Desmosome-enriched fractions were isolated from bovine muzzle epidermis either as desmosome-tonofilament complexes using a procedure involving treatment at pH9 or in the form of desmosomal residue fractions using a modification of the citric acid buffer (pH 2.3) method of Skerrow and Matoltsy [1]. Major polypeptides of high molecular weights (mol. wt.) were separated by gel electrophoresis, individual polypeptide bands were excised, and protein was eluted and used for immunization. Guinea pig antibodies raised against two prominent polypeptides of high mol. wt. (250,000 and 215,000) showed, on nitrocellulose paper blots of desmosome-tonofilament polypeptides separated by gel electrophoresis, extensive cross-reaction between a group or large polypeptides characteristic of desmosome-containing fractions, most notably polypeptides of 250 K, 215 K, 200 k, 175 K, and 164 K. These antibodies allowed, when used in immunofluorescence microscopy, the specific localization of desmosomal junctions (i) in sections through epithelia-containing tissue (e.g., epidermis, mucosae of tongue and esophagus, cornea, mammary gland, small intestine, liver, thymus, urothelium of bladder) and myocardium; (ii) on dissociated cells from these tissues; (iii) on various epithelial cells grown in culture; an (iv) in tumor-like proliferations of cultured epithelial cells injected into nude mice. Individual desmosomes could be visualized and resolved at the light microscopic level. No reaction was found in cells devoid of desmosomes and on other classes of intercellular junctions. Electron microscopic localization using immunoperoxidase techniques indicated that these proteins are located in, or close to, the desmosomal plague structure. It is proposed to use such antibodies against desmosomal proteins as markers specific to this so far only morphologically define class of junctions. Use of these markers will (i) improve identification and classification of intercellular junctions; (ii) facilitate determinations of the specific patterns of distributions of desmosomes and desmosomal protein in various cells and tissue; (iii) allow studies of formation and disintegration of desmosomes, and of the biosynthesis and possible recycling of their constituents; and (iv) provide tissue group-specific markers valuable in histology and diagnosis, especially for identification of epithelial and carcinoma cells.
Higher life-expectancy and progresses in operative medicine and anaesthesiology increasingly confront the anaesthesist with the necessity of performing anaesthesia in elderly patients. The most frequent disease in this group of patients are related to the cardiovascular system (hypertension, coronary heart disease, arrhythmia and myocardial insufficiency. Preoperative cardiovascular therapy with regard to anaesthesia is outlined.
The diagnostic determination of residual urine with radionuclides is a method without complications for the patient. It can be carried out without loss of time wherever nuclear medicine facilities are available using conventional measuring instruments. The radiation exposition is negligible. The special advantage of this method is the by-passing of the catheterization of the bladder. This means that the risks of a potential lesion of the urethra and infection of the urinary passages which are iatrogenically promoted are avoided. In keeping the standardized conditions the examination results are identical with those of the catheter method.
Changes in the negative change densities on the outside (by raising the phosphatidic acid concentration) or on the inside of the red blood cell membrane (by a breakdown of the polyphosphoinositides and a concomitant diacylglycerol production) do not explain the last step in echinocytosis, the release of spectrin-free vesicles.

Our investigations have shown the following: 1. 8-Chloro-6-(2-fluorophenyl)-1-methyl-4H-imidazo[1,5-a] [1,4]benzodiazepine (midazolam, Ro 21-3981, Dormicum) produces no adverse haemodynamic changes in healthy subjects. 2. The reduction in peripheral resistance becomes apparent mainly in the case of raised baseline values, as, for example, in hypertensive subjects, but it can also be of significance prior to anaesthesia, when there may be raised systemic vascular resistance due to stress. 3. Venous pooling, which leads to a decrease in pre-load and subsequent lowering of the cardiac index, can have a compensatory effect in cardiac insufficiency. Under certain conditions, as for example, in the case of imminent volume deficit, a marked fall in pressure is to be expected. 4. The haemodynamic effects of midazolam are thus limited to vascular reactions. Under certain circumstances volume therapy may be necessary before the drug is used.
8-Chloro-6-(2-fluorophenyl)-1-methyl-4H-imidazo[1,5-a] [1,4]benzodiazepine (midazolam, Ro 21-3981, Dormicum) is a new benzodiazepine which, when compared with its predecessors diazepam (Valium) and flunitrazepam (Rohypnol), has several outstanding advantages: We conducted hemodynamic investigations with 0.15 mg/kg b.w. midazolam in a total of 28 coronary patients divided into 3 groups. The new water-soluble benzodiazepine is at the dose given useful in the induction of and as an adjuvant to anesthesia because of its hemodynamic effect.
Reported in this paper are 50 pyogenic infections, with anaerobic germs being cultured in 40,7 per cent of the cases, as well as 105 examinations of patients with flour, with Haemophilus vaginalis having been isolated in 20 cases. Widespread occurrence of such pathogens and severity of infections caused by such bacteria call for close cooperation between clinical practitioners and microbiologists, as no safe diagnosis is practicable without involvement of the latter. Reference is made to possible therapies, using chloremphenicol and metronidazole.
Epidural opiate as a single analgesic during labour failed to relieve pain sufficiently. According to a synergistic effect of epidural opiate and local anaesthetic a combination of both drugs appears to be useful for obstetric analgesia. In a double-blind-study a single epidural morphine-application at an early stage of labour led to prolonged intervals of reinjection during conventional epidural anaesthesia with bupivacaine. The reduced dosage of local anaesthetic improves neonatal status and reduces the necessity of extractional aids. In addition the long-lasting postpartum pains. Side-effects, such as vomiting and pruritus, are rare.
In 75 patients epidural opiates were applied for relief of chronic cancer pain. In order to avoid local infection during long-term therapy part of the catheter was placed subcutaneously. Different opiates were used separately or in combination with local anaesthetics to define the degree and duration of pain relief after epidural opiate application. Haemodynamic and respiratory parameters, changes in lower extremity blood supply and other side-effects were recorded during epidural pain therapy. Epidural opiate application cause a long-lasting reduction of pain, which may become restricted during long-term or repeated use, especially after a period of systemic opiate therapy. Side-effects, for example slight respiratory depression in the first hour after injection, indicate an initial phase of resorption beeing followed by a long-lasting reduction of pain without attendant symptoms. Keeping in mind certain precautions epidural opiate therapy is superior to systemic opiate application.
Epidural opiate application was performed in 75 patients with chronic pain due to abdominal cancer. To reduce the risk of local infection, the epidural catheter was in part placed subcutaneously. Degree and duration of analgesia were determined after epidural morphine (with and without bupivacaine), pethidine or fentanyl. Hemodynamic and respiratory parameters were measured and side-effects were registered. The longest duration of analgesia was observed after epidural morphine. In long-term treatment there was a reduced efficiency and the frequency of daily injections increased, especially in those patients who already had received systemic opiates prior to the epidural opiate administration. This may be due to tolerance of the spinal receptors. A combination of epidural morphine with small doses of local anesthetics caused prolonged action and delayed the onset of tolerance. Besides slight influences on respiratory function, which may be referred to the initial period of systemic absorption, there were no relevant side-effects. Regarding certain precautions the epidural application of morphine may have advantages in comparison to systemic analgetics in treatment of chronic pain.

The only two children of clinically healthy parents both developed an acute lymphocytic leukemia of the T-cell type, one with a mediastinal mass, one without. Extensive laboratory studies revealed a combination of the following unusual circumstances. (1) The injection of leukemic bone marrow into BALB/c-nu/nu mice led to an explosive simultaneous development of disseminated lymphomatous tumors with murine karyotype. (2) HLA typing and MLC testing of all four family members revealed sharing of HLA-A,D and DRw determinants between the parents and pointed to the appearance of suppressor cell activity with the outbreak of the acute leukemia of one sibling. (3) Parental lymphocytes gave a low response to mitogen stimulation, suggesting a subclinical cellular immune defect. It is proposed that the siblings inherited from each parent a defective immune response factor, possibly related to HLA-D/DRw antigens, that predisposed to acute T-cell leukemia. The neoplastic process might have been triggered by a transferable agent.

The healing rates in carpal tunnel syndrome following surgical decompression are known to be excellent only in those cases which mainly show signs and symptoms without marked and permanent muscular atrophy of the thenar (i.e. without advanced damage to the motor units of the median nerve). In accordance with the findings of CURTIS (1973) we have added microsurgical techniques in a group of patients with very advanced disease. In a first series definite clinical recovery of the motor units of the median nerve was found in 80%. These clinical findings are now confirmed by the electrophysiological findings in a group of cases examined pre- and postoperatively by identical electrophysiological parameters. The results are discussed. To avoid additional damage to the nerve, experience in microsurgical techniques is of the utmost importance.
