Effect of Cryptotympana atrata extract on compound 48/80-induced anaphylactic reactions.

We studied the effect of the aqueous extract of Cryptotympana atrata Fabricius on anaphylactic reactions. Cryptotympana atrata completely inhibited compound 48/80-induced systemic anaphylactic shock at doses of 0.5 and 1.0 g/kg. When Cryptotympana atrata was given as pretreatment at concentrations ranging from 0.025 to 1.0 g/kg, the plasma histamine levels induced by compound 48/80 were reduced in a dose-dependent manner. Cryptotympana atrata also inhibited histamine release from rat peritoneal mast cells (RPMC) by compound 48/80. In addition, when Cryptotympana atrata was added, the level of cAMP in RPMC transiently and significantly increased about 14-fold compared with that of control cells. Our studies provide evidence that Cryptotympana atrata may be beneficial in the treatment of nonspecific anaphylactic reactions.

Inhibition of immediate-type allergic reaction by Rosa davurica Pall. in a murine model.

We studied the effect of Rosa davurica Pall. (Rosaceae) fruits (RdF) on immediate-type allergic reactions. RdF completely inhibited compound 48/80-induced systemic anaphylactic shock at the dose of 1 g/kg. When RdF was given as pretreatment, at concentrations ranging from 0.0001 to 1 g/kg, the serum histamine levels induced by compound 48/80 were reduced in a dose-dependent manner. RdF inhibited the passive cutaneous anaphylaxis activated by anti-dinitrophenyl (DNP) IgE antibody dose dependently. RdF also inhibited the histamine release induced by compound 48/80 or anti-DNP IgE from the rat peritoneal mast cells (RPMC). Moreover, RdF had a significant inhibitory effect on anti-DNP IgE-induced tumor necrosis factor-alpha production from RPMC. These results indicate that RdF may contain compounds with actions that inhibit mast cell degranulation in the rat.

Inhibition of lipopolysaccharide plus substance P-induced tumor necrosis factor-alpha production from astrocytes by Chongmyung-Tang.

Astrocytes have the capacity to secrete or respond to a variety of cytokines. In this study, we have examined whether an aqueous extract of Chongmyung-Tang (CmT) inhibits production of tumor necrosis factor-alpha (TNF-alpha) from primary cultures of mouse astrocytes. CmT (1 and 10 microg/ml) significantly inhibited the TNF-alpha production by astrocytes stimulated with lipopolysaccharide (LPS) and substance P (SP). Interleukin-1 (IL-1) has been shown to elevate TNF-alpha production from LPS-stimulated astrocytes while having no effect on astrocytes in the absence of LPS. We therefore examined whether IL-1 mediated inhibition of TNF-alpha production from primary astrocytes by CmT. Treatment of CmT (1 and 10 microg/ml) to astrocytes stimulated with both LPS and SP decreased IL-1 production significantly. In addition, reverse-transcribed polymerase chain reaction analysis demonstrated that significantly reduced level of the TNF-alpha mRNA was expressed in astrocytes treated with CmT. Our results suggest that CmT inhibits TNF-alpha production by reducing TNF-alpha mRNA expression.

Inhibition of immunoglobulin E production by Poncirus trifoliata fruit extract.

Immunoglobulin E (IgE) is the principal immunoglobulin involved in immediate hypersensitivities and chronic allergic diseases. The effect of an aqueous extract of Poncirus trifoliata (L) Raf. (Rutaceae) fruits (PTFE) on in vivo and in vitro IgE production was investigated. PTFE dose-dependently inhibited the active systemic anaphylaxis and serum IgE production induced by immunization with ovalbumin, Bordetella pertussis toxin and aluminum hydroxide gel. PTFE strongly inhibited interleukin 4 (IL-4)-dependent IgE production by lipopolysaccharide-stimulated murine whole spleen cells. In the case of U266 human IgE-bearing B cells, PTFE also showed an inhibitory effect on the IgE production. These results suggest that PTFE has an anti-allergic activity by inhibition of IgE production from B cells.

Inhibition of mast cell-dependent anaphylaxis by sodium salicylate.

Sodium salicylate (NaSal) is a commonly used agent with a wide pharmacological spectrum. The objective of the present study was to investigate the effect of NaSal on anaphylaxis. NaSal (10-1 and 1 mm) significantly inhibited systemic anaphylaxis induced by compound 48/80 in rats. NaSal also significantly inhibited local anaphylaxis activated by anti-dinitrophenyl (DNP) immunoglobulin E (IgE). NaSal (10-1 and 1 mm) significantly inhibited histamine release from rat peritoneal mast cells (RPMC) activated by compound 48/80 or anti-DNP IgE. Northern-blot analysis demonstrated that a significantly reduced level of the mRNA of L-histidine decarboxylase was expressed in mast cells treated with NaSal, compared with that without NaSal. NaSal (10-2 and 10-1 mm) had a significant inhibitory effect on anti-DNP IgE-induced tumour necrosis factor-alpha secretion from RPMC. The level of cyclic AMP in RPMC, when NaSal (1 mm) was added, transiently and significantly increased about sixfold compared with that of basal cells. These results suggest a possible use of NaSal in managing mast cell-dependent anaphylaxis.

Inhibition of human tumor growth by 2'-hydroxy- and 2'-benzoyloxycinnamaldehydes.

2'-Hydroxycinnamaldehyde (HCA) was isolated from Cinnamomum cassia Blume (Lauraceae) and 2'-benzoyloxycinnamaldehyde (BCA) was prepared by the reaction of HCA and benzoyl chloride. HCA and BCA strongly inhibited in vitro growth of 29 kinds of human cancer cells and in vivo growth of SW-620 human tumor xenograft without the loss of body weight in nude mice. HCA prevented adherence of SW-620 cells to the culture surface but did not inhibit oncogenic K-Ras processing, implying its antitumor mechanisms at the cellular level.

Predictors of patient satisfaction in an outpatient plastic surgery clinic.

Although outcome measurement in medical care has traditionally included various aspects of clinical and functional status, patient satisfaction is another important metric in the assessment of health care quality. Determining which factors contribute most to patient satisfaction can assist health care providers in improving care. In an era of diminishing resources, satisfaction ratings can further aid third-party payers in determining who provides those services. The authors used the ordinal logistic regression method to develop a predictive model for patient satisfaction in an outpatient plastic surgery clinic. Consecutive patients who attended a university outpatient plastic surgery clinic were asked to complete the Visit Specific Patient Satisfaction Questionnaire (VSQ) after their clinic visit. Type of clinic (e.g., hand clinic, aesthetic clinic, breast clinic) and demographic variables like age, gender, race, and education were added to the questionnaire to control for possible confounding effects. The authors constructed an ordinal logistic regression model using the overall visit response as the dependent variable and the eight other response categories as independent variables. A total of 345 patients completed the VSQ, which takes each patient less than 2 minutes to complete. The patient response rate was more than 95%. Statistically significant predictors included (1) personal manner of physician (odds ratio [OR], 18.0; p = 0.0002), (2) time spent with physician (OR, 4.7; p = 0.0099), (3) length of time to get an appointment (OR, 4.6; p = 0.0055), and (4) explanation of what was done (OR, 3.9; p = 0.0263). There was no statistically significant association between overall satisfaction scores and the following factors: (1) length of wait in the clinic (OR, 2.7; p = 0.0747), (2) getting through to the clinic by phone (OR, 0.71; p = 0.5439), (3) convenience of the clinic's location (OR, 2.3; p = 0.1368), and (4) technical skills of the physician (OR, 1.0; p = 0.9974). The predictive model was adjusted for possible confounding due to the type of clinic and demographic variables. In this study the most important predictors of patient satisfaction were those related to efficient clinic operation (scheduling of appointments and clinic waiting time) and the quality of the patient-physician interaction. Clinic facilities (like clinic location and ease of phone contact) were not significant predictors. Because patients often have difficulty assessing the technical skills of physicians, this variable was not a significant predictor. To improve patient satisfaction in plastic surgery outpatient clinics, efforts and resources should be directed toward expedient and empathic delivery of care.

A differential index assignment scheme for tree-structured vector quantization.

A differential index (DI) assignment scheme is proposed for the image encoding system in which a variable-length tree-structured vector quantizer (VLTSVQ) is adopted. Each source vector is quantized into a terminal node of VLTSVQ and each terminal node is represented as a unique binary vector. The proposed index assignment scheme utilizes the correlation between interblocks of the image to increase the compression ratio with the image quality maintained. Simulation results show that the proposed scheme achieves a much higher compression ratio than the conventional one does and that the amount of the bit rate reduction of the proposed scheme becomes large as the correlation of the image becomes large. The proposed encoding scheme can be effectively used to encode MR images whose pixel values are, in general, highly correlated with those of the neighbor pixels.

The impact of environmental factors on emergency medicine resident career choice.

OBJECTIVE: To evaluate the impact of environmental factors on emergency medicine (EM) resident career choice. METHODS: Program directors of all U.S. EM residencies were surveyed in November 1997. A 22-item questionnaire assessed resources allocated to research, fellowship availability, academic productivity of faculty and residents, and career choices of residency graduates. RESULTS: The response rate was 83%. The program director (mean+/-SD) estimates of resident career choice were as follows: 27.8+/-19.1% pursued academic positions with emphasis on teaching, 5.4+/-9.8% pursued academic positions with emphasis on research, and 66.8+/-23.1%, pursued private practice positions. In addition, 5.70+/-6.13% of the residency graduates were estimated to seek fellowship training. Univariate analyses demonstrated that increasing departmental funding for research, having substantial resource availability (defined as having at least two of the following: dedicated laboratory space; support for a laboratory research technician/assistant, a clinical research nurse or study coordinator, a statistician, or an assistant with a PhD degree), a greater number of peer-reviewed publications by residents (r = 0.22; p = 0.08), and a greater number of peer-reviewed publications by faculty (r = 0.26; p = 0.04) positively correlated with the percentage of graduates who pursue academic research careers. Using multiple regression, however, increasing intramural funding and the presence of substantial resource availability were the only variables predictive of resident pursuit of an academic research career. CONCLUSION: Modification of the EM training environment may influence the career choices of graduates. Specifically, greater commitment of departmental funds and support of resources for research may enhance the likelihood of a trainee's choosing an academic research career.

Inhibition of mast cell-dependent anaphylaxis by succinic acid.

We have examined the effect of succinic acid on anaphylaxis. Succinic acid (100 mM) significantly inhibited systemic anaphylaxis induced by compound 48/80 in mice and dose-dependently inhibited local anaphylaxis activated by anti-dinitrophenyl IgE. Further 10 and 100 mM significantly inhibited histamine release from rat peritoneal mast cells activated by compound 48/80 or anti-dinitrophenyl IgE. In addition succinic acid (0.1 and 1 mM) had a significant inhibitory effect on anti-dinitrophenyl IgE-induced tumour necrosis factor-alpha secretion from rat peritoneal mast cells. The level of cyclic AMP in rat peritoneal mast cells, when succinic acid (100 mM) was added, transiently and significantly increased about 4 times compared with that of basal cells. These results suggest a possible use of succinic acid in managing mast cell-dependent anaphylaxis.

The evaluation of antianaphylactic effect of Oryza sativa L. in rats.

This study was carried out to examine the effect of methanol extract of Oryza sativa L. (Dong-Jin in Korean, abbreviate as Os-DJ hereafter) on anaphylaxis. Os-DJ (10(-5) to 1 g/kg) dose-dependently inhibited systemic anaphylaxis induced by compound 48/80 in rats. When Os-DJ was pretreated at concentration ranging from 10(-5) to 1 g/kg, the serum histamine levels were reduced in a dose-dependent manner. Os-DJ (1 g/kg) also significantly inhibited local anaphylaxis activated by anti-dinitrophenyl (DNP) IgE. Moreover, Os-DJ dose-dependently inhibited the histamine release from rat peritoneal mast cells (RPMC) activated by compound 48/80 or anti-DNP IgE. These results indicate that Os-DJ possess antianaphylactic activity by inhibition of histamine release from mast cells in vivo and in vitro.

Shini-san inhibits mast cell-dependent immediate-type allergic reactions.

Shini-San has been used for treatment of allergic disease in Korea. However, its effect in experimental models remains unknown. The mast cell plays a pivotal role in initiating allergic response by secreting intracytoplasmic granular mediators such as histamine. The present report describes an inhibitory effect of Shini-San on mast cell-mediated immediate-type allergic reactions. Topical application of compound 48/80 can induce an ear swelling response in normal (WBB6F1(-)+/+) mice but not in congenic mast cell-deficient WBB6F1-W/WV mice. Shini-San inhibited concentration-dependent mast cell-dependent ear swelling response induced by compound 48/80 in normal mice. Shini-San inhibited concentration-dependent passive cutaneous anaphylaxis induced by anti-dinitrophenyl (DNP) immunoglobulin E (IgE) in rats by topical application. Shini-San also inhibited in concentration-dependent fashion the histamine release from the rat peritoneal mast cells by compound 48/80 or anti-DNP IgE. Moreover, Shini-San had a significant inhibitory effect on compound 48/80-induced systemic anaphylactic reaction. These results indicate that Shini-San inhibits immediate type allergic reactions by inhibition of mast cell degranulation in vivo and in vitro.

Lavender oil inhibits immediate-type allergic reaction in mice and rats.

We studied the effects of lavender oil on mast cell-mediated immediate-type allergic reactions in mice and rats. Lavender oil (1:500, 1:100, 1:10, 1:1, 1:0) inhibited concentration-dependently mast cell-dependent ear swelling response induced by compound 48/80 in mice by both topical and intradermal application. Lavender oil (1:500, 1:100, 1:10, 1:1, 1:0) inhibited concentration-dependently passive cutaneous anaphylaxis induced by anti-dinitrophenyl (DNP) IgE in rats by both topical and intradermal application. Lavender oil (1:500, 1:100, 1:10, 1:1, 1:0) also inhibited concentration-dependently the histamine release from the peritoneal mast cells by compound 48/80 or anti-DNP IgE. Moreover, lavender oil (1:1000, 1:100, 1:10, 1:0) had a significant inhibitory effect on anti-DNP IgE-induced tumour necrosis factor-alpha secretion from peritoneal mast cells. These results indicate that lavender oil inhibits immediate-type allergic reactions by inhibition of mast cell degranulation in-vivo and in-vitro.

Effect of ionizing radiation on the differentiation of ROS 17/2.8 osteoblasts through free radicals.

Although the acceleration of bone regeneration by radiation has been reported, the mechanisms of action of radiation on bone are unclear. The present results indicate that ionizing radiation-stimulated differentiation could result from the generation of reactive oxygen species during radiation exposure. The free radical release is considered as the most important mechanism of bone effect by radiation treatment. In addition, we report that radiation induced transient activation of c-Jun N-terminal kinase/stress-activated protein kinase (JNK/SAPK) activation and the transcription factor, AP-1. The JNK and AP-1 activation is mediated with radiation-released free radicals in ROS 17/2.8 osteoblasts. These results indicate that ionizing radiation at a single dose of up to 5 Gray stimulates differentiation of ROS 17/2.8 osteoblasts via free radial release which may affect JNK/SAPK and AP-1 activities.

Asiasari radix inhibits immunoglobulin E production on experimental models in vitro and in vivo.

Immunoglobulin (Ig) E is the principal Ig involved in immediate hypersensitivities and chronic allergic diseases. The hallmark of these disorders is increased IgE production. The effect of an aqueous extract of the roots of Asiasari radix (ARAE) on an in vivo and in vitro IgE production was investigated. ARAE dose-dependently inhibited the active systemic anaphylaxis and serum IgE production induced by immunization with ovalbumin, Bordetella pertussis toxin and aluminum hydroxide gel. ARAE strongly inhibited IL-4-dependent IgE production by lipopolysaccharide- stimulated murine whole spleen cells. In the case of U266 human IgE-bearing B cells, ARAE also showed an inhibitory effect on the IgE production. These results suggest that ARAE has an anti-allergic activity by inhibition of IgE production from B cells.

Action of Sosiho-Tang on systemic and local anaphylaxis by anal administration.

The herbal formulation Soshiho-Tang (SS-Tang) has been used against allergic disease for generations, and still occupies an important place in traditional medicine in Korea. Previously, we reported that SS-Tang potently inhibited mast cell- mediated anaphylaxis when orally administered. In this study, we investigated the effect of SS-Tang by anal administration in anaphylaxis responses. SS-Tang dose-dependently inhibited compound 48/80-induced systemic anaphylaxis with doses of 10(-4) to 1 g/kg 1 h before anally administered. Of special note, SS-Tang inhibited systemic anaphylaxis completely with a dose of 1 g/kg. SS-Tang reduced plasma histamine levels induced by compound 48/80 significantly. However, the mortality was 100% when SS-Tang was administered after compound 48/80 treatment. SS-Tang (10(-1) g/kg) also inhibited passive cutaneous anaphylaxis activated by anti-dinitrophenyl IgE antibody by 30.9%. These results provide evidence that anal therapy of SS-Tang may be beneficial in the treatment of systemic and local anaphylaxis.

Effect of Schizonepeta tenuifolia extract on mast cell-mediated immediate-type hypersensitivity in rats.

We investigated the effect of an aqueous extract of Schizonepeta tenuifolia (STAE) on mast cell-mediated immediate-type hypersensitivity. STAE inhibited systemic allergic reaction induced by compound 48/80 in rats dose-dependently. STAE also inhibited plasma histamine levels induced by compound 48/80. STAE inhibited local allergic reaction activated by anti-dinitrophenyl (DNP) IgE. In addition, STAE does-dependently inhibited histamine release from rat peritoneal mast cells (RPMC) activated by compound 48/80 or anti-DNP IgE. However, STAE had a significant enhancing effect on anti-DNP IgE-induced tumor necrosis factor-alpha (TNF-alpha) production from RPMC. These results indicate that STAE inhibits immediate-type hypersensitivity and suggest that STAE can selectively activate the TNF-alpha production from RPMC.
