The pupil cycle time (PCT) was measured in 22 HIV-positive patients and 22 age/sex-matched controls. Of the HIV-positive group, 13 (59%) met the Centre for Disease Control classification of acquired immunodeficiency syndrome (AIDS) defining illness. There was a highly statistically significant difference in the PCT between the HIV-infected group and the control group (p < 0.0001, Student's t-test). Within the HIV-infected group there was no statistically significant difference in the PCT between those patients with full-blown AIDS and the others. This study suggests that HIV infection may be associated with subclinical ocular autonomic dysfunction even in the earlier stages of HIV infection.
A combination of a single drop of each of 0.5% tropicamide and of 10% phenylephrine hydrochloride instilled after routine applanation tonometry gave consistent pupil dilatation in a population with cataract studied over 5 years. The dilatation allows reproducible photographic documentation of cataracts in long-term studies.
Nine patients with acquired immunodeficiency syndrome (AIDS) and cytomegalovirus (CMV) retinitis on maintenance therapy with ganciclovir: 9(1,3-dihydroxy-2-propoxymethyl) guanine (DHPG) at high dose (30 mg/kg/week) or low dose (20 mg/kg/week) were tested every 1-2 weeks for CMV isolation from blood, saliva, and urine. Duration of therapy ranged from 1.5 to 12 months (average 5.3 months). During pretreatment and low-dose and high-dose maintenance therapy, CMV was isolated from 48/59 (81%), 90/211 (43%), and 40/290 (14%) of specimens, respectively. Three patients with progressive retinitis had viraemia more frequently than did six patients with stable retinitis, CMV being isolated from 29/47 (62%) and 17/121 (14%) of blood samples, respectively.
Total serum tryptophan concentrations were measured in 74 cataractous patients and 124 controls. No significant differences were observed in the mean values for the cataractous and normal subjects, 11.4 +/- 2.6 and 11.3 +/- 2.2 micrograms/ml respectively, or in the range of values, 7.4-17.1 and 7.3-17.4 micrograms/ml respectively. These results do not support previous reports that serum tryptophan is elevated in cataractous subjects.
A retrospective review of school children presenting as partially sighted or 'blind' from cataract shows they account for 91 of 531 (17.1%) cases of visual impairment in a 9-year period, 1977-1985. An apparent peak of incidence occurred in 1983, but there was no obvious aetiology for this peak.

Vision is the primary mode of learning and if it is absent or impaired, other pathways must be stimulated. The range of appropriate services available to visually handicapped children is explained in relation to the school run by the Royal Victorian Institution for the Blind in Melbourne. A period of residential education may be needed for children with multiple impairment. Those whose handicap is primarily visual can usually live at home and have their education integrated with that of their normal peers. Home-based services and parent support systems for preschool and kindergarten age children are outlined. To be successful, they require sharing confidential medical and education information. The school provides a major input to a discussion group of all professionals dealing with such children. Information from the group includes the causes, and educational implications of visual handicap in Victorian children. The commonest pathologies are macular problems and congenital cataract.

A prospective study extending over 12 years has surveyed 107 latent diabetics and 172 clinical diabetics during pregnancy. Among the clinical diabetics background retinopathy was evident or developed during pregnancy in 40 cases. Eleven cases showed proliferative changes, only 1 such case appearing de novo during gestation. There was a higher incidence of fetal loss in the diabetic than the latent diabetic group, and this loss was highest in those with proliferative retinal disease. Pregnancy was to associated with any increased risk to the mother of progression of retinal changes and visual loss. The only known direct relationship, namely, that between duration of diabetes and ocular complications, is confirmed.
A new microsurgical system is presented for human and experimental corneal grafting. The system is based on novel methods of corneal cutting and holding which simplify collection procedures and minimise operator manipulation during transplantation.
After preliminary testing with pig, dog, rabbit, monkey, cat, and human cadaver eyes the CLCC system has been used for penetrating and lamellar keratoplasty in human subjects. Clinical experience covers a broad range of corneal pathology. Detailed operative instructions and observations are given. In the experimental situation the CLCC is being used to achieve deep lamellar corneal dissection for a continuing study of non-penetrating keratoplasty.
