuana abuse is typified by a diminution of ambition, productivity, and motivation. However, it has been contended that amotivational syndrome is induced not only by marijuana but also by amphetamine and its analogs, cocaine and volatile solvents. Since we positively support this view, we diagnosed the case as amotivational syndrome after long-term methamphetamine abuse. This was also a rare criminal case of amotivational state without hallucinations and delusions after methamphetamine abuse. We suggested that the crime committed in this case was closely related to crime induced by economic problems in residual states of schizophrenic offenders. This could be a case of both ACOA and methamphetamine dependence. There were unresolved alcohol- and drug-related problems in this case. Therefore, careful early intervention in a crisis, cooperation with the authorities and the institutions concerned, and comprehensive rehabilitation should be employed to resolve such alcohol- and drug-related problems.

Expression of inducible nitric oxide synthase and its involvement in pulmonary granulomatous inflammation in rats.

Two types of pulmonary granulomatosis were produced in rats by intratracheal instillation of zymosan or silica. In both models, immunostaining with anti-rat monoclonal antibody for inducible nitric oxide synthase (iNOS), ANOS11, showed that the intensity of iNOS immunoreactivity in the inflammatory lesions peaked at 3 days and declined thereafter. Immunohistochemical double staining and in situ hybridization demonstrated the expression of iNOS in neutrophils, monocyte-derived macrophages, and bronchiolar epithelial cells in the pulmonary lesions. Electron spin resonance spectroscopy revealed the production of an excessive amount of nitric oxide (NO) in the pulmonary lesions. Immunostaining with a polyclonal antibody against nitrotyrosine indicated the formation of nitrotyrosine residues in the granulomatous lesions, particularly in the periphery of the lesions, providing indirect evidence for the generation of peroxynitrite anion in the zymosan- or silica-instilled lungs. Administration of N omega-nitro-L-arginine methyl ester or S-methylisothiourea sulfate, which significantly suppressed NO production, resulted in marked reduction of monocyte/macrophage infiltration as well as in inhibition of induction of monocyte chemoattractant protein-1 in the lesions. These data indicate that NO and its more reactive product peroxynitrite anion may be important mediators of granuloma formation in the lung.

[Standardization of serum lipid examination: an attempt in Oita].

There is an obvious difference in currently available clinical laboratory data between hospitals. With the advent of information systems, the clinical laboratory information in the future must be standardized so that it can be used in medical treatment and health control throughout the life patients time. Establishing publicly acknowledged standards is essential for standardization to be translated into practice. The standards for clinical laboratory examination in Japan are approved by consensus in the Japanese Committee for Clinical Laboratory Standards (JCCLS). However, standards have yet to be established for most laboratory examinations. This poses is a major obstacle in promoting the standardization. Standardization should be implemented carefully with consideration of the worldwide trend. That is probably the reason why the consultation and approval at JCCLS have been delayed, but it is hoped that standards will be established as soon as possible. The committee for quality control survey in Oita Medical Association organized a Lipid Unit of the Sectional Committee for Standardization in 1994 and has since implemented standardization according to the lipid standardization program of the U.S. Centers for Disease Control and Prevention (CDC) with some modifications. Measures for improvement have been adopted for a trial period in parallel with four surveys, and in September, 1995 all 9 hospitals passed the standardization test performed on the basis of the CDC provisions for certification. The Oita Medical Association plans to expand the lipid standardization to all parts of the prefecture using the trial of standardization by the Lipid Unit as a model.
